Gut microbial and functional alterations lead to metagenomic signatures for midgut neuroendocrine tumor patients and for carcinoid syndrome.

SUMMARY: We report the case of a 41-year-old Japanese woman with visual field disturbances during late pregnancy. At 39 weeks of gestation, she was diagnosed with bitemporal hemianopsia at the ophthalmology department. An MRI revealed a symmetrical pituitary gland enlargement, compressing the optic chiasm. An emergency cesarean section was performed immediately, resulting in the delivery of a male infant weighing 3,112 grams. Laboratory tests indicated low serum free thyroxine (T4), thyroid-stimulating hormone (TSH), cortisol, luteinizing hormone, and follicle-stimulating hormone. The patient

Postoperative Considerations Following Pituitary Surgery: A Guide for Clinicians.

Transsphenoidal surgery represents the primary treatment for pituitary adenomas (other than prolactin-secreting) and other sellar/suprasellar masses including craniopharyngiomas. Endocrine evaluation for anterior hypopituitarism and water metabolism disorders is indicated in all patients. Transient fluid restriction postoperatively in patients without vasopressin deficiency reduces the risk of hyponatremia and hospital readmission. In Cushing disease, the postoperative serum cortisol levels in the first postoperative days guide further management. In acromegaly, remission is established primar

Is the SPLUNC1-Orai1 axis a critical determinant of lung health?

Short palate lung and nasal epithelial clone 1 (SPLUNC1; gene name BPIFA1) is a secreted protein that is highly expressed in the nasopharyngeal and pulmonary systems. By data mining, we found that SPLUNC1 is also expressed in other organs, including the kidneys and the pituitary gland. SPLUNC1 is an asthma and cystic fibrosis gene modifier that also inversely correlates with the severity of bronchiectasis. Orai1 is a plasma membrane Ca2+ channel that is an essential regulator of the immune system. We previously found that SPLUNC1 binds to Orai1, causing it to be ubiquitinated, internalized and

Clinical characteristics and comparative analysis of PD-1/PD-L1 inhibitor-induced pituitary dysfunction versus other pituitary disorders.

This study aims to investigate the clinical characteristics of programmed cell death protein-1 (PD-1) or programmed cell death ligand-1 (PD-L1) inhibitors-induced pituitary injury (pituitary-irAE) by comparing with other pituitary diseases. We retrospectively reviewed clinical records of patients who were admitted for pituitary-irAE, idiopathic isolated ACTH deficiency (IIAD), and lymphocytic hypophysitis (LYH) from January 2015 to June 2024. Among 3689 patients treated with PD-1/PD-L1 inhibitors, 0.596% patients developed pituitary-irAE. The mean age of onset was 59.41 ± 10.69 years, with 13

Orthotopic pituitary tumors generated by stereotaxic GC cell injection in immunocompetent rats.

Innovative treatment strategies for pituitary tumors are necessary to limit the disease burden and to improve survival in cases of carcinomas. The paucity and inaccuracy of available preclinical models substantially hamper pituitary research and drug discovery. Hence, we describe a novel method to generate orthotopic pituitary tumors via stereotaxic injection of somatotroph GC cells into the pituitaries of immunocompetent Wistar Furth rats. Tumor growth was monitored by repeated 7 Tesla magnetic resonance imaging. The procedure consistently led to rapidly expanding intra- and suprasellar growt

A core outcome set for pituitary surgery research: an international delphi consensus study.

PURPOSE: This study aimed to develop a core outcome set (COS) for pituitary surgery to enhance the quality, efficiency and effectiveness of future pituitary adenoma surgery research. METHODS: Thirty-three outcomes were identified through a systematic review of pituitary adenoma surgery outcomes and a study on patient-reported measures. These were presented in an online survey to healthcare professionals (HCPs), patients and caregivers. In the first round, participants scored each outcome's importance on a 5-point scale (1-strongly disagree; 5-strongly agree) and could also suggest additional

Analysis of sex-differential gene expression on the target of approved drug.

Sex is a key piece of patient information but is often not actively considered in drug use. This is partly due to the lack of molecular evidence at the gene expression level beyond sex chromosomes and sex hormones. We aim to investigate how sex differences in tissue-specific gene expression relate to FDA-approved drugs using the latest database of The Genotype-Tissue Expression (GTEx) V10. Our analysis reveals that 91.4% of FDA-approved drug target genes exhibit sex-differential expression in at least one tissue. The tissues with the most pronounced sex differences include subcutaneous adipose

Infertility as a Rare Manifestation of Pituitary Involvement in Granulomatosis With Polyangiitis: A Case Report.

