First use of Trumenba (MenB-fHbp) vaccine to control a nursery outbreak of serogroup B invasive meningococcal disease involving children previously immunised with Bexsero (4CMenB), England, November 2023.

In November 2023, the UK Health Security Agency was notified of PCR-confirmed group B (MenB) invasive meningococcal disease (IMD) in a 3-year-old child (Case A), followed by probable IMD in a 2-year-old (Case B, culture and PCR tests negative) attending the same nursery. An incident management team (IMT) was convened. Both children were fully vaccinated with the MenB vaccine 4CMenB (Bexsero, GSK Biologicals). All 39 children attending the nursery and nine staff received ciprofloxacin chemoprophylaxis preceded by pharyngeal swabbing. Pharyngeal swabbing yielded two MenB isolates matching Case A

[Molecular Detection of Neisseria meningitidis Serogroup C in an Adolescent with Atypical Presentation: A Silent Threat].

Neisseria meningitidis is an important pathogen that can cause invasive meningococcal diseases, especially with serogroups A, B, C, W and Y, which can be seen endemic or sporadic. Early diagnosis and treatment of invasive meningococcal disease is life-saving. Delay in diagnosis of meningitis infection, especially in the adolescent age group, which initially presents with atypical symptoms, can further increase the risk of morbidity and mortality. In this case report the clinical evaluation and diagnostic process of a 15-year-old female patient who admitted to the emergency department with abdo

Live-attenuated chikungunya vaccine in children: a randomized phase 2 trial.

Currently, no licensed vaccine is available against chikungunya virus (CHIKV) in children aged less than 12 years. In this phase 2, observer-blind, randomized, dose-response trial we evaluated the tolerability (solicited adverse events (AEs)), safety (unsolicited AEs) and immunogenicity of a live-attenuated CHIKV vaccine (VLA1553) in healthy children aged 1-11 years in endemic countries. Here we provide a prespecified interim analysis to 28 days after vaccination. Participants (n = 304) received either a half dose (n = 119) or a full dose (n = 124) of VLA1553, or active control meningococcal v

Investigation of invasive Neisseria meningitidis serogroup Y ST1466 case increases in New York State.

Bacterial meningitis and septicemia caused by Neisseria meningitidis is a serious infection that requires immediate medical attention and prompt treatment. In the United States, cases of N. meningitidis serogroup Y increased sharply in 2023, leading to a CDC health advisory issued in March 2024, alerting public health agencies and healthcare providers of this surge. N. meningitidis serogroup Y is of particular concern because these strains demonstrate higher levels of resistance to the antimicrobials ciprofloxacin and penicillin. At the Wadsworth Center Bacteriology Laboratory (WCBL), all N. m

Development of a rapid, simple multiplex PCR-dipstick assay for the detection of Neisseria meningitidis serogroups in clinical isolates.

The rapid identification and serogrouping of Neisseria meningitidis are crucial for effectively managing invasive meningococcal disease (IMD) patients and their close contacts, particularly in developing countries with limited laboratory resources. We developed a simple multiplex polymerase chain reaction (PCR)-dipstick DNA chromatography (mPCR-dipstick) assay for detecting six major serogroups (A, B, C, W, Y, and X) prevalent worldwide. The assay performance, sensitivity, and specificity were evaluated using 116 unencapsulated and encapsulated N. meningitidis, 29 other Neisseria spp., and 11

Meningococcal Serogroup Y Meningitis Reveals Inborn Factor B Deficiency.

Complement factor B (FB) deficiency is an extremely rare alternative pathway (AP) defect predisposing to invasive infections, with only three families reported to date. We investigated a 14-year-old girl who developed Neisseria meningitidis serogroup Y meningitis complicated by bacteremia and cerebral venous sinus thrombosis. Complement function was assessed by TCA and AP50 hemolytic assay, FB quantification, and functional reconstitution with purified proteins. Genetic analysis of CFB was performed by next-generation sequencing. TCA, C3, and C4 were normal, whereas AP50 was undetectable. Plas

Clinical impact and cost-effectiveness of vaccinating infants and adolescents against invasive meningococcal B disease in the Netherlands.

BACKGROUND: Invasive meningococcal B disease (IMD-B) causes morbidity and mortality among infants and adolescents in the Netherlands. While multiple vaccines against IMD-B are licensed by the European Medicines Agency, none of them is currently part of the Dutch National Immunization Program (NIP). We evaluated the clinical impact and cost-effectiveness of different IMD-B vaccination strategies for Dutch infants and adolescents. METHODS: We developed a static, single-cohort Markov model to estimate the lifetime number of IMD-B cases and deaths prevented, as well as the incremental cost-effect

An observational, cohort, multi-centre, open label phase IV extension study comparing IPV immune responses to preschool dTaP-IPV booster vaccines in children whose mothers received or did not receive an IPV-containing pertussis vaccine during pregnancy in England.

A diphtheria-tetanus-acellular pertussis-inactivated polio combination vaccine (dTaP-IPV) was offered as part of the UK antenatal vaccination programme from 2012 to July 2024. Prior research established that infants of mothers who received a dTaP-IPV vaccine in pregnancy have significantly reduced poliovirus-specific neutralising antibodies after their primary immunisation series compared with infants of non-dTaP-IPV vaccinated mothers. We investigated whether sufficient poliovirus-specific neutralising antibody titres are achieved in these children following the pre-school dTaP-IPV booster va

Rapid and recent evolution of fetA phase variability among hyperinvasive MenW:cc11 disease isolates.

Neisseria meningitidis serogroup W (MenW) clonal complex 11 isolates are a major worldwide cause of invasive meningococcal disease. This lineage has exhibited significant plasticity with multiple evolutionary events occurring during international and national spread. Phase variation is a major mechanism for the modulation of surface antigen expression including for FetA, a major immunogenic meningococcal antigen. Alterations in switching rates occur due to changes in the repeat numbers of phase-variable genes. Analysis of 1521 MenW:cc11 carriage and disease isolates detected evidence of ongoin

Evaluation of the impact of different notification methods for unvaccinated individuals in a ACWY meningococcal vaccination campaign.

INTRODUCTION: Invasive meningococcal disease (IMD) in Spain remains rare, yet rising trends in serogroups W and Y prior to 2019 prompted the national recommendation to replace adolescent meningococcal C vaccination with the quadrivalent meningococcal ACWY (MenACWY) vaccine and to implement a catch-up campaign for individuals born between 2001 and 2007. In the Region of Murcia, the COVID-19 pandemic delayed implementation until September 2021. After an initial mass-vaccination phase and a long period without active outreach, coverage remained suboptimal, prompting a multi-phase reminder strateg

Emergence of a novel cefotaxime- and ciprofloxacin-resistant strain of serogroup Y Neisseria meningitidis sequence type 23 clonal complex in China.

OBJECTIVES: Neisseria meningitidis is a major pathogen of septicemia and meningitis, with a fatality rate of 10%-15%. Cefotaxime resistance in N. meningitidis remains rare, and has not yet been reported in internationally disseminated serogroup Y (MenY) ST-23 clonal complex (cc23) strains. This study aimed to characterize the molecular epidemiology of a novel cefotaxime- and ciprofloxacin-resistant MenY cc23 clone. METHODS: During the period of 2023-2025, 10 MenY isolates were collected from patient (n=1), close contact (n=8), and carrier (n=1) in Shanxi, China. The minimum inhibitory concent
