Clinical manifestations of injured tracheal and esophageal branches of the recurrent laryngeal nerve following thyroid surgery.

PURPOSE: Tracheal branch (TB) and esophageal branch (EB) injuries of the recurrent laryngeal nerve (RLN) during thyroid surgery are underexplored. In this study, we examined postoperative clinical manifestations in these injuries. METHODS: Using a head-mounted 2.5× magnifying loupe, TBs and EBs were isolated to assess branching patterns and postoperative outcomes. RESULTS: TBs comprised 1–3 branches, most commonly two, whereas EBs comprised 1–2 branches, usually one. Clinical symptoms were more frequent after TB injury than in controls, with 81.6% of patients showing chest tightness or dyspnea

A Randomized Trial Evaluating Intraoperative Ischemic Preconditioning of Parathyroid Glands During Total Thyroidectomy: A Signal for Earlier Parathyroid Function Recovery.

BACKGROUND: Ischemic preconditioning of parathyroid glands (IPCP) is biologically plausible but clinical evidence is limited. In this single-center randomized trial (ChiCTR2000039788), we compared IPCP versus control during total thyroidectomy with central neck dissection. METHODS: A total of 135 patients with differentiated thyroid carcinoma were analyzed (IPCP, n = 67; control, n = 68). IPCP consists of three cycles of 60-second occlusion followed by 60-second reperfusion of the ipsilateral superior and inferior thyroid arteries. The baseline characteristics, postoperative hypoparathyroidis

Autoimmune polyendocrinopathy candidiasis ectodermal dystrophy (APECED): a case series and experience from a UK tertiary paediatric hospital.

OBJECTIVES: Autoimmune polyendocrinopathy candidiasis ectodermal dystrophy (APECED) is a multi-system autoimmune disorder caused by AIRE gene variants. Diagnosis can be challenging and often delayed. A specialised clinic for children and young people with APECED established in 2024 at Bristol Royal Hospital for Children, recruited patients from South-West England. We describe the diagnostic journey, disease progression and management challenges for 4 patients with APECED. CASE PRESENTATION: All cases had a history of fungal nail infections and chronic mucocutaneous candidiasis preceding the d

Optimal extent of thyroidectomy in clinically node-negative unilateral papillary thyroid carcinoma >1 cm and ≤4 cm.

BACKGROUND: The optimal extent of thyroidectomy for papillary thyroid carcinoma (PTC) measuring >1 cm and ≤4 cm remains a subject of debate. This study aimed to determine the optimal surgical extent for clinically node-negative (cN0) unilateral PTC measuring >1 cm and ≤4 cm and to identify factors associated with recurrence risk. METHODS: We conducted a retrospective analysis of 403 patients with unilateral cN0 PTC measuring 11-40 mm. Propensity score-matched (PSM) analysis was performed using five covariates including age, sex, tumor size, central neck dissection, and follow-up duration. Pat

Perioperative outcomes of transoral endoscopic vs. robotic thyroidectomy: a systematic review and meta-analysis.

BACKGROUND: Transoral endoscopic thyroidectomy vestibular approach (TOETVA) and transoral robotic thyroidectomy (TORT) have emerged as scarless alternatives to conventional open thyroidectomy, offering excellent cosmetic outcomes. However, the comparative perioperative outcomes between these two minimally invasive techniques remain to be systematically evaluated. This meta-analysis aimed to compare the perioperative outcomes of TOETVA and TORT in patients undergoing thyroid surgery. METHODS: A systematic search was performed in PubMed, Embase, the Cochrane Library, and Web of Science from inc

Early Postoperative Intact Parathyroid Hormone Dynamics After Curative Parathyroidectomy for Sporadic Primary Hyperparathyroidism: Trajectories, Predictors of Eucalcemic PTH Elevation, and 18-Month Outcomes.

OBJECTIVE: Eucalcemic postoperative parathyroid hormone elevation (PePTH) after curative parathyroidectomy for sporadic primary hyperparathyroidism is common, yet its temporal profile and clinical significance remain unclear. Transient hypoparathyroidism also occurs after surgery, but the immediate course of intact PTH (iPTH) recovery is poorly defined. We therefore characterized day-by-day postoperative calcium, phosphate, and iPTH trajectories through postoperative day (POD) 5 and evaluated preoperative predictors of PePTH at 6 months and 18-month biochemical outcomes. METHODS: In this pros

Unexpected severe hypercalcemia in a 6-year-old child with hypoparathyroidism and feeding difficulties.

Treatment of hypoparathyroidism with activated vitamin D and calcium carbonate is associated with increased risk of hypercalcemia, hypercalciuria, and nephrocalcinosis. These complications are usually due to inadvertent overdose of medications. We present a case of a 6-year-old boy with hypoparathyroidism, sensorineural deafness, and renal dysplasia (HDR) syndrome and feeding difficulties who developed severe hypercalcemia twice, which was caused not just by medication. Both episodes of calcium overload were related to excessive dietary calcium intake from enteral formula. After rehydration an

First Reported Use of Recombinant Parathyroid Hormone in Kenny-Caffey Syndrome Type 2: A Case Report and Literature Review.

Background/Objectives: Hypoparathyroidism (HPT) is a disorder caused by the insufficient production of parathyroid hormone (PTH). Its main features include decreased serum calcium, increased serum phosphorus, and abnormal bone modeling. In children, HPT is most commonly due to genetic disorders. Among rare genetic syndromes that can include HPT in their clinical spectrum is Kenny-Caffey syndrome (KCS) type 2. Conventional therapy for HPT primarily consists of oral calcium and active vitamin D metabolites. The major limitation of conventional therapy is hypercalciuria with an increased risk of

Anthropometric Indices and Markers of Atherothrombotic Risk in Subjects with Primary Hyperparathyroidism.

Background: Both primary hyperparathyroidism (PHPT) and chronic hypoparathyroidism (HypoPT) are associated with the onset and development of cardiovascular diseases (CVDs). In particular, PHPT is accompanied by the presence of elevated atherothrombotic risk, while the importance of traditional and new anthropometric indices to reflect the cardiovascular risk remains uncertain in this condition. This study aims to investigate whether novel and traditional anthropometric indices distinguish PHPT and whether these indices are correlated with atherothrombotic risk. Methods: A total of 40 subjects

Endocrine and neurological complications in total versus completion thyroidectomy: a systematic review and meta-analysis.

BACKGROUND: The optimal extent of thyroidectomy remains controversial. Total thyroidectomy (TT) offers definitive disease control but it may raise the complications and completion thyroidectomy (CT) following initial lobectomy might lower the morbidity. This meta-analysis and systematic review compared TT and CT using endocrine and neurological outcomes in the postoperative period. METHODS: We searched databases of PubMed, Scopus, Embase, Web of Science and Google Scholar in accordance with PRISMA and until September 1, 2025.
