Association between low-carbohydrate diet scores and macrovascular events or all-cause mortality in adults with type 1 diabetes in Japan.

AIMS/INTRODUCTION: The effects of lifestyle factors on macrovascular events or all-cause mortality remain unclear in type 1 diabetes. We examined the associations of diet, physical activity, and sleep with these outcomes among adults with type 1 diabetes living in Japan. MATERIALS AND METHODS: This multicenter prospective cohort included 352 adults with type 1 diabetes from the Juntendo-Aso Type 1 Diabetes cohort. Lifestyle factors were assessed by questionnaire. Dietary intake was evaluated using a brief-type self-administered diet history questionnaire, from which Total, Plant, and Animal l

Transition from pediatric to adult care for patients with type 1 diabetes mellitus: more than just a handover.

Type 1 diabetes (T1D) is a chronic autoimmune disease that commonly presents during childhood or adolescence and requires lifelong multidisciplinary care. The transition from pediatric to adult diabetes services is a pivotal stage often accompanied by poorer glycemic control, reduced clinic attendance, and increased risk of disengagement from healthcare. This review summarizes current evidence on the principles, challenges, and effective strategies for transitioning adolescents and young adults with T1D from pediatric to adult care. A review of the literature was conducted, focusing on transit

Kimura Disease in an Adolescent With Type 1 Diabetes Mellitus: A Rare Co-occurrence of T Helper 2-Driven Eosinophilic Lymphadenopathy and Autoimmune Endocrinopathy.

Kimura disease (KD) is a rare, chronic T helper 2 (Th2)-driven inflammatory disorder characterized by painless head-and-neck lymphadenopathy or subcutaneous masses, peripheral and tissue eosinophilia, and markedly elevated serum immunoglobulin E (IgE). It predominantly affects young men of Asian descent. Although KD is regarded as an allergic or immune-dysregulatory rather than a classically autoimmune condition, it has been described in association with nephrotic syndrome, suggesting an underlying systemic immunologic disturbance. Its co-occurrence with type 1 diabetes mellitus (T1DM), an arc

Types 1 diabetes mellitus and Graves' disease in Sanjad-Sakati syndrome: a case report in Saudi Arabia.

Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder caused by TBCE gene variants and characterized by congenital hypoparathyroidism, severe growth retardation, developmental delay, and dysmorphic features. We report a nine-year-old Saudi boy with genetically confirmed SSS due to a homozygous 12-bp deletion in exon 3 of the TBCE gene (c.155_166del12bp; p.S52_G55del), who presented with hypocalcemic convulsions at three weeks of age. At three years, he developed type 1 diabetes mellitus (T1DM) with profound insulin deficiency and positive anti-islet cell and anti-GAD-65 antibodie

Type 1 diabetes metabolomics and multi-omics: Current applications, limitations, and future directions - A narrative review.

Type 1 diabetes mellitus (T1DM) is an autoimmune disease characterized by immune-mediated destruction of pancreatic β-cells, resulting in absolute insulin deficiency and lifelong dependence on exogenous insulin. This disease, if not treated, leads to acute and chronic complications affecting the cardiovascular, renal, and retinal systems and introduces significant psychosocial burdens. But even with advances in insulin therapy, optimal glycemic control remains difficult to achieve. T1DM progression spans from autoantibody seroconversion to overt diabetes, accompanied by dynamic metabolic pertu

The β-cells in type 1 diabetes mellitus: findings from human pancreas studies.

Type 1 diabetes mellitus (T1DM) is a chronic autoimmune disease characterized by long-term and dynamic interactions between the immune system and pancreatic β-cells, ultimately leading to severe insulin deficiency. A paradigm shift in T1DM research has occurred in the past few years, with research now focusing on the human disease because of access to human pancreatic tissue and pancreas-draining lymph nodes. Advances in high-resolution imaging, single-cell and spatial multi-omics, and histopathology based on artificial intelligence have profoundly reshaped our understanding of disease initiat

Impact of vaccination on the development of type 1 diabetes in children and vaccine safety in pediatric patients with T1DM: A systematic review.

Type 1 diabetes mellitus (T1DM) is a chronic autoimmune disease associated with increased susceptibility to infections, making immunization an important component of pediatric diabetes care. Despite the established benefits of vaccination, concerns persist regarding its potential role in triggering autoimmune diseases, including T1DM. This systematic review evaluated the impact of vaccination on the risk of developing T1DM in children and assessed vaccine safety in those with established disease. PubMed and Embase were searched for studies published between 2015 and 2025 in accordance with PRI

CD180-Associated B-Cell Alterations in Type 1 Diabetes and the In Vitro Effects of Chlorogenic Acid: An Integrated Transcriptomic and Experimental Study.

