Autosomal dominant hypocalcemia type 1: status quo of tailored management and future perspectives.

A 28-day-old female infant presented with clonic seizures secondary to hypocalcemia (calcium [Ca], 5.7 mg/dL) and hypoparathyroidism (intact PTH, 7 pg/mL). Despite the initiation of oral alfacalcidol therapy, she experienced recurrent episodes of generalized convulsions or focal tetany, despite calcium lactate administration during febrile episodes. Her younger sister exhibited frequent irritability until day 38 of life due to hypocalcemia (Ca, 7.7 mg/dL) and hypoparathyroidism (intact PTH, 5 pg/mL). She experienced 3 febrile generalized seizures during infancy despite oral alfacalcidol treatm

Importance of oxyphil cells for parathyroid hormone/calcium concentrations in primary hyperparathyroidism: a retrospective observational study.

BACKGROUND: In patients with primary hyperparathyroidism (pHPT), the importance of oxyphil cells for hormonal activity is uncertain. METHODS: This retrospective observational study analyzed the histopathological findings of postoperative specimens from patients with pHPT (single gland disease). The indication for surgery was based on increased plasma intact parathyroid hormone (iPTH) concentrations and the presence of symptoms typical of pHPT. To quantify approximate oxyphil cell content (OCC), we determined the percentage of oxyphil cells observed in each specimen. To quantify the approximat

Therapeutic opportunities for CKD-associated osteoporosis in adults with kidney failure imply screening.

BACKGROUND: Fracture risk in patients with chronic kidney disease (CKD) is excessively high. A recent European consensus provides guidance on the comprehensive screening for CKD-associated osteoporosis in patients with CKD stages G4-5D. However, data on the yield and therapeutic implications of this approach in patients with kidney failure is lacking. METHODS: All prevalent dialysis patients aged >50 years or postmenopausal with life expectancy >1 year at a single center were invited for comprehensive bone health assessment, including the evaluation of risk factors, calcium intake, mineral me

Bariatric Surgery and Metabolic Bone Disease: Crosstalk between Muscle, Adipose Tissue, and Bone.

After bariatric surgery (BS), there is an increased risk of bone health issues due to restricted nutrient intake, malabsorption, insufficient supplement adherence, and reduced mechanical loading from significant weight loss. We aimed to provide a review of the current understanding of post-BS human physiology and the relevant animal models employed to investigate bone metabolism. Muscle-derived signals influence bone metabolism through mechanisms beyond just mechanical forces, highlighting the relevance of skeletal muscle as an endocrine organ to regulate bone health. BS patients often have hi

Adaptive and Behavioral Phenotype in Pediatric 22q11.2 Deletion Syndrome: Characterizing a High-Risk Neurogenetic Copy Number Variant.

22q11.2 deletion syndrome (22q11.2DS) is the most common recurrent microdeletion in humans and a prototypical high-risk neurogenetic copy number variant (CNV) associated with a broad spectrum of neurodevelopmental and psychiatric disorders, including intellectual disability (ID), autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), anxiety, and psychotic symptoms. This hemizygous deletion encompasses multiple genes involved in brain development and neural circuit function, contributing to marked phenotypic variability and multisystem involvement. In pediatric popula

Peri-kidney transplant management in autosomal dominant hypocalcaemia type 1.

Autosomal dominant hypocalcaemia type 1 is rare and clinically challenging. Altered calcium handling may lead to progressive nephrocalcinosis and chronic kidney disease. We present the first known report of a child with ADH1 caused by the genetic variant c.2528C > A; p.Ala843Glu, who successfully underwent kidney transplantation without simultaneous parathyroid gland transplant aged 11yrs. We outline our reasoning for this and our management strategy for maintaining calcium homeostasis post-transplant over a 4-year period.

Genetic Mapping of the 22q11.2 Deletion Syndrome (DiGeorge Syndrome) Microdeletion Types Revealed Novel Candidate Breakpoints.

Background: 22q11.2 deletion syndrome (DiGeorge Syndrome) is a rare disorder that involves a de novo hemizygous microdeletion within the 22q11.2 chromosomal locus. Individuals affected by this condition display a wide array of clinical phenotypes as well as haplotype sequences, which render understanding the genotype-phenotype relationship quite difficult. Additionally, the complex structure of the 22q11.2 low-copy repeats (LCRs), which usually inhibits sequencing efforts, has complicated the study of possible breakpoints that instigate the deletion events. In this study, 22q11.2 deletion synd

Morning urinary calcium/creatinine ratio as a sex-specific screening tool for detecting hypercalciuria in hypoparathyroidism.

BACKGROUND: Hypercalciuria is a relevant complication in patients with treated hypoparathyroidism. While hypercalciuria is usually estimated by 24-hour urinary calcium excretion (24hUCa), the calcium/creatinine ratio (CaCrR) of a morning urine sample is more often used in pediatric endocrinology due to feasibility. This study aims to compare these two approaches in adult patients with hypoparathyroidism. METHODS: In a cohort of 184 systematically evaluated patients with primary hypoparathyroidism (93 % postoperative, 73 % female), the 24hUCa was compared with the CaCrR of a morning spot urine

Markers of Mineral Metabolism in Children With CKD Stages 2 to 5D.

