Comparison between enzyme-linked immunospot assay and intracellular cytokine flow cytometry assay of cytomegalovirus-specific T-cell response in healthy participants.

Human cytomegalovirus (HCMV) usually establishes a lifelong latent infection after primary infection. Reactivation occurs sporadically and is controlled by cell-mediated immune response (CMIR). Monitoring of CMIR against CMV is mandatory in immunocompromised patients to adjust immunosuppressive drugs to prevent serious end-organ damage after CMV reactivation. Intracellular staining (ICS) and enzyme-linked immunospot (ELISPOT) quantifying CMV-specific T-cells are generally used as surrogate markers for CMIR against CMV. Whether the results of these 2 methods correlate well is not known. This st

A pilot study of the CMV inSIGHT T-cell immunity panel to assess cytomegalovirus (CMV) cell mediated immunity (CMI) in allogeneic hematopoietic cell transplant (HCT) recipients.

BACKGROUND: Cytomegalovirus T-cell mediated immunity (CMV CMI) is essential for CMV control after allogeneic hematopoietic cell transplantation (HCT). Clinical experience with commercially available CMV CMI assays in letermovir (LTV) recipients is limited. OBJECTIVE: To evaluate the performance of the CMV inSIGHT T-cell immunity panel (Eurofins Viracor, Lenexa, KS) and factors associated with positive CMV CMI in LTV recipients. We also correlated the CMV inSIGHT with an in-house CMV-induced T-cell activation assay. STUDY DESIGN: In this prospective, observational study, the CMV inSIGHT was p

Letermovir does not affect long-term polyclonal immune reconstitution after allogeneic hematopoietic stem cell transplantation with ATG-based GvHD prophylaxis.

INTRODUCTION: Letermovir (LET) is an effective prophylaxis for human cytomegalovirus (HCMV) reactivations in HCMV-seropositive patients after allogeneic stem cell transplantation (allo-SCT). HCMV promotes polyfunctional T-cell responses, leading to HCMV-specific immune reconstitution (IR), thus contributing to polyclonal IR. METHODS: We retrospectively analyzed HCMV-seropositive patients undergoing allo-SCT with ATG-based GvHD prophylaxis to assess the impact of LET on timing and quality of polyclonal IR. Two cohorts were identified: a pre-emptive treatment (PET) historical cohort transplante

Virus-specific resident memory T cell networks sustain immunity in human oral mucosa.

BACKGROUND: Durable protection against viral infections relies on tissue-resident memory T (TRM) cells at pathogen entry sites. The oral cavity is a critical mucosal barrier frequently exposed to viral pathogens; however, the phenotypic characteristics and spatial organization of virus-specific T cells in the human oral mucosa remain poorly understood. OBJECTIVE: This study aimed to define how virus-specific T cells maintain long-term residency in the oral mucosa and to identify cellular interactions supporting their persistence. METHODS: We profiled predicted virus-specific T cells in the b

Congenital cytomegalovirus infection drives oligoclonal expansion of cytotoxic γδ T cells from early fetal progenitors.

Gamma delta (γδ) T cells emerge early during human gestation and are uniquely equipped to protect the fetus and infant following infection, due to their innate-like recognition of conserved molecular ligands. Following congenital cytomegalovirus infection (cCMV), Vδ1 T cells have been shown to expand, differentiate, and upregulate cytotoxic mediators, but display markedly restricted γδTCR diversity compared to CMV-infected adults. Early fetal γδ T cells are biased toward rapid effector function and comprise a distinct ontological "layer" that can be distinguished from late gestation γδ T cells

The Memory Inflation Response Against Spread-Defective CMV Requires Priming-Independent CD4+ T Cell Help.

Memory inflation (MI) is a distinctive CD8+ T cell response to chronic cytomegalovirus (CMV) infection, characterized by non-contracting, functional populations that are predominantly effector differentiated. Harnessing such responses has received growing attention as a promising vaccination strategy, using spread-defective CMV-vectors. However, many underlying mechanistic aspects of CD8+ MI responses, including their dependency on CD4+ T cell help, remain unclear. Using a Rag KO transfer model, we demonstrate that CD4+ T cells enhance effector differentiation of inflationary CD8+ T cells when

MAPK-activated protein kinase 2 orchestrates memory T-cell inflation in cytomegalovirus infection.

