The impact of COVID-19 on all-cause mortality and life expectancy: findings from a population-based cohort in uMkhanyakude district, northern KwaZulu-natal.

BACKGROUND: Despite the comprehensive death reporting system in South Africa, significant gaps persist in our understanding of the pandemic's impact in rural areas. METHODS: We computed monthly death counts stratified by demographic factors from a population-based demographic surveillance cohort in rural KwaZulu-Natal. We fit seasonally adjusted negative binomial regression models to estimate excess mortality during the COVID-19 pandemic period (5 March 2020 to 31 December 2021) compared to the pre-pandemic period (1 January 2015 to 4 March 2020). We then assessed the relative risk of death b

Neutrophils suppress adaptive αβ T cells but not the innate γδ T cells and natural killer cells.

The presence of polymorphonuclear neutrophils (PMNs) in the tumor microenvironment is associated with lower survival rates for cancer patients. Presumably due to the immunosuppressive activity of neutrophils (also known as Myeloid-Derived Suppressor Cell (MDSC) activity). We investigated whether this MDSC activity shows any selectivity in suppressing specific lymphocyte subsets. Our data show that CD4+ and CD8+ T cells, both naive and memory fractions, are sensitive to PMN-MDSC activity. Similarly, regulatory T cells, believed to play an important immunosuppressive function themselves, were af

Beyond the embryo: reframing recurrent implantation failure through immunopriming and endometrial immune dysregulation.

Recurrent implantation failure (RIF) remains one of the most debated and emotionally challenging conditions in assisted reproductive technology (ART). Despite remarkable advances in embryology, including blastocyst culture, time-lapse imaging and preimplantation genetic testing for aneuploidy (PGTA), a significant proportion of patients continue to fail implantation even after transfer of morphologically good-quality or euploid embryos. Contemporary reproductive medicine has largely adopted an embryocentric model of implantation, often defining RIF based upon repeated failed embryo transfers.

T-cell and monocyte DNA methylation and Wnt dysregulation in common variable immunodeficiency: an exploratory study.

BACKGROUND: Many patients with common variable immunodeficiency (CVID) develop inflammatory/autoimmune complications associated with T-cell and monocyte dysregulation which may involve epigenetic modifications. OBJECTIVE: To characterize genome-wide CpG methylation in CD3+ T-cells and CD14+ monocytes in CVID and relate epigenetic alterations to gene expression and circulating proteins. METHODS: Whole-genome bisulphite sequencing was performed on purified CD3+ T-cells and CD14+ monocytes from three CVID patients with inflammatory complications and three healthy controls per cell type. Differe

Peptide optimization to improve antigen presentation and antitumor responses in sarcoma.

Sarcomas are rare and heterogeneous malignancies for which therapeutic options remain limited, particularly in advanced disease. Standard treatments, including surgery, radiotherapy, chemotherapy, and approved targeted agents, provide marginal improvements in progression-free and overall survival, highlighting the need for immunotherapeutic strategies. Therapeutic cancer vaccines represent a promising strategy, but clinical efficacy has been limited by low antigen immunogenicity and an immunosuppressive tumor microenvironment (TME). Enhancing antigen presentation and T-cell receptor (TCR) enga

Single-cell mass cytometry cartography highlights systemic lupus erythematosus and Sjögren's disease immune differences.

OBJECTIVES: Autoimmune diseases such as primary Sjögren's disease (pSjD) and systemic lupus erythematosus (SLE) share clinical and serological features, including type I interferon signatures, autoantibody formation, and chronic inflammation, but the underlying immunopathology is not the same. To support the development of targeted therapies, we aimed to systematically characterize disease-specific alterations in peripheral immune cell subsets of pSjD and SLE compared to healthy donors (HD). METHODS: We analyzed peripheral blood mononuclear cells from 15 HD, 12 pSjD and 21 SLE patients by mas

Xeno-free production of lentiviral particles: implementing hPL as FBS replacement in cell culture.

Commonly used for delivering fluorescent or bioluminescent reporters to target genes, lentiviral vectors are an "easy-to-clone" vehicle to study up- and downregulation of gene expression induced by drug inhibitors, cytokine stimulants, chemicals, and environmental toxicants. To produce viral particles safely, several plasmid constructs are delivered to human embryonic kidney cells (HEK293-T), which use the cellular machinery to package and assemble non-replicative lentiviruses with high efficiency. However, the cell culture of HEK293-T cells still largely relies on the use of fetal bovine seru

Peripheral immune profiles separate disease activity stages in birdshot uveitis.

INTRODUCTION: Birdshot uveitis (BU) is a chronic, debilitating posterior uveitis strongly associated with HLA-A*29, supporting a suspected autoimmune etiology. We used high-dimensional mass cytometry and clinical stratification to characterize the peripheral immune landscape in BU and identify disease activity-dependent signatures. METHODS: In this cross-sectional study, we included 36 treated BU patients and 31 age- and sex-matched healthy controls (HCs). All participants underwent comprehensive ophthalmologic evaluation with multimodal imaging. Patients were stratified by an expert ophthalm

Model-based integration of chimeric antigen receptor T-cell kinetics and tumor features to predict therapeutic outcomes in patients with B-cell malignancies.

INTRODUCTION: Chimeric antigen receptor (CAR) T-cell therapy is a standard treatment for several hematological malignancies, yet patient-specific responses and toxicities remain hard to predict. Although multiple covariates and risk scores exist, their prognostic accuracy remains limited. METHODS: We prospectively measured CAR T-cell kinetics in peripheral blood of 92 patients with B-cell malignancies using multicolor flow cytometry. To explore underlying mechanisms, we developed a mechanistic model of CAR T-cell and tumor cell interactions. RESULTS: We captured four phases of CAR T-cell kin

Immune cell fibrotic plasticity: mesenchymal-like reprogramming of macrophages, neutrophils and T cells.

