Genes and cells associated with sublingual allergen immunotherapy revealed by integrated transcriptome analysis in allergic rhinitis patients.

BACKGROUND: Sublingual allergen immunotherapy (SLIT) is known as an effective therapy for allergic rhinitis (AR), although its efficacy varies across different treatment duration and individuals. The factors associated with the duration and response of SLIT need to be explored. METHODS: Twenty-seven AR patients undergoing SLIT and three healthy volunteers were recruited. Peripheral blood mononuclear cells were isolated for RNA sequencing, and key results were further examined by quantitative PCR (qPCR) in available PBMC samples. Time-correlated and response-associated genes were identified an

Thirty years in, regulatory T cells are ready for medicine.

Regulatory T (Treg) cells are a specialized subset of CD4+ T cells indispensable for the establishment and maintenance of immunological tolerance. Treg cells employ diverse mechanisms of immune regulation mediated by a broad repertoire of immunosuppressive molecules under the control of the lineage-specifying transcription factor FoxP3. Functional defects in Treg cells disrupt immune homeostasis and are implicated in the pathogenesis of autoimmune diseases, chronic inflammatory disorders, and cancer. In this article, we revisit three decades of progress since Sakaguchi's seminal discovery of C

Wolf in Sheep's Clothing: A Case of Sarcoidosis Exacerbating Asthma.

As a leading cause of chronic respiratory morbidity worldwide, asthma remains a complex disorder characterized by inflammation, recurrent exacerbations, and episodic symptoms. Pulmonary sarcoidosis is a potential misdiagnosis for asthma. Symptoms such as shortness of breath, chest tightness, and dry cough can be present in both conditions, making differentiation difficult. This case highlights the diagnostic challenges and clinical overlap between severe asthma and sarcoidosis in a patient with uncontrolled respiratory symptoms, underscoring the importance of considering other etiologies when

Real-world insights into genomic reprogramming during CAR-T cell manufacturing in India.

BACKGROUND: While CAR-T cell therapy has transformed outcomes in B-cell malignancies, most genomic insights originate from clinical trials. There is a paucity of data describing molecular changes during CAR-T manufacturing in real-world practice, particularly for non-US CAR constructs. METHODS: This was an observational study conducted at a tertiary care center in India where a total of 4 set of paired samples comprising leukapheresis starting material (n=4) and the corresponding final CAR-T cell product (n=4) were analyzed. Analysis was done using NanoString nCounter® platform with a targete

Emerging CAR-Treg and regulatory T cell therapies for inflammatory bowel diseases: a systematic review of a new era of treatment.

BACKGROUND: Inflammatory bowel disease (IBD) is a chronic immune-mediated disorder characterized by dysregulated mucosal immunity. Despite major advances in therapies, a high proportion of patients do not achieve or sustain remission. Cellular immunotherapies have emerged as a mechanistically distinct therapeutic strategy with potential relevance for refractory disease. In the present paper, the authors aim to evaluate the rationale and current evidence supporting chimeric antigen receptor (CAR)-engineered immune cells and regulatory T (Treg)-cell-based therapies in IBD. Preclinical studies an

T cell exhaustion in sepsis: mechanisms, biomarkers, and immune-reversal strategies.

Sepsis is increasingly recognized as a dynamic immune disorder in which early hyperinflammation may coexist with or progress to profound immunosuppression. T-cell exhaustion is a central feature of this immune paralysis and is characterized by lymphopenia, impaired proliferation, reduced effector cytokine production, inhibitory receptor upregulation, metabolic dysfunction, and defective antibacterial immunity. Recent studies have expanded the mechanistic landscape of sepsis-induced T-cell exhaustion beyond the classical PD-1/PD-L1 axis. TIGIT-mediated suppression of CD4+ T-cell immunity, extra

Polyarteritis nodosa and sarcoidosis presenting with myalgia and myositis-mimicking magnetic resonance imaging findings: A report of two cases.

