Detection of Natural Killer and Natural Killer T Cells Responses among Patients with Chronic Hepatitis C Infections in Sana'a City-Yemen.

Natural killer (NK) cells and natural killer T (NKT) cells contribute to the immune defense against hepatitis C virus (HCV). However, chronic hepatitis C (CHC) develops when there is a dysregulation of both innate and adaptive immune mechanisms. This case-control study was conducted to evaluate the immunological and clinical profiles of patients with chronic hepatitis C (CHC) in Sana'a, Yemen. The work focused on measuring T cell (CD3+), NK cell (CD56+), and NKT like cells (CD3 + CD56+) populations, along with examining viral load, hematological and biochemical indicators, and exploring how th

Complex dynamics behavior in a generalized diffusive multiple delays within-host model for HBV infection with capsids and adaptive immune response in a heterogeneous viral environment.

In this paper, we investigate the dynamics of a new chronic hepatitis B model through environmental-driven infection in heterogeneous and homogenous space. The model includes spatial diffusion, five time delays, and a general incidence function. The well-posedness of solutions, including the existence of global solutions, uniqueness, non-negativity, ultimate boundedness, is established. The reproduction numbers for virus infection, CTL immune response, antibody immune response, CTL immune competition and antibody immune competition, respectively, are calculated. By employing the Lyapunov funct

Immune correlates of outcomes after nucleos(t)ide analogue withdrawal in chronic hepatitis B: direct evidence, mechanistic context, and candidate biomarkers.

Nucleos(t)ide analogue (NA) discontinuation in chronic hepatitis B (CHB) can produce two clinically divergent trajectories: a minority of patients achieve sustained hepatitis B surface antigen (HBsAg) loss or off-treatment viral control, whereas others develop hepatitis flares without virological benefit. Recent longitudinal studies have begun to define the host immune correlates of these outcomes. This Mini Review separates direct evidence from NA-withdrawal cohorts from mechanistic evidence obtained in other CHB settings. Direct human data suggest that beneficial outcomes are associated with

High HBV antigen levels drive loss of vaccine-induced CD8+ T-cell function in carrier mice over time.

BACKGROUND AND AIM: Chronic hepatitis B virus (HBV) infection persists due to the dysfunction of virus-specific CD8+ T cells. High HBV antigen levels impair the success of therapeutic vaccination. The influence of viral titers on vaccine-induced immunity remains to be fully elucidated. We compared the kinetics of vaccine responses in low- and high-titer HBV carrier mice to identify tipping points and dissect the decisive steps determining the outcome of therapeutic vaccination. METHODS: We established low- and high-titer persistent HBV replication in the livers of immunocompetent C57BL/6J mic

RNAi-mediated HBV antigen shutdown enhances the antiviral immune effects of PEGIFNα via T and B cell crosstalk remodeling.

BACKGROUNDS/AIMS: PEGylated interferon-α (PEGIFNα) shows promise in treating chronic hepatitis B (CHB), yet patient response remains suboptimal. While suppressing hepatitis B virus (HBV) antigens by RNA interference (RNAi) could enhance PEGIFNα efficacy in CHB patients, the underlying immunological mechanisms remain obscure. METHODS: Using our newly established extracellular humanized IFNAR (IFNAR-hEC) mouse model of chronic HBV infection, we evaluated the efficacy of a GalNac-conjugated siRNA (GalNac-siHBV) alone or in combination with PEGIFNα. Phenotypic and functional characteristics of im

ANKRD11 deficiency reprograms CD8+ T cell differentiation to enhance immunity in chronic infection and cancer.

CD8+ T cell dysfunction is a major obstacle to hepatitis B virus (HBV) clearance and antitumor immunity. Here, using a humanized mouse model, we identify a T cell receptor targeting a clinically relevant HBV epitope and reveal ANKRD11 as a key epigenetic regulator of CD8+ T cell dysfunction in chronic infection and tumors. Ankrd11 knockout in CD8+ T cells enhances HBV-specific T cell proliferation and effector differentiation, especially under immunosuppressive conditions, via AP-1 family gene upregulation. Loss of Ankrd11 both drives the conversion of progenitor exhausted T cells into termina

Cirrhosis Impairs Reconstitution of the Circulating T-Lymphocyte Compartment After Direct-Acting Antiviral Therapy for Hepatitis C Virus Infection.

