GH Response to Glucagon in Transition: Role of Body Mass Index and Etiology in Childhood-onset GH Deficiency.

CONTEXT: The glucagon stimulation test (GST) is increasingly used as an alternative to the insulin tolerance test for diagnosing persistent GH deficiency (GHD) during transition, though its accuracy and appropriate cutoff values are still uncertain. OBJECTIVE: To investigate the GH response to GST in transitional-age patients with childhood-onset GHD (CO-GHD), with a focus on the influence of body mass index (BMI) and the underlying etiology. PATIENTS AND METHODS: A total of 180 patients with CO-GHD (median age 17.39 years) underwent GST. They were grouped based on the number of pituitary de

Adrenal Crisis Induced by Zoledronic Acid in Two Patients With Hypopituitarism: A Case Report and Literature Review.

Adrenal insufficiency (AI) is characterized by inadequate steroid hormone production and is frequently a consequence of hypopituitarism, which is also associated with increased risk of osteoporosis due to deficiencies in growth hormone, gonadotropins, and other pituitary hormones. Zoledronic acid (ZA), a widely used bisphosphonate, is associated with acute phase reaction (APR) that may trigger adrenal crisis in susceptible individuals. We describe two patients with hypopituitarism and osteoporosis who developed adrenal crisis shortly after their first ZA infusion, despite stable physiological

Pituitary dysfunction in trans-sellar encephalocele: a two-case report and a literature review.

Trans-sellar encephalocele (TSE) is a rare congenital anomaly of the cranial base characterized by herniation of intracranial contents through the sellar floor. Although clinical presentations are diverse, the prevalence and characteristics of endocrine dysfunction due to hypothalamic-pituitary involvement are unclear. This study aimed to clarify endocrine function in patients with TSE and emphasized the importance of multidisciplinary management. Here, we present two adult cases of TSE with progressive visual impairment and evident pituitary hormone deficiencies, and conducted a comprehensive

Safety of immune checkpoint inhibitors: A systematic review of disproportionality analysis studies.

BACKGROUND: Immunotherapy has fundamentally changed the treatment landscape for both solid and hematologic malignancies over the past decade. However, an evolving body of literature has reported immune-related adverse events (irAEs) involving several organs in patients treated with immune checkpoint inhibitors (ICIs). Our study aimed to systematically review published disproportionality analyses on ICIs, to summarize the current state of methodology and potential strengths of the analysis, while highlighting safety signals generated for these pharmacological groups. METHODS: A PubMed, Scopus,

A systematic review of stereotactic radiosurgery for pituitary metastases.

BACKGROUND: Pituitary gland metastases (PGM) are extremely rare, and their ideal management is poorly defined. Radiosurgery for PGM may obviate the need for tumor excision, decreasing surgical morbidity in a patient population with an otherwise life-limiting illness. Given this, we sought to summarize existing literature on outcomes of PGM treated with stereotactic radiosurgery (SRS). METHODS: A systematic review was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA). PubMed and Science Direct databases were queried from their inception

An overview of hypopituitarism's causes.

The widespread application of tumor therapies such as immune checkpoint inhibitors and the emergence of new infectious diseases such as COVID-19 are promoting the continued expansion of the cause spectrum of hypopituitarism, making its scope significantly beyond traditional causes such as pituitary tumors and craniocerebral trauma. Faced with this evolution, a comprehensive and in-depth understanding of its etiology has become a top priority, which has also put forward new requirements for clinical diagnosis and differential diagnosis. This review aims to systematically sort out and deeply exp

Radiosurgery in recurrent and persistent Cushing's Disease: a Systematic Review and Meta-Analysis.

Cushing's disease (CD) is a severe systemic metabolic disorder caused by elevated levels of cortisol sustained by a pituitary neuroendocrine tumor. Endoscopic trans-sphenoidal surgery (ETS) is the first-line treatment, but does not achieve disease remission in all patients. For patients with persistent or recurrent Cushing's disease, stereotactic radiosurgery (SRS) has been reported as an effective treatment option. This review and meta-analysis assesses the available evidence to systematically highlight the current strengths and limitations of SRS in the treatment of CD. In May 2025, followin

Exploring the Role of Skull Base Anatomy in Surgical Approach Selection and Endocrinological Outcomes in Craniopharyngiomas.

Background/Objectives: Craniopharyngiomas (CPs) are rare, generally benign tumors predominantly located in the sellar and suprasellar regions, associated with significant morbidity and complex surgical management. Despite high overall survival rates, patients frequently experience complications including visual impairment, pituitary dysfunction, diabetes insipidus (DI), and hypothalamic syndrome. Among these, hypothalamic obesity (HO) represents one of the most clinically challenging sequelae, often occurring early, lacking standardized medical treatment, and leading to substantial comorbidity

Hypopituitarism in patients with prolactinomas: a narrative review.

Pituitary adenomas are the most frequent cause of hypopituitarism in adults, due to the mass effect of the lesion on the pituitary gland, and/or their treatments (particularly surgery and/or radiotherapy). Prolactinomas represent the most frequent histotype of hormone-secreting pituitary adenomas. As is well known, high prolactin levels induce a suppression of the gonadotropic axis and subsequent hypogonadotropic hypogonadism. Overall, the reported prevalence of hypopituitarism is highly heterogeneous in the different studies, depending on the definition used and the cohort examined. Treatment

Neuroendoscopic Trans-ventricular Cyst Drainage Prior to Tumour Resection Using a Two-Staged Approach for Cystic Craniopharyngioma: A Case Report and Literature Review.

Craniopharyngiomas are benign suprasellar tumours that can present with symptoms of raised intracranial pressure, visual impairment, or endocrine dysfunction. They are classified into the more common adamantinomatous and less common papillary tumours. Cystic variants may cause obstructive hydrocephalus, necessitating urgent intervention. This video case-based review presents the case of a 61-year-old male who presented with a two-month history of headaches, cognitive decline, and visual changes. Imaging revealed a large 2.8 x 2.2 x 2.2 cm suprasellar mass with cystic components extending into

Gamma knife radiosurgery vs fractionated radiotherapy : gender difference in efficacy and toxicity.

Although radiation therapy is used less frequently in the treatment of functioning and nonfunctioning pituitary adenomas, it still plays a role to control volume and secretion. Radiation therapy remains to be considered in the therapeutic algorithm for these diseases, as mentioned in all recent national and international guidelines. Recent studies have focused on the use of stereotactic approaches, either given as a single dose or as a fractionated one. However, some patients may still benefit from conformal fractionated radiotherapy. The choice among these procedures is currently based mainly