Diseases presenting with muscle symptoms are diverse. We report two cases initially suspected of idiopathic inflammatory myopathies that were ultimately diagnosed as polyarteritis nodosa and sarcoidosis, respectively. Case 1 was a 30-year-old man with fever, limb muscle pain, and elevated inflammatory markers. Muscle magnetic resonance imaging showed high signal intensity on fat-suppressed T2-weighted images, and biopsy revealed vasculitis with fibrinoid necrosis, leading to a diagnosis of polyarteritis nodosa. Case 2 was a 70-year-old man with proximal muscle pain and weakness and elevated mu

Tonsillar Tfh cells contribute to the pathogenesis of IgA nephropathy in collaboration with memory B cells.

INTRODUCTION: Immunoglobulin A nephropathy (IgAN) is a chronic glomerular disease characterized by mesangial IgA deposition, which can progress to end-stage renal failure. Tonsillectomy is used as a therapeutic intervention to slow disease progression and provide early clinical benefit. However, the immune mechanisms within the palatine tonsils that contribute to IgAN pathogenesis remain incompletely understood. METHODS: To define the tonsillar immune environment that regulates pathological antibody production, we analyzed T and B lymphocyte subsets in relation to renal function. We also cond

Isolated subcutaneous sarcoidosis presenting as multiple nodules: a rare case with diagnostic challenges.

INTRODUCTION AND IMPORTANCE: Isolated subcutaneous sarcoidosis is an uncommon form of sarcoidosis, typically presenting as firm, painless nodules beneath clinically normal skin. Its rarity, especially in male patients or in atypical anatomical locations, can mimic malignant or infectious processes and cause significant diagnostic uncertainty. Recognizing this entity is important to prevent misdiagnosis and unnecessary interventions. CASE PRESENTATION: A 51-year-old man presented with multiple progressively enlarging subcutaneous nodules over the scalp, face, trunk, and extremities for 1 year,

Milder Form of Vici Syndrome Due to Novel Missense Variant in Epg5 Gene Affecting Splicing: A Case Report.

Objective. Vici syndrome is a rare neurodevelopmental disorder with multisystem involvement, caused by mutations in the EPG5 gene encoding a protein involved in autophagy. It includes dysgenesis of the corpus callosum, cataracts, hypopigmentation, cardiomyopathy, and immuno-deficiency. Here we report a case of a 9-year-old boy of Roma ethnicity with a milder form of Vici syndrome and a novel variant in the EPG5 gene. Methods. DNA and RNA were extracted from the whole blood. Whole exome sequencing was performed and analysed with an in-house bioinformatics pipeline. A mini-gene assay was perform

Integrative transcriptomic analysis and experimental validation identify GPR97 and PROK2 as novel genes upregulated in acute myocardial infarction.

Acute myocardial infarction (AMI) is a major global health burden. Current diagnostic reliance on high-sensitivity cardiac troponin (hs-cTn) is limited by its lack of disease specificity and inability to reveal upstream molecular and immune mechanisms. This study aimed to identify robust core genes and their functional networks in AMI via integrated multi-dataset analysis and experimental validation. Three peripheral blood transcriptomic datasets (GSE60993, GSE61144, GSE97320) were obtained from GEO. Differentially expressed genes (DEGs) were identified using limma, with common DEGs extracted.

Mechanisms of therapeutic antibodies immunogenicity: infliximab oligomeric aggregates increase T-cell response and uptake by dendritic cells involving mannose-sensitive receptor CD206.

INTRODUCTION: Aggregation of therapeutic antibodies (Ab)s is now recognized as a major product-related factor associated with increased potential of immunogenicity. Regulatory agencies have established limits for the presence of (sub)visible particles in commercialized products. These particles are considered a potential risk factor for the development of unwanted immune responses. However, little is known concerning the role of oligomeric Abs aggregates of nanometric size in Abs' immunogenicity. The objective of this work was to better understand the role and the mechanisms of T-cell response

Efineptakin alfa (NT-I7) improves overall survival and induces immune niches in murine tumors.

