Aldosterone synthase inhibitors may alter how primary care treats hypertension.

Hypertension guidelines recommend adding antihypertensive therapies until the blood pressure (BP) is below a specific target. If the BP is not below target with 3 medications, including a diuretic, then it is labeled resistant hypertension, and then we investigate for secondary causes, such as primary aldosteronism (PA). PA testing seems complex with 3 results: aldosterone, renin, and the aldosterone-renin ratio. Hence, these tests are ordered infrequently, and the diagnosis of PA is often delayed or never made. Also, heart failure, chronic kidney disease, and other conditions can have elevate

The Rare Case of a Sertoli Cell Tumor of the Adrenal Gland in a Man With Pathological Dexamethasone Suppression Test: A Case Report.

Sertoli cell tumors are rare gonadal stromal tumors. Extragenital manifestations, particularly in the adrenal gland, are exceptionally uncommon. We present a case of an adrenal Sertoli cell tumor in a male patient, presenting with a pathological (1 mg overnight) dexamethasone suppression test. An 82-year-old otherwise healthy man was referred for evaluation of an incidentally detected left adrenal mass. The patient was asymptomatic with no clinical signs of catecholamine excess, hypercortisolism, hyperaldosteronism, or B-symptoms. Initial CT scan revealed a 44 × 42 mm lesion, which increased t

Endocrine Hypertension in Children and Adults: Beyond Primary Aldosteronism.

Endocrine hypertension refers to elevated blood pressure resulting from an identifiable hormonal cause that is potentially treatable. Endocrine hypertension occurs in ∼30% of young adults with hypertension (18-40 years), in 30-50% of patients with resistant hypertension, most commonly due to primary aldosteronism, and can occur in a small yet clinically significant subset of children with hypertension. Other causes of endocrine hypertension include excess mineralocorticoid production, cortisol excess or impaired cortisol metabolism, disorders of the mineralocorticoid receptor or epithelial sod

Hyperaldosteronism in the Pathophysiology and Management of Cardiovascular-Kidney-Metabolic Syndrome.

Cardiovascular-kidney-metabolic (CKM) syndrome is defined by the pathologic interconnections between obesity, diabetes, chronic kidney disease and cardiovascular disease. An important and underrecognized pathophysiologic factor underlying CKM syndrome is obesity-induced aldosterone excess. This excess aldosterone causes structural and functional alterations in the kidneys, heart and vasculature promoting development of chronic kidney disease, cardiovascular disease and heart failure. Inhibition of aldosterone signalling is a pillar in the treatment approach to CKM syndrome. Pharmacologic optio

Incidence of Adrenal Masses Eventually Discovered in Patients Screened for Primary Aldosteronism.

CONTEXT: Biochemical screening for endocrine hypertension may be complex; the known presence of an adrenal mass markedly increases the pretest probability for adrenal hypertension and may inform diagnostic efforts. OBJECTIVE: Model the potential incidence of adrenal masses that may be detected by routine adrenal imaging in patients with suspected endocrine hypertension. DESIGN: Retrospective cohort study. SETTING: Provincial administrative and clinical databases for radiology and biochemistry, Alberta, Canada. PARTICIPANTS: Patients with hypertension and any measurement of aldosterone-reni

Persistent Renin Suppression After Unilateral Adrenalectomy for Primary Aldosteronism: Implications for Adverse Outcome?

CONTEXT: Suppressed renin concentrations in medically treated patients with primary aldosteronism (PA) are associated with a worse cardiometabolic outcome compared to patients with normalized renin levels. No similar study has been conducted in patients with PA after unilateral adrenalectomy. OBJECTIVE: This study investigates the prevalence and impact of persistent renin suppression after unilateral adrenalectomy in patients with lateralized PA. METHODS: We retrospectively included 77 patients who underwent adrenalectomy for lateralized PA. All patients had postoperative renin levels availa

Evaluating the impact of adrenalectomy and mineralocorticoid receptor antagonists for primary aldosteronism on left ventricular remodeling: a systematic review and meta-analysis.

