Evaluation of Current Screening Thresholds for the Detection of Primary Aldosteronism.

BACKGROUND: In 2025, the Endocrine Society (ES) updated the guidelines for primary aldosteronism (PA), introducing more inclusive diagnostic criteria: plasma renin activity (PRA) ≤1 ng/mL per hour, plasma aldosterone concentration (PAC) ≥7.5 ng/dL by liquid chromatography-tandem mass spectrometry, and aldosterone-to-renin ratio >15. We compared PA positivity by comparing these updated criteria against broader permissive criteria (PRA <1 ng/mL per hour, PAC >5 ng/dL, without an aldosterone-to-renin ratio requirement). METHODS: We retrospectively analyzed deidentified adults with PRA and PAC me

The Importance of Disease-specific Growth Charts for Children with Congenital Adrenal Hyperplasia.

BACKGROUND: Children with congenital adrenal hyperplasia (CAH) due to 21-hydroxylase deficiency typically have height, weight, and body mass index (BMI) growth patterns that differ from the general population due to increased androgen and/or glucocorticoid exposures. With the recent surge in the development of new therapies, CAH-specific growth charts are needed to evaluate the effectiveness of these new treatments. METHODS: Retrospective data from patients aged 0 to 20 years with classic CAH, confirmed by hormonal testing and/or CYP21A2 genotyping, from 2 large clinical databases were analyz

Visualization of relative 11β-hydroxylase (CYP11B1) activity in human cortisol-producing adrenocortical adenoma (CPA) using mass spectrometry imaging-based enzyme histochemistry.

Enzymes play essential roles in numerous biochemical reactions, making it necessary to evaluate both their localization and activity. While conventional enzyme activity detection methods require specific antibodies and colorimetric reactions for each target compound, a recent method using mass spectrometry imaging enables the visualization of enzyme distribution and activity by applying substrates directly to tissue sections and detecting the resulting enzyme reaction products generated on these sections. Notably, the 11β-hydroxylase (CYP11B1, EC 1.14.15.4) reaction plays a key role in stress

Pheochromocytoma as a manifestation of Neurofibromatosis type 1: Clinical presentation, diagnosis, and genetic analysis.

OBJECTIVES: Neurofibromatosis type 1 (NF1) is a genetic multisystem disorder characterized by café-au-lait macules, neurofibromas, and other clinical features. Patients with NF1 have an increased risk of developing various tumours, including pheochromocytomas and paragangliomas (PPGLs), rare neuroendocrine tumours that secrete catecholamines. The prevalence of PPGL in NF1 patients varies, with some studies reporting rates up to 7.7%. Diagnosis relies on clinical symptoms such as hypertension, headaches, and palpitations, alongside biochemical and imaging studies. CASE PRESENTATION: This repor

Endocrine Hypertension in Children and Adults: Beyond Primary Aldosteronism.

Endocrine hypertension refers to elevated blood pressure resulting from an identifiable hormonal cause that is potentially treatable. Endocrine hypertension occurs in ∼30% of young adults with hypertension (18-40 years), in 30-50% of patients with resistant hypertension, most commonly due to primary aldosteronism, and can occur in a small yet clinically significant subset of children with hypertension. Other causes of endocrine hypertension include excess mineralocorticoid production, cortisol excess or impaired cortisol metabolism, disorders of the mineralocorticoid receptor or epithelial sod

Primary Aldosteronism and the Kidneys.

Primary aldosteronism (PA) is the most common and modifiable form of secondary hypertension, being responsible for 5-10% of all cases of hypertension and over 20% of cases of resistant hypertension. PA is characterized by autonomous aldosterone secretion from 1 or both adrenal glands, independent of regulation by renin and angiotensin II. Although the adverse cardiovascular consequences garner the majority of attention in regard to its clinical impact, PA leads to important detrimental effects on kidney health also. This includes early glomerular hyperfiltration followed by albuminuria, steepe

Sodium-Glucose Cotransporter 2 Inhibitors in Patients with Primary Aldosteronism.

KEY POINTS: Primary aldosteronism patients on mineralocorticoid receptor antagonists face high cardio-renal risks, but evidence for effective add-on therapies remains limited. Adding sodium-glucose cotransporter 2 inhibitors to mineralocorticoid receptor antagonists reduced 3-year mortality by 6.6%, major adverse cardiovascular events by 2.7%, and major adverse kidney events by 9.7% absolute risk. Mineralocorticoid receptor antagonist-sodium-glucose cotransporter 2 inhibitor combination therapy offers a promising strategy to improve long-term survival and organ preservation in primary aldoster

Congenital adrenal hyperplasia in children: a comprehensive review of clinical spectrum, diagnostic strategies, and management approaches.

INTRODUCTION: Congenital adrenal hyperplasia (CAH) comprises a group of rare autosomal recessive disorders and is a leading cause of primary adrenal insufficiency in children. Most cases are due to classic 21-hydroxylase deficiency. Earlier diagnosis has been made possible with newborn screening, which has also reduced the risk of adrenal crises. This review is based on a search of PubMed, Embase, Scopus, and Medline for articles published from 2000 to 2025, using the terms "congenital adrenal hyperplasia", "21-hydroxylase deficiency", "primary adrenal insufficiency", and "steroidogenesis" wit

Thirty-three years of X-linked adrenoleukodystrophy diagnosis at a Brazilian reference center: diagnostic patterns and key findings.

X-linked adrenoleukodystrophy (X-ALD) is the most common peroxisomal disorder. Although the quantification of very long-chain fatty acids (VLCFA) is an old technique implemented to diagnose and monitor, the limited number of experienced clinical biochemists in this field and also the limited availability of technical resources have resulted in scarce reports on the prevalence of X-ALD in developing countries like Brazil. This retrospective cross-sectional study analyzed all records of Brazilian patients diagnosed with X-ALD in LAM/SGM/HCPA from November 1992 to June 2026. Plasma VLCFA levels w

DUR-Net+: Semi-Supervised Abdominal CT Pheochromocytoma Segmentation via Dynamic Uncertainty Rectified and Prior Knowledge From SAM-Med3D.

Pheochromocytoma is a rare urological adrenal tumor disease. Automated segmentation of pheochromocytomas from computed tomography (CT) is essential for diagnosis and treatment. However, this task is a challenging one due to issues such as blurred boundaries, irregular shapes, variations in location and size, and the lack of annotated images for training. To address these issues, we propose a semi-supervised framework for pheochromocytoma segmentation that primarily consists of a dynamic uncertainty rectification mechanism and a supervised strategy based on SAM-Med3D prior knowledge. First, we
