Reduced FOXP3 expression and its association with genetic ancestry in neuromyelitis Optica spectrum disorder: a Colombian cohort.

BACKGROUND: Autoimmune disorders are characterized by impaired immune tolerance, largely mediated by CD4⁺ regulatory T cells (Tregs), whose function depends on the transcription factor FOXP3. OBJECTIVES: To compare FOXP3 expression levels between patients with neuromyelitis optica spectrum disorder (NMOSD) and healthy controls from Bogotá, Colombia, and to explore their association with genetic ancestry. METHODS: FOXP3 expression was quantified from peripheral blood mononuclear cells using RNA-based analysis. Genomic ancestry proportions were estimated using ancestry-informative markers. RE

Eligibility of Cystic Fibrosis Transmembrane Conductance Regulator (CFTR) modulator therapies: cohort of cystic fibrosis registry of Türkiye.

BACKGROUND: Acute pancreatitis is a common cause of hospitalisation characterised by inflammation of the pancreas. While mechanical, toxic and iatrogenic factors typically cause it, post-oesophagogastroduodenoscopy (EGD) pancreatitis is extremely rare. This report examines a case of acute pancreatitis following EGD, aiming to highlight this rare but significant complication. CASE DESCRIPTION: A 46-year-old woman with a history of breast cancer, anxiety, vitamin D deficiency and gastro-oesophageal reflux disease underwent an EGD, which revealed and led to the removal of duodenal polyps. Six ho

Measuring evidential phenomena for complex reasoning patterns about a mass of evidence.

BACKGROUND: Carpal tunnel syndrome (CTS) is the most common compressive neuropathy affecting the upper extremity, yet evidence-based guidelines for its diagnosis and treatment are lacking. We set out to expose any potential discrepancies in CTS practice attitudes based on surgeon's academic background, residency training, clinical experience, and other factors. METHODS: This was an online survey-based study. Members of the American Association for Hand Surgery (AAHS) were sent an electronic mail request (n = 817). The online questionnaire consisted of 12 questions that queried surgeons' appro

Challenges in Autologous Peripheral Blood Stem Cell Collection in a Patient With IgM Myeloma and Cryoglobulinaemia.

Cold agglutinins are well recognized to complicate stem cell harvests. There is recognition that in both the collection, the processing, and reinfusion of the stem cells to the patient, the product could be exposed to lower temperatures, and this could affect the cold agglutinins within. There is little published evidence on peripheral blood stem cell (PBSC) collection in patients with cryoglobulinaemia. We would like to present a gentleman who has IgM myeloma and Type 1 cryoglobulinaemia. The patient was young and fit, and the plan was to consolidate his induction chemotherapy with an autolog

Takotsubo cardiomyopathy with immune-mediated comorbidities: a case report.

BACKGROUND: Takotsubo cardiomyopathy (TCM) is an under-recognized cardiovascular syndrome, leading to myocardial infarction and left ventricular systolic dysfunction, in the absence of detectable coronary artery lesion. The pathophysiologic mechanisms underlying the condition remain unclear. Whilst previously believed to be a result of circulating high levels of catecholamines due to preceding severe emotional or physical stress, giving it its colloquial name of 'broken heart syndrome', newer evidence suggesting accompanying systemic inflammatory activation hints at a mechanism of acute stress

Rethinking Catatonia in Neurodevelopmental Conditions: Toward a Refined Typology and Research Framework.

Catatonia is a transdiagnostic syndrome observed across psychiatric, medical, and developmental disorders. In the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision (DSM-5-TR), it is not classified as a stand-alone disorder but as a specifier or syndrome that accompanies other conditions. This review examines the current state of knowledge on catatonia in children, adolescents, and individuals with neurodevelopmental disorders, particularly autism spectrum disorder and intellectual developmental disorders. Several DSM-5-TR criteria overlap with baseline behavio

Monoclonal gammopathy of renal significance presenting as cryoglobulinaemic vasculitis in a frail elderly patient.

We report the case of a woman in her late 70s with frailty and treatment-resistant hypertension who presented with marked albuminuria, purpuric rash and elevated serum free light chains. Renal biopsy confirmed monoclonal gammopathy of renal significance (MGRS) with Type I cryoglobulinaemia. She was treated with bortezomib and dexamethasone, resulting in clinical and biochemical remission. However, she experienced a relapse within months, requiring re-initiation of therapy with additional agents. Despite supportive measures, she later developed complications of infection and fluid overload, whi

The Mouth Tells the Tale: Kaposi Sarcoma as the First Clue to Undiagnosed AIDS.