Memory T-cell inflation is a distinctive immunological phenomenon observed during persistent viral infections such as cytomegalovirus (CMV). Unlike conventional memory T-cell responses, which contract after infection resolution, a subset of CMV-specific T cells undergoes a progressive and sustained expansion, termed "inflation", which is thought to be critical for long-term immune surveillance. The molecular mechanisms that govern memory T-cell inflation remain incompletely understood, yet they are pivotal for understanding immune persistence and designing strategies against chronic viral infe

A federated digital twin reveals cytomegalovirus reactivation impairs CAR-T cell therapy via IL-15-mediated cytokine competition in B-Cell lymphoma.

BACKGROUND: Cytomegalovirus (CMV) reactivation occurs in 30 - 40 % of seropositive patients receiving chimeric antigen receptor T-cell (CAR-T) therapy for B-cell lymphoma and is strongly associated with treatment failure. However, the causal immunological mechanism driving this failure whether through direct viral cytopathic effects, T-cell exhaustion, or resource competition remains undefined. Furthermore, existing predictive models lack the mechanistic insight needed to guide intervention. We developed a privacy-preserving, mechanistic digital twin to test the "cytokine sink" hypothesis, wh

Integrative immunoinformatics and structural modeling for the rational design of a multi-epitope vaccine candidate against human cytomegalovirus.

Human cytomegalovirus (CMV) is a globally widespread pathogen associated with significant morbidity in immunocompromised individuals. Despite its clinical importance, no licensed vaccine is currently available. This study aimed to design a rational multi-epitope vaccine candidate targeting CMV using an integrative approach combining immunoinformatics and structural biology. Viral proteins were screened to identify epitopes with high affinity for B cells, cytotoxic T cells (CTLs), and helper T cells (HTLs) using the Immune Epitope Database (IEDB). Selected epitopes were filtered according to th

Cytomegalovirus-induced T cell responses accelerate Alzheimer's disease progression in mice.

Infections have long been implicated as causative factors in Alzheimer's disease (AD). Multiple studies have further suggested a key role for herpesviruses, such as cytomegalovirus (CMV). Using transgenic 3xTg-AD mice, we demonstrate that systemic infection with the β-herpesvirus murine CMV (MCMV) accelerates the development of cognitive decline, tauopathy and synaptic loss in the hippocampus, all of which are key features of AD. Accelerated disease progression after infection was associated with substantial lymphocyte infiltration into the brain, dominated by MCMV-specific effector memory CD8

γδ T cells show distinct responses to CMV after stem cell transplantation.

Cytomegalovirus (CMV) reactivation is a frequent complication after allogeneic hematopoietic stem cell transplantation (aHSCT) and critically shapes immune reconstitution. However, the clonal and functional dynamics of T cell responses to CMV remain insufficiently defined. In this study, we combined longitudinal single-cell RNA sequencing with paired T cell receptor sequencing to track γδ and αβ T cell clones at clonal resolution across five post-transplant time points in patients with and without CMV reactivation. This integrative approach revealed marked, patient-specific expansion of non-Vγ

A comparative performance evaluation of the VIDAS cytomegalovirus (CMV) interferon-gamma release assay (IGRA) and T-SPOT CMV for measuring CMV-specific T-cell responses in allogeneic hematopoietic stem cell transplant recipients receiving letermovir prophylaxis.

UNLABELLED: Assessment of cytomegalovirus (CMV) cell-mediated immunity (CMV-CMI) using standardized, commercially available interferon-gamma release assays (IGRAs) may improve the clinical management of CMV infection in allogeneic hematopoietic stem cell transplant recipients (allo-HCT). Studies comparing the performance of CMV IGRA assays have been conducted in this setting, revealing frequent discrepancies between assays. Here, we compared the performance of a newly released VIDAS CMV IGRA (bioMérieux) and the T-SPOT. CMV (Oxford Immunotec) was evaluated in a cohort of 85 allo-HCT recipients

Protective Role of Donor KIR B Haplotype in Cytomegalovirus Reactivation Following T-Cell-Depleted Hematopoietic Stem Cell Transplantation.

