Repeated short corticotropin testing in patients with polymyalgia rheumatica.

OBJECTIVE: To investigate adrenal function and the prevalence of glucocorticoid (GC) induced adrenal insufficiency (GIAI) in patients with polymyalgia rheumatica (PMR), the first 18 months after diagnosis. METHODS: Newly diagnosed patients with PMR underwent short corticotropin tests before commencement of GC treatment and after 10 weeks, 6 months, and 12 months. Patients still on GC or with GIAI after 12 months were invited to a test after 18 months. GIAI was defined as a stimulated serum cortisol level <420 nmol/l. RESULTS: : Sixty patients with PMR were enrolled. The mean (S.D.) stimulate

Sexual Dysfunction and Its Relationship With Hypogonadism and Myelopathy in Male Patients With X-Linked Adrenoleukodystrophy.

X-linked adrenoleukodystrophy (ALD) is a genetic disorder with neurological and endocrine manifestations, including myelopathy and hypergonadotropic hypogonadism. Many patients experience sexual dysfunction but the underlying cause has not been clarified. The aim of this study was to assess the prevalence of sexual dysfunction and its relationship with biochemical hypergonadotropic hypogonadism (i.e., low testosterone and high luteinizing hormone [LH] levels) and/or myelopathy. Male ALD patients from "the Dutch ALD cohort" at the Amsterdam University Medical Centers were questioned regarding s

The Significance of High Expression of Circulating miR-222-3p in Patients With Unilateral Primary Aldosteronism.

CONTEXT: The significance of circulating microRNAs (miRNAs) in primary aldosteronism (PA) is still largely unknown. OBJECTIVE: We compared the profiles of circulating miRNAs between unilateral primary aldosteronism (UPA) and bilateral hyperaldosteronism (BHA) and investigated the potential role of a newly identified UPA-related miRNA in the pathogenesis of PA. METHODS: We initially conducted a comprehensive expression analysis of circulating miRNAs using a quantitative polymerase chain reaction panel in a small number of cases matched for background factors, followed by validation analysis w

Is It Time to Retire Aldosterone Suppression Testing?

Primary aldosteronism (PA) is the most common endocrine hypertension. For decades, PA diagnosis has required proving nonsuppressibility of aldosterone following maneuvers modulating the renin-angiotensin-aldosterone pathway. This includes oral salt suppression, intravenous saline suppression, captopril suppression, and others. Grounded in rational first principles from pathophysiologic considerations and small, early pathophysiologic studies following Conn's initial PA description, such testing has been widely recommended. However, a modern understanding of PA pathophysiology and critical appr

Rethinking Primary Aldosteronism: The 2025 John H. Laragh Research Award.

Dr John Laragh, a pioneer in the field of hypertension, held a fundamental belief in the need to challenge existing dogmas in medicine to enhance our scientific understanding of disease and advance patient care. Perhaps in no area of hypertension does this ring truer with than primary aldosteronism (PA). Following its initial description in the mid-1950s with an initial surge in diagnoses soon thereafter, PA was ultimately relegated to be considered a "zebra" of hypertension felt to be responsible for only a small percentage of cases. In turn, diagnostic and treatment pathways went largely unc

Pathology of adrenal tumours: recent advances.

Changes in the nomenclature and classification of adrenal gland diseases are the result of advances in understanding the pathogenesis, germline susceptibility and the clonal-neoplastic nature of diseases of the adrenal gland. Although numerous classification systems have been proposed, the Weiss system remains the standard for distinguishing benign from malignant adult adrenal cortical tumours, but the Helsinki system and the reticulin algorithm are proving to be increasingly useful in difficult cases. Subtypes of adrenal cortical neoplasms, such as myxoid and oncocytic, as well as those occur

Approach to the Patient with Cushing's Syndrome: Use of Anticoagulation Therapy.

There is an increased awareness on the higher hypecoagulability risks in patients with Cushing's syndrome (CS), but management remains controversial. Here, we present 4 illustrative cases of CS that exemplify some "grey areas" on venous thromboembolism (VTE) prevention-when to start, how long to continue, what to use, and when to stop. The cases span the initiation of prophylaxis at diagnosis of active CS, periprocedural management around inferior petrosal sinus sampling, perioperative prophylaxis after transsphenoidal surgery (TSS), and discontinuation decisions in medically controlled diseas

Impact of Tumor Arterial Embolization on Hemodynamics During Surgical Resection of Pheochromocytoma and Paraganglioma in Children.

