Association between plasma aldosterone after saline infusion test and nocturnal hypertension in patients with suspected primary aldosteronism.

BACKGROUND: Nocturnal hypertension (NH) is a significant risk factor for target organ damage and all-cause mortality. Post-saline infusion test plasma aldosterone concentration (post-SIT PAC) serves as an indicator of autonomous aldosterone secretion level, but its link to NH is uncertain. This study aims to explore the association between post-SIT PAC levels and NH in patients with suspected primary aldosteronism (PA). METHODS: Participants were classified into 3 groups after a saline infusion test (SIT): negative (post-SIT PAC < 5 ng/dL, n = 86), borderline (post-SIT PAC 5-10 ng/dL, n = 180

Broadening Primary Aldosteronism Screening: Alignment Across Contemporary Guidelines.

Fewer than 2% of eligible patients are screened for primary aldosteronism, despite evidence that early detection and targeted therapy are associated with lower cardiovascular and kidney morbidity. Recent updates to major hypertension and endocrine guidelines reflect growing recognition that primary aldosteronism is far more prevalent than previously understood and that broader, more practical screening approaches are needed. These recommendations increasingly extend screening beyond resistant hypertension to adults with stage 2 hypertension and even to all individuals with hypertension. They a

Endocrinological aspects of sarcopenic obesity.

BACKGROUND: Sarcopenic obesity (SO) is a multifactorial condition characterized by the coexistence of excess adiposity and reduced skeletal muscle mass and function. Its development reflects a complex interaction of metabolic, inflammatory, and endocrine mechanisms that disrupt the balance between anabolic and catabolic processes. MAIN FINDINGS: Endocrine dysfunction is a major driver of the altered adipose-muscle crosstalk characteristic of SO. Hormonal imbalance amplifies mitochondrial dysfunction, oxidative stress, and chronic inflammation, leading to reduced muscle quality and increased v

Liquid chromatography-tandem mass spectrometry-based urinary steroid profiling applied to primary aldosteronism diagnosis.

Steroid hormones are essential regulators of physiological homeostasis, which can help in diagnosing primary aldosteronism (PA). However, current methodologies for urinary steroid analysis face critical limitations, including narrow analyte coverage and tedious sample preparation workflows. Since the liquid chromatography-tandem mass spectrometry (LC-MS/MS) is considered the preferred technique for steroid measurement, there is a great need to establish an LC-MS/MS method combined with simplified and efficient sample pretreatment for multi-steroid profiling. We establish a novel LC-MS/MS metho

Pooled diagnostic accuracy of 68Ga-pentixafor PET/CT for aldosterone-producing lesions in primary aldosteronism.

BACKGROUND: Primary aldosteronism is a leading cause of secondary hypertension and is associated with increased cardiovascular risk. Accurate localization of aldosterone-producing lesions is essential to guide curative treatment. Although adrenal venous sampling (AVS) remains the gold standard for subtyping, it is invasive and not widely available. Recent advances in molecular imaging, including 68Ga-Pentixafor PET/CT targeting CXCR4, have shown potential for non-invasive localization of functional adrenal lesions. PURPOSE: To assess the pooled diagnostic accuracy of 68Ga-Pentixafor PET/CT fo

Adrenal Emergencies.

The adrenal glands drive physiologic homeostasis, with dysregulation in any direction causing multisystem dysfunction. Adrenal excess states include hyperaldosteronism which manifests with refractory hypertension and electrolyte abnormalities including hypernatremia and hypokalemia. Paragangliomas including pheochromocytoma can cause multisystem end-organ dysfunction due to catecholaminergic storm, which require rapid blood pressure control with phentolamine and identification of lesions amenable to surgical resection. Adrenal insufficiency states in contrast can result in hypotension and deco

Subclinical Primary Aldosteronism and eGFR Decline Over Time.

KEY POINTS: This prospective study of 976 adults measured the association between subclinical primary aldosteronism and kidney outcomes. Among a general healthy adult population, subclinical primary aldosteronism was associated with steeper decline in eGFR over time. Primary aldosteronism pathophysiology spans a wide unrecognized continuum that parallels the risk for kidney disease. BACKGROUND: Primary aldosteronism, an overt form of renin-independent aldosterone production, leads to steeper eGFR decline compared with primary hypertension. Mounting evidence suggests that milder forms of renin

An interpretable machine learning model for preoperative prediction of aldosterone secretion and CYP11B2 status of adrenal gland.

BACKGROUND: To investigate whether a machine learning (ML) model integrating CT-based radiomics and clinical features can noninvasively evaluate the aldosterone secretion and CYP11B2 status of the adrenal gland, using Shapley Additive Explanations (SHAP) for model interpretation. METHODS: Patients who underwent adrenalectomy with CYP11B2 immunohistochemistry (IHC) confirmation between January 2022 and February 2025 were analyzed retrospectively. Radiomics models were developed based on different radiomics features selected by ElasticNet. Clinical features were used to construct the clinical m

The ARRplus: a model for renin-dependent and sex-specific aldosterone cut-offs that improves screening for primary aldosteronism.

OBJECTIVES: Screening for primary aldosteronism (PA) using the aldosterone:renin ratio (ARR) has suboptimal diagnostic accuracy, particularly in females. We assessed whether accuracy could be improved using sex-specific relationships of aldosterone with renin. METHODS: Plasma aldosterone and renin were measured in 442 females and 491 males prospectively screened for PA. Patients were divided into test (n=717) and validation (n=216) cohorts. Sex-specific cut-offs for renin-dependent aldosterone concentrations and the ARR were developed using logistic regression models in the test cohort. Diagn

Optimal Cut-Off Values of the Aldosterone-Renin Ratio in Different Age Groups for Primary Aldosteronism Screening.

