Multivalvular Cardiac Involvement from Giant Hepatic Metastases of an Ileal Neuroendocrine Tumor.

Intestinal neuroendocrine tumors (NETs) are rare, slow-growing neoplasms arising from enterochromaffin cells, capable of secreting vasoactive substances that may cause carcinoid syndrome (CS) and clinically significant valvular heart disease. These tumors are often diagnosed at advanced stages due to nonspecific gastrointestinal symptoms, and distant metastases, particularly to the liver and lymph nodes, are common at presentation. Hormonal dysregulation can lead to chronic diarrhea, flushing, and bronchospasm, while carcinoid heart disease (CHD) contributes substantially to morbidity and mort

Carcinoid Heart Disease: Surgical Timing, Right Ventricular Risk Stratification and Operative Strategy.

Carcinoid heart disease is a progressive right-sided valvulopathy caused by serotonin and other vasoactive mediators released by metastatic neuroendocrine tumours. As oncological therapies have extended survival, cardiac disease has become a leading determinant of mortality. Operative mortality has decreased to 5-6% in contemporary high-volume centres, and long-term survival appears increasingly determined by tumour biology rather than cardiac disease when surgery is appropriately timed. The principal determinant of operative outcome is preoperative right ventricular function; symptom-based re

Multivalvular Carcinoid Heart Disease: The Role of Echocardiography in Diagnosis and Selection for Heterotopic Bicaval Valve Implantation.

Background and Clinical Significance: Carcinoid heart disease (CHD) is an uncommon valvular manifestation of neuroendocrine tumours, usually affecting right-sided cardiac valves. Left-sided involvement is rare and is generally associated with bronchopulmonary carcinoid, right-to-left shunting, or markedly elevated circulating vasoactive substances. Therapeutic decision-making is particularly challenging in advanced disease when severe tricuspid regurgitation occurs in patients at prohibitive surgical risk. Case Presentation: We report the case of a 61-year-old male patient with progressive dys

From Carcinoid Syndrome to Carcinoid Heart Disease: The Cost of Delayed Cardiac Surveillance.

Carcinoid heart disease (CHD) is a serious cardiac manifestation of carcinoid syndrome (CS), characterized by progressive fibrotic degeneration of cardiac valves and right-sided heart failure. We report the case of a 72-year-old man with a history of metastatic small-intestinal neuroendocrine tumor and CS diagnosed four years earlier, who presented with worsening dyspnea and peripheral edema. Transthoracic echocardiography revealed characteristic findings of advanced CHD, including severely thickened, retracted, and immobile tricuspid and pulmonary valve leaflets, resulting in severe tricuspid

Clinical and Echocardiographic Characteristics of Neuroendocrine Tumours and Carcinoid Heart Disease: Insights from a Contemporary Canadian Cohort.

BACKGROUND: Neuroendocrine tumours (NETs) are uncommon cancers that secrete vasoactive hormones. Around half of those with metastatic small bowel NETs develop carcinoid syndrome, and historically, up to 50% develop carcinoid heart disease (CHD). We evaluated clinical characteristics, echocardiographic features, and outcomes of patients with NETs and CHD in a contemporary cohort to identify factors associated with CHD development and mortality. METHODS: We identified patients with NETs who underwent an echocardiogram at a tertiary cancer centre-affiliated laboratory (2013-2023). Patients were

Niacin deficiency in carcinoid syndrome: Definitions, biomarkers, and a pragmatic framework for clinical assessment.

Niacin deficiency has been recognized for decades in patients with carcinoid syndrome, but its prevalence, biological meaning, and optimal assessment remain uncertain because definitions and biomarkers vary across studies. We conducted a scoping review, in accordance with PRISMA-ScR guidance, to map how niacin deficiency has been conceptualized and assessed in carcinoid syndrome, including mechanistic hypotheses, biochemical and functional measures, and overt clinical pellagra. PubMed was searched from inception to February 2026, with additional citation chasing. Twenty-four sources were inclu

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Liposomal amphotericin B (AmBisome) is a lipid-associated formulation of the broad-spectrum polyene antifungal agent amphotericin B. It is active against clinically relevant yeasts and moulds, including Candida spp., Aspergillus spp. and filamentous moulds such as Zygomycetes, and is approved for the treatment of invasive fungal infections in many countries worldwide. It was developed to improve the tolerability profile of amphotericin B deoxycholate, which was for many decades considered the gold standard of antifungal treatment, despite being associated with infusion-related events and nephr

Urine 5-HIAA and quality of life in neuroendocrine neoplasia. Is there an association?

BACKGROUND: To determine whether urine 5-HIAA (u5HIAA) concentrations correlate with health-related quality of life (HRQL) in patients with neuroendocrine neoplasms (NEN). METHODS: A retrospective single-centre cross-sectional study was conducted including 386 patients with histological or radiological diagnosis of NEN. Patients completed standardized questionnaires at clinic appointments. Spot u5HIAA concentrations were correlated with Patient Health Questionnaire-9 (PHQ-9) depression scores, Generalized Anxiety Disorder-7 (GAD-7) anxiety scores, European Organisation for Research and Treatm

Pregnancy in a patient with metastatic functioning midgut neuroendocrine tumor receiving lanreotide after peptide receptor radionuclide therapy.

