KEY POINTS: These 2 phase 3 trials assessed upacicalcet, an intravenous calcimimetic, in hemodialysis patients with secondary hyperparathyroidism. ≥30% reduction in intact parathyroid hormone levels occurred in 42% (Parathyroid Hormone [PTH] Attenuation Trial in Hemodialysis-1) and 44% (Parathyroid Hormone [PTH] Attenuation Trial in Hemodialysis-2) with upacicalcet versus 17% and 17%, respectively, with placebo (strata-adjusted; both P < 0.001). Asymptomatic hypocalcemia was more frequent with upacicalcet, and symptomatic hypocalcemia was infrequent; gastrointestinal events were similar to pla

DiGeorge syndrome is associated with a wide spectrum of cardiovascular anomalies. We present a rare and anatomically complex arch anomaly in this context. A male newborn with prenatally diagnosed DiGeorge syndrome and a right aortic arch was suspected of having a left pulmonary artery arising from the ascending aorta. Postnatal multimodal imaging revealed a discontinuous left pulmonary artery originating anomalously from the left common carotid artery via a left-sided ductus arteriosus, which later developed significant stenosis. To preserve left pulmonary artery flow and promote its growth, p

Background: Primary hyperparathyroidism parathyroid hormone (PTH) hypersecretion is difficult to recognise when calcium is normal and difficult to distinguish from secondary hyperparathyroidism once PTH is found to be elevated. These are separate problems arising at separate points in care. We developed and evaluated two interpretable models corresponding to these decision points using an internal-external validation design. Methods: We analysed 2788 adults undergoing PTH testing across three campuses of one hospital (development n = 1843; internal-external validation n = 445 and n = 500); tho

BACKGROUND: Parathyroid carcinoma is a rare endocrine malignancy that remains difficult to distinguish preoperatively from benign causes of primary hyperparathyroidism due to overlapping clinical and biochemical features. This study evaluates the serum parathyroid hormone (PTH)-to-tumor volume ratio as a potential parameter for differentiating parathyroid carcinoma from benign parathyroid lesions.
METHODS: A retrospective analysis was conducted on 344 patients who underwent surgery for primary hyperparathyroidism between 2017 and 2024. Patients were divided into carcinoma (n = 8) and benign l

Severe skeletal complications at the onset of primary hyperparathyroidism are uncommon in contemporary Western clinical practice because routine serum calcium measurement usually allows early diagnosis. We describe a case of primary hyperparathyroidism in which delayed diagnosis, despite suggestive symptoms, led to an extremely severe presentation characterized by multiple fractures, osteolytic lesions, and prolonged postsurgical hungry bone syndrome requiring several months of intensive treatments and hospitalization. During hospitalization, the patient developed potentially life-threatening

OBJECTIVE: The triglyceride-glucose (TyG) index is an easily accessible novel indicator of insulin resistance. This study aimed to investigate the relationship between the TyG index and the homeostatic model assessment of insulin resistance (HOMA-IR) in patients with primary hyperparathyroidism (PHPT), compare these parameters with healthy controls, and evaluate whether the TyG index could serve as a potential new parameter in patients undergoing parathyroidectomy.
METHODS: We retrospectively analyzed PHPT patients diagnosed between October 2023 and May 2024 in a tertiary endocrinology clinic

Multiple endocrine neoplasia type 1 (MEN1), characterized by multiglandular endocrine tumors such as hyperparathyroidism (HPT), is an autosomal dominant disorder caused by mutations of the MEN1 gene. In patients with chronic kidney disease (CKD), abnormalities in calcium-phosphate metabolism may obscure this diagnosis, making the recognition of MEN1 challenging. A 68-year-old man with end-stage renal disease underwent an ABO-compatible living-donor kidney transplantation (KT); however, persistent hypercalcemia prompted further evaluation, which revealed recurrent HPT with bilateral parathyroid

A 55-year-old man with hypercalcemic primary hyperparathyroidism underwent preprocedural localization with 99mTc-MIBI SPECT/CT, demonstrating a right inferior parathyroid adenoma. As the patient refused surgery, radiofrequency ablation was performed. Calcium normalized progressively, while parathyroid hormone (PTH) levels remained mildly elevated at 6 months. Follow-up 99mTc-MIBI SPECT/CT showed no residual adenomatous uptake, correctly predicting delayed biochemical cure, with subsequent normalization of PTH at 1 year. This case highlights the potential value of 99mTc-MIBI SPECT/CT as an earl

INTRODUCTION: Chronic kidney disease progression leads to bone loss and increased fracture and mortality risk. The myokine irisin, produced by muscle during exercise, has been shown to exert protective effects on bone in healthy animals and exhibits a potential interaction with PTH in vitro. The aims of this study were to determine the effect of irisin on bone in CKD-MBD and determine if an interaction exists with PTH in vitro and in vivo.
METHODS: Early and mature IDG-SW3 osteocytes were treated with recombinant irisin, PTH, or both. Mineralization and osteocyte cell markers were assessed. I

Water-clear cell parathyroid adenoma (WCCPA) is an exceptionally rare subtype of parathyroid adenoma characterized by large, clear, glycogen-rich cells. A small number of cases have been reported in the literature. This report presents a rare case of bilateral multifocal WCCPA in a young female. A 34-year-old female presented with generalized bone pain and fatigue. Laboratory evaluation revealed hypercalcemia (12.9 mg/dL) and markedly elevated PTH (488 pg/mL), consistent with primary hyperparathyroidism (PHPT). Neck ultrasonography and computed tomography identified bilateral inferior parathyr

Secondary hyperparathyroidism is a major contributor to skeletal fragility in chronic kidney disease, yet fracture risk remains high despite parathyroid hormone-lowering therapy. In a rat model of chronic kidney disease, Allen et al. showed that combining the prebiotic fiber inulin with a calcimimetic further suppresses elevated bone turnover while improving cortical bone architecture and mechanical properties. These findings highlight the potential of gut-targeted interventions to complement established parathyroid hormone-lowering strategies for improving skeletal health.