BACKGROUND: Pheochromocytoma and paraganglioma in children present significant intraoperative hemodynamic challenges during surgical resection due to catecholamine release. Preoperative tumor arterial embolization is used to reduce bleeding and stabilize hemodynamics, but its efficacy in pediatric patients remains unclear. METHODS: A retrospective cohort study included 31 children (aged 3-18 years) undergoing resection of pheochromocytoma or paraganglioma at Beijing Children's Hospital from January 2019 to June 2025. Patients were divided into embolization (n = 12) and control (n = 19) groups

Comprehensive analysis of germline and somatic SDHA alterations reveals rare incidental germline variants and prognostic implications of somatic loss in breast cancer.

Multigene panel testing (MGPT) has increased the detection of SDHA pathogenic/likely pathogenic (P/LP) variants in cancer patients, but their clinical interpretation remains challenging due to low penetrance and lack of gene-specific variant interpretation guidelines. This study aimed to evaluate the prevalence and clinical relevance of germline and somatic SDHA P/LP variants in cancer patients. A total of 1,699 consecutively referred cancer patients underwent MGPT between 2021 and 2023. Population controls were obtained from the gnomAD database. When available, tumour samples were analysed fo

Pro-tumoural effects of vitamin C supplementation in a zebrafish model for SDHB-associated PPGL.

Phaeochromocytomas and paragangliomas (PPGLs) are rare neuroendocrine, chromaffin cell-derived tumours of the adrenal medulla or paraganglia. Germline pathogenic variants in succinate dehydrogenase subunit B (SDHB) are most prevalent and associated with malignancy and poor prognosis. Treatment options are limited, and therapy development is hindered by knowledge gaps concerning the pathomechanism and lack of suitable models. Previously, homozygous sdhb mutant zebrafish larvae showed disease characteristics, and adult heterozygous mutants represented human heterozygous SDHB carriers, portraying

PBK as a Potential Biomarker and Therapeutic Target for Metastatic Pheochromocytomas and Paragangliomas.

BACKGROUND: Metastasis is the major determinant of prognosis in pheochromocytomas and paragangliomas (PPGLs), yet reliable biomarkers remain lacking. PDZ-binding kinase (PBK), a serine-threonine kinase of the MAPK kinase family, regulates mitosis and oncogenic signaling and has been implicated in tumorigenesis and progression in multiple malignancies. However, its role in PPGLs has not been explored. METHODS: Bioinformatic analyses of public transcriptomes identified a novel metastasis-related molecule, PBK, and investigated its correlation with clinical features and prognosis, with findings

Preserved Adrenal Function After Left Renal Cell Carcinoma With Tumor Thrombus Resection and Right Adrenal Vein Ligation: A Case Report.

BACKGROUND/AIM: Adrenal insufficiency is a recognized complication following adrenalectomy and, less commonly, nephrectomy due to the anatomical and functional relationship between the adrenal glands and kidneys. While unilateral adrenalectomy is typically well tolerated due to compensation by the contralateral adrenal gland, adrenal insufficiency may still occur, particularly in cases involving bilateral disease. Recognizing and managing this risk is critical in postoperative care to prevent life-threatening adrenal crises. CASE REPORT: We present a case of left renal cell carcinoma (RCC) wi

Update and Practical Recommendations for the Use of Medical Treatment of Cushing Syndrome.

Medical treatment of hypercortisolism may be necessary for a high proportion of patients with Cushing syndrome (CS), including those who are not candidates for curative surgery. It may also be used in the presurgical period when hypercortisolism is severe, as long-term treatment following surgical failure or recurrence after surgery, or while waiting for the effects of pituitary radiation in Cushing disease. Currently available medical treatments include adrenal steroidogenesis inhibitors that block cortisol secretion (ketoconazole, levoketoconazole, metyrapone, osilodrostat, mitotane, and eto

Neurological manifestations of Allgrove syndrome in patients carrying a potentially founder p.Ser263Pro variant in the AAAS gene.

Allgrove syndrome (AS) is a rare, multisystem, autosomal recessive disorder characterized by the triad of symptoms: achalasia, alacrimia and ACTH-resistant adrenal insufficiency. Various and nonspecific neurological symptoms can also develop over time, "blurring" the typical course of this underdiagnosed condition. The incidence of Allgrove syndrome is unknown. Orphanet database reports fewer than 100 published cases, but according to the literature review, at least 206 patients have already been described. The pathogenic variant p.Ser263Pro is one of the most recurrent aberrations affecting t
