Impact of the KARAZ platform's behavioral interventions and incentives on diabetic glycemic control in Saudi Arabia.

INTRODUCTION: The glucose management indicator (GMI) and time-in-range (TIR) are important glycemic parameters calculated from continuous glucose monitoring (CGM) values. KARAZ, a hybrid Internet of things-artificial intelligence platform, was developed in Saudi Arabia to help manage diabetes mellitus. The complex nature of self-care and behavior changes associated with diabetes mellitus requires breaking large behaviors into achievable ones and related incentives. AIM: This study explored how tiny habits as a behavioral intervention and incentive system affect glycemic control among KARAZ Pl

New-onset Fulminant Type 1 Diabetes Following COVID-19 Vaccination.

A 31-year-old Japanese man was admitted with a slight fever and epigastric pain. He had received his third Moderna Coronavirus disease 2019 (COVID-19) vaccine dose (Spikevax, mRNA-1273) 16 days before his visit. His serum amylase level was elevated, and computed tomography found pancreatic enlargement. Acute pancreatitis was diagnosed, and the patient was treated with fasting and intravenous fluids. However, on day 3 of hospitalization, his blood glucose level had increased to 320 mg/dL. His serum and urinary C-peptide were remarkably low (≤0.03 ng/mL and ≤0.6 μg/day, respectively). Finally, f

Extracellular(Serum) Levels of Matrix Metalloproteinases in Pediatric Type 1 Diabetes Mellitus and Association with Diabetic Ketoacidosis and Cerebral Edema.

BACKGROUND: Type 1 diabetes mellitus (T1DM) in children is associated with acute complications such as diabetic ketoacidosis (DKA) and the severe risk of diabetic ketoacidosis-related cerebral edema (DKACE). Matrix metalloproteinases (MMPs) are implicated in inflammation and tissue remodeling, potentially contributing to these complications. This study explores the role of MMPs as biomarkers in pediatric T1DM patients with DKA and DKACE. METHODS: We conducted a systematic cross-sectional study at Jiangxi Children's Hospital, enrolling 56 pediatric patients with T1DM, categorized into three gr

Causal relationship between type I diabetes mellitus and atrial fibrillation: A Mendelian randomization study.

BACKGROUND: Patients with type 1 diabetes mellitus have been at heightened risk for developing atrial fibrillation. We aimed to investigate whether this association is causal using Mendelian randomization. METHODS: Using publicly available genome-wide association studies data, we selected single nucleotide polymorphisms significantly associated with type 1 diabetes mellitus as instrumental variables. We employed inverse variance-weighted, weighted median, MR-Egger regression, simple mode, and weighted mode methods within a two-sample Mendelian randomization framework to assess the causal rela

A Case of Pancreatic Metastasis of Renal Cancer Coexisting With a Pancreatic Neuroendocrine Tumor in a Patient Diagnosed With Fulminant Type 1 Diabetes Mellitus.

Fulminant type 1 diabetes mellitus (F1DM) is a subtype of type 1 diabetes that typically arises from viral infections or exposure to anticancer drugs. In the absence of these triggers, F1DM rarely develops concurrently with pancreatic tumors. This report presents a unique case of F1DM occurring alongside a pancreatic neuroendocrine tumor (PanNET) and pancreatic metastasis of renal cancer. A 72-year-old man with a history of right nephrectomy for renal cancer 20 years prior presented with a sudden onset of severe thirst. Laboratory investigations revealed hyperglycemia, ketoacidosis, and deplet

The Usefulness of the Glucose Management Indicator in Evaluating the Quality of Glycemic Control in Patients with Type 1 Diabetes Using Continuous Glucose Monitoring Sensors: A Cross-Sectional, Multicenter Study.

The Glucose Management Indicator (GMI) is a biomarker of glycemic control which estimates hemoglobin A1c (HbA1c) based on the average glycemia recorded by continuous glucose monitoring sensors (CGMS). The GMI provides an immediate overview of the patient's glycemic control, but it might be biased by the patient's sensor wear adherence or by the sensor's reading errors. This study aims to evaluate the GMI's performance in the assessment of glycemic control and to identify the factors leading to erroneous estimates. In this study, 147 patients with type 1 diabetes, users of CGMS, were enrolled.

MALAT1 SNP (rs619586) shows a protective effect against type 1 diabetes mellitus, while the miR-146a SNP (rs57095329) is linked to an increased risk of developing the disease.

BACKGROUND: Type 1 diabetes mellitus (T1DM), one of the most distinct and intricate metabolic diseases, is characterized by the death of pancreatic β-cells, which leads to a lack of insulin secretion and, ultimately, hyperglycemia. The condition is more common in children and adolescents. Long non-coding RNAs (LncRNAs) and microRNAs (miRNAs) regulate gene expression at the post-transcriptional level. They are essential to the control of several vital physiological processes. Many diseases have been linked to genetic variations in LncRNAs and miRNAs, though, their significance in type 1 diabete

Inflammatory Mediators in the Oral Fluids and Blood Samples of Type 1 Diabetic Patients, According to Periodontal Status-A Systematic Review.

