Takotsubo Cardiomyopathy due to Guillain-Barré Syndrome and Thyroid Storm: A Case Report.

Takotsubo cardiomyopathy (TCM), also known as stress cardiomyopathy, affects 0.02% of hospitalized patients and is primarily triggered by emotional stressors, although unusual medical situations have also been documented. We report the case of a 36-year-old female patient who presented with symptoms of infection (oropharyngeal pain and diarrhea) 10 days before admission. Upon arrival at the emergency department, she exhibited progressive weakness, lower cranial nerve involvement, and areflexia, necessitating invasive mechanical ventilation support. Diagnostic evaluation revealed sinus tachycar

Reversible Cerebral Vasoconstriction Syndrome with Hyperthyroidism.

A 55-year-old woman presented with a recurrent thunderclap headache triggered by bathing, showering, and the Valsalva maneuver. Laboratory investigations revealed hyperthyroidism, whereas magnetic resonance imaging demonstrated a convexity subarachnoid hemorrhage and vasospasm of the cerebral arteries. The patient was diagnosed with reversible cerebral vasoconstriction syndrome (RCVS) and hyperthyroidism. The association between RCVS and hyperthyroidism has received only limited attention in literature. We herein report a case of RCVS with hyperthyroidism and discuss the association between RC

Thyroid Storm in a Twin Pregnancy Presenting as Severe Hypertension and Preterm Labor.

BACKGROUND: Thyroid storm is a rare, life-threatening endocrine emergency in pregnancy with significant maternal and neonatal implications. Clinical symptoms may mimic those of hypertensive disorders in pregnancy. Prompt diagnosis and multidisciplinary management are critical for reducing maternal and neonatal morbidity and mortality. CASE: A 31-year-old woman with a dichorionic diamniotic twin pregnancy, chronic hypertension, short cervix, and poorly controlled Graves' disease presented at 22 weeks' gestation with vaginal bleeding and severe-range blood pressures. She was diagnosed with thyr

Endovascular intervention and therapeutic plasma exchange in the treatment of cerebral venous thrombosis associated with thyrotoxicosis: illustrative case.

BACKGROUND: Cerebral venous thrombosis (CVT) is a rare cerebrovascular disorder that predominantly affects younger individuals. Its development is influenced by multiple factors, including hypercoagulability, hemodynamic abnormalities, and venous vessel wall injury. Thyrotoxicosis is an uncommon but increasingly recognized predisposing factor for CVT. OBSERVATIONS: The authors present the case of a young female with thyrotoxicosis-induced CVT who achieved successful recovery following endovascular intervention and therapeutic plasma exchange (TPE). LESSONS: This report emphasizes the clinica

Coronary Artery Bypass Grafting in the Presence of Severe Hyperthyroidism: A Case Report.

BACKGROUND In coronary artery bypass grafting, hyperthyroidism with autoantibodies, as seen in Basedow disease, is uncommon and rarely leads to severe thyrotoxicosis. However, surgery can trigger this condition, and documented cases have resulted in poor outcomes. We describe the case of an incidental hyperthyroidism finding on preoperative evaluation for coronary artery bypass grafting for triple-vessel coronary artery disease. CASE REPORT A 58-year-old man presented with exertional dyspnea, chest pain, and postprandial vomiting. Coronary angiography revealed severe triple-vessel disease. Aft

Approach to the patient with thyroid storm.

Thyroid storm, or thyrotoxic crisis, is a rare but life-threatening hypermetabolic state arising from severe thyrotoxicosis with a high risk of multiorgan failure. It typically occurs in individuals with untreated or poorly controlled hyperthyroidism, most often Graves' disease, though various other disorders may also precipitate it. Common triggers include infection, surgery, trauma, abrupt discontinuation of antithyroid medication, severe emotional or physical stress, and iodine exposure. Clinical manifestations involve hyperthermia, tachyarrhythmia, cardiac and respiratory failure, and hepa

Thyrotoxicosis.

