Effects of polycystic ovary syndrome on liver, heart, muscle, and pancreatic-related diseases.

BACKGROUND: PCOS is far more than an ovarian disorder; it is a systemic metabolic crisis affecting 11-13% of women. This review maps its extra-ovarian reach into the liver, heart, muscle, and pancreas. METHODS: We analyzed how the "pathophysiological quartet"-insulin resistance, hyperandrogenism, inflammation, and oxidative stress-coordinates systemic damage. RESULTS: PCOS prevalence skyrockets to 28.3% in women with obesity. Key findings include a 51.61% prevalence of MASLD in obese patients and a 2-4 fold increase in cardiovascular and type 2 diabetes risk. These manifestations are rooted

Prevalence of polycystic ovary syndrome among adolescents depending on the adopted diagnostic criteria.

INTRODUCTION: PCOS is a common endocrine disorder that impacts hormonal, metabolic, and reproductive health. Its prevalence is steadily increasing, including in the pediatric population, yet there is still a lack of research thoroughly examining the problem of teenage PCOS. AIM: The purpose of the study was to determine the prevalence and clinical-hormonal profile of PCOS among teenage patients with hyperandrogenism and/or menstrual disorders, depending on the adopted diagnostic criteria. METHODS: An observational study was conducted on a group of 289 patients referred for diagnosis due to h

Androgenetic alopecia in polycystic ovary syndrome: a cutaneous marker of systemic metabo-inflammatory and endocrine dysfunction.

Polycystic ovary syndrome (PCOS) is a multifaceted endocrine-metabolic disorder in which androgenetic alopecia (AGA) serves as a prominent clinical marker of profound systemic dysregulation, extending beyond simple hyperandrogenism. This condition imposes a significant psychosocial burden on affected women, yet its complete pathophysiology remains incompletely understood, leading to therapeutic plateaus. This review advances the central hypothesis that alopecia in PCOS results from a synergistic failure of local follicular metabolic signaling and genetic predisposition. This framework posits t

Challenges and knowledge gaps in sex differences in cardio-kidney-metabolic syndrome across the lifespan.

Cardiovascular-kidney-metabolic (CKM) syndrome represents a continuum of interrelated adiposity, insulin resistance, cardiovascular disease, kidney dysfunction, and metabolic disturbances that evolve across the lifespan. Emerging evidence demonstrates that both biological sex and sociocultural gender significantly shape CKM risk, progression, and clinical expression. CKM syndrome pathogenesis reflects complex multisystem interactions involving adipose tissue dysfunction, neurohormonal activation, inflammatory signaling, and vascular impairment, all of which exhibit important sex-specific patte

Polyendocrine metabolic ovarian syndrome, the new name for polycystic ovary syndrome: a multistep global consensus process.

Polyendocrine metabolic ovarian syndrome (PMOS), previously named polycystic ovary syndrome (PCOS), affects one in eight women. However, the term PCOS is inaccurate, implying pathological ovarian cysts, obscuring diverse endocrine and metabolic features, and contributing to delayed diagnosis, fragmented care, and stigma, while curtailing research and policy framing. Building on an international mandate for change, we outline an unprecedented, rigorous, multistep global consensus process for the name change. Funding and governance were established with engagement of 56 leading academic, clinica

Hypertension and dyslipidemia in women with PCOS: a population-based multiregister study in Sweden.

STUDY QUESTION: What is the impact of PCOS and the hyperandrogenic (HA) PCOS phenotype on the risk of developing hypertension and dyslipidemia? SUMMARY ANSWER: PCOS is an independent risk factor for the development of hypertension and dyslipidemia, with the risk being higher among women with the HA PCOS phenotype. WHAT IS KNOWN ALREADY: PCOS is an established risk factor for insulin resistance and the metabolic syndrome. However, prospective data regarding the risk of hypertension and dyslipidemia in population-based cohorts of women with PCOS are limited. STUDY DESIGN, SIZE, DURATION: This

Redox-endocrine triad in PCOS: can vitamin D, myo-inositol, and melatonin synergize as bioactive cocktails?

Polycystic ovary syndrome (PCOS) is a heterogeneous endocrine-metabolic disorder in which reproductive dysfunction coexists with insulin resistance, chronic low-grade inflammation, and heightened oxidative stress (OS). Increasing evidence indicates that these abnormalities are not independent phenomena but components of a self-perpetuating redox-endocrine network that sustains hyperandrogenism, anovulation, and metabolic impairment. This review critically synthesizes experimental, translational, and clinical data to examine whether vitamin D, myo-inositol, and melatonin, three widely used but

Elevated Adrenal Steroids, Advanced Bone Ages, and Skewed Adult Heights in Trans Male Adolescents: Indications for Comprehensive Evaluation.

PURPOSE: Several studies demonstrate endogenous androgen excess in adult trans male patients, but data are lacking in adolescents. Identifying hyperandrogenism in growing children is relevant, as it can be the first sign of serious adrenal conditions and can be associated with advanced skeletal maturity and premature growth cessation. We sought to identify the percentage of adolescent trans male patients who, prior to gender affirming treatment, had biochemical hyperandrogenism relative to reference ranges and to a comparison group. METHODS: We conducted a retrospective chart review of patien

A rise in prepubertal DHEA in the spiny mouse reveals the effect of adrenarche on the reproductive axis.