INTRODUCTION: Changes in age- and sex-related markers for chronic kidney disease (CKD)-mineral and bone disorder (MBD) (CKD-MBD) across CKD stages 2 to 5D have not yet been studied in children. METHODS: In this cross-sectional study, we investigated age, and where applicable, sex-related z-scores of 10 key markers for CKD-MBD in 170 children with stages 2-5D. RESULTS: We identified distinct CKD stage-dependent changes in CKD-MBD markers. In CKD stage 2, elevated serum sclerostin (z-score: 0.97), total fibroblast growth factor 23 (FGF23) (z-score: 0.72) and alkaline phosphatase (AP) (z-score:

TrpC5 silencing reduces cell invasion and migration and enhances radiosensitivity in papillary thyroid carcinoma cells.

INTRODUCTION: Carcinoma (PTC) is the most common malignant tumor derived from thyroid follicular cells and represents the most common pathological type of thyroid malignancy. Cancer metastasis and radiosensitivity are important factors that limit the treatment of PTC. This study aimed to investigate the regulatory effects of transient receptor potential channel C5 (TrpC5) on the proliferation, invasion, migration, and radiosensitivity of PTC. METHODS: Human papillary thyroid carcinoma cell lines TPC-1 and B-CPAP were transfected with TrpC5 siRNA, pcDNA-TrpC5, or their corresponding negative c

Constitutive activation of TGF-β receptor type I promotes cortical and cancellous bone formation in adult mice.

Transforming growth factor β (TGF-β) is a pluripotent cytokine that plays a pivotal role in regulating bone remodeling. In this study, we investigated the skeletal phenotype of 24-week-old transgenic female mice expressing a constitutively active TGF-β receptor type I (TβRI) under the control of inducible Mx1-Cre promoter. Poly(I:C) injection was used to induce expression of TβRI to generate Mx1;TβRICA mice. In Mx1;TβRICA mice, serum calcium levels were increased, while parathyroid hormone (PTH) levels were decreased. Micro-computed tomography (μCT) analysis revealed a significant increase in

Discovery and characterization of canvuparatide, a once-weekly parathyroid hormone analog for the treatment of hypoparathyroidism.

Hypoparathyroidism is a rare endocrine disorder characterized by hypocalcemia, hyperphosphatemia, and low or undetectable levels of parathyroid hormone (PTH). Advanced treatments that precisely maintain blood calcium levels within the normal range to improve disease symptoms and outcomes are needed. Canvuparatide (formerly known as MBX 2109) is a once-weekly investigational PTH analog that undergoes a controlled-release conversion to a biologically active peptide through an intramolecular cyclization reaction controlled by temperature and pH. Here we demonstrate the biologically active PTH ana

Fibrosis-Driven Surgical Risk After Thyroid Nodule Ablation: Quantitative Clinicopathological Determinants of Complications in Post-Ablative Thyroidectomy-A Retrospective Cohort Study.

BACKGROUND: Thermal ablation is increasingly used for selected benign and low-risk thyroid nodules, yet some patients still require thyroidectomy for regrowth, persistent symptoms, or new oncologic concern. The surgical and pathological impact of ablation-induced remodeling remains incompletely defined. We aimed to characterize postablation thyroidectomy outcomes and identify histological correlates of perioperative morbidity. METHODS: We conducted a single-center retrospective cohort study of patients undergoing thyroidectomy after radiofrequency or ethanol ablation (2021-2025). Clinical and

Parathyroid Carcinoma Mimicking Thyroid Hyalinizing Trabecular Tumor: A Case Report and Review of Atypical Presentations of Parathyroid Carcinoma.

We present a parathyroid carcinoma with hyalinizing trabecular architecture and a prominent spindle-cell component. The tumor closely mimicked a hyalinizing trabecular tumor of the thyroid due to its trabecular growth, eosinophilic hyalinized stroma, and nuclear features including grooves and intranuclear pseudoinclusions. Recognition of this rare pattern is important to avoid misclassification.

Densitometry Versus Bioimpedance for Modeling Vitamin D-Endocrine and Metabolic Associations in Pediatric Obesity: A Cross-Sectional Parallel-Modality Analysis.

Background/Objectives: It has been previously shown that bioimpedance assessment (BIA) systematically underestimates adiposity compared to densitometry analysis (DXA), though the methods correlate strongly. However, whether DXA outperforms BIA for physiology modeling-using vitamin D as a sentinel signal-remains uncertain. We compared DXA and BIA side-by-side to model (i) adiposity-25(OH)D associations, (ii) mediation-style links with metabolic outcomes, and the vitamin D-PTH-calcium axis. Methods: We performed a cross-sectional analysis of 165 children with simple obesity and no vitamin D prop