Neuroendocrine tumors (NETs) are rare neoplasms that present significant challenges during pregnancy, particularly in the context of metastatic disease or carcinoid syndrome requiring systemic therapy. Data on the safety of somatostatin analogs during pregnancy remain limited and are primarily derived from studies involving patients with acromegaly. We report the maternal and fetal outcomes in a pregnant patient diagnosed with a metastatic NET of the ileocecal valve, complicated by carcinoid syndrome. The patient had previously undergone treatment with 177Lu-DOTATATE, debulking surgery, and la

Ovarian metastasis from neuroendocrine tumor presenting as hot flashes: A case report and literature review.

Neuroendocrine metastases to the ovaries are extremely rare, with approximately 100 cases reported in the English literature. The most frequent primary source is a small bowel neuroendocrine tumor (NET), particularly of ileal origin, and patients often present with widespread metastases and carcinoid syndrome. We report the case of a 46-year-old North African woman who presented with hot flashes, motility diarrhea, and a cutaneous rash of the neckline persisting for 3 months, initially misattributed to perimenopause. Imaging revealed bilateral ovarian masses classified as ORADS 4. Surgical exp

Cryo-EM structures of a pentameric ligand-gated ion channel in liposomes.

Primary malignant tumors of small bowel constitute only about 1-2% of all gastrointestinal neoplasms. Although neuroendocrine tumors (NETs) are relatively rare, they still represent the second most common malignancy of the small bowel (after adenocarcinoma). Clinical manifestations include abdominal pain, bowel obstruction, diarrhea, weight loss and bleeding. The differential diagnosis of obscure gastrointestinal bleeding can sometimes be challenging for endoscopy as well as for radiology methods. We present the case of an 80-year-old man suffering from severe hypochromic anemia. Routine endos

Efficacy of Oxymetazoline 0.1% in Acquired Blepharoptosis: A Systematic Review and Meta-Analysis of Randomized Clinical Trials.

PURPOSE: Abnormal eyelid drooping is a defining feature of blepharoptosis, a disorder that can affect the superior vision field. The standard of care is surgical intervention but may be associated with complications. There is emerging evidence that the direct-acting α-adrenergic agonist ophthalmic solution, oxymetazoline, is effective for blepharoptosis. This systematic review and meta-analysis aimed to evaluate the efficacy and safety of oxymetazoline in the management of blepharoptosis. PATIENTS AND METHODS: A total of five databases were searched. The inclusion was limited only to randomiz

Gastric neuroendocrine tumor presenting with carcinoid syndrome.

Carcinoid tumors are rare neoplasms, most frequently found in the gastrointestinal tract, responsible for the production of neuroendocrine mediators. Carcinoid syndrome is even rarer and consists of a set of symptoms characteristic of the release of these mediators into the systemic circulation. We present an interesting case of a patient with carcinoid syndrome as a late manifestation of a gastric neuroendocrine tumor with, highlighting the importance of knowing how to identify the carcinoid syndrome.

Surgical Management of Neuroendocrine Tumor Liver Metastases.

Patients with neuroendocrine tumor liver metastases (NETLMs) may develop carcinoid syndrome, carcinoid heart disease, or other symptoms from overproduction of hormones. Hepatic resection and cytoreduction is the most direct treatment of NETLMs in eligible patients, and cytoreduction improves symptoms, may reduce the sequelae of carcinoid syndrome, and extends survival. Parenchymal-sparing procedures, such as ablation and enucleation, should be considered during cytoreduction to maximize treatment of multifocal tumors while preserving healthy liver tissue. For patients with large hepatic tumor

[An unusual tricuspid regurgitation].

Tricuspid regurgitation can be due to different causes and mechanisms. Among these, cardiac involvement in carcinoid disease is a rare cause of tricuspid valve disease with a peculiar echocardiographic aspect. We report the case of a 59-year-old woman, with no past medical history, who was recently found to have a heart murmur and signs of right heart failure. The echocardiogram showed a remodeling of valvular and subvalvular apparatus, resulting in massive regurgitation. This finding prompted further diagnostic investigations, leading to the diagnosis of carcinoid syndrome with cardiac involv

Mixed Trabecular and Mucinous Carcinoid Arising in the Background of a Mature Ovarian Cystic Teratoma.

Ovarian carcinoid tumors, originating from mature ovarian cystic teratomas (dermoid cysts), are rare neuroendocrine tumors. The synchronous occurrence of trabecular and mucinous carcinoid tumors within a single ovarian teratoma is infrequent and has limited documentation. We present the case of a 67-year-old postmenopausal woman presenting with abdominal pain, constipation, and urinary retention, but without features of carcinoid syndrome. Ultrasonography identified a complex, multiloculated cystic mass in the right ovary. Laboratory assessments, including CA-125 levels, were unremarkable. The