Type 1 diabetes mellitus (T1DM) is an autoimmune disease involving progressive loss of pancreatic β-cells. Although B cells have been implicated in T1DM progression, the molecular characteristics associated with disease-related B-cell states remain unclear. Here, we investigated B-cell-associated molecular alterations in T1DM and explored the potential immunomodulatory effects of a natural compound. Public T1DM transcriptomic datasets were analyzed using differential expression analysis, WGCNA, and machine-learning approaches. Single-cell RNA sequencing was used to characterize Cd180 expressio

Speckle Tracking Analysis of the Left and Right Heart in Type 1 Diabetes Patients with End-Stage Renal Disease Awaiting Simultaneous Pancreas-Kidney Transplantation.

Background/Objectives: Diabetic cardiomyopathy is an under-recognized complication of type 1 diabetes mellitus (T1DM), often co-existing with diabetic nephropathy. This study investigates whether patients with T1DM and end-stage renal disease (ESRD) undergoing simultaneous pancreas-kidney transplantation (SPKT) show echocardiographic signs of cardiomyopathy prior to surgery and explores whether cardiac changes are more attributable to diabetes itself or to chronic kidney disease. Methods: We retrospectively analyzed preoperative echocardiographic data of 16 SPKT patients with T1DM and ESRD, us

Early Sound-Based Meal Onset Detection Using Hidden Markov Models (HMMs).

OBJECTIVE: Meal detection is essential for automated insulin delivery systems in patients with Type 1 Diabetes (T1D), as meals are the primary drivers of rapid postprandial glucose excursions. Current closed-loop systems depend on manual meal announcements, which are often inaccurate or omitted, reducing safety and autonomy. To address this, we developed an automatic meal detector leveraging physiological sound sequences from the body. METHODS AND PROCEDURES: The dataset comprised 50 meal recordings from ten participants (aged 24-30 years) using a throat-mounted microphone. Twenty recordings

Capturing human-machine interplay in automated insulin delivery systems through day-to-day variability of the automated correction index (ACI-V%).

BACKGROUND: The Automated Correction Index (ACI) reflects the proportion of algorithm-driven correction boluses relative to total bolus insulin in advanced hybrid closed-loop (AHCL) systems. While mean ACI has been associated with glycemic outcomes in patients with type 1 diabetes mellitus (T1DM), the clinical significance of its day-to-day variability (ACI-V%) remains unknown. METHODS: In this multicenter real-world study, 330 pediatric and adult patients with T1DM using the MiniMed™ 780G system were studied. ACI-V% was defined as the coefficient of variation of daily ACI values over a 30-da

Thyroid storm revealing suspected evolving autoimmune polyglandular syndrome type 2 with steroid-limited assessment of autoimmune adrenal insufficiency: a case report.

BACKGROUND: Autoimmune polyglandular syndrome type 2 (APS-2) is a rare autoimmune disorder characterized by the coexistence of autoimmune adrenal insufficiency with autoimmune thyroid disease and/or type 1 diabetes mellitus. Its heterogeneous and progressive presentation frequently delays diagnosis, particularly during acute endocrine emergencies where one dominant manifestation may obscure concurrent glandular dysfunction. Recognition of evolving autoimmune disease before overt adrenal insufficiency develops remains particularly challenging, especially when emergency corticosteroid therapy is

MenSC-EV-derived HSPA5 activates the AKT/mTOR axis to suppress ferroptosis in Type 1 diabetes mellitus-associated ovarian dysfunction.

Type 1 diabetes mellitus (T1DM) is associated with ovarian dysfunction, including menstrual irregularities, impaired ovarian reserve, and premature reproductive aging, yet effective therapeutic strategies to safeguard ovarian function against metabolic stress remain limited. Mesenchymal stem cells (MSCs) have emerged as pivotal regulators of tissue homeostasis, with their paracrine effects largely mediated by extracellular vesicles (EVs). Here, we demonstrate that menstrual blood-derived endometrial stem cell-EVs (MenSC-EVs) alleviate streptozotocin (STZ)-induced ovarian dysfunction and inhibi