Hyperthyroidism is a diagnosis existing along a spectrum of severity. Patients present with a variety of signs and symptoms: tachycardia, elevated heart rate, anxiety, changes in mental status, gastrointestinal disturbances, and hyperthermia. Management of subclinical hyperthyroidism and thyrotoxicosis without thyroid storm is heavily dependent on outpatient evaluation. Thyroid storm is the most severe form of hyperthyroidism with the highest mortality. Management of thyroid storm follows a stepwise approach, with resuscitation and detection of the precipitating cause being paramount. Special

The molecular mechanism of melatonin in regulating osteoporosis based on the RANKL/OPG signaling axis.

Melatonin is a promising drug for improving bone mass in postmenopausal women. This study investigated the mechanism behind the regulation of melatonin on osteoblast differentiation. Mouse embryonic osteoblast precursor MC3T3-E1 cells were treated with melatonin or transfected with cathepsin D (CTSD) vectors. A cellular model was established by H2O2 treatment. Alkaline phosphatase (ALP) staining and Alizarin Red S staining were performed. An osteoporosis mouse model was established by ovariectomy (OVX) and treated with melatonin or transfected with a CTSD knockdown vector. Bone biomechanical t

Hemichorea as the Initial Presentation of Graves' Disease.

Chorea is a hyperkinetic movement disorder characterized by involuntary, spontaneous, irregular, purposeless movements that flow unpredictably from one body part to another. An overlap between movement disorders and neuroendocrine abnormalities is widely recognized; however, chorea is a rare complication of hyperthyroidism. In this case report, we present a case of unilateral upper limb chorea as a manifestation of hyperthyroidism. We describe a clinical case of a 21-year-old male who presented repeatedly to the emergency department (ED) with acute-onset, involuntary proximal movements of the

Double trouble in pregnancy: Complete hydatidiform mole coexisting with a live fetus and maternal crisis: A case report and comprehensive literature review.

Complete hydatidiform mole with a coexisting live fetus (CHMCF) is an exceptionally rare, high-risk obstetric condition, most often seen in dichorionic twin pregnancies, with an estimated incidence of 1 in 20,000-100,000. Maternal complications frequently include vaginal bleeding, early-onset preeclampsia, thyrotoxicosis, and, occasionally, progression to gestational trophoblastic neoplasia (GTN). Fetal outcomes are unpredictable, ranging from spontaneous loss to live birth. We report a case of a 25-year-old gravida 3 para 2 at 24 + 2 weeks' gestation presenting with vaginal bleeding. [Amiodarone-induced hyperthyroidism : complex diagnosis and treatment].

Amiodarone is among the most effective antiarrhythmic drugs and it is frequently used in the management of both supraventricular and ventricular arrhythmias, particularly in patients with impaired cardiac function. Due to its high iodine content, amiodarone therapy is associated with multiple adverse effects, most notably on thyroid function. One of the most significant endocrine complications is amiodarone-induced thyrotoxicosis. Two main subtypes have been identified - Type 1 and Type 2 - along with mixed forms, whose diagnosis may be challenging but remains essential, as therapeutic strateg

Spinal Anaesthesia for Complete Molar Pregnancy With Biochemical Thyrotoxicosis and Ventricular Ectopy.

Gestational trophoblastic disease (GTD) presents unique anaesthetic challenges due to the combined risks of haemorrhage, cardiopulmonary compromise, and endocrine dysfunction. Complete hydatidiform mole is frequently associated with markedly elevated human chorionic gonadotropin (β-hCG) concentrations, which may result in biochemical or clinical thyrotoxicosis through thyroid-stimulating hormone receptor activation. Anaesthetic technique selection during uterine evacuation remains contentious. General anaesthesia (GA) is often favoured because it allows airway control and rapid escalation in t

From Functional Disorder to a Life-Threatening Emergency: A Rare Adolescent Case of Thyrotoxicosis.

