BACKGROUND: The optimal surgical approach for patients with low-risk differentiated thyroid cancer (DTC) remains controversial. Current evidence comparing long-term outcomes between total thyroidectomy and hemithyroidectomy is limited by short follow-up and a lack of comprehensive patient-reported outcomes.
OBJECTIVE: To compare the effectiveness of total thyroidectomy versus hemithyroidectomy in patients with low-risk DTC, evaluating oncologic outcomes, complications, thyroid function, quality of life, and healthcare costs.
METHODS: We conducted a multicenter retrospective cohort study of 8

DiGeorge syndrome is associated with a wide spectrum of cardiovascular anomalies. We present a rare and anatomically complex arch anomaly in this context. A male newborn with prenatally diagnosed DiGeorge syndrome and a right aortic arch was suspected of having a left pulmonary artery arising from the ascending aorta. Postnatal multimodal imaging revealed a discontinuous left pulmonary artery originating anomalously from the left common carotid artery via a left-sided ductus arteriosus, which later developed significant stenosis. To preserve left pulmonary artery flow and promote its growth, p

INTRODUCTION: Hypoparathyroidism is a rare endocrine disorder caused by deficient secretion or action of parathyroid hormone (PTH), leading to hypocalcaemia, hyperphosphataemia, and disrupted calcium-phosphate homeostasis. Postsurgical hypoparathyroidism is the most common form, whereas autoimmune, genetic, metabolic, and infiltrative causes are less frequent. Chronic PTH deficiency is associated with renal, skeletal, neuromuscular, ocular, and soft tissue complications that impair quality of life.
AREAS COVERED: Conventional treatment with oral calcium and active vitamin D corrects hypocalca

BACKGROUND: Heterozygous pathogenic variants in the transcription factor GATA binding protein 3 (GATA3) cause hypoparathyroidism, sensorineural deafness and renal dysplasia (HDR) syndrome, also known as Barakat syndrome. Although not typically associated with the disease, hypomagnesemia has been reported. We aimed to determine its prevalence in HDR syndrome and explore how GATA3 impacts magnesium balance.
METHODS: Retrospective clinical and laboratory data from 66 patients with GATA3 variants were collected via European Rare Kidney Diseases Reference Network (ERKNet) and European Reference Ne

Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder caused by TBCE gene variants and characterized by congenital hypoparathyroidism, severe growth retardation, developmental delay, and dysmorphic features. We report a nine-year-old Saudi boy with genetically confirmed SSS due to a homozygous 12-bp deletion in exon 3 of the TBCE gene (c.155_166del12bp; p.S52_G55del), who presented with hypocalcemic convulsions at three weeks of age. At three years, he developed type 1 diabetes mellitus (T1DM) with profound insulin deficiency and positive anti-islet cell and anti-GAD-65 antibodie

Hypoparathyroidism (HypoPT) is a relatively rare condition characterised by inadequately low levels of calcium and low levels of parathyroid hormone (PTH), or a complete absence of PTH. In consequence, the body loses its ability to correctly regulate the calcium and phosphate metabolism. Low calcium levels (hypocalcemia) and high phosphorus levels (hyperphosphataemia) are observed in the blood serum of patients. Conventional therapy remains the treatment of choice, involving calcium supplementation (most often calcium carbonate) and active forms of vitamin D, such as alfacalcidol or calcitriol

BACKGROUND: The influence of body mass index (BMI) on perioperative outcomes following total thyroidectomy (TT) remains controversial. We performed a systematic review and meta-analysis to evaluate the association between BMI and perioperative complications after TT.
METHODS: Twelve observational studies including 22,145 patients undergoing elective TT were analyzed. BMI was categorized per WHO criteria. Primary outcomes were recurrent laryngeal nerve (RLN) palsy and hypoparathyroidism. Secondary outcomes included operative time, length of stay (LOS), postoperative bleeding, and surgical site

BACKGROUND: Postoperative hypocalcemia and hypoparathyroidism are two common complications after total or subtotal/near-total for papillary thyroid carcinoma (PTC). This study aimed to identify significant predictors of early postoperative hypocalcemia and hypoparathyroidism and develop prediction nomograms for risk stratification in PTC patients.
METHODS: A total of 1036 PTC patients who underwent total or subtotal/near-total thyroidectomy were retrospectively enrolled. Multivariable logistic regression with prespecified variables was performed to identify independent predictors for two outc

INTRODUCTION: Identification and preservation of parathyroid glands (PGs) in children is particularly challenging due to their small size and frequent ectopic location. Near-infrared autofluorescence (NIRAF) enables real-time intraoperative recognition of PGs; however, its application in pediatric patients remains poorly documented. Accurate PG identification is essential to reduce postoperative hypoparathyroidism.
METHODS: We conducted a prospective observational case series including patients <18 years undergoing thyroidectomy or parathyroidectomy with the assistance of a NIRAF probe. Detec

