Anti-N-Methyl-D-Aspartate Receptor Encephalitis With Autonomic Instability and Focal Seizures Without Detectable Ovarian Teratoma.

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis is a severe but potentially reversible form of autoimmune encephalitis, and it typically affects young women. It can be challenging to differentiate from other conditions that cause symptoms such as seizures and psychiatric and autonomic manifestations. We present the case of a previously healthy young woman without any significant past medical history who developed acute-onset, progressive neuropsychiatric disturbance, focal impaired awareness seizures, cognitive impairment, and intermittent autonomic disturbance manifesting as sinu

Particle-size-associated differences in follicular accessibility, granulosa cell oxidative injury, and oocyte developmental competence following polystyrene exposure.

Polystyrene particles are emerging environmental contaminants with potential reproductive hazards, but whether particle size influences access to the follicular microenvironment and subsequent oocyte dysfunction remains unclear. Here, fluorescent and non-fluorescent PS particles with nominal diameters of 0.1, 0.5, and 1.0 μm were compared using mouse cumulus-oocyte complexes, ovarian tissue, and KGN granulosa-like cells under equal-mass and theoretically matched particle-number exposure conditions. Smaller PS particles showed greater follicular-associated fluorescence, with particle-associated

NNMT enhances hyperandrogenism in polyendocrine metabolic ovarian syndrome through epigenetic remodelling.

Polyendocrine metabolic ovarian syndrome (PMOS) features intertwined reproductive, endocrine and metabolic defects, yet its initiating mechanisms remain incompletely understood. Here we show that an epigenetic mechanism driven by nicotinamide N-methyltransferase (NNMT) in granulosa cells contributes to disease progression in PMOS. We observe that Nnmt overexpression in mouse granulosa cells reduces histone H3 lysine 27 trimethylation (H3K27me3), leading to increased retinoic acid receptor beta (RARB) expression. RARB, in turn, amplifies ovarian androgen synthesis. The resulting androgen excess

Microglia as candidate links between hormonal signals and neuronal circuits in polycystic ovary syndrome: evidence gaps and a research agenda.

Polycystic ovary syndrome (PCOS) combines reproductive endocrine dysfunction with metabolic, autonomic, and neurobehavioral phenotypes, yet the contribution of brain microglia remains uncertain. We conducted a structured narrative review that separates human PCOS evidence, PCOS-like animal findings, PCOS-related cellular observations, and non-PCOS mechanistic bridges. Only three heterogeneous PCOS-like studies directly assessed microglial outcomes, and none established a conserved microglial state, microglial necessity, a complete hormone-microglia-neuron causal chain, or human relevance. We t

Delayed Ovarian Teratoma Resection in Anti-N-Methyl-D-Aspartate (NMDA) Receptor Encephalitis Despite Immunotherapy: Implications for Early Source Control.

Anti-N-methyl-D-aspartate (NMDA) receptor encephalitis is a severe autoimmune neurologic disorder frequently associated with ovarian teratomas in adult women. Tumor removal is a central component of management and is associated with improved neurologic outcomes. We report a 33-year-old woman with confirmed anti-NMDA receptor encephalitis complicated by refractory seizures, movement disorder, and prolonged ventilator dependence in whom ovarian teratoma resection was delayed after the family initially declined surgical intervention. Prior to surgical intervention, the patient received high-dose

Functional hypothalamic amenorrhea: A call for global awareness and research.

Functional hypothalamic amenorrhea (FHA) is a common yet under-recognized cause of secondary amenorrhea resulting from reversible suppression of the hypothalamic-pituitary-ovarian axis. Triggered by chronic stress, low energy availability, disordered eating, and/or excessive exercise, FHA leads not only to anovulation and hypoestrogenism but also to broader neuroendocrine disruption involving the hypothalamic-pituitary-adrenal and thyroid axes. Although historically associated with extreme trauma and the "female athlete triad," FHA occurs worldwide and remains significantly underdiagnosed acro

Fertility preservation in girls and adolescents with Turner syndrome: lessons for reproductive care in differences of sex development - a mini-review.

Current knowledge regarding fertility preservation (FP) in girls is derived predominantly from pediatric oncology cohorts; however, FP is increasingly being applied to non-oncological conditions associated with impaired fertility, including differences of sex development (DSD). Turner syndrome (TS), characterized by accelerated germ cell depletion beginning during fetal life, represents the DSD condition with the greatest clinical experience in FP. This mini-review summarizes current evidence on ovarian tissue cryopreservation and oocyte cryopreservation in TS. Available data support the techn

Ovarian endometrioid adenofibroma in a perimenopausal woman with intermittent vaginal bleeding: a case report.