Thyroid storm is a rare but life-threatening endocrine emergency that requires rapid recognition and multidisciplinary care. We report the case of a 16-year-old female patient admitted to the emergency department with acute confusion, fever, and tachycardia, in whom severe thyrotoxicosis was confirmed. Her Burch-Wartofsky score was 40, consistent with an impending thyroid storm. Graves' disease had been previously diagnosed, but treatment was delayed, and influenza B infection acted as a precipitating factor. Management with antithyroid drugs, beta-blockers, and intensive care led to clinical

Bone microarchitecture assessed by HR-pQCT in syndrome of inappropriate secretion of thyrotropin with thyrotoxicosis.

UNLABELLED: This is the first study to investigate bone microarchitecture in patients with syndrome of inappropriate secretion of thyrotropin (SITSH). Patients with SITSH had impaired bone microarchitecture compared with healthy controls, and the bone loss was associated with the degree of thyrotoxicosis. PURPOSE: This study aims to evaluate bone microarchitecture through high-resolution peripheral quantitative computed tomography (HR-pQCT) in patients with syndrome of inappropriate secretion of thyrotropin (SITSH). METHODS: This cross-sectional study enrolled 32 patients with SITSH. All pat

Peripartum Management of Refractory Graves' Thyrotoxicosis.

Graves' disease (GD) is the most common cause of hyperthyroidism, characterized by autoimmune stimulation of the thyroid gland and typically managed with antithyroid drugs (ATDs), radioiodine (RAI) therapy, or surgery. While most patients respond to standard antithyroid therapy, a minority exhibit refractory thyrotoxicosis, posing significant clinical challenges. Pregnancy further complicates management due to immunological shifts, increased risk of adverse maternal and fetal outcomes, and limitations in therapeutic options. We present the case of a 39-year-old woman with severe, persistent Gr

When Thyrotoxicosis Meets a Latent Plaque: Unmasking a Type I Myocardial Infarction.

Thyrotoxicosis is a rare but significant precipitant of acute coronary syndrome (ACS), especially in young individuals without traditional cardiovascular risk factors. We present the case of a 36-year-old male with no past medical history who arrived with substernal chest pain, elevated troponins, and sinus rhythm without ST changes on ECG. Thyroid ultrasound revealed a large goiter, and labs confirmed severe thyrotoxicosis. Coronary angiography showed 85% stenosis of the proximal-to-mid left anterior descending artery, managed successfully with stent placement. This case demonstrates how an e

Sequential Amiodarone-Induced Thyroid Dysfunction: From Myxedema Coma to Late-Onset Type 1 Thyrotoxicosis After Drug Withdrawal.

Our report underscores three key points: (1) the prolonged metabolic effects of iodine load and tissue toxicity even after amiodarone cessation, (2) the possibility of reversible amiodarone-induced hypothyroidism preceding subsequent thyrotoxicosis, and (3) the need for long-term thyroid function monitoring, even after amiodarone withdrawal.

A Phenome-Wide Association and Mendelian Randomization Study of Genetically Predicted Membranous Nephropathy with Multiple Diseases.

INTRODUCTION: Membranous nephropathy constitutes a significant proportion of cases leading to end-stage renal disease. In such instances, the accurate distinction between primary and secondary forms is crucial for proper treatment. Consequently, we systematically examined the link between genetically predicted membranous nephropathy and various diseases. METHODS: The phenome-wide association approach assessed the links between genetically predicted membranous nephropathy and 2,408 diseases. Two-sample Mendelian randomization was then employed to explore causal relationships using methods like

Ammi visnaga (Visnaga daucoides) & Ammi majus as a novel thyrotherapeutic phytoalternative for rat with L-thyroxine-induced hyperthyroidism.

Hyperthyroidism is a worldwide health problem mainly associated with potential health consequences such as heart failure, liver dysfunction, thyroid eye disease, hypercalcemia, hyperglycemia, dehydration & nervousness; moreover, it could turn into emergent thyrotoxicosis. Right now, there are limited options to regress hyperthyroidism, including surgery, ablation, & Thionamides, while each of them is fraught with risk, making the clinical decision very complicated. Thus, finding a sustainable, safe alternative with Thionamides' potency but with no/ limited adverse effects is a distinct clinica
