Acute bacterial meningitis at Boston City Hospital during 12 selected years, 1935-1972.

Data are presented on the occurrence of and mortality rate from acute bacterial meningitis at Boston City Hospital during 12 years between 1935 and 1972 selected in relation to the introduction of potent antibacterial agents. The most frequent causative organisms were Streptococcus pneumoniae, Neisseria meningitidis, and Haemophilus influenzae, but large proportions were caused by other gram-positive cocci and gram-negative bacilli. The greatest reduction in mortality rate after antibiotics became widely used was in patients with meningococcal and influenzal meningitis who were less than or eq

Report of the first hemispheric meeting on meningococcal disease.

The first Hemispheric meeting on meningococcal meningitis was held in São Paulo and Brasília on 23-28 February 1976. Organized by the Pan American Health Organization in collaboration with the Government of Brazil and other PAHO Member States, the meeting had three principal aims: to review the general subject of cerebrospinal meningitis and the special situation in Brazil; to analyze experience gained in Brazil regarding laboratory diagnosis, treatment, and reduction of mortality among hospitalized patients; and to develop prevention and control strategies based on a review of available scien

[Epidemiologic analysis morbidity with meningococcal infections in the USSR].

The authors present the analysis of the incidence of epidemic cerebrospinal meningitis in the USSR from 1937 to 1974, and of meningococcus infection from 1965 to 1974. A rise of the meningococcus infection incidence from 1969 to 1974 was recorded 28 years after the elevation of 1940-1942 and was 1.5 times below this latter rise. The rise in 1969-1974 was characterized by marked signs peculiar to the infection with the droplet transmission mechanism; among those who contracted the disease prevalence was seen among children aged under 14 years (63-72%). A marked affection of juveniles was noted.

Vaccination against meningococcal group A disease in Finland 1974-75.

Meningococcal group A polysaccharide vaccine was used in children 3 months to 5 years of age in the winter 1974-75 during an epidemic caused by sulphanamide-resistant group A meningococci. In 3 provinces, the vaccine was offered on a double-blind basis, using Haemophilus influenzae type b polysaccaride vaccine as control. Approximately 100 000 children were vaccinated with either one of the vaccines. In the Kymi province, approximately 22 000 children representing 90% of the child population of this age received the meningococcus vacine. Infants below 18 months received a booster dose of vacci

Meningococcal carrier rates in parts of eastern Nigeria.

A survey of the meningococcal carrier rate was conducted in the Nsukka area (eastern Nigeria) during an interepidemic period. It was found that approximately 10% of 1126 individuals studied carried one serogroup or other. The predominant serogroup was B (58.1%), followed by C (24.2%). The highest percentage of isolates was from the age group 11-20 years (12.00%), followed by the age group 6-10 years (10.5%). The proportion of isolations was only 6.0% in the age group 31 years and above. Isolates from urban communities were resistant to sulfadiazine: 71.43% (group A), 63.9% (group B), 80.0% (gr

[Isolation of menincococci in Palermo during a course of seasonable recrudescence].

Three strains of Neisseria meningitidis were isolated from young patients during an outbreak of cerebrospinal fever in Palermo, fourteen from their family contacts. All strains from patients and ten of the strains from contacts fell into group C, two of the others into group B; two were untypable strains. All the strains belonging to group C exhibited sensitivity to sulphonamides and to other antibiotics tested. Strains of group B appeared resistant to sulphonamides and to combined trimethoprim-sulphamethoxazole; they were also slightly resistant to streptomycin and tetracyclines.

[CSF. changes in partially treated bacterial meningitis (author's transl)].

Review of 102 admissions of children (one month to seven years of age) with bacterial meningitis is presented. 46 of the patients (45%) had been treated previously with one or more antibiotics. Previous treatment made culture of CSF negative in a statistically significant (p less than 0.001) number of cases, mainly in meningococcal meningitis. There were no significant differences in the levels of CSF glucose, proteins and WBC's between both groups. A decrease of the percentage PMN/lymphocytes was found in the CSF of the partially treated group. In 7 out of 102 no more than two CSF pathologica

Recent survey of infectious meningitis in adults: review of laboratory findings in bacterial, tuberculous, and aseptic meningitis.

Infectious meningitis in adults was reviewed to establish the frequency of meningitis due to each causative agent and to reexamine the laboratory parameters that help to distinguish aseptic, bacterial, and mycobacterial meningitis. Aseptic meningitis occurred 2.2 times more often than bacterial and mycobacterial meningitis combined. The most common nonviral causative agent was the pneumococcus (23 cases) followed by the tubercle bacillus (11 cases) and the meningococcus (5 cases). Cerebrospinal fluid (CSF) Gram stain was the most useful study to rule in a bacterial cause: 89% of cases of bacte

Pathogenesis of cutaneous lesions in acute meningococcemia in humans: light, immunofluorescent, and electron microscopic studies of skin biopsy specimens.

Biopsy specimens of the skin were taken from 10 patients with acute meningococcemia who exhibited mainly maculopurpuric lesions. The specimens were studied by light, electron, and immunofluorescent microscopy in an attempt to obtain information on the pathogenesis of vascular injury. Light microscopy disclosed a large number of Neisseria meningitidis organisms, both in the endothelial cells and being phagocytized by neutrophils. Vascular injury was characterized (by means of both light and electron microscopy) by endothelial necrosis, thrombosis, and necrosis of other elements of the vascular

Rifampin.