Ovarian endometrioid adenofibroma (EA) is an exceptionally rare benign epithelial tumor of the ovary with nonspecific clinical and imaging manifestations, and its diagnosis relies mainly on pathological examination. This case describes a 55-year-old perimenopausal woman with a 1-year history of intermittent vaginal bleeding and a progressively enlarging right adnexal mass. Imaging revealed a solid mass in the right adnexal region with minimal blood flow signal. Serum tumor markers were within normal ranges, and the sex hormone profile showed no evidence of estrogen excess. Serial ultrasonograp

Association of glaucoma diagnosis with sex hormone-related diagnoses and hormone-replacement therapy within a national U.S. database.

OBJECTIVE: To understand the potential influence of gonadocorticoids on glaucoma susceptibility. METHODS: We conducted a retrospective cohort study using TriNetX, a multicentre federated health research network, to create cohorts of patients with sex hormone-related diagnoses, including those that lead to increased or decreased estrogen or testosterone states. These were compared with corresponding propensity score-matched control cohorts. Associations between sex hormone states and glaucoma diagnoses/procedures were assessed using standardized differences to identify imbalance. Glaucoma diag

A TikTok take on polycystic ovarian syndrome: a cross-sectional study.

BACKGROUND: TikTok has rapidly become a dominant social media platform, particularly popular among younger users, with 32% of its users under the age of 20, and up to 60% female. Its ease of content creation and engagement-focused algorithm often lead to the spread of misinformation, potentially harming patients. The main objective of this study was to assess the content, reliability, and overall reach of TikTok videos made by medical professional creators vs. other creators about polyendocrine metabolic ovarian syndrome (PMOS), previous known as polycystic ovarian syndrome (PCOS). METHODS: A

SIRT3-Dependent Actions of Berberine: From Molecular Therapies to Toxicity.

Berberine, an alkaloid with broad bioactive properties, exerts its pleiotropic effects by modulating SIRT3, a mitochondrial NAD+-dependent deacetylase. This review examines the dual role of SIRT3 as a target of berberine in the management of various disorders while also addressing its associated toxicological profiles. The antihyperglycemic effects of berberine are mediated through SIRT3 inhibition, which promotes hyperacetylation of complex I subunits, degradation of MPC1, an increased AMP/ATP ratio, AMPK activation, and suppression of PEPCK1, collectively resulting in the inhibition of gluco

Preconception incretin therapy reduces pregnancy and cardiometabolic outcomes in polycystic ovary syndrome.

BACKGROUND: Polycystic ovary syndrome (PCOS), now polyendocrine metabolic ovarian syndrome (PMOS), is associated with reproductive and cardiometabolic complications. Glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic peptide (GIP) receptor agonists (RAs) improve obesity-related outcomes, but their impact on pregnancy outcomes in PMOS remains unclear. METHODS: We conducted a target trial emulation using the TriNetX global federated health research network. Women ≥18 years with PCOS/PMOS receiving pre-conception GLP-1/dual GLP-1/GIP RA therapy (liraglutide, semaglutide or tirz

Anti-Müllerian hormone in relation to adiposity and metabolic indices in polyendocrine metabolic ovarian syndrome.

INTRODUCTION: Anti-Müllerian hormone (AMH) is widely used as a marker of ovarian reserve and is also incorporated into the diagnostic assessment of polyendocrine metabolic ovarian syndrome (PMOS). Although PMOS is strongly associated with metabolic dysfunction and abnormal adipose tissue distribution, the relationship between AMH concentrations and adiposity, and especially visceral adiposity remains unclear. This study aimed to evaluate the association between serum AMH levels and selected adiposity and metabolic markers in women with PMOS. METHODS: This cross-sectional study included 1147 w

Neuroendocrine regulation of adipose partitioning in polyendocrine metabolic ovarian syndrome: the gut-brain-adipose axis.

