Severe hypomagnesemia and secondary hypocalcemia associated with vonoprazan in a patient with type 2 diabetes.

Hypomagnesemia is a well-known complication of long-term proton pump inhibitor usage; the stronger acid-suppressive effect of potassium-competitive acid blockers (P-CABs) may increase the risk of hypomagnesemia to a similar or greater extent. However, reports of P-CAB-associated hypomagnesemia are scarce. A 78-year-old Japanese man with a history of type 2 diabetes and myocardial infarction, who was receiving low-dose aspirin and long-term vonoprazan, presented with epigastric discomfort. Laboratory testing revealed significant hypocalcemia, hypomagnesemia, and low intact parathyroid hormone l

The physiological effects of induced prepartum subclinical hypocalcemia in multiparous Holstein cows.

Subclinical hypocalcemia (SCH) is prevalent during the periparturient period in dairy cows due to increased calcium (Ca) demand for milk and colostrum synthesis. Transient SCH has been shown to activate a negative feedback loop, thereby improving Ca homeostasis. Here, we investigate the impact of induced SCH during the prepartum period on Ca homeostasis pre- and postpartum, and milk production. Thirty multiparous Holstein cows were enrolled 21 d before the expected due date, in a randomized complete block design, and were assigned to receive either a 6-h continuous intravenous infusion of sali

Hypoparathyroidism and Salivary Gland Dysfunction: A Single Center Case-Control Study.

INTRODUCTION: Hypoparathyroidism, an endocrine disorder causing hypocalcemia from inadequate PTH secretion, affects multiple systems and may correlate with salivary dysfunction and poor oral health. This study aimed to investigate the effects of hypoparathyroidism on salivary calcium levels, flow rate, and parotid gland structure, exploring the implications for oral health of this patient population. MATERIALS AND METHODS: Conducted as a case-control study between January and June 2024, our research included 86 participants: 43 with hypoparathyroidism and 43 in a healthy control group. We com

Fascin expression is associated with aggressive clinicopathological features in parathyroid tumors.

BACKGROUND: Parathyroid tumors are classified by the World Health Organization as adenomas, atypical tumors, or carcinomas. While the loss of parafibromin expression and a Ki-67 labeling index >5% are well known markers of malignancy, the role of fascin, an actin-bundling protein implicated in cancer invasion and metastasis, has not yet been investigated in parathyroid tumors. METHODS: We examined fascin expression by immunohistochemistry in 19 carcinomas, 16 atypical tumors, 23 adenomas and 14 normal parathyroid tissues. Staining for parafibromin and Ki-67 was also performed. Fascin expressi

White-to-brown adipose switching promotes bladder cancer progression.

The invasion of bladder cancer into perivesical adipose tissue represents a watershed event in tumor progression, accompanied by a dramatic deterioration in clinical outcomes. However, the underlying molecular mechanisms governing this cancer-adipose tissue crosstalk remain poorly elucidated. Here, we systematically characterize a bidirectional regulatory network between bladder cancer cells and perivesical adipocytes. Our findings demonstrate that bladder cancer cells secreted parathyroid-hormone-related protein (PTHrP), which induces browning of perivesical adipose tissue through activation

Endocrine-liver bidirectional regulation: From mechanistic research to multidisciplinary clinical practice.

A close bidirectional regulatory network exists between endocrine disorders and liver dysfunction, and their reciprocal imbalance is a key driver of metabolic liver diseases and chronic liver diseases. Based on the review, this letter systematically appraises its value in elucidating bidirectional crosstalk mechanisms across the thyroid, parathyroid, pancreas, adrenal, and sex hormone axes with the liver, while identifying limitations in mechanistic depth and clinical translation. Aligned with current clinical needs and research hotspots, we propose targeted research directions, including stre

Clinical and prognostic significance of parathyroid hormone-related protein in breast cancer: a systematic review and meta-analyses of observational studies in women.

Parathyroid hormone-related protein (PTHrP) is produced by normal breast epithelial cells and is a cause of hypercalcemia in breast and other cancers. However, there are conflicting data regarding its role(s) in breast cancer biology. This study aimed to clarify PTHrP's potential function(s) in breast cancer by systematically examining its association with tumor characteristics, patient outcomes and hypercalcemia in women. We designed a predetermined strategy based on PRISMA 2020 guidelines. Seven databases were interrogated to identify studies of women with breast cancer reporting association

Markers of bone turnover, parathyroid hormone, calcium, and magnesium concentrations in horses with acute colitis.