Symmetrical, bilateral intracranial calcifications, commonly involving the basal ganglia, constitute the characteristic features of Fahr's syndrome, which is an uncommon neurometabolic condition. Its incidence in conjunction with congenital parathyroid agenesis, however, has rarely been reported. The present case emphasises how crucial it is to identify the metabolic origins of intracranial calcification and differentiate Fahr's syndrome from primary familial brain calcification to prevent repeated neurological consequences through adequate therapy. A 26-year-old man with congenital absence of

A woman with chronic hypoparathyroidism following total thyroidectomy was treated with palopegteriparatide, a long-acting parathyroid hormone replacement therapy, for more than 4 years. Previous reports have suggested that stopping parathyroid hormone therapy may lead to severe rebound hypocalcemia and increased calcium requirements due to a "hungry bone syndrome." After discontinuation of palopegteriparatide, the patient resumed conventional treatment with calcium supplements and active vitamin D at doses similar to those used before starting hormone therapy; the alfacalcidol dose was subsequ

BACKGROUND: Preserving parathyroid glands during thyroidectomy remains challenging, particularly in remote-access procedures. Mitoxantrone hydrochloride injection (MHI) is a local lymphotropic tracer that may improve intraoperative discrimination of parathyroid glands during gasless unilateral axillary endoscopic thyroidectomy (GUAET), but evidence remains limited. We evaluated whether intraoperative MHI was associated with a lower rate of incidental parathyroidectomy (IP) during GUAET and described parathyroid autotransplantation (PAT) as a separate management outcome.
METHODS: We performed

BACKGROUND: Synchronous medullary thyroid carcinoma (MTC) and papillary thyroid carcinoma (PTC) are uncommon thyroid malignancies, whereas mixed-component cervical lymph node metastases containing both tumor components within the same metastatic deposit are exceptionally rare. This case highlights the diagnostic value of integrating discordant site-specific fine-needle aspiration (FNA) findings with serum biomarkers for preoperative recognition of this unusual presentation.

BACKGROUND: Conventional open thyroidectomy provides reliable exposure for hemithyroidectomy and central neck dissection but leaves a visible anterior cervical scar. Remote-access approaches may improve cosmetic outcomes but often require specialized equipment, longer operative time, and greater technical complexity. This study evaluated the feasibility and perioperative outcomes of a modified supraclavicular approach in selected patients with unilateral papillary thyroid microcarcinoma (PTMC).
METHODS: This single-center retrospective cohort study included 130 patients with unilateral PTMC w

Patients who undergo total thyroidectomy after Roux-en-Y gastric bypass are subject to higher risks of postoperative hypocalcemia due to altered gastrointestinal anatomy, but guidelines for management do not exist. Preoperative evaluation of patient's calcium and vitamin D levels should be performed to ensure that any baseline deficiencies are corrected. Postoperative management should include a step-up approach starting with oral calcium and vitamin D supplementation with IV supplementation when needed. In severe cases that progress to permanent hypoparathyroidism, consideration for surgical

BACKGROUND: We evaluated experience with probe-based near-infrared autofluorescence (NIRAF) system (PTeye) in parathyroid gland (PTG) identification during total thyroidectomy.
METHODS: In this retrospective study, patients undergoing total thyroidectomy using PTeye (2020-2022; n = 84) were compared with a historical visual-identification cohort (2018-2019; n = 103).
RESULTS: PTG identification rates were comparable, although 42% of PTGs detected with PTeye were initially visualized with low or moderate confidence. Significantly fewer patients in the PTeye cohort had PTGs incidentally identi

BACKGROUND: The objective of this study was to verify that a strategy involving prophylactic exposure of the inferior parathyroid glands and preservation of their venous blood flow-achieved through early identification and protection of their venous drainage-can enhance the success rate of maintaining glandular perfusion and effectively reduce the incidence of postoperative hypoparathyroidism.
METHODS: This study included 600 patients who underwent total thyroidectomy with concomitant bilateral central neck dissection between November 2019 and December 2025. Patients were divided into two gro

Intraoperative neuromonitoring (IONM) is commonly used during thyroid surgery to facilitate recurrent laryngeal nerve (RLN) identification and potentially reduce related postoperative complications. However, the efficacy of routine IONM in decreasing complication rates remains debated. In this study, we investigated the impact of IONM on postoperative complications after thyroidectomy. This retrospective single-center study included 214 thyroidectomy operations performed between 2021 and 2024. Patients were divided into 2 groups: Group 1 (n = 107) underwent surgery with routine IONM, and Group

BACKGROUND: Chromosome 22q11.2 deletion syndrome (22q11.2DS), historically known as DiGeorge syndrome, is a common microdeletion disorder characterized by a wide range of multisystem manifestations. While Whole Exome Sequencing (WES) is increasingly used as a first-tier diagnostic tool for complex infant presentations, it has limited sensitivity for detecting copy-number variants such as microdeletions. Parental consanguinity adds further diagnostic complexity by increasing the likelihood of co-occurring single-gene variants.