Persistently elevated parathyroid hormone (PTH) after parathyroidectomy is often attributed to residual disease but may reflect altered calcium (Ca) homeostasis. The impact of denosumab on perioperative PTH interpretation remains unclear. A 77-year-old woman presented with a hypercalcemic crisis and was initially managed at another hospital. During treatment with hydration and elcatonin (40 units twice daily), denosumab (60 mg) was administered, resulting in a rapid decline in corrected Ca (c-Ca). Eleven days later, at presentation to our institution, her c-Ca was 9.9 mg/dL (SI: 2.48 mmol/L; r

PURPOSE: Primary hyperparathyroidism (PHPT) is an endocrine disorder in which one or more parathyroid glands oversecrete parathyroid hormone, leading to hypercalcemia and potential complications such as kidney stones, bone fractures, and other systemic issues. Machine learning (ML) is extensively applied to diagnose medical diseases due to its high accuracy and ease of implementation.
METHODS: The microarray expression dataset (GSE83421) was selected from the Gene Expression Omnibus Database (GEO) for analysis. Differential expression analysis was performed with false discovery rate (FDR) cor

BACKGROUND: Heterozygous pathogenic variants in the transcription factor GATA binding protein 3 (GATA3) cause hypoparathyroidism, sensorineural deafness and renal dysplasia (HDR) syndrome, also known as Barakat syndrome. Although not typically associated with the disease, hypomagnesemia has been reported. We aimed to determine its prevalence in HDR syndrome and explore how GATA3 impacts magnesium balance.
METHODS: Retrospective clinical and laboratory data from 66 patients with GATA3 variants were collected via European Rare Kidney Diseases Reference Network (ERKNet) and European Reference Ne

Larynx-trachea-thyroid-parathyroid allotransplantation can restore essential laryngeal functions, but postoperative endocrine changes remain poorly understood. This study assessed thyroid and parathyroid function in four recipients. All developed early thyroid hormone alterations consistent with euthyroid sick syndrome, followed by variable endocrine outcomes. Two experienced thyrotoxicosis associated temporally with graft rejection, while another developed hypothyroidism and elevated parathyroid hormone during late rejection. Transient parathyroid hormone suppression was also observed. In con

Hypoxia-inducible factor prolyl hydroxylase (HIF-PH) inhibitors are widely used to treat renal anemia in patients with chronic kidney disease (CKD). While roxadustat, an HIF-PH inhibitor, is known to induce central hypothyroidism via thyroid hormone receptor-beta agonism, thyroid dysfunction associated with other HIF-PH inhibitors, such as enarodustat, remains unrecognized. We report the case of a 32-year-old male on peritoneal dialysis (PD) with underlying subclinical mild primary hypothyroidism and erythropoiesis-stimulating agent (ESA)-resistant renal anemia secondary to primary hyperparath

INTRODUCTION: Near-infrared autofluorescence (NIRAF) is an emerging adjunctive tool that facilitates intraoperative identification of parathyroid glands. Although it has shown promise in the setting of thyroidectomy, its role in parathyroidectomy for primary hyperparathyroidism (PHPT) remains unclear.
METHODS: A systematic review was conducted according to PRISMA guidelines. Four databases were searched to identify studies comparing NIRAF with conventional visual identification of parathyroid glands during parathyroidectomy. The primary outcome was cure rates and secondary outcomes included o

Sanjad-Sakati syndrome (SSS) is a rare autosomal recessive disorder caused by TBCE gene variants and characterized by congenital hypoparathyroidism, severe growth retardation, developmental delay, and dysmorphic features. We report a nine-year-old Saudi boy with genetically confirmed SSS due to a homozygous 12-bp deletion in exon 3 of the TBCE gene (c.155_166del12bp; p.S52_G55del), who presented with hypocalcemic convulsions at three weeks of age. At three years, he developed type 1 diabetes mellitus (T1DM) with profound insulin deficiency and positive anti-islet cell and anti-GAD-65 antibodie

Calcimimetics are increasingly used to control hypercalcemia before parathyroidectomy. Although treatment-induced degenerative changes have been reported in parathyroid adenomas, their effects on preoperative imaging and surgical planning remain unclear. A 70-year-old woman with primary hyperparathyroidism was administered preoperative evocalcet (2 mg/day). After ∼2 months, serum calcium levels decreased to the hypocalcemic range, prompting drug discontinuation before surgery referral. Upon presentation, corrected serum calcium normalized to 9.1 mg/dL (SI: 2.28 mmol/L; reference range, 8.8-10.

Hyperparathyroidism (HPT) may persist in patients after kidney transplantation (KT). A better understanding of which patients are likely to have persistent HPT after KT would help in the development of optimal management strategies. Our aim was to examine the prevalence of persistent HPT after KT in a contemporary cohort and to identify risk factors associated with persistent HPT at 6 months. We followed 104 KT subjects in a longitudinal prospective cohort at Cho Ray Hospital, Vietnam, from October 2023 to May 2025. Persistent HPT was defined as a parathyroid hormone (PTH) level of 150 pg/mL a
