T cell dysfunction and metabolic disruption in chronic hepatitis C virus infection.

Hepatitis C virus (HCV) infection remains a major global health burden and a leading cause of chronic liver disease, cirrhosis, and hepatocellular carcinoma. Despite the availability of highly effective direct-acting antivirals, sustained immune dysfunction and long-term complications continue to challenge disease management. Chronic HCV infection is facilitated by multiple viral evasion mechanisms, including rapid sequence variation, disruption of innate antiviral signaling, and altered natural killer cell function. A key feature of disease progression is the dysfunction of virus-specific CD4

Compensatory relationships determine the impact of TGF-β on the humoral immune response to hepatitis B surface antigen.

Despite an effective vaccine against the hepatitis B virus (HBV), there are about 250 million people living with chronic HBV (CHB) worldwide and one million deaths annually. Most children and about 5% of adults exposed to HBV will fail to clear the virus, developing a lifelong infection. Hepatitis B surface antigen (HBsAg)-specific antibody (HBsAb) is protective in uninfected individuals and is considered a component of a functional cure. Immune factors that play important roles in regulating inflammation, such as TGF-β, IL-10, and regulatory T cells (Tregs), may also contribute to CHB pathoge

Engineering a dual-antigen mRNA vaccine to restore immune control in chronic hepatitis B.

Therapeutic vaccination for chronic hepatitis B (CHB) remains challenging, as persistent immune tolerance to hepatitis B surface antigen (HBsAg) impedes anti-HBsAg seroconversion. Here we show that screening antigen combinations incorporating HBsAg identifies preS1-HBsAg as the optimal pairing. The resulting dual-antigen mRNA vaccine elicits robust hepatitis B virus (HBV)-specific immune responses in chronic HBV mouse models, leading to near-complete viral genome clearance, marked reduction of multiple HBV antigens, and serological conversion. Mechanistic analyses reveal that preS1 serves as t

A Review of Recent Advances in Chimeric Antigen Receptor (CAR) T-Cell Therapy for Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) is the most common form of primary liver cancer and poses a major global health burden. It remains a leading cause of cancer-related death worldwide, with persistently high incidence and mortality in regions affected by chronic viral hepatitis, cirrhosis, alcohol-related liver disease, and metabolic dysfunction-associated steatotic liver disease. Although surgical resection, liver transplantation, locoregional therapies, molecular targeted agents, and immune checkpoint inhibitors have improved treatment options, outcomes for advanced HCC remain unsatisfactory. HBV-Specific T Cell Responses for Short-Term Hepatitis Progression Prediction: A Comparative, Interpretable Machine Learning Study.

BACKGROUND & AIMS: As central determinants of viral control and immunopathology, hepatitis B virus (HBV)-specific T-cell responses provide more clinically meaningful information than routine biochemical and virological markers. Nevertheless, most existing prediction models rely primarily on clinical parameters and rarely incorporate HBV-specific T-cell responses, potentially limiting their predictive performance and biological interpretability. To develop an accurate and interpretable model for predicting hepatitis progression, we evaluated diverse machine learning (ML) methods integrating mul

Long-term antigen reduction does not achieve durable HBV control in mice but enhances therapeutic vaccine efficacy.

BACKGROUND & AIMS: Chronic HBV infection has an increasing death toll, but curative therapies are lacking. The study aimed to determine whether long-term suppression of viral antigens using RNA-interference restores HBV-specific immunity and achieves durable virus control, and how long HBV antigens must be suppressed to enable therapeutic vaccination to restore immunity. METHODS: HBV-transgenic or AAV-HBV-infected, HBV-carrier mice were treated for up to 7 months with liver-directed small interfering RNAs (siRNAs) or short-hairpin RNAs (shRNAs) that target all HBV transcripts. The antiviral e

A Distinct Immune Signature Characterizes Cirrhosis in Ethiopian Individuals With Chronic HBV Infection.

