Primary hyperaldosteronism is the most common cause of secondary hypertension, with a prevalence estimated between 5% and 15%. Despite this, it remains largely underdiagnosed due to often non-specific clinical manifestations and insufficient screening. However, primary hyperaldosteronism is associated with significantly higher cardiovascular morbidity and mortality compared to essential hypertension, regardless of blood pressure levels and usual cardiac remodeling. Recent studies have highlighted a direct link between primary hyperaldosteronism and atrial fibrillation-the most common sustained

BACKGROUND: Blood pressure (BP) monitoring is crucial for the management of hypertensive disorders of pregnancy. Cuffless, continuous BP-monitoring devices have been developed, but have yet to be validated during pregnancy.
OBJECTIVES: To describe the case of a 37-year-old patient presenting primary hyperaldosteronism, who suffered from severe preeclampsia during her first pregnancy. She chose to wear a cuffless BP monitoring device (AKTIIA) during her second pregnancy while undergoing three 24-h ABPM sessions. She developed preeclampsia, leading to hospitalisation and premature birth at 32 w

Conn's syndrome or primary aldosteronism (PA) due to aldosterone-producing adrenal adenoma (APA) is the most frequent cause of hypokalemia and secondary hypertension. We are reporting a Primary Aldosteronism(PA) case in an indian male, who presented with recurrent lower limb weakness & was admitted twice for management of his symptoms. On evaluating, laboratory reports revealed that his symptoms are due to refractory severe hypokalemia, metabolic alkalosis, raised serum aldosterone levels, and low plasma renin activity. Transtubular potassium gradient (TTKG) was suggestive of ongoing renal pot

Renovascular disease and primary hyperaldosteronism are among the leading causes of high blood pressure in young adults. Collaboration between cardiologists and endocrinologists is crucial in addressing the underlying causes of resistant hypertension. In this article, we provide updated information on the clinical context in which renal artery stenosis is suspected, and the appropriate investigation methods for confirmation. The decision to revascularize is based on the etiology of the stenosis, patient age, and comorbidities. In brief, we aimed to provide a cardiologist's perspective on a sig

BACKGROUND: Hypokalemic paralysis is a rare but potentially life-threatening neuromuscular emergency characterized by the sudden onset of flaccid weakness due to reduced serum potassium. While inherited channelopathies represent primary causes, secondary etiologies-particularly endocrine, renal, and metabolic disorders-are more frequently encountered and require timely recognition to avoid complications.
CASE SUMMARY: We report four patients presenting with acute flaccid paralysis associated with documented hypokalemia (serum potassium <3.5 mmol/L). The first case involved a pregnant woman wi

Hypertension is a significant comorbidity in the New World, and its prevalence is rising. Around 5%-10% of cases with hypertension have secondary hypertension. Adrenal gland disease is among the common causes of secondary hypertension. We present a 57-year-old male with uncontrolled hypertension and a history of intracranial hemorrhage. The lab evaluation of the patient revealed a K+ of 3.8 mEq/L, Na + of 138 mEq/L, Renin of 2.6 mIU/L, Aldosterone of 47.3 ng/dL, consistent with primary hyperaldosteronism (PHA). The patient's abdominal computed tomography (CT) revealed an adrenal mass measuring

Primary aldosteronism (PA) is a common disorder, and the condition is underdiagnosed; the prevalence could reach one-third in patients with hypertension, and thyroid carcinoma is the second most common cancer, with papillary thyroid carcinoma (PTC) accounting for 90% of cases. Lymph node metastasis is common in PTC. However, pressure symptoms, including invasion of major local veins and the vagus nerve, are extremely rare. The association between primary hyperaldosteronism and PTC is rare. The interaction between genetic and environmental factors could explain the association. Whether the coex

Conn syndrome is an adrenal gland adenoma that causes primary hyperaldosteronism, with a prevalence of <1% in the world population. Therefore, this case report presents a patient with Conn syndrome co-existing with hyperthyroidism. A 26-year-old female was admitted with muscle spasms in the left arm and muscle weakness in both legs for a day, palpitations, excessive sweating, nocturia, polyuria, and polydipsia. The patient had a history of hyperthyroidism for 10 years and routinely took anti-hypertension (Nifedipine), anti-thyroid, and potassium supplements. Physical examination showed the pre

Primary hyperaldosteronism is a common and treatable cause of secondary arterial hypertension. Case finding with the aldosterone-to-renin ratio is recommended in all patients with arterial hypertension. If the case finding test is positive, a confirmatory test should be performed, followed by imaging of the adrenal glands and adrenal vein sampling. Treatment depends on the cause. Laparoscopic adrenalectomy is the treatment of choice for unilateral secretion and medical therapy with mineral corticoid receptor antagonists for bilateral secretion.

