A Rare Cause of False-Positive 18 F FDG Uptake in Testicular Adrenal Rest Tumors Mimicking Malignancy in a Patient With Congenital Adrenal Hyperplasia : A Complete Metabolic Response on FDG PET/CT Following Treatment.

Testicular adrenal rest tumors (TARTs) are benign lesions believed to arise from pluripotent steroidogenic cells within the testes, typically developing under chronic stimulation by adrenocorticotropic hormone. We present the case of a male with congenital adrenal hyperplasia (CAH) who underwent FDG PET/CT due to bilateral testicular masses, demonstrating false-positive FDG avidity suggestive of malignancy. Following appropriate corticosteroid therapy, repeat imaging revealed complete metabolic resolution of the lesions. This case illustrates a rare false-positive finding on FDG PET/CT and to

Evidence of Iron Accumulation in Cerebral Adrenoleukodystrophy: A Potential Novel Disease Mechanism.

In this first application of Quantitative Susceptibility Mapping Source Separation to cerebral adrenoleukodystrophy, we uncovered alterations in iron and myelin within lesions and normal appearing white matter. As validation, we demonstrate abnormal iron accumulation in those same compartments within primary brain tissue. A gradient of microglial activation to absence parallels the transition from normal white matter to demyelinated lesion. In perilesional white matter, increases in myelin peroxidation and Acyl-CoA synthetase long-chain family member 4 are observed. Thus, the unifying mechanis

Primary Adrenal BCOR::CCNB3 Sarcoma: Fine Needle Aspiration Cytology Findings.

Pediatric adrenal tumors are uncommon, with the most common being peripheral neuroblastic tumors. Other neoplasms like adrenal cortical tumors, pheochromocytoma, and myelolipoma may also be encountered in this age group. Rare instances of mesenchymal tumors such as Ewing sarcoma and rhabdomyosarcoma have been described. We report the fine needle aspiration cytology (FNAC) findings of the first case of BCOR-altered sarcoma occurring in the adrenal gland. A 13-year-old boy presented with severe abdominal pain. Imaging revealed a mass in the right adrenal gland. FNAC showed loosely cohesive clust

Targeting ER stress in adrenocortical carcinoma: Celastrol as a novel therapeutic candidate.

BACKGROUND: Adrenocortical carcinoma (ACC) is a rare, aggressive malignancy with limited treatment options. Mitotane, the only approved drug, has modest efficacy and frequent adverse effects, underscoring the urgent need for new therapeutic approaches. Celastrol, a triterpene compound derived from traditional Chinese medicine, exhibits anti-inflammatory activity and broad anti-tumor effects, including sensitization of cancer cells to chemo- and radiotherapy. PURPOSE: In this study, we investigated for the first time the anti-cancer potential of celastrol in the ACC H295R cell line. METHODS:

Diurnal Regulation and Gene-Specific Vulnerability of Oxidative Alcohol-Metabolizing Enzymes to Circadian Disruption.

Oxidative alcohol metabolism in the liver relies on sequential enzymatic reactions involving alcohol dehydrogenase (ADH), cytochrome P450 2E1 (CYP2E1), and aldehyde dehydrogenase (ALDH) isozymes. However, the circadian regulation of these enzymes, their susceptibility to genetic, environmental, and metabolic disruption, and their functional implications toward alcohol-mediated tissue injury remain incompletely defined. To address this gap, we performed a comprehensive integrative analysis of the publicly available circadian transcriptome datasets spanning genetic clock disruption, acute sleep

Primary Adrenal Gland Lymphoma: Report of 13 Cases-A Retrospective Multicenter Polish Lymphoma Research Group Analysis.

Introduction: The existence of primary adrenal gland lymphoma (PAGL) has been debated due to lack of lymphoid tissue in the adrenal glands. PAGL is extremely rare, accounting for less than 1% of all types of lymphomas. The aim of this study was to analyze patients with PAGL in Polish population. Material and Methods: We retrospectively reviewed 13 adult patients with PAGL diagnosed in Polish Hematological Centers. Results: A total of 13 patients (5 women and 8 men) with PAGL were included into the study. The median age at the diagnosis was 69.1 years (range: 31-85). The most common histologica

Expression of YME1 Like 1 ATPase Increases With the Stage of Adrenocortical Carcinoma Tissue and Is Associated With Poor Patient Prognosis.

BACKGROUND/AIM: Adrenocortical carcinoma (ACC) is an endocrine tumor arising in the adrenal cortex. Although its incidence is extremely low, it is highly malignant, rapidly proliferating, and infiltrating surrounding organs, resulting in a poor prognosis. YME1 Like 1 ATPase (YME1L1) is an ATP-dependent metalloprotease that regulates mitochondrial proteostasis. Recently, a correlation between YME1L1 expression and the prognosis of several cancers has been reported. However, no studies have examined the expression level of YME1L1 mRNA in ACC tissues or the relationship between YME1L1 expression

Use of osilodrostat in a block-and-replace and titration strategy for cushing's syndrome: a position paper by French experts.

The primary treatment for active Cushing’s Syndrome (CS) is the surgical removal of the causative tumor. Alternative therapeutic strategies, particularly pharmaceutical approaches, are required when surgery is not feasible, unsuccessful, or in cases of recurrence. Osilodrostat, an adrenal steroidogenesis inhibitor, is approved in Europe and USA for adult patients with CS. The recommended strategy involves gradual titration to achieve eucortisolism, but this approach has limitations. Delayed disease control can be harmful in severe cases, and frequent clinical and laboratory monitoring may be c

Current insights into monitoring of congenital adrenal hyperplasia.

The management of 21-hydroxylase deficiency (21OHD), the most common form of congenital adrenal hyperplasia, remains challenging as both over- and undertreatment with hormone replacement therapy are associated with short and long-term complications. Monitoring of treatment efficacy typically combines clinical assessment with biochemical evaluation by measuring specific steroids. Currently, androstenedione and 17-hydroxyprogesterone are the most commonly measured biomarkers, and their concentrations are interpreted using available reference intervals. However, inter-center variation in the conc

7-T Potassium (39K) MRI to Assess Muscle K+ Depletion in Primary Aldosteronism.

Background Although 98% of potassium (K+) resides intracellularly, current clinical diagnostics assess only extracellular K+ concentrations. Noninvasive imaging of tissue K+ distribution could provide novel insights into pathophysiologic processes in diseases such as primary aldosteronism (PA). PA is characterized by excessive aldosterone production, which leads to electrolyte imbalances, hypertension, and increased cardiovascular risk. Purpose To determine whether potassium 39 (39K) MRI combined with sodium 23 (23Na) MRI can help detect alterations in skeletal muscle K+ and Na+ distribution i

Pitfalls in the management of undiagnosed secondary adrenal insufficiency: a case report and review of the literature.

BACKGROUND: Prolonged cortisol deficiency in undiagnosed central adrenal insufficiency can lead to severe hypotonic hyponatremia due to inappropriate vasopressin secretion and malnutrition caused by inhibition of orexigenic signals. Notably, although hydrocortisone-induced recovery can trigger osmotic demyelination and refeeding syndromes, no previous report has simultaneously described these complications and documented significant decreases in vasopressin levels, along with changes in urine osmolality and volume before and after hydrocortisone administration. CASE PRESENTATION: A 48-year-ol