BACKGROUND AND AIMS: The immune cell phenotype of African patients living with chronic hepatitis B virus (HBV) infection is rarely described, despite high infection rates. This study investigated the differences in immune cell phenotypes in four distinct clinical phases of disease in an Ethiopian cohort. METHODS: Two hundred eighty-seven patients with untreated HBV infection, and no human immunodeficiency virus and hepatitis C virus infection, were included at Adama Hospital, Ethiopia. Patients were categorized based on clinical parameters into the following groups: low-replicative infection,

Emerging Immune-Based Therapeutic Strategies in Hepatocellular Carcinoma.

Hepatocellular carcinoma (HCC) remains a leading cause of cancer-related mortality worldwide despite recent advances in systemic therapy. The introduction of immune checkpoint inhibitors has substantially expanded treatment options for advanced disease; however, durable responses are observed only in a subset of patients. Increasing evidence indicates that this limited efficacy is largely driven by the complex immune landscape of HCC, which is shaped by disease aetiology, tumour microenvironment-mediated immune suppression, and impaired innate immune surveillance. Chronic viral hepatitis and m

HIV dominates T-cell immune responses in HIV-HBV co-infection and is associated with increased HIV reservoir transcription.

Due to the effectiveness of antiretroviral therapy (ART), Human Immunodeficiency Virus (HIV) infection is now managed as a chronic condition. However, people with HIV (PWH) remain susceptible to co-infections, including Hepatitis B virus (HBV), with 15% of PWH in Africa being co-infected. While co-infections may accelerate disease progression and increase AIDS-related mortality, their effects on T-cell responses and HIV reservoir size are unclear. This study compared T-cell responses in people with HIV-HBV co-infection, HIV or HBV alone, and assessed the effects of HBV on the HIV reservoir siz

Anti-HBsAg antibody mAb19-LS enhances antiviral immunity in humans with chronic hepatitis B.

Chronic infection with hepatitis B virus (HBV) is characterized by persistent expression of hepatitis B surface antigen (HBsAg), which is associated with profound immune tolerance. Although nucleos(t)ide analogue therapy effectively suppresses viral replication, it neither eliminates HBV nor reverses virus-specific immune dysfunction. Here, we report the results of two parallel first-in-human, dose-escalation studies evaluating a single infusion of mAb19-LS, a long-acting IgG1 monoclonal antibody targeting HBsAg, in individuals with chronic HBV infection receiving nucleos(t)ide analogue therap

Chronic HBV Infection Disrupts CCL5-Secreting cNK Cells and Attenuates Liver Accumulation and Activation of DCs and HBV-Specific T Cells.

Understanding the immunological mechanisms underlying chronic hepatitis B virus (HBV) infection-induced immune tolerance is critical for developing effective therapeutic strategies, yet the mechanisms by which natural killer (NK)-cell subsets regulate dendritic cell (DC)-mediated antiviral immunity during chronic HBV infection remain unclear. Here, we demonstrated that chronic HBV infection disrupted intrahepatic DCs accumulation and activation by suppressing CC chemokine ligand 5 (CCL5) production from hepatic NK cells. Importantly, chronic HBV infection reduced the number of hepatic conventi

Modular delivery of co-stimulatory signals through a PD-1-based immunoswitch receptor improves the functionality of Hepatitis B Virus-specific engineered T cells.

In chronic hepatitis B virus infection (cHBV), T-cell responses are skewed. This prevents the elimination of HBV-infected hepatocytes, contributes to hepatocellular carcinoma development, and limits the efficacy of T-cell therapies. To counteract the immune checkpoint PD-1/PD-L1-interaction in adoptive T-cell therapy, we developed PD-1-based immunoswitch receptors that convert PD-L1 engagement into co-stimulation. We designed immunoswitch receptors linking a PD-1 ectodomain to intracellular CD28, 4-1BB, or OX40 signaling domains and expressed them in HBV-specific chimeric antigen receptor (CAR

PD-1/PD-L1 blockade as part of combination strategies toward functional cure of chronic hepatitis B.