Kaposi sarcoma (KS) is a vascular neoplasm associated with human herpesvirus-8 (HHV-8), most commonly seen in individuals with advanced human immunodeficiency virus (HIV)/AIDS. We report the case of a 52-year-old male patient with no known HIV history who presented with a three- to four-month history of worsening sore throat, oral pain, diarrhea, weight loss, and systemic symptoms including night sweats and fatigue. The patient was eventually diagnosed with oral KS based on pathological results. This case highlights KS as a potential initial manifestation of advanced HIV/AIDS and the importanc

Immune reconstitution syndrome-mediated acute fulminant tuberculous myopericarditis in young postpartum patient: a case report.

BACKGROUND: Postpartum symptom deterioration of tuberculosis is associated with the condition that is termed immune reconstitution syndrome (IRS). In the postpartum period, restitution of cellular immunity mediates the hypersensitivity reaction to Mycobacterium tuberculosis (MTB) antigen that hypothetically results in acute fulminant tuberculosis myopericarditis. CASE SUMMARY: A 29-year-old woman develop acute respiratory distress 5 days following caesarean section. On admission vital sign indicating haemodynamic instability. Chest X-ray showed extensive pulmonary consolidation which signifie

Minor salivary gland biopsy in the diagnosis of definite ocular sarcoidosis in paediatric granulomatous uveitis.

BACKGROUND: Non-infectious paediatric granulomatous uveitis (PGU) is a rare disease that is idiopathic in more than half of affected children. The diagnosis of definite ocular sarcoidosis (OS) must be supported by the presence of non-caseating granulomas detected in biopsy, and is therefore a challenge in children with PGU. This study investigated the utility of minor salivary gland biopsy (MSGB) in the diagnosis of definite OS in PGU. METHODS: Twenty-six consecutive children with PGU diagnosed between 2018 and 2023 and with a systematically performed MSGB within 3 months of the diagnosis wer

Movement Disorders in Toxoplasmosis: A Systematic Review.

BACKGROUND: Movement disorders are an uncommon but important manifestation of central nervous system toxoplasmosis. Their phenotypes, lesion patterns, and outcomes have not been systematically characterized. METHODS: A PRISMA-based systematic review identified all published patient-level reports of central nervous system toxoplasmosis with movement disorders. Eligible case reports and case series confirmed infection via serology, neuroimaging, polymerase chain reaction, or histopathology. Extracted data included demographics, immune status, clinical and neuroimaging features, lesion location,

Progressive Multifocal Leukoencephalopathy in a Young Medication-Noncompliant HIV Patient: A Case Report and Literature Review.

Progressive multifocal leukoencephalopathy (PML) is a rare and debilitating opportunistic infection of the central nervous system (CNS) caused by the John Cunningham (JC) virus. It predominantly affects immunocompromised individuals, including those with advanced HIV infection. The incidence of PML in people living with human immunodeficiency virus (HIV) has significantly declined in the era of antiretroviral therapy (ART). We report a case of PML in a young patient with a history of medication non-compliance, presenting with progressive neurological deficits, underscoring the importance of ad

Disseminated mycobacteriosis and activation of latent opportunistic infections in the course of immune reconstitution inflammatory syndrome (IRIS) in a patient with AIDS: A case report.

Advanced HIV infection leads to profound immunosuppression and increased susceptibility to opportunistic infections. Resumption of antiretroviral therapy (ART) may result in immune reconstitution inflammatory syndrome (IRIS), which manifests as a worsening of an existing infection or the emergence of new ones. Disseminated mycobacteriosis caused by Mycobacterium avium complex (MAC) is one of the most severe opportunistic infections in the course of AIDS. The aim of this paper is to present the diagnostic and therapeutic difficulties in a AIDS patient who developed IRIS with generalized MAC inf

Using a Sequential Skills Training Model to Increase Oral-Motor Skills for Consuming Table-Textured Foods.

Background: Quantifying tumor-stroma architecture on routine hematoxylin and eosin slides may refine risk stratification in bladder cancer (BCa). We developed a convolutional neural network to segment whole-slide images, compute the mixed tumor-stroma ratio (MTSR), evaluate its prognostic value across multicenter cohorts, explore underlying molecular programs through multi-omics analysis, and construct a preoperative multiparametric MRI (mpMRI) radiomics model to estimate MTSR noninvasively. Methods: The ResNet50 convolutional network was customized using The Cancer Genome Atlas BCa slides lab
