Experiences of Healthcare Professionals in Home Based-Caring for Children With Type 1-Diabetes in Vulnerable Positions: A Qualitative Study.

INTRODUCTION: Providing home-based care to children with type 1 diabetes (T1D) places healthcare professionals (HCPs) in complex and demanding practice contexts. Outside the clinic, HCPs must adapt their expertise to families' everyday lives, navigate shifting roles and manage emotional and relational challenges, particularly when working with vulnerable families. These situations often involve competing priorities, social constraints and complex family dynamics that challenge conventional care approaches and require additional competencies and support. Despite this, HCPs' perspectives remain

Comparative effects of different exercise modalities and doses on lipid metabolism in patients with type 2 diabetes mellitus: a systematic review with Bayesian dose-response network meta-analysis.

OBJECTIVE: This review aimed to determine whether the effects of structured exercise on triglycerides (TG) and low-density lipoprotein cholesterol (LDL-C) differed according to exercise modality and weekly training volume in adults with type 2 diabetes mellitus (T2DM). METHODS: Seven electronic databases were searched from inception to 18 December 2025. Randomized controlled trials were eligible when they enrolled adults with T2DM, evaluated a structured exercise intervention, and reported TG or LDL-C outcomes. Comparative effects across exercise categories were then estimated with a Bayesian

Association of a history of type 2 diabetes with short-term mortality in patients with decompensated cirrhosis.

BACKGROUND: The association between type 2 diabetes mellitus (T2DM) and mortality in patients with liver cirrhosis remains controversial. This study aimed to evaluate whether a history of T2DM influences short-term all-cause mortality in hospitalized patients with decompensated liver cirrhosis (DLC). METHODS: We conducted a retrospective study of hospitalized patients with DLC at three medical centers in China between November 2019 and June 2024. The study endpoint was the 6-month mortality. The association between T2DM and mortality was assessed using multivariable logistic regression, and s

Association of current HbA1c with concurrent multiorgan screening abnormalities and the incremental value of routine clinical characteristics in type 2 diabetes: a single-center cross-sectional study in Wenzhou, China.

BACKGROUND: Glycated hemoglobin A1c (HbA1c) is central to glycemic management but mainly reflects recent average glycemia rather than long-term cumulative exposure or existing organ abnormalities. We assessed whether adding age, sex, diabetes duration, body mass index (BMI), and systolic blood pressure to HbA1c improved the identification of concurrent vascular, renal, and peripheral nerve abnormalities in patients with type 2 diabetes mellitus (T2DM). METHODS: This was a single-center cross-sectional analysis of a prospectively maintained clinical registry at a standardized metabolic disease

A machine learning-driven and explainable prediction model for in-hospital mortality in patients with diabetes mellitus and congestive heart failure: integrating XGBoost and SHAP approaches.

BACKGROUND: Patients with congestive heart failure (CHF) and type 2 diabetes mellitus (T2DM) are at elevated risk of in-hospital mortality, particularly in intensive care units (ICUs). Conventional scoring systems may not fully capture the clinical complexity of such comorbidities. This study aimed to construct an interpretable machine learning model to predict in-hospital mortality in this high-risk group. METHODS: A retrospective study included 573 ICU patients with CHF and T2DM admitted to three hospitals in Henan, China (Jan 2025-Jun 2026). Seven predictors-body mass index (BMI), myocardi

Metabolomics in the Prediction and Early Diagnosis of Type 2 Diabetes Mellitus: Advances and Applications.

Early detection and intervention can prevent the onset and progression of type 2 diabetes mellitus (T2DM); however, achieving early diagnosis and accurate prediction remains challenging. Metabolomics, a potent diagnostic tool in systems biology, makes it possible to systematically identify important biomarkers linked to type 2 diabetes, which provides vital proof for risk assessment and early diagnosis. This review summarizes research advances in metabolomics for T2DM prediction and early detection since 2010 by incorporating both the reported prospective and cross-sectional metabolic evidence

Gut virome remodeling after fecal microbiota transplantation is associated with virus-bacteria co-variation and metabolic response in obesity and type 2 diabetes.

Fecal microbiota transplantation (FMT) modifies gut bacterial composition and improves metabolic outcomes in subjects with obesity and type 2 diabetes (ObT2), but bacterial changes alone do not fully explain the heterogeneity in clinical response. We performed virus-like particle (VLP)-enriched metagenomic sequencing coupled with bulk-DNA metagenomic sequencing on longitudinal fecal samples from a randomized controlled FMT trial in ObT2. Using random forest classification, post-FMT virome changes showed accuracy comparable to bacteriome changes (84.8% vs 81.4%) in distinguishing LDL responders

A Qualitative Descriptive Study of Motivation States for Physical Activity Among Middle-Aged Adults With Type 1 Diabetes Mellitus.

BACKGROUND: Midlife adults with type 1 diabetes mellitus (T1DM) face unique physiological and psychological challenges when balancing physical activity and rest. The Wants and Aversions for Neuromuscular Tasks model accounts for competing desires and aversions for movement and rest, addressing the limitations of general physical activity frameworks. Thus, we employed a qualitative descriptive study guided by the Wants and Aversions for Neuromuscular Tasks model to explore momentary motivational states governing movement and rest among midlife adults with T1DM following an exercise-support inte

A V-type starch-lycopene complex with enhanced cis-lycopene content alleviates hyperglycemia and glycogen fragility in type 2 diabetic db/db mice via gut microbiota.

Type 2 diabetes mellitus (T2DM) is a major global health challenge. Resistant starch type 5 (RS5), formed by complexing starch with guest components into V-type crystalline structures, represents a promising approach for glycemia control. Native lycopene has anti-diabetic potential but limited bioactivity. Here we developed a V-type corn starch-lycopene RS5 complex (CS-LYC) and evaluated its effects in T2DM db/db mice, focusing on glycemic control and gut microbiota-liver glycogen connection. CS-LYC contained 26.07% resistant starch and 11.8 mg/g lycopene, with complexation promoting isomeriza

Severe Suspected Metformin-Associated Lactic Acidosis With Relatively Preserved Serum Creatinine at Presentation: Sequential Intermittent Hemodialysis and Continuous Venovenous Hemodialysis.

Metformin-associated lactic acidosis (MALA) is an uncommon but potentially fatal complication of metformin therapy in which extracorporeal treatment may be required for severe acidemia or markedly elevated lactate levels. We report the case of a 47-year-old man with type 2 diabetes mellitus treated with metformin who presented with encephalopathy, profound hypoglycemia, hypothermia, and severe high-anion-gap metabolic acidosis. Lactate was 24.1 mmol/L and peaked at 25.5 mmol/L, while serum total carbon dioxide was <5 mmol/L. Despite the severity of the metabolic disturbance, serum creatinine w