Granulomatosis with polyangiitis is a rare autoimmune disease that affects small-sized blood vessels. It most commonly involves the lungs, kidneys, and the areas around the nose and throat, but can also affect other organs, including the nervous system. Enlargement of the pituitary gland may cause a mass effect, and pituitary dysfunction can occur. Among pituitary hormone abnormalities, hypogonadotropic hypogonadism is more common than other hormonal deficiencies. Pituitary involvement may arise at any stage of the disease, sometimes even as the initial presentation. We report a rare case of g

Thyroid Health and Selenium: The Critical Role of Adequate Intake from Fetal Development to Adolescence.

The thyroid gland plays a crucial role in regulating metabolism and supporting development through the production of the hormones T4 and T3. These hormones are essential during childhood for nervous system myelination, physical growth, puberty, skeletal and dental maturation, and overall metabolic balance. In early infancy, when the hypothalamic-pituitary-thyroid axis is still immature, thyroid dysfunction can result in a range of long-term complications. The metabolism and action of thyroid hormones depend not only on iodine but also on other vital micronutrients, particularly selenium (Se).

The Glucocorticoid Receptor Co-Chaperone FKBP51 in the Adrenal Cortex Is Not Involved in Regulating Hypothalamic-Pituitary-Adrenal Activity in the Mouse.

The FK506 binding protein 51 (FKBP51) is a stress-modulating protein implicated in stress-related psychiatric disorders. While FKBP51 has been extensively studied in the brain and other tissues, its role in the adrenal gland, a key part of the stress response, remains unexplored. To investigate Fkbp5 gene expression and FKBP51 function in the adrenal gland, we examined expression in C57Bl/6 male and female mice and generated an adrenal cortex-specific Fkbp5 knockout (ASFkbp5-/-). We found Fkbp5 mRNA expression throughout the adrenal gland in both male and female mice. However, deletion of Fkbp

Sheehan's syndrome complicated with Graves' disease in a woman with a 20-year history of hypothyroidism.

Sheehan's syndrome is a disorder caused by ischemic necrosis of the pituitary gland following postpartum hemorrhage, typically resulting in pituitary dysfunction that can affect the adrenal, thyroid, and gonadal axes. Clinically, patients may present with dysfunction of a single axis or multiple axes. The co-occurrence of Sheehan's syndrome and Graves' disease is extremely rare, posing a significant diagnostic challenge. Herein, we report the case of a woman in her early 50s with a 28-year history of Sheehan's syndrome, which had led to complete pituitary dysfunction, who had been managing hyp

The Quiet Burden of Iron: A Rare Case of Hereditary Hemochromatosis in Pakistan.

Hereditary hemochromatosis is characterized by excessive iron absorption through the gut and its deposition in the body. In many instances, symptoms do not arise until significant organ damage has occurred. Diagnosing a case of hereditary hemochromatosis is difficult even after the appearance of symptoms due to the diverse clinical presentations. Genetic testing should be done, whenever possible, to determine the underlying genetic abnormality. We present the case of a 42-year-old man who came to us with complaints of generalized body swelling and shortness of breath on exertion. He was found

Modeling of T cell-mediated autoimmune pituitary disease using human induced pluripotent stem cell-originated organoid.

Anti-pituitary-specific transcription factor (PIT)-1 hypophysitis is an autoimmune disease characterized by hormone secretion impairment from PIT-1-expressing pituitary cells, accompanied by malignancies with ectopic PIT-1 expression. Cytotoxic T cells (CTL) targeting PIT-1-positive cells have been implicated in disease development, yet direct evidence is lacking. As human leukocyte antigen (HLA)-matching is required for modeling T cell-mediated autoimmune diseases, we employ induced pluripotent stem cells (iPSC) to generate pituitary organoids harboring the patients' HLA haplotype and cocultu

Apparent Clitoromegaly in a Newborn: A Case of Congenital Adrenal Hyperplasia.