This study sought to establish age-specific optimal cut-off values for the aldosterone-to-renin ratio (ARR) in screening for primary aldosteronism (PA). The analysis included 281 patients with confirmed PA and 1242 with essential hypertension (EH), categorized into four age groups: ≤39, 40-49, 50-59, and ≥60 years. A positive correlation was observed between ARR and age in both patient groups. Receiver operating characteristic analysis determined the optimal ARR cut-offs to be 2.3, 2.8, 3.1, and 3.6 for the respective age groups, with all demonstrating high diagnostic accuracy. Subtype- and treatment-based analysis of calcium dynamics in primary aldosteronism.

OBJECTIVE: Primary aldosteronism (PA) adversely affects cardiovascular and renal health and has been linked to increased urinary calcium excretion, elevated parathyroid hormone (PTH) levels, and reduced bone mineral density. Although adrenalectomy and mineralocorticoid receptor antagonists (MRAs) are established treatments, their differential effects on calcium dynamics across PA subtypes remain unclear. DESIGN: We conducted a retrospective observational study, analyzing a large, well-characterized cohort on calcium metabolism in PA. A total of 352 patients with PA and 57 controls were evalua

Machine learning-based prediction of primary aldosteronism subtype using comprehensive clinical features.

Primary aldosteronism (PA) has two major subtypes: unilateral (uPA) and bilateral (bPA). Although several diagnostic models for subtype classification have been reported, the optimal combination of algorithms and clinical features remains unclear. This study aimed to identify machine learning models and clinical features that contribute to PA subtype prediction. A total of 274 PA patients who underwent successful adrenal venous sampling (AVS) at a single center were analyzed. Overall, 196 endocrine features were comprehensively collected and classified into four categories: A, PA-related featu

Clinical implications of aldosterone responsiveness to adrenocorticotropic hormone stimulation in two major subtypes of primary aldosteronism.

OBJECTIVE: This study examined differences in aldosterone responsiveness to adrenocorticotropic hormone (ACTH) between the unilateral aldosterone-producing adenoma (APA) and bilateral idiopathic hyperaldosteronism (IHA) subtypes of primary aldosteronism (PA) and the impacts on vascular function and treatment outcomes. METHODS: This retrospective study enrolled 97 patients with PA (63 with APA and 34 with IHA) who underwent adrenal venous sampling (AVS) with ACTH stimulation. During AVS, baseline and ACTH-stimulated plasma aldosterone concentration (PAC) and cortisol levels were also assessed.

7-T Potassium (39K) MRI to Assess Muscle K+ Depletion in Primary Aldosteronism.

Background Although 98% of potassium (K+) resides intracellularly, current clinical diagnostics assess only extracellular K+ concentrations. Noninvasive imaging of tissue K+ distribution could provide novel insights into pathophysiologic processes in diseases such as primary aldosteronism (PA). PA is characterized by excessive aldosterone production, which leads to electrolyte imbalances, hypertension, and increased cardiovascular risk. Purpose To determine whether potassium 39 (39K) MRI combined with sodium 23 (23Na) MRI can help detect alterations in skeletal muscle K+ and Na+ distribution i

Big Data Approach to Assessment of an Aldosterone-Renin Ratio for Detection of Potential Surgical Cases of Primary Aldosteronism.

BACKGROUND: Prior retrospective studies of aldosterone-renin ratio (ARR) measures were confounded by selection bias. METHODS: To define ARR sensitivity independent of case definition, administrative health and diagnostic imaging repositories in Alberta, Canada, were used to construct a 5-component definition for primary aldosteronism (PA) for the most easily recognizable cases. Components included hypertension, hypokalemia, adrenal mass, suppressed renin, and nonsuppressed aldosterone. ARR was omitted from the definition, permitting independent evaluation of the ARR using receiver operating c

Fewer Pills, Lower Bills: Antihypertensive and Cost Outcomes of Adrenalectomy in Primary Aldosteronism.

BACKGROUND: Primary aldosteronism (PA) can be treated surgically or medically depending on disease lateralization and surgical candidacy. There is a dearth of data directly comparing antihypertensive medication trajectories and costs between these strategies. PATIENTS AND METHODS: We performed a retrospective cohort study of patients with new PA diagnoses and adrenal vein sampling to assess antihypertensive medication outcomes and treatment costs using Optum's de-identified Clinformatics® Data Mart Database (2004-2022). Patients were stratified by receipt of adrenalectomy versus medical manag

Biochemical success after selective adrenal arterial embolization predicts long-term regression of aldosterone-mediated cardiac remodeling.

BACKGROUND: Selective adrenal arterial embolization (SAAE) is a minimally invasive, vascular intervention targeting aldosterone-producing tissue in primary aldosteronism (PA). While surgery and mineralocorticoid receptor antagonists are standard therapies, long-term cardiac outcomes after SAAE remain poorly defined. METHODS: In this prospective cohort study, 129 patients with adrenal vein sampling-confirmed unilateral PA who declined adrenalectomy underwent SAAE. Biochemical outcomes were classified by Primary Aldosteronism Surgical Outcome (PASO) criteria as complete (CS, n = 71) or incomple

Partial pressure of oxygen-guided adrenal venous sampling in primary aldosteronism.

CONTEXT: Adrenal venous sampling (AVS) plays a pivotal role in treatment optimization for primary aldosteronism (PA) to minimize cardiovascular risks. However, technical difficulties often hinder accurate cannulation to the adrenal veins (AVs). OBJECTIVE: This study aimed to explore the distribution of partial pressure of oxygen (pO2) in the AV and neighboring veins, inspired by our awareness of the lighter red color of adrenal venous blood than the others. METHODS: We enrolled 179 PA patients who underwent AVS from 2021 to 2024. During AVS, we collected residual blood samples from bilateral