PURPOSE: Polyendocrine Metabolic Ovarian Syndrome (PMOS), formerly polycystic ovary syndrome (PCOS), is a common, complex, and heterogeneous endocrine disorder characterized by reproductive and metabolic dysregulation that significantly increases long-term cardiometabolic risk. Growing evidence indicates that dysregulated adipose tissue partitioning, resulting in preferential visceral and ectopic fat accumulation, is a major determinant of metabolic dysfunction in PMOS. The presence of metabolic abnormalities in lean women with PMOS further suggests that mechanisms beyond obesity and total fat

Molecular stratification of ovarian insufficiency via novel MicroRNA signatures and target gene suppression.

INTRODUCTION: Diminished ovarian reserve (DOR) and premature ovarian failure (POF) are ovarian dysfunctions before age 40, with miRNAs implicated in their pathophysiology. This study evaluated IGF1R, POU2F, and potential regulators miR-507, miR-761, and miR-4770 across healthy, DOR, and POF groups. METHODS: Follicular fluid (FF), granulosa cells (GCs), and blood serum (BS) were obtained from 25 healthy controls undergoing ART for male-factor infertility, 25 DOR, and 15 POF patients. IGF1R/POU2F expression in GCs and miRNA levels in all samples were assessed by qRT-PCR. RESULTS: All miRs were

Insulin signaling in PCOS/PMOS: molecular insights and clinical implications.

Insulin signaling in polycystic ovary syndrome (PCOS) or Polyendocrine Metabolic Ovarian Syndrome (PMOS) involves selective pathway alterations that link metabolic disease to reproductive pathology. In metabolic pathways, impaired PI3K/Akt signaling limits insulin-dependent GLUT4-mediated glucose intake, while ovarian steroidogenic responses to insulin are preserved. Compensatory pancreatic β-cell insulin secretion and reduced hepatic insulin clearance lead to hyperinsulinemia, suppressing sex hormone-binding globulin, increasing free androgen bioavailability, and amplifying ovarian androgen p

Modulating the Gut-Ovarian Axis: Mechanistic Rationale and the Limits of Current Evidence for Probiotics in Female Reproductive Aging.

Female reproductive aging is characterized by progressive follicular depletion, deterioration of oocyte quality, and endocrine decline, with oxidative stress, mitochondrial dysfunction, immune-inflammatory remodeling, and granulosa-cell loss contributing to ovarian functional deterioration. This critical narrative review, supported by a structured literature search, evaluates mechanistic, preclinical, and human evidence for probiotic and microbiota-targeted modulation of the gut-ovarian axis. The current evidence base remains predominantly preclinical. In premature ovarian insufficiency (POI),

Trypanosoma brucei infection remodels the uterine immune environment and drives neuroendocrine dysfunction.

Human African Trypanosomiasis (HAT) or sleeping sickness is a systemic parasitic infection caused by the protozoan parasite Trypanosoma brucei. HAT is associated with substantial immunological, metabolic, and neurological pathology. Although reproductive dysfunction has previously been recognised in both human and experimental T. brucei infection, whether parasites can directly infiltrate the female reproductive tract (FRT), and how infection may reshape the FRT immune landscape remains poorly understood. Using a murine model of T. brucei infection we reveal that parasites are localised in the

Comparative Efficacy and Safety of Liraglutide Versus Metformin in Women With Polyendocrine Metabolic Ovarian Syndrome (PMOS): A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

Polyendocrine metabolic ovarian syndrome (PMOS), previously known by the name 'polycystic ovary syndrome (PCOS),' is a common and complex endocrine-metabolic disorder affecting approximately one in eight women. PMOS is increasingly regarded as a multisystem disorder rather than only a gynaecological condition. Exclusive of weight loss, metformin has been the mainstay pharmacological therapy in the treatment of PMOS; a new contender for the management of this condition is liraglutide. Liraglutide is a glucagon-like peptide-1 receptor agonist (GLP-1 RAs) and has gained increasing interest becaus

The Gut-Insulin Axis in Polycystic Ovary Syndrome: A Narrative Review of Gut Dysbiosis, Inflammation, and Emerging Therapeutic Strategies.

Polycystic ovary syndrome (PCOS) is a complicated, endocrine-endemic disorder with a prevalence rate of 8-13% in reproductive-age women in the world, which manifests through insulin resistance, hyperandrogenism, and reproductive dysfunction. Despite its high global prevalence, existing therapeutic interventions focus predominantly on symptom management rather than targeting the heterogeneous and multifactorial pathophysiology of the syndrome. Intestinal barrier integrity is destroyed by dysbiosis of the gut microbiota and induces endotoxemia and chronic low-grade inflammation through signallin