BACKGROUND: Adrenarche, characterized by rising adrenal dehydroepiandrosterone (DHEA) levels, precedes puberty and has been implicated in reproductive maturation, and disorders like polycystic ovary syndrome (PCOS). However, the mechanisms by which DHEA influences the hypothalamic-pituitary-gonadal (HPG) axis remain poorly understood, largely due to the absence of an appropriate animal model. The spiny mouse (genus Acomys), which has a prolonged juvenile phase and endogenous prepubertal DHEA production, provides a valuable system to investigate the effects of early-life exposure to DHEA. Excess female adrenal androgen secretion is a consequence of in utero androgenic excess in sheep.

ABSTRACT: Excess androgenic steroid exposure during development in female sheep alters metabolic and reproductive function in postnatal life, with phenotypes reminiscent of health-relevant aspects of Polyendocrine Metabolic Ovarian Syndrome (PMOS) in women. We hypothesised that altered steroid exposure during in utero life would alter female postnatal adrenal function. Prenatal steroid excess was created by fetal steroid injections, on d62 and d82 of gestation, designed to separately create excess androgenic exposure (testosterone propionate, n = 14), oestrogenic exposure (diethylstilbestrol,

Approach to the patient: investigation of postmenopausal androgen excess.

Androgen excess in postmenopausal women presents a challenging clinical conundrum. A crucial component in management involves identification of the underlying etiology with a focus on detection of potentially malignant pathology or underlying genetic syndromes. A basic understanding of androgen physiology in women and the associated alterations during the menopausal transition is required to accurately risk stratify this cohort and streamline investigations. Over the course of this article, we will propose an approach to investigation and management of postmenopausal androgen excess that focus

Case Report: Ovarian dysgerminoma mimicking a sex cord-stromal tumor with hyperandrogenism: diagnostic challenges and molecular insights.

Ovarian dysgerminoma, a malignant germ cell tumor, rarely presents with endocrine manifestations. This report describes a rare case of an 8-year-old girl with virilization symptoms and markedly elevated serum testosterone, initially suggestive of a steroid-producing sex cord-stromal tumor. Histopathological examination revealed a dysgerminoma with highly unusual architectural patterns, including pseudoglandular, follicular, and tubular structures, along with prominent luteinized stromal cells. Immunohistochemistry was diagnostic, showing germ cell markers (SALL4+, OCT3/4+, CD117+) and β-HCG-po

Where are we in the diagnosis and treatment of hirsutism? A narrative review.

BACKGROUND: Hirsutism is a prevalent androgen-dependent condition that significantly affects women's physical and psychological health. Despite decades of study, major gaps persist in understanding local androgen metabolism, the role of insulin resistance, and individualized treatment responses. This review aims to provide a contemporary update that integrates recent molecular, diagnostic, and therapeutic advances. OBJECTIVE: To critically appraise the current understanding of hirsutism, with an emphasis on: (1) emerging concepts in local androgen action and follicular sensitivity, (2) evolvi

Advances in pharmacological treatment for congenital adrenal hyperplasia.

PURPOSE OF REVIEW: Congenital adrenal hyperplasia (CAH), most commonly caused by 21-hydroxylase deficiency, remains associated with substantial morbidity despite life-saving glucocorticoid replacement. This review is timely because several novel therapies have recently emerged with the potential to improve disease control while reducing glucocorticoid burden. RECENT FINDINGS: Recent advances in CAH management include modified-release hydrocortisone, which better mimics physiological cortisol secretion and may improve androgen control with lower glucocorticoid exposure. Steroid-reducing agents

Analytical validation of a dried blood microsampling method to measure androstenedione, 17α‑hydroxyprogesterone, and 11‑ketotestosterone for congenital adrenal hyperplasia monitoring.

Measurement of androstenedione (4-AD) and 17α-hydroxyprogesterone (17-OHP) in blood is crucial for diagnosing and monitoring endocrine disorders such as congenital adrenal hyperplasia (CAH), which results from enzymatic defects in the steroidogenesis pathway. CAH patients require lifelong hormone replacement therapy to manage adrenal insufficiency and control androgen excess. Microsampling offers a less invasive alternative to frequent venous blood sampling, particularly in paediatric CAH patients, and enables at-home monitoring. Including 11-ketotestosterone (11-KT), an emerging biomarker in

Polyendocrine metabolic ovarian syndrome in pregnancy: pathophysiology and outcomes.

Polyendocrine metabolic ovarian syndrome (PMOS), formerly known as polycystic ovary syndrome, is a chronic condition driven by genetic, epigenetic and lifestyle factors. The resulting endocrine changes, including hyperinsulinaemia and hyperandrogenism, underpin the clinical manifestations of PMOS and are amplified by overweight and obesity. Reproductive features of PMOS, including ovulatory disturbance, endometrial abnormalities, subfertility and an increased rate of pregnancy and birth complications, have long been documented in these patients, but awareness in clinical practice has only incr

Adjunctive berberine improves hormonal, metabolic, and inflammatory profiles in women with polycystic ovary syndrome: a retrospective case-control study.

BACKGROUND: Polycystic ovary syndrome (PCOS) is often associated with androgen excess, insulin resistance, and low-grade inflammation. While progesterone is standard for regulating menstrual cycles, additional therapies are often needed to address metabolic and inflammatory dysfunctions. Berberine has shown insulin-sensitizing and anti-inflammatory actions that may complement standard hormonal therapy. OBJECTIVES: This study was designed to evaluate whether adding oral berberine to luteal-phase progesterone improves anthropometric, hormonal, metabolic, and inflammatory indicators in women wit