Background: Cytomegalovirus (CMV) reactivation is a major complication after hematopoietic stem cell transplantation (HSCT). Natural killer (NK) cells help control CMV through killer-cell immunoglobulin-like receptors (KIRs) and their HLA ligands, but donor-derived CMV-specific T-cells may confound the interpretation of NK-mediated effects. Methods: We analyzed 276 HLA-matched (10/10) adults receiving ATG-based T-cell-depleted myeloablative HSCT with a known donor and recipient CMV serostatus. The donor and recipient KIR genotypes were scored by the Cooley B-content score (0-4; ≥2 = high). Cli

Early CMV-specific T cell immune reconstitution after kidney transplantation: A prospective AIM-based cohort study.

OBJECTIVES: Cytomegalovirus (CMV) remains a major cause of morbidity following kidney transplantation (KT). We applied flow cytometry-based phenotyping and activation-induced marker (AIM) assays to characterize early immune reconstitution and CMV-specific T-cell dynamics after KT and to determine whether early AIM responses are associated with clinically significant CMV infection (CS-CMVi). METHODS: Twenty-nine adult KT recipients were followed prospectively for 12 months: 13 donor CMV-positive/recipient CMV-negative (D+/R-) and 16 CMV-seropositive recipients (R+). CMV-specific CD4+ and CD8+

Human leukocyte antigen genotype modulates cytomegalovirus- and Epstein-Barr virus-specific T-cell response in healthy donors for optimized virus-specific T-cell donor selection.

INTRODUCTION: Reactivation of cytomegalovirus (CMV) and Epstein-Barr virus (EBV) is a major complication in immunocompromised patients, and understanding donor variability in antiviral immunity is essential for optimizing adoptive T-cell therapy. This study aimed to identify immunogenetic determinants of virus-specific T-cell responses in healthy donors to guide donor selection. METHODS: CMV- and EBV-specific T-cell responses were characterized using flow cytometry and IFN-g ELISpot after stimulation with peptide pools, followed by integrated analyses correlating the response magnitude with h

Immunophenotype and proviral landscape of HTLV-1c infection and pulmonary disease.

BACKGROUND: Human T-lymphotropic virus 1 (HTLV-1) integrates into host DNA, resulting in life-long infection that underlies malignancy, inflammatory disease, and early all-cause mortality. HTLV-1 subtype-C endures as an endemic infection in Central Australia, and is associated with pulmonary disease. The cellular and viral features driving HTLV-1c pathogenesis remain poorly understood. METHODS: We recruited a cohort of 41 First Nations participants from Alice Springs Hospital in Central Australia. We analysed plasma biomarker sVCAM1, and three circulating CD4+ T-cell phenotypes by flow cytome

Cytotoxic Vδ2+ T cell subsets expand in response to malaria in human tonsil and spleen organoids.

Vaccine effectiveness against malaria is dramatically reduced in malaria-exposed compared to malaria-naïve populations, potentially due to altered immune responses in secondary lymphoid organs following repeated infection. Newly developed human tonsil and spleen organoids, which replicate key features of B and T cell immunity, provide an exciting opportunity to overcome challenges of other models and to improve our understanding of innate-adaptive interactions in lymphoid tissue. The objectives of this study were to use these organoids to investigate the impact of malaria parasites on 1) cells

Dynamic immune modulation in fasciolosis: From protective to permissive responses.

This review examines the evolution of the host immune response throughout Fasciola hepatica and Fasciola gigantica infection (fasciolosis). Newly Excysted Juveniles (NEJ) migrate to the peritoneum, where they transiently trigger a protective Th1 immune response that clears most parasites within approximately one week. The remaining flukes penetrate the hepatic parenchyma and initiate a non-protective Th2 immune response. This response promotes parasite survival and reduces hepatic damage by preventing excessive immune damage and inducing rapid fibrous tissue repair. Finally, a mixed regulatory

Plasmodium yoelii infection induces lung injury by modulating type 2 conventional dendritic cells autophagy via the STAT3-IRF4 signaling.

Malaria is an infectious disease caused by Plasmodium that severely impacts human health, often resulting in lung injury. Classical type 2 dendritic cells (cDC2) in the lungs play a crucial role in the pathogenesis of asthma and infectious diseases; however, their specific functions during Plasmodium infection remain poorly understood. In this study, we demonstrated a significant accumulation and activation of cDC2 in the lungs of mice infected with Plasmodium. While the phagocytosis ability of activated cDC2 decreases, it promotes the differentiation of CD4+ T cells towards Th1 cells, thereby