Hypocalcemia is frequent in horses with colitis. Information on serum parathyroid hormone (PTH) concentrations and its association with bone turnover biomarkers in horses with colitis is lacking. We aimed to determine the association between serum bone resorption biomarkers (C-terminal telopeptide of type I collagen [CTX-I]) and bone formation (osteocalcin [OCN]) with blood PTH, total calcium (tCa), ionized calcium (iCa), phosphorus (Pi), and total magnesium (tMg) concentrations, and mortality in horses with acute colitis. A total of 163 horses were divided into colitis (n = 127) and healthy (

[A case report of severe hypocalcemia secondary to Fahr's syndrome].

Fahr's syndrome is a rare neurological disease characterized by bilateral intracranial calcifications associated with disturbances in calcium-phosphate metabolism, most commonly due to hypoparathyroidism. We report a 56-year-old man presenting with generalized seizures and behavioral changes. Laboratory evaluation revealed severe hypocalcemia, hyperphosphatemia, and low parathyroid hormone levels. Brain computed tomography demonstrated symmetrical calcifications involving the basal ganglia and other subcortical structures, consistent with Fahr's syndrome. Diagnosis was established through the

Immunohistological parathormone production changes and angiogenesis in normal and adenomatous human parathyroid glands.

INTRODUCTION: Parathyroid tumor function is not always positively correlated with tumor size, complicating the disease picture. Here, we compared parathormone (PTH) and vascular endothelial growth factor (VEGF)-A expression intensities in normal and adenomatous parathyroid glands, as well as the number of vessels, as determined by CD31 expression. METHODS: Eight adenomatous and six normal samples were compared for the expression intensities of PTH and VEGF-A, expression granule area of VEGF-A per unit area (/900 μm2), and number of blood vessels. We then examined relationships between the exp

Advances in the management of osteopenia: a case series.

BACKGROUND: Osteopenia is a pre-pathological condition characterized by reduced bone mineral density that may lead to increased fracture risk and to the onset of osteoporosis. Standard treatments for managing osteopenia include supplementation of calcium, vitamin D, and vitamin K. Interestingly, recent studies suggest D-chiro-inositol and α-lactalbumin may enhance bone health through mechanisms such as insulin sensitization and improved nutrient absorption; furthermore, D-chiro-inositol is thought to be a specific mediator of osteoclast activity by inhibiting the expression of several osteocla

Primary hyperparathyroidism in a child with tuberous sclerosis.

Primary hyperparathyroidism in patients with tuberous sclerosis (TS) is extremely rare: only three cases have been reported previously, two of them in pediatric patients. An 11-year-old male patient with a clinical and molecular diagnosis of TS presented with mild symptomatic hypercalcemia in routine laboratory tests. Biochemical evaluation confirmed hypercalcemia with hypophosphatemia and inadequate parathyroid hormone levels, with radiological signs of hyperparathyroidism. Ultrasound demostrated hyperplasia of the left superior parathyroid gland, and a sestamibi and technetium-99m scintigram

Bone health after 3 years of ETI therapy: a real-world study from the Danish cystic fibrosis cohort.

BACKGROUND: Malnutrition, inflammation, limited exercise, and low muscle mass increase the risk of cystic fibrosis (CF) bone disease (CFBD). Additionally, dysfunction of the CF transmembrane conductance regulator (CFTR) directly affects bone-forming cells. Elexacaftor/Tezacaftor/Ivacaftor (ETI) could influence these aspects. We examined the impact of ETI on bone health in Danish people with CF. METHODS: This nationwide cohort study evaluated changes in bone status in adults with CF after ETI treatment. Dual energy X-ray absorptiometry (DXA) was used to assess bone mineral density Z-scores in

Gene dosage effects of 22q11.2 copy number variants on in-vivo measures of white matter axonal density and dispersion.

22q11.2 deletion (22qDel) and duplication (22qDup) carriers have an increased risk of neurodevelopmental disorders and exhibit altered brain structure, including white matter microstructure. However, the underlying cellular architecture and age-related changes contributing to these white matter alterations remain poorly understood. Neurite orientation dispersion and density imaging (NODDI) was used on mixed cross-sectional and longitudinal data to examine group differences and age-related trajectories in measures of axonal density (i.e., intracellular volume fraction; ICVF), axonal orientation