BACKGROUND: Primary aldosteronism (PA) is characterized by autonomous aldosterone production, which leads to left ventricular (LV) remodeling and adverse cardiovascular outcomes. This meta-analysis aimed to compare the effects of two treatment strategies, mineralocorticoid receptor antagonists (MRAs) and adrenalectomy, on the regression of LV remodeling in patients with PA. METHODS: Embase, PubMed, and Cochrane Library were searched for studies reporting the effect of adrenalectomy or medical therapy on reversing LV mass index (LVMI) in patients with PA. The outcome of interest was the reduct

Screening and Diagnosis of Primary Aldosteronism in Patients Using Renin-Angiotensin System Inhibitors.

This study investigated whether patients receiving renin-angiotensin system inhibitors (RASIs) can undergo reliable screening for primary aldosteronism (PA) without discontinuation of therapy. Consecutive patients with hypertension who underwent PA screening at our hospital between 2016 and 2024 while on RASIs were recruited. Plasma aldosterone concentration (PAC) and direct renin concentration (DRC) were measured at three time points: pre-washout, post-washout, and post-captopril-challenge test (CCT). Subsequently, the aldosterone-to-renin ratio (ARR) was calculated, and PAC, DRC, and ARR wer

Somatic Drivers in Aldosterone-producing Adenomas and Nodules: Insights from a Brazilian Cohort of Primary Aldosteronism.

BACKGROUND: The Brazilian population represents a mosaic of genetic diversity resulting from admixture ancestries. Given reported disparities in primary aldosteronism (PA) genetics across ethnicities, we investigated the genetic spectrum of aldosterone-producing adenomas (APAs) and nodules (APNs) with classical histology in a Brazilian cohort. METHODS: We included 62 lesions (1 case with bilateral APAs) from 61 consecutive patients (median age at PA diagnosis 49 years, 59% women) with PA and classical histology, defined by CYP11B2 immunostaining (HISTALDO consensus). Somatic DNA was extracted

A novel 2D and 3D model for primary adrenocortical carcinoma of advanced and metastasized stage co-secreting cortisol, aldosterone, testosterone, 18-oxocortisol and 18-hydroxycortisol.

Adrenocortical carcinoma (ACC) is a highly aggressive malignancy with poor survival rates and few treatment options. Preclinical models are indispensable to further strengthen our understanding of disease progression and development of novel therapeutic treatments. Here, we report the establishment of a new cell line named ZUC-1 originating from the resection of an advanced primary ACC and its characterization at the genomic, cellular and molecular level. ZUC-1 cells were successfully propagated as monolayer cultures and three-dimensional spheroids. LC-MS/MS analysis revealed for ZUC-1 cells c

Emerging Medical Therapies for Primary Aldosteronism.

Medical therapy for primary aldosteronism has been stagnant for decades, relying on mineralocorticoid receptor antagonists, which block downstream signaling of aldosterone rather than aldosterone production. This approach typically leads to reactive elevation of aldosterone production, and possible implications of its nongenomic effects. In addition, steroidal mineralocorticoid receptor antagonist use is limited by cross-reactivity with other nuclear receptors and concern for hyperkalemia, particularly in kidney insufficiency. These limitations have propelled a rising interest in therapies tha

Impacts of the use of chemiluminescent enzyme immunoassay-measured plasma aldosterone concentration in the diagnostic process of unilateral primary aldosteronism: a real-world multicenter study in Japan.

Unilateral primary aldosteronism (UPA) is characterized by a severe clinical phenotype and can be cured by adrenalectomy. Establishing accurate cutoff values that indicate the need for adrenal venous sampling (AVS) is crucial. Therefore, we aimed to identify appropriate cutoff values for screening and confirmatory testing to predict UPA by LC-MS/MS-equivalent plasma aldosterone concentration (PAC) using chemiluminescent enzyme immunoassay (CLEIA). A retrospective cohort analysis was conducted as part of the JPAS-II study of 443 patients diagnosed with PA using CLEIA-measured PAC, of whom 179 w