Tertiary lymphoid structures (TLSs) are emerging as good predictive biomarkers of response to cancer immunotherapy. However, therapeutic strategies to induce these structures are currently limited. We evaluated the therapeutic benefit of efineptakin alfa (NT-I7), a long-acting form of IL-7, as well as its ability to remodel the tumor immune microenvironment and induce TLSs in murine lung and colorectal tumor models. NT-I7 improved overall survival in tumor-bearing mice. It also increased the abundance of T, B, dendritic cells, and stem-like CD8 T cells and promoted the formation of immune aggr

Hepatocellular Carcinoma Associated with Hepatic Sarcoidosis and Budd-Chiari Syndrome: A Case Report and Review of the Literature.

Hepatic sarcoidosis may progress to cirrhosis, whereas Budd-Chiari syndrome (BCS) is characterized by a hepatic venous outflow obstruction and it is associated with the risk of liver injury. The coexistence of hepatic sarcoidosis and BCS is extremely rare, and hepatocellular carcinoma (HCC) has not been previously reported in this setting. We describe the case of a 71-year-old woman with cutaneous sarcoidosis who presented with HCC and was diagnosed with BCS and hepatic granulomas with advanced fibrosis. This case suggests that these two conditions may act additively or synergistically to crea

Humanized biparatopic nanobody-based CAR-T cells overcome antigen-heterogeneity in multiple myeloma.

BACKGROUND: Chimeric antigen receptor (CAR) T-cell therapy targeting B-cell maturation antigen (BCMA) shows activity in multiple myeloma (MM), yet relapse remains common owing to heterogeneous BCMA expression and soluble BCMA (sBCMA). Improving BCMA-directed CAR-T therapy requires persistence, enhanced antitumor activity, tolerance to low antigen density and resistance to sBCMA-mediated inhibition. METHODS: We identified anti-BCMA VHHs from a humanized phage display library, constructed monospecific and biparatopic CARs, and evaluated their function and optimal designs in preclinical models,

Strict carbohydrate restriction enables volume-based FDG-PET/CT to outperform SUVmax for risk stratification in cardiac sarcoidosis.

BACKGROUND: FDG-PET/CT SUVmax reflects focal metabolic intensity but not the patchy inflammatory burden of cardiac sarcoidosis (CS). We evaluated whether volume-based total cardiac metabolic volume (tCMV) and total cardiac metabolic activity (tCMA), measured after optimized suppression, predict long-term major adverse cardiac events (MACE) better than SUVmax. METHODS: Forty-eight consecutive patients with CS classified by JCS 2016 criteria underwent FDG-PET/CT after 24-h strict carbohydrate restriction without high-fat supplementation and delayed cardiac imaging at 120 min. tCMV, tCMA, and SU

A novel frameshift variant in TTC29 associated with total asthenozoospermia and male infertility.

BACKGROUND: Severe sperm motility defects are an important cause of male infertility and are frequently associated with sperm flagellar abnormalities. TTC29 encodes an evolutionarily conserved axonemal protein implicated in sperm flagellar assembly and motility, yet the pathogenic mechanisms of TTC29 variants remain incompletely understood. METHODS: A Chinese man with total asthenozoospermia was investigated using whole-exome sequencing, Sanger sequencing, transmission electron microscopy, sperm immunofluorescence, and heterologous expression analysis in HEK293 T cells. Site-Specific Immunological Changes Induced by Narrowband UVB in Acral and Non-Acral Vitiligo.

Acral vitiligo responds less well to NB-UVB than non-acral disease, but site-specific immunologic changes during therapy are incompletely defined. The objective of this study was to compare NB-UVB-associated changes in T cell subsets and soluble mediators in acral versus non-acral vitiligo. Thirty patients with non-segmental vitiligo were enrolled. Ten underwent paired biopsies (acral and non-acral) before and after 6 months of NB-UVB for flow cytometry (Tregs, CD8+ T cells, CD8+CD45RO+ memory T cells, CD8+CD69+CD103+ TRM).