BACKGROUND: Direct-acting antivirals (DAAs) achieve sustained virological response (SVR) in over 95% of patients with chronic hepatitis C virus (HCV) infection. Although viral eradication allows partial immune restoration, it remains unclear whether established cirrhosis limits recovery of the circulating T lymphocyte compartment after SVR. METHODS: We performed a phenotypic and functional analysis of circulating T-helper (Th) and T-cytotoxic (Tc) lymphocytes in patients with chronic HCV infection without fibrosis (HCV-NoF, n = 21) or with cirrhosis (HCV-cirrhosis, n = 20), as well as in alco

Research Progress on Mechanisms of Immune Tolerance Induced by Hepatitis B Surface Antigen in Chronic HBV Infection.

The sustained elevation of hepatitis B surface antigen (HBsAg) facilitates the progression of chronic hepatitis B virus (HBV) pathogenesis, and initiates and sustains host immune tolerance. Immune tolerance is a major factor contributing to viral persistence and viral clearance failure. HBsAg exerts regulatory effects on innate defenses and adaptive responses, limiting the activity of dendritic cells (DCs), natural killer (NK) cells, monocytes/macrophages, T lymphocytes, and B cells. Furthermore, it fosters a tolerogenic environment via interconnected regulatory cell networks, immune checkpoin

Sequential combination of anti-PD-1 antibody and PEG-IFNα in chronic hepatitis B patients: correlates of HBsAg loss with enhanced T-cell responses.

Pegylated interferon-α (PEG-IFNα) exhibits suboptimal efficacy in achieving a functional cure in patients with chronic hepatitis B (CHB), partially due to impaired hepatitis B virus (HBV)-specific T-cell function. We conducted a pilot study (ClinicalTrials.gov: NCT06457477) to explore the efficacy, safety, and immunological changes associated with a response-guided sequential strategy of anti-PD-1 antibody (αPD-1) and PEG-IFNα. Ten virologically suppressed CHB patients receiving nucleos(t)ide analogues (NAs), all of whom had previously received PEG-IFNα and had HBsAg levels ≤200 IU/mL at enrol

Immunological consequences of delayed exposure to hepatitis A virus: evidence consistent with CD8+ T-cell-mediated immunopathology.

BACKGROUND: Hepatitis A virus (HAV) infection has traditionally occurred during early childhood in high-endemic settings but is increasingly affecting adolescents and adults in regions undergoing epidemiological transition, where improvements in sanitation delay primary exposure. Adult HAV infection is associated with more severe clinical disease, and prior work has implicated CD8 + cytotoxic T lymphocyte responses and cytokine-driven bystander immune activation in hepatocyte injury; however, the immunological correlates underlying age-dependent severity in populations undergoing active epidem

Phase-specific polarization of peripheral helper T cells influences immunopathology and viral control in HBV infection.

BACKGROUND: CD4+ T cells orchestrate antiviral immunity but can also drive immunopathology in hepatitis B virus (HBV) infection. Peripheral helper T (Tph) cells are implicated in extra-lymphoid inflammation, yet their functional roles during HBV infection remain elusive. METHODS: We utilized single-cell RNA sequencing (scRNA-seq) to analyze intrahepatic and blood CD4+ T cells from HBV-infected patients at various disease phases, including immune tolerant (IT), immune activation (IA), acute resolving (AR), and chronic resolved (CR). Multicolor immunohistochemistry (mIHC), single-cell multiplex

Robust HLA-B-restricted CD8+ T-cell responses in chronic HBV infection.

BACKGROUND & AIMS: The function and quantity of CD8+ T-cell responses is superior in acute vs. chronic HBV infection. However, whether different HBV-derived epitopes are targeted in these distinct courses of HBV infection remains unclear. METHODS: We screened peripheral blood mononuclear cells from 56 patients with chronic HBV infection genotype D and 32 patients with acute/resolved HBV infection to identify responses to overlapping peptides (18mers) covering the full HBV genotype D proteome. We then performed experimental fine-mapping of the minimal optimal epitopes. RESULTS: Patients with

Understanding HBV immunopathogenesis to inform future immune-based therapies.