Adrenal cavernous hemangiomas are rare benign venous malformations characterized by vascular dysmorphogenesis. A 53-year-old male patient was referred for a surgical consultation with a 5 cm heterogeneous, lipid-poor, incidental left adrenal mass. It was detected on a computed tomography (CT) scan performed for left upper quadrant pain. His past medical history was significant for poorly-controlled hypertension on a multi-drug regimen, anxiety, headaches, insomnia, and palpitations. Laboratory testing performed by his primary care physician revealed elevated plasma catecholamines, which were c

Hyperaldosteronism often clinically manifests as hypertension with mild to moderate hypokalemia. However, in rare cases, it can also present primarily with severe hypokalemia. In this case report, we will be discussing our patient, a 43-year-old woman who presented with abdominal cramps and vomiting, along with severe muscle cramps resulting in an inability to walk. She was found to have an undetectable level of serum potassium and prolonged QT interval on her ECG. After aggressive electrolyte correction and further imaging and endocrinology studies, she was diagnosed to have primary hyperaldo

Primary hyperaldosteronism is a common cause of secondary hypertension and is often underdiagnosed. While bilateral adrenal hyperplasia is the most frequent form, the condition may evolve, and a unilateral aldosterone-producing adenoma can emerge, especially in cases presenting with worsening biochemical abnormalities. We report a case of a 60-year-old male patient with a history of bilateral adrenal hyperplasia who presented with life-threatening, asymptomatic hypokalemia (1.41 mmol/L), refractory to intravenous potassium supplementation. Electrocardiography revealed flattened T waves and pro

Robot-assisted surgery has revolutionized minimally invasive procedures, offering superior three-dimensional visualization and mobile instruments suitable for smaller areas. For this reason, robotic retroperitoneal adrenalectomy (RRA) is emerging as an ideal procedure for this technology. This study aimed to assess the outcomes of the first 10 consecutive cases of this procedure using the RAS Hugo™ platform. Conducted between July 2023 and February 2024, the surgeries were performed with patients in the prone position, accessing the retroperitoneal space using standard endoscopic techniques. H

BACKGROUND: Primary aldosteronism (PA) is a common treatable cause of hypertension. When caused by unilateral adrenal disease, it is potentially curable by adrenalectomy. However, specialized tests and other factors may delay definitive treatment. We assessed the time to adrenalectomy (TTA) for patients worldwide.
METHODS: We conducted an international, multicentre retrospective study involving 39 centres from 15 countries to determine the total time taken from the first presentation to adrenalectomy and the intervals between each stage (screening, confirmatory, subtyping, and adrenalectomy).

Primary hyperaldosteronism is an underdiagnosed cause of hypertension despite its increasing prevalence and strong association with cardiovascular morbidity and mortality. Early recognition is critical. Individuals with primary aldosteronism generally present with treatment-resistant hypertension and hypokalemia. Here, we present the case of a patient who experienced cardiac arrest due to severe hypokalemia and was subsequently diagnosed with primary hyperaldosteronism, confirmed through adrenal vein sampling. The condition was successfully treated with a left adrenalectomy.

The mineralocorticoid receptor (MR) plays a pivotal role in skin homeostasis, inflammation, and repair, interacting closely with the glucocorticoid receptor (GR) to regulate various physiological and pathological processes. Dysregulation of MR signaling has been implicated in several skin disorders, including psoriasis, atopic dermatitis, and impaired wound healing. Furthermore, studies have shown that patients with primary hyperaldosteronism exhibit epidermal hyperplasia, impaired differentiation, increased immune cell infiltrates, and elevated pro-inflammatory cytokines due to MR overactivat

BACKGROUND: Primary aldosteronism is a common but underdiagnosed cause of endocrine hypertension that contributes to global cardiovascular morbidity and mortality. It is characterized by renin-independent hyperaldosteronism that originates from adrenal lesions-the majority of which are found to harbor aldosterone-driver somatic mutations in genes encoding ion-transporting proteins. These mutations disrupt intracellular calcium homeostasis, facilitating a pathological increase in aldosterone synthase expression and aldosterone production. Elucidating the exact mechanisms causing aldosterone exc