There is currently little information on the immune profile of adult type 1 diabetes patients diagnosed with periodontal disease. The aim of this systematic review is to bring together the known evidence of which inflammatory markers, measured in salivary flow or gingival crevicular fluid and serum blood, are present in both pathologies. Following the Preferred Reporting Items for Systematic reviews and Meta-Analys guidelines, we systematically searched in the PubMed, Web of Science, Scopus and Cochrane Library databases for studies on the associations of different chemokines with type 1 diabe

Advancing Telemedicine Using Smart Insulin Pens with Continuous Glucose Monitoring and Telecommunication Systems: A Case Series.

Background: Multiple daily injections (MDIs) have been a mainstay for insulin delivery by persons with type 1 diabetes mellitus (T1DM). "Smart" insulin pens (SIPs) offer several advantages over traditional insulin pens, such as a memory function, bolus calculator, and reminders for patients to take their insulin. SIPs can integrate with CGM, allowing for the collection of accurate insulin and glucose data, which can integrate into combined reports. Using these technologies along with telecommunication modalities provides an infrastructure to improve the way in which healthcare can be delivered

Dimethyl Fumarate Protects Diabetes-Induced Testicular Toxicity in Rats: Investigations on Nrf-2/HO-1 and β-Catenin/OCT-4 Signalling Pathways.

Type 1 Diabetes Mellitus (T1DM) is associated with the destruction of insulin-producing β-cells and is characterized by prolonged hyperglycemia. This can increase oxidative stress which eventually damages the male reproductive system, disturbs the production of testosterone, alters semen quality and histological architecture, and increases testicular apoptosis. Dimethyl Fumarate (DMF) is an Nrf-2 activator that has been explored as possible protective agent in several preclinical studies, although its exact role in germ-cell toxicity is still not well-known. In the present study, an attempt ha

Genetically predicted amino acids related to neural regulation mediate the association between diabetes mellitus and postherpetic neuralgia: a Mendelian randomization study.

BACKGROUND: Postherpetic neuralgia (PHN) and diabetes mellitus frequently coexist in clinical settings; however, the causal relationship between them remains unclear. Moreover, the potential mediating role of amino acids related to neural regulation in this association has not been fully explored yet. METHODS: Univariable Mendelian randomized (UVMR) was utilized to examine the causal relationship between various subtypes of diabetes mellitus and PHN, with the inverse variance weighted method as the main approach. Multivariable MR (MVMR) was conducted to assess the direct effect of diabetes me

Parent Feedback on the Reducing Emotional Distress for Childhood Hypoglycemia in Parents (REDCHiP) Intervention: A Qualitative Analysis.

OBJECTIVES: Severe hypoglycemia is more common among young children with type 1 diabetes mellitus (T1DM) than older youth, and parents report significant hypoglycemia fear (HF). Parents experiencing HF describe constant and extreme worry about the occurrence of hypoglycemia and may engage in potentially risky behaviors to avoid hypoglycemia. Our team developed and tested a behavioral intervention, Reducing Emotional Distress for Childhood Hypoglycemia in Parents (REDCHiP), to decrease HF in parents of young children with T1DM. Here, we qualitatively analyzed parent feedback to refine and optim

The Therapeutic Potential of Dipeptidyl Peptidase 4 Inhibitors and Glucagon-Like Peptide-1 Receptor Agonists in Diabetic Peripheral Neuropathy.

Diabetic peripheral neuropathy (DPN) is one of the commonest complications of diabetes mellitus (DM). Current therapeutic approaches largely focus on pain management. However, less evidence is available on the clinical potential of two widely prescribed drug categories in DM management: dipeptidyl peptidase 4 inhibitors (DPP-4is) and glucagon-like peptide-1 receptor agonists (GLP-1RAs). In this review, we discuss evidence from both experimental and clinical studies on the potential utility of these drugs in the management of DPN. Immunohistochemical data indicate that agents in both categories

The NGAL as a prognostic biomarker of kidney injury in children and adolescents with type 1 diabetes mellitus: A systematic review and meta-analysis.

AIMS: A major complication of type 1 diabetes is diabetic kidney disease (DKD). Albuminuria and impaired glomerular filtration rate are the main characteristics of DKD. Neutrophil gelatinase-associated lipocalin (NGAL) levels may rise even in the early stages of DKD, even in patients with normoalbuminuria. We present the first systematic review and meta-analysis examining the prognostic role of NGAL exclusively in pediatric patients with type 1 diabetes. METHODS: A search through major databases was carried out until September 24, 2024, investigating the prognostic role of NGAL in kidney inju

Exploring glucagon-like peptide-1 receptor agonists as potential disease-modifying agents in autoimmune diseases.

BACKGROUND: Glucagon-like peptide-1 receptor (GLP-1R) agonists are emerging with therapeutic agents for the treatment of two of the most prevalent metabolic disorders: diabetes and obesity. However, the causal relationship between GLP-1R agonists and autoimmune diseases is still unclear. METHODS: The available cis-eQTLs for drug target genes (GLP-1Rs) were used as proxies for exposure to GLP-1R agonists. Obesity and type 2 diabetes mellitus (T2DM) were used as positive controls to ensure the reliability of the genetic instrument. Mendelian randomization (MR) was performed to reveal the causal