This case report elucidates the diagnostic trajectory of a female newborn, presenting with apparent clitoromegaly, ultimately diagnosed with congenital adrenal hyperplasia (CAH). The patient was born in a prominent obstetrics and gynecology center in Tbilisi, Georgia, where the anomaly was promptly identified following a physiologically normal pregnancy and labor. Despite the relative infrequency of such cases in our center, particularly among term infants, the handling of this case was swift and successful. Comprehensive examinations were conducted, with meticulous documentation of clitoromeg

50 YEARS OF NEWBORN SCREENING FOR CONGENITAL HYPOTHYROIDISM: EVOLUTION OF INSIGHTS IN ETIOLOGY, DIAGNOSIS AND MANAGEMENT: Transient or permanent congenital hypothyroidism: from milestones to current and future perspectives.

ABSTRACT: Endometriosis is a prevalent condition where tissue similar to the uterine lining grows outside the uterus, causing pain and infertility. Diagnosing endometriosis typically requires invasive procedures such as laparoscopy. MicroRNAs (miRNAs) have emerged as promising noninvasive biomarkers for various diseases, including endometriosis. However, studies have shown inconsistent miRNA expression patterns across populations. This study aims to validate circulating miRNAs as biomarkers for endometriosis in Indian women, addressing the limited validation data available for this population.

Hexavalent chromium neurotoxicity in Caenorhabditis elegans targeted GABAergic, cholinergic, and dopaminergic neurons, and contributed to an aged phenotype.

Hexavalent chromium [Cr(VI)] is a ubiquitous environmental pollutant and potent toxicant. Cr(VI) exposure impacts millions of people around the world, primarily through inhalation or ingestion. Such exposures are best known for contributing to cancers of the lung, sinus, and nasal passage, damage to kidneys and liver, and contact dermatitis. Growing evidence indicates exposure contributes to neurological conditions, but knowledge gaps persist regarding Cr(VI) neurotoxicity. Human studies report Cr(VI) contributes to autism spectrum disorders, motor neuron disease, olfactory dysfunction, and im

Multidisciplinary orthodontic and prosthetic treatment of an eleven-year-old patient with a solitary median maxillary central incisor: A case report.

Solitary median maxillary central incisor (SMMCI) syndrome is a rare developmental disorder in which a patient presents with only one upper central incisor located in the midline, along with some other features - such as deviant sella turcica and pituitary gland morphology, panhypopituitarism, hypothyroidism, and hypoplasia of the inner genitals - that may or may not be present. Although some authors consider it a mild manifestation of holoprosencephaly, others classify it as a distinct entity. The aim of this case report is to describe the orthodontic treatment of an 11-year-old female patien

A case report of acute Sheehan syndrome with a review of 29 existing reports from 1990 to 2024: is it still considered a rare disease?

BACKGROUND: Sheehan's syndrome (SS) can occur as a consequence of massive postpartum hemorrhage (PPH). Although traditionally considered rare, acute SS may be underestimated, especially in resource-limited settings, where even minor obstetric complications other than the commonly recognized uterine atony could lead to significant clinical consequences. CASE REPORT: A 32-year-old pregnant woman (gravida 1, para 0) at 37 weeks and 6 days of gestation developed massive PPH complicated by hemorrhagic shock, cardiac arrest, and an emergent hysterectomy, with an estimated blood loss of 20 L. Despit

Lupus-Associated Autoimmune Hypophysitis: A Rare Case.

Autoimmune hypophysitis (AH) is a rare inflammatory disorder of the pituitary gland, and its association with systemic lupus erythematosus (SLE) is extremely uncommon. We describe a 36-year-old man with newly diagnosed SLE who later developed fatigue, infertility, polyuria, and polydipsia. Hormonal evaluation revealed hypogonadotropic hypogonadism and low IGF-1. MRI showed pituitary stalk thickening and loss of the posterior pituitary bright spot, consistent with AH. Central diabetes insipidus (CDI) was confirmed by urine testing. The patient improved clinically after initiation of desmopressi

A Decade With Sheehan's Syndrome: A Case Report and Personal Experience.

OBJECTIVE: Sheehan's syndrome (SS) is a rare endocrine disorder caused by ischemic necrosis of the anterior pituitary gland secondary to massive postpartum hemorrhage (PPH) and shock. It commonly leads to multiple hormonal deficiencies, such as adrenal insufficiency, hypothyroidism, and hypopituitarism. Affected individuals may present with signs and symptoms including weight gain or loss, cold intolerance, hair loss, menstrual irregularities, and hypotension. Diagnosis is often delayed due to its nonspecific presentation and overlap with conditions like postpartum depression and chronic fatig