Chronic hepatitis B virus (HBV) infection remains a major cause of cirrhosis and hepatocellular carcinoma (HCC) worldwide, with more than 250 million people living with chronic infection despite effective prophylactic vaccination. While current treatments provide durable suppression of viral replication and reduce liver-related complications, they rarely achieve functional cure, defined as sustained hepatitis B surface antigen (HBsAg) loss after treatment cessation. Viral persistence is maintained by intrahepatic covalently closed circular DNA and HBV DNA integration, which sustain antigen pro

The therapeutic potential of epimedium and its bioactive flavonoids in hepatitis and cirrhosis: an integrative review.

Hepatitis and cirrhosis constitute a substantial global health burden, with therapeutic options remaining largely confined to antiviral agents and liver transplantation. Traditional Chinese medicine has long employed Epimedium species (Berberidaceae) for hepatoprotective indications, with contemporary investigations identifying prenylated flavonoids-principally icariin and its active metabolite icaritin-as the principal bioactive constituents. This integrative review systematically examined 33 high-impact studies published up to March 2026, selected based on a comprehensive search of PubMed, W

Functional Heterogeneity of Hepatitis B Virus-Specific CD8+ T Cells Mediates the Clinical Outcomes of Infection.

BACKGROUND: Hepatitis B virus (HBV)-specific CD8+ T cells play a crucial role in viral clearance. However, in patients with chronic hepatitis B infections, sustained immune activation leads to functional impairment of these HBV-specific CD8+ T cells. The goal of this study was to clarify whether these virus-specific CD8+ T cells are exhausted or reflect an incomplete response derived from the initial T cell repertoire. METHODS: In this study, HBV core 18-27 epitope-specific CD8+ T cells were identified by proliferation through in vitro antigen stimulation. Peptide-specific CD8+ T cells recogn

Dysregulated transcription in core- and pol-specific CD8 T cells can be targeted by HDAC inhibition to improve T-cell function in chronic hepatitis B.

BACKGROUND & AIMS: Virus-specific T-cell dysfunction is a hallmark of chronic hepatitis B (CHB), and its reversal may represent a potential strategy for achieving an HBV cure. A deeper understanding of the T-cell correlates of viral persistence and infection control is needed to clarify the role of CD8 T cells in CHB pathogenesis and to identify novel therapeutic targets for effective T-cell reconstitution. METHODS: Core- and polymerase-specific CD8 T cells from untreated, highly viremic, HBeAg-negative patients with CHB were analyzed by RNA sequencing and compared with those from patients wh

Conjugated bilirubin reduces hepatitis E virus infectivity by modulating immunometabolic processes.

Despite the landmark discovery of the hepatitis E virus (HEV) four decades ago and its status as the leading cause of viral hepatitis worldwide, the infection remains overlooked in numerous countries. Currently, there are no approved treatments for hepatitis E or licensed vaccines available for global distribution. The clinical presentation of infection is influenced by the host's immune response and may also be affected by metabolic factors. We previously reported an immunomodulatory role for conjugated bilirubin (cBR) in hepatitis A virus infection, and antiviral effects of this metabolite h

Cell-autonomous co-stimulatory function of membrane-bound CD100 promotes activation and differentiation of HBcAg-specific CD8+ T cells.

BACKGROUND: CD100/SEMA4D functions as a critical co-stimulatory molecule in T cell responses, existing in both membrane-bound (mCD100) and soluble (sCD100) forms. Previous studies have shown that CD100 enhances CD8+ T cell responses indirectly by promoting the maturation and activation of antigen-presenting cells, thereby facilitating viral clearance. However, whether mCD100, highly expressed on resting T cells, can also directly regulate CD8+ T cell function upon ligand engagement remains unclear. METHODS: We examined the impact of CD100 deficiency on the functional phenotype and differentia

CD96 marks a phenotypically distinct checkpoint-associated HCV-specific CD8+ T-cell subset featuring memory-associated states.

INTRODUCTION: Hepatitis C virus (HCV) infection frequently progresses to chronicity, where persistent antigen exposure drives differentiation of virus-specific CD8+ T cells toward an exhausted phenotype. This state is defined by progressive upregulation of inhibitory receptors and functional impairment of antigen-specific T cells. Because patients span distinct clinical stages from acute infection to spontaneous resolution or therapy induced sustained virological response, HCV provides a unique model to study how T-cell exhaustion is established, maintained, and potentially reversed. While PD1