Fibrosis has traditionally been viewed as a fibroblast- and myofibroblast-centred process, in which immune cells regulate matrix deposition primarily through inflammatory mediators and paracrine signalling. However, advances in single-cell profiling, spatial analyses and lineage-resolved approaches are beginning to challenge this rigid division of cellular labour. Emerging evidence indicates that, in persistently injured and maladaptively repairing tissues, selected immune cell populations can acquire matrix producing, cytoskeletal remodelling and mesenchymal-like functions, thereby contributi

B Cell Deficiency Protects Against the Development of c-MET and Beta-Catenin Driven Hepatocellular Carcinoma in Mice.

Hepatocellular carcinoma (HCC), a lethal form of liver cancer, continues to show rising mortality rates despite advancements in immunotherapies. This highlights the need for more effective treatments. Recent studies linked the presence of B cells within human tumors, including HCC, to improved responses to immunotherapy and better prognoses. Specifically, tumor-infiltrating B cells have been associated with reduced cancer cell proliferation, suggesting that B cell-mediated responses may be harnessed as a potential therapeutic strategy against HCC. However, in advanced HCC, IL-10-producing regu

A spatiotemporally programmed neutrophil-hitchhiking polymeric prodrug potentiates cancer immunotherapy.

Enhancing tumor accumulation after systemic administration while minimizing off-target hepatic deposition remains a major challenge for effective cancer therapy. Herein, we developed a "prime-and-hitch" two-step nanoplatform strategy to address this issue. A PEGylated Ce6 prodrug (CPD) was administered via a single low-dose intratumoral injection, enabling sustained intratumor retention for up to 10 days. Laser irradiation induced a temporally controlled acute inflammatory response, with inflammatory cytokines and neutrophil infiltration peaking at 1 h. In this optimal window, an acidic tumor

IL-37a facilitates allergen immunotherapy in a house dust mite-induced mouse model of allergic asthma.

In recent years, the incidence of allergic asthma has increased dramatically. Allergen immunotherapy (AIT) is an effective approach to achieve long-term remission of allergic asthma and has gained widespread attention. However, current AIT suffers from low efficacy and long treatment duration. In this study, we found that IL-37a transgenic mice with allergic asthma exhibited reduced pulmonary inflammation, decreased Th2 cytokines, and lower allergen-specific IgE following AIT. Mechanistic studies revealed increased IL-37+ Bregs, suggesting that IL-37a may promote AIT through Bregs. Our study d

Could post-transplant cyclophosphamide benefit autoimmune disease patients following autologous hematopoietic stem cell transplantation?

Autologous hematopoietic stem cell transplantation (auto-HSCT) has emerged as an effective therapeutic option for selected patients with severe autoimmune diseases, particularly relapsingremitting multiple sclerosis. However, disease relapse remains a significant challenge, suggesting the persistence of autoreactive lymphocytes and incomplete restoration of sustained immune tolerance. Post-transplant cyclophosphamide (PTCy) is widely employed following allogeneic HSCT for graftversus- host disease prophylaxis. Here, we propose that incorporation of PTCy into auto-HSCT protocols may further imp

Oxidative Stress, Post-Translational Modifications, and Type 1 Diabetes Pathogenesis.

Type 1 diabetes (T1D) is an organ-specific autoimmune disease that destroys insulin-producing beta-cells. Insulin is a vital hormone for regulating glycemia that often undergoes post-translational modifications during processing from pre-proinsulin to biologically active insulin. While necessary for proper glucose homeostasis, insulin is also an autoantigen recognized by autoreactive T cells in T1D. Cellular stress pathways including endoplasmic reticulum stress and oxidative stress mediated by an increase in reactive oxygen species (ROS) synthesis and/or a concomitant decrease in antioxidant

The effect of low-temperature-induced immune activation in elderly idiopathic facial palsy: the association between Th1/Th2 imbalance and facial nerve inflammation.

The pathogenesis of idiopathic facial palsy in the elderly has not been fully elucidated. Recent studies have shown that exposure to low temperatures may participate in the pathological process through immune-mediated pathways. This article reviews that cold stimulation can disrupt the Th1/Th2 immune balance, manifested as reduced secretion of Th1-type cytokines and upregulation of Th2-type cytokines, thereby inducing peripheral inflammatory responses around the facial nerve, leading to nerve edema, demyelination, and conduction dysfunction. Due to the immune aging characteristics of the elder

Turning cold tumors hot: Strategies to enhance antitumor immunity.

Immunologically cold tumors remain a major obstacle to effective cancer immunotherapy because they lack sufficient immune infiltration and fail to sustain productive antitumor immune responses. Although substantial progress has been made in understanding the mechanisms underlying immune evasion, only a minority of patients achieve durable benefit from current immunotherapeutic approaches. In this review, we discuss emerging strategies aimed at converting cold tumors into immune-responsive lesions by enhancing tumor antigenicity and promoting effective immune priming. We examine how these appro

Hyaluronan fragments modulate inflammation in preeclampsia: a focus on Th17/Treg balance.

Pregnant women with preeclampsia (PE) exhibit an exacerbated inflammatory immune response. Hyaluronan (HA), a damage-associated molecular pattern (DAMP), is increased in the plasma of women with PE and may differentially modulate immune responses depending on its molecular weight. Forty-five women were investigated, including 20 with PE, 20 normotensive (NT) pregnant women, and 5 healthy non-pregnant (NP) women. Plasma samples were obtained for TNF, IL-10, HA, and soluble CD44 (sCD44) analyses by ELISA. Expression of the CD44 receptor, as well as intracytoplasmic transcription factors T-bet (T
