Singular Riemannian foliations, variational problems and principles of symmetric criticalities.

BACKGROUND: Osteoporosis is a major global health concern, increasing the risk of fragility fractures, morbidity, and mortality. Conventional diagnostic tools such as dual-energy X-ray absorptiometry (DXA) have limitations in accurately predicting fracture risk. Recent studies suggest that microRNAs (miRNAs) play a crucial role in bone metabolism and may serve as potential biomarkers for osteoporosis and fracture susceptibility. OBJECTIVE: This study aimed to identify differentially expressed miRNAs in osteoporotic and non-osteoporotic trauma patients with proximal femur fractures and assess

Clinical and Microbiological Characteristics of Meningococcal Eye Infections: Retrospective National Surveillance in England, 2010-2022.

OBJECTIVES: Neisseria meningitidis is primarily associated with severe systemic infections but can infect the eye and periocular tissues. Most meningococcal eye infections have a mild prognosis, but there is a significant increase in the risk of invasive disease. UK public health guidelines recommend chemoprophylaxis for meningococcal eye infection cases and contacts. This study involved a clinical and microbiological analysis of meningococcal eye infections in England over a 13-year period. METHODS: The analysis included all English cases of eye infection with confirmed isolation of N. menin

Patient Preferences for Neuromyelitis Optica Spectrum Disorder (NMOSD) Treatments: Results from a Discrete Choice Experiment Study in the USA.

INTRODUCTION: Benefit-risk profiles and mode/frequency of administration vary among the four US-approved treatments for anti-aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4-Ab+ NMOSD), but no published studies have reported treatment preferences among patients with NMOSD in the USA. Therefore, the objective of this study was to quantify preferences and predict treatment choices among patients with AQP4-Ab+ NMOSD in the USA. METHODS: A cross-sectional, web-based discrete choice experiment survey was administered to patients with self-reported AQP4-Ab+ NMOSD. Responde

Use of the GSK MenACWY-CRM/MenB-4C Pentavalent Meningococcal Vaccine Among Persons Aged ≥10 Years: Recommendations of the Advisory Committee on Immunization Practices - United States, 2025.

Meningococcal disease is a serious bacterial infection caused by Neisseria meningitidis. Serogroups B, C, W, and Y cause the majority of cases of this disease in the United States. These serogroups are targeted by different meningococcal vaccines available in the United States. Two quadrivalent (serogroups A, C, W, and Y) meningococcal conjugate vaccines (MenACWY) (MenACWY-CRM [Menveo, GSK] and MenACWY-TT [MenQuadfi, Sanofi Pasteur]) and two serogroup B meningococcal vaccines (MenB) (MenB-4C [Bexsero, GSK] and MenB-FHbp [Trumenba, Pfizer]) are licensed for use in the United States and recommen

The impact of introducing meningococcal C/ACWY booster vaccination among adolescents in Germany: a dynamic transmission modelling study.

BACKGROUND: In Germany, primary vaccination against invasive meningococcal disease (IMD) serogroup C aims to reduce the highest burden of IMD in infants aged 12–23 month. Due to another IMD-peak in adolescents, we modelled the potential impact of introducing adolescent boosters with conjugate meningococcal C or ACWY (MenC/MenACWY) vaccines. METHODS: We built an age- and serogroup-structured dynamic-transmission model for Germany, which we calibrated to national surveillance data in 2005–2019. We simulated five vaccination scenarios of either continuing with the current MenC primary vaccination

Assessing the potential impact of the 20-valent (PCV20) and an adult 21-valent (aPCV21) pneumococcal conjugate vaccine on invasive pneumococcal disease in England.

BACKGROUND: Several higher-valent pneumococcal conjugate vaccines (PCVs) have been licensed recently, including a 20-valent PCV (PCV20) for children and adults and 21-valent PCV (aPCV21) licensed for adults. We assessed the potential for aPCV21 to further reduce the burden of invasive pneumococcal disease (IPD) in England. METHODS: IPD cases are electronically reported to the UKHSA and pneumococcal isolates are routinely submitted to the UKHSA national reference laboratory for confirmation and serotyping. We used national enhanced surveillance data for all IPD cases confirmed in England (July

Development of a rapid, simple multiplex PCR-dipstick assay for the detection of Neisseria meningitidis serogroups in clinical isolates.

The rapid identification and serogrouping of Neisseria meningitidis are crucial for effectively managing invasive meningococcal disease (IMD) patients and their close contacts, particularly in developing countries with limited laboratory resources. We developed a simple multiplex polymerase chain reaction (PCR)-dipstick DNA chromatography (mPCR-dipstick) assay for detecting six major serogroups (A, B, C, W, Y, and X) prevalent worldwide. The assay performance, sensitivity, and specificity were evaluated using 116 unencapsulated and encapsulated N. meningitidis, 29 other Neisseria spp., and 11

An observational, cohort, multi-centre, open label phase IV extension study comparing IPV immune responses to preschool dTaP-IPV booster vaccines in children whose mothers received or did not receive an IPV-containing pertussis vaccine during pregnancy in England.

A diphtheria-tetanus-acellular pertussis-inactivated polio combination vaccine (dTaP-IPV) was offered as part of the UK antenatal vaccination programme from 2012 to July 2024. Prior research established that infants of mothers who received a dTaP-IPV vaccine in pregnancy have significantly reduced poliovirus-specific neutralising antibodies after their primary immunisation series compared with infants of non-dTaP-IPV vaccinated mothers. We investigated whether sufficient poliovirus-specific neutralising antibody titres are achieved in these children following the pre-school dTaP-IPV booster va

Rapid and recent evolution of fetA phase variability among hyperinvasive MenW:cc11 disease isolates.

Neisseria meningitidis serogroup W (MenW) clonal complex 11 isolates are a major worldwide cause of invasive meningococcal disease. This lineage has exhibited significant plasticity with multiple evolutionary events occurring during international and national spread. Phase variation is a major mechanism for the modulation of surface antigen expression including for FetA, a major immunogenic meningococcal antigen. Alterations in switching rates occur due to changes in the repeat numbers of phase-variable genes. Analysis of 1521 MenW:cc11 carriage and disease isolates detected evidence of ongoin

Emergence of a novel cefotaxime- and ciprofloxacin-resistant strain of serogroup Y Neisseria meningitidis sequence type 23 clonal complex in China.

OBJECTIVES: Neisseria meningitidis is a major pathogen of septicemia and meningitis, with a fatality rate of 10%-15%. Cefotaxime resistance in N. meningitidis remains rare, and has not yet been reported in internationally disseminated serogroup Y (MenY) ST-23 clonal complex (cc23) strains. This study aimed to characterize the molecular epidemiology of a novel cefotaxime- and ciprofloxacin-resistant MenY cc23 clone. METHODS: During the period of 2023-2025, 10 MenY isolates were collected from patient (n=1), close contact (n=8), and carrier (n=1) in Shanxi, China. The minimum inhibitory concent

Beyond the Central Nervous System: Neisseria meningitidis as an Unusual Cause of Urogenital Infection.

Neisseria meningitidis is a well-known cause of invasive diseases such as meningitis and septicemia, but is increasingly recognized as an emerging urogenital pathogen. We report the case of a 36-year-old man who presented to a tertiary hospital in Riyadh, Saudi Arabia, with persistent dysuria and purulent urethral discharge. Despite initial treatment for nonspecific urethritis and subsequent management for chronic prostatitis with levofloxacin and azithromycin, his symptoms persisted.
