INTRODUCTION: Non-functioning adrenal incidentalomas (NFAI) are defined by 1 mg-dexamethasone suppression test (F-1mgDST) cortisol levels < 1.8 µg/dL. INTRODUCTION AND IMPORTANCE: Focal nodular hyperplasia (FNH) is a benign liver lesion usually associated with vascular abnormalities. Neuroendocrine tumors (NETs), on the other hand, frequently metastasize to the liver and typically appear as hypervascular lesions. The coexistence of FNH and diffuse hepatic metastases from an ileal NET is exceptionally rare and represents a major diagnostic challenge because both entities may present as hypervascular liver lesions, potentially leading to misdiagnosis and inappropriate management.

A 4-month-old female-intact German Shepherd mixed breed dog was evaluated for polyuria, polydipsia, oral discomfort, tachypnea, and pyrexia. Blood testing documented total and ionized hypercalcemia with an increased parathyroid hormone concentration. Computed tomography identified bone abnormalities of the mandibule and mild, diffuse pulmonary changes. Ultrasonography of the neck documented one mildly enlarged parathyroid gland that was surgically removed. Histopathology indicated diffuse chief cell hyperplasia. The ionized calcium concentration normalized within 7 days postoperatively.