Chronic hepatitis B (CHB) affects 260 million people worldwide. Despite advances in antiviral therapies, functional cure, characterized by sustained loss of hepatitis B surface antigen (HBsAg) and durable viral control after treatment cessation, remains infrequent. The PD-1/PD-L1 immune checkpoint pathway plays an important role in the exhaustion of hepatitis B virus (HBV)-specific T cells, thereby limiting the immune system's ability to clear HBV. Blocking the PD-1/PD-L1 pathway has emerged as a promising strategy to overcome this immune exhaustion to some extent and reinvigorate antiviral T-

Fate of hepatitis D virus-specific CD8+ T cells during bulevirtide monotherapy in patients with chronic hepatitis delta.

BACKGROUND & AIMS: Bulevirtide reduces viremia in chronic hepatitis D virus (HDV) infection, but long-term treatment is required in most patients to prevent relapse. Sustained treatment responses may be fostered by therapy-induced amelioration of HDV-specific CD8+ T-cell responses that are exhausted because of high viremia and antigen loads during chronic HDV infection. We therefore studied the effect of bulevirtide monotherapy on HDV-specific CD8+ T-cell repertoire, phenotype, and functionality. METHODS: Twenty-eight patients with HDV infection and cirrhosis starting bulevirtide treatment we

HCV-Specific T-Cell Recovery After a Year of SVR Following DAA Therapy.

Direct-acting antiviral (DAA) drugs lead to sustained virologic response (SVR) in almost all treated patients with chronic hepatitis C virus (HCV) infection. Despite this success, the long-term effects of HCV clearance by DAA therapy on T-cell-mediated antiviral immunity remain unclear, particularly concerning the restoration of immune function and its role in providing subsequent protection against reinfection. Our results demonstrated enhanced HCV-specific cytokine production by CD4+ T cells in vitro, particularly in the activated CD38+ and CD38+ HLADR+ subsets 1 year after DAA-mediated cure

Downregulated HIPK2 Expression in T Cells Is Associated with Occult Hepatitis B Virus Infection.

Occult hepatitis B virus (HBV) infection (OBI) has the same transmission routes and clinical outcomes as overt HBV infection. However, due to the lack of reliable biomarkers, its occurrence and potential clinical harm are often overlooked. We previously found that the prevalence of OBI increases with age among individuals vaccinated at birth, but the transcriptome characteristics of their immune cells remain unknown. In this study, fifteen subjects born between 1987 and 1993 and vaccinated perinatally were recruited. They were divided into OBI, inactive chronic HBsAg carriers (ICHC) and health

From exhaustion to functional cure: frontiers in reversing HBV-specific T-cell immunity.

Chronic hepatitis B (CHB) remains a leading cause of liver-related morbidity and mortality, affecting approximately 254 million people worldwide and accounting for more than 800,000 deaths annually. Although nucleos(t)ide analogues (NAs) durably suppress viral replication, the annual rate of HBsAg loss is only ~1%, leaving functional cure as the foremost unmet clinical need. Exhaustion of HBV-specific CD8+ T cells is now widely accepted as the central immunological barrier to viral clearance. Single-cell sequencing, spatial transcriptomics and epigenome profiling have collectively redefined th

Hepatic T-cell dysfunction in chronic liver disease and hepatocellular carcinoma: a spatial systems-immunology framework for precision immunotherapy.

BACKGROUND: Chronic liver diseases, including chronic viral hepatitis, metabolic dysfunction-associated steatohepatitis (MASH), cirrhosis, and hepatocellular carcinoma (HCC), are characterized by progressive immune dysregulation driving chronic inflammation, fibrosis, and hepatocarcinogenesis. Despite hepatic T-cell dysfunction being a defining characteristic of these conditions, it is primarily examined through discrete molecular pathways instead of as an integrated component of the hepatic immune system. This reductionist perspective has limited mechanistic understanding and the development

Cellular and molecular networks governing precursor exhausted CD8+ T cells in chronic liver disease: Implications for immunotherapy.

Chronic liver diseases of diverse etiologies, along with primary liver cancers, represent major global health burdens with limited curative options. CD8+ T-cell exhaustion is a central barrier to effective immune control in these pathological settings. However, precursor exhausted CD8+ T cells (Tpex), a stem-like subset with self-renewal and proliferative potential, retain the ability to generate effector-like progeny and sustain long-term immune surveillance. In this review, we first provide a definition of Tpex to distinguish them from other related CD8+ T-cell states, including terminally e