In 1971, rifampin was approved for treatment of pulmonary tuberculosis and asymptomatic carriers of Neisseria meningitidis. At present, the approved indications remain the same. However, rifampin in conjunction with at least one other antituberculous drug may be of great value in therapy of extrapulmonary tuberculosis and infections due to other susceptible mycobacteria. In addition, results of clinical trials in leprosy have been highly encouraging. Rifampin appears to induce light chain proteinuria in a majority of patients and has been implicated in suppression of both humoral and cell-medi

Bacterial meningitis in Navojo Indians.

An analysis of 219 confirmed cases of bacterial meningitis among Navajo Indians during a 5-year period, July 1, 1968, through June 30, 1973, revealed that 56 percent were caused by Haemophilus influenzae, 26 percent by Neisseria meningitidis, 6 percent by Mycobacterium tuberculosis, and 6 percent by other organisms. The annual incidence of H. influenzae meningitis (17.7 per 100,000 persons) and that of pneumococcal meningitis (8.0 per 100,000) were much higher than the rates for these diseases reported from other population groups. The annual incidence of meningococcal meningitis (2.0 per 100,

[Immunochemical study of the meningococcal cell wall].

Meningococcus strains--standard and those isolated from the cerebrospinal fluid of patients suffering from the cerebrospinal meningitis were grown on fluid and solid protein-free nutrient media. Electron microscopic study showed that in the strains isolated from patients the wall was much thicker than in the standard strains. The cell wall preparations were obtained by its solution at pH 9.0 and separation from the microbial residue. In the cultures grown on the solid medium the cell was dissolved already in its washing at pH 7.2. It was revealed in gel filtration that the cell wall preparatio

Intracranial infections: investigation by computerized axial tomography.

The value of computerized axial tomography (CAT) in the management of intracranial infections is described and the findings compared with those of other conventional neuroradiological investigations. The most important contribution is in the diagnosis and localization of cerebral abscess. CAT provides not only information of the nature of the lesion but also the number of loculi and the presence of multiple lesions; it is the investigation of choice for follow-up of patients with cerebral abscess treated either by operation or conservatively. CAT is invaluable in the investigation of other inf

Acute bacterial meningitis: where do children die?

The case-fatality rate for acute bacterial meningitis in childhood is still disappointingly high. A review of 687 cases of acute bacterial meningitis including 72 deaths, identified in a defined population of children under ten years of age, was done to determine where patients were treated and where they died. Ten per cent of all hospital admissions were to infectious-disease hospitals, 16 per cent to teaching hospitals, and 74 per cent to general, non-teaching ("regional-board") hospitals. The case-fatality rates associated with these hospitals were three per cent, three per cent and 11 per

[Therapy of meningitis (author's transl)].

For the specific and unspecific treatment of purulent meningitis, penicillin, ampicillin and chloramphenicol are usually sufficient. Only resistant pathogens (Klebsiella, Pseudomonas among others) and meningitis in infants require other treatment. Cephalosporins and aminoglycosides only come into consideration for the treatment of very rare exceptional cases. The still relatively poor prognosis of purulent meningitis is largely independent of the efficacy of the treatment. Delay in beginning therapy, lack of intensive care, primary diseases and complications are principally responsible for thi

[Clinical study of septicaemia (author's transl)].

The internationally accepted term septicaemia is used to describe illnesses in which pathogenic microorganisms are present in the blood. Septicaemia should be defined according to the causative organism, the portal of entry and the underlying disease. In the last 16 years the causative organisms in 788 cases of septicaemia in our hospital were found to be gram-positive cocci in 28.1% gram-negative rods in 37.6%, and other organisms in 24.i%. Infections with Psuedomonas in particular have become more frequent in recent years. The range of causative organisms in septicaemia varies considerably w

[Role of viral-bacterial associations in meningitis in children].

A total of 50 section cases of meningitis in children were investigated. All patients revealed acute viral respiratory infections with generalization, including lesions of the central nervous system (CNS). In 14 children moreover meningococcal infection was diagnosed. In 30 children lesions of the CNS were due to mixed bacterial microflora. In 6 children along with acute viral respiratory infections (AVRI) mycoplasmosis was also revealed. Etiology of the process was determined on the basis of characteristic structural changes in the CNS and other organs, findings of virological and bacteriolog

[Healthy meningococcal carriers in 1973 in Greece. Sensitivity of the isolated strains to minocycline, rifampicin and sulphadiazine (author's transl)].

The frequency of healthy carriers of meningococci in Greece in 1973 has been studied by examining 1105 nasopharyngeal swabs from 731 recruits, during the four recruitment periods of this year. The frequency of healthy carriers among inductees within 24 hours from the arrival in the Camp was 38.9%. After a stay of 35 to 40 days in the training Camp the frequency of healthy carriers rose to 66.4%. Among all the soldiers examined, 24.5% were carriers of meningococci of group B, 13.2% of non-typable strains, 8.1% of autoagglutinable strains, 4.1% of meningococci of group A, 3.7% of meningococci of
