Streptococcus pyogenes endometritis- a population-based study on the causative emm-types, case clustering and disease severity.

PURPOSE: Streptococcus pyogenes is a cause of endometritis, a severe infection in women of reproductive age. S. pyogenes is classified into emm-types based on variations of the emm-gene. We aimed to compare the distribution of emm-types in endometritis to that of controls with bacteraemia, and to assess associations between emm-types and disease severity or clustering of cases. METHODS: Vaginal or cervical cultures of S. pyogenes from Departments for gynaecology and obstetrics in Skåne, between 2012 and 2020, were identified. Clinical data on patients were retrieved from medical records. Incl

Early use of methylene blue in the management of refractory group A streptococcal toxic shock syndrome with multi-organ failure in the intensive care unit.

An independent, community-dwelling man in his seventies with a medical history of type 2 diabetes mellitus and atrial fibrillation presented to the emergency department with acute-onset confusion. He rapidly developed refractory shock complicated by multi-organ failure, which required multiple vasopressors for haemodynamic support and renal replacement therapy for kidney failure. The early initiation of methylene blue was associated with his rapid stabilisation and improvement. He was found to have an upper extremity wound with group A Streptococcus, leading to the diagnosis of group A Strepto

Primary Streptococcal Peritonitis in a Postpartum Patient: A Rare but Life-Threatening Condition.

We present the case of a 43-year-old postpartum woman who developed primary streptococcal peritonitis with a fulminant septic course requiring intensive care. Initial symptoms were nonspecific and improved temporarily, but the patient returned the next day with recurrent pain and markedly elevated inflammatory markers. Imaging confirmed peritonitis, and despite empirical antibiotics, the patient deteriorated into septic shock. Laparotomy revealed purulent peritonitis without a clear source, and Streptococcus pyogenes was detected via PCR and intraoperative cultures. The patient responded to es

Clindamycin demonstrates clinical effectiveness for bacteremic M1UKStreptococcus pyogenes pneumonia: A case report and in vitro evidence of notable antitoxin effects on M1UK.

We report a case of bacteremic M1UKStreptococcus pyogenes pneumonia in an immunocompetent patient, which improved with adjunctive clindamycin therapy. In vitro, clindamycin consistently suppressed expression of streptococcal pyrogenic exotoxin A (speA) in M1UK strains but not in the M1global strain, whereas levofloxacin showed no suppressive effect. These findings support a potential antitoxin role of clindamycin in M1UK infections.

Invasive Streptococcus dysgalactiae subsp. equisimilis infections in a super-aged society: a Japanese nationwide observational study (2024-2025).

BACKGROUND: Streptococcus dysgalactiae subsp. equisimilis (SDSE), historically considered less virulent than Streptococcus pyogenes, has emerged as an important invasive pathogen, particularly in aging societies. Data on nationwide clinical and molecular epidemiology in super-aged populations remain limited. We aimed to identify admission abnormalities associated with the 28-day mortality for invasive SDSE (iSDSE) infections in Japanese adults and evaluate the molecular epidemiology of this emerging pathogen through a large-scale surveillance. METHODS: A prospective nationwide surveillance of

Invasive group A Streptococcal infections in Franche-Comté, France (2022-2023).

Following the withdrawal of COVID-19-related barrier measures, a sharp rise in group A streptococcal (GAS) circulation has been reported in several countries. To describe a series of invasive group A streptococcal (iGAS) infections in a French region after the COVID-19 pandemic. We conducted a descriptive retrospective study on confirmed and probable cases of iGAS that occurred between September 2022 and August 2023 in six public hospitals in Franche-Comté (France). A total of 116 patients (99 adults and 17 children) were analysed. The main clinical presentations of iGAS infections were skin a

The novel prophage lysin Lys1459 exhibits broad-spectrum antibacterial activity via triple-binding domain.

UNLABELLED: Phage-encoded lysins represent a promising class of antimicrobial agents against drug-resistant bacteria due to their ability to degrade bacterial cell walls. This study characterizes a novel prophage-derived lysin, Lys1459, from Streptococcus uberis. We expressed and purified the full-length Lys1459 protein, along with its three individual domains (CHAP, A2, and CBD), to systematically evaluate its antibacterial spectrum, biochemical properties, and therapeutic efficacy. The full-length Lys1459 demonstrated potent lytic activity, achieving a 3-log reduction in Streptococcus agalac

Pediatric Invasive Group A Streptococcus Characteristics Before, During, and After COVID-19.

Pediatric invasive Group A Streptococcus (iGAS) infections two years before, during, and a year after the Coronavirus Disease 2019 (COVID-19) pandemic were analyzed in an urban hospital in the Northeastern United States. Invasive GAS was diagnosed in 38 patients. There were as many infected children post-COVID as in the prior 4.5 years combined. The post-pandemic surge was most striking in late spring. The median age was youngest during the post-pandemic year at 2.3 years. During and after the pandemic, about half the children had concurrent virus detection, and two-thirds had no known chronic

Genome-informed metabolomic re-analysis identifies serum-associated amino acid signatures in sepsis-associated bacterial pathogens.

BACKGROUND: Antibiotic-resistant bacterial infections remain a major global health threat, particularly in hospital settings. Standard antimicrobial susceptibility testing provides essential clinical information, but it does not fully capture pathogen metabolic adaptation under host-like conditions. Integrating isolate whole-genome sequencing with metabolomics may help prioritize condition-associated metabolic signatures linked to bacterial physiology and antibiotic response. METHODS: We performed a secondary integrative re-analysis of publicly available isolate whole-genome sequencing and ma

Retropharyngeal and parapharyngeal abscesses in children - a 9.5-year retrospective single-center analysis followed by literature review.

Introduction: Deep neck infections are severe complications of the inflammatory processes in the upper respiratory tract. They can be divided into retropharyngeal, parapharyngeal, peritonsillar, and submandibular abscesses, depending on the affected region. Deep neck infections are considered acute bacterial complications with a progressive course and life-threatening nature, and that is why they require an immediate, broad-spectrum, intravenous antibiotic therapy. Aim: The aim of our retrospective analysis was to characterize the population of children affected by retro- and/or parapharyngeal

Host-response trajectories in necrotizing fasciitis: host-pathogen interactions, immune evasion, and immunoparalysis.

Necrotizing fasciitis, a prototypical form of necrotizing soft-tissue infection, is a rapidly progressive and life-threatening soft-tissue infection characterized by extensive fascial necrosis, systemic toxicity, tissue hypoperfusion, septic shock, and multiorgan dysfunction. Although early recognition, urgent surgical source control, appropriate antimicrobial therapy, toxin suppression when indicated, and organ support remain the foundations of management, the dynamic host-response states that link microbial virulence, local tissue injury, impaired pathogen clearance, perfusion failure, and s

Retroperitoneal infection associated with Streptococcus dysgalactiae subsp. equisimilis bacteremia and lower-extremity cellulitis: a case report.

Streptococcus dysgalactiae subsp. equisimilis (SDSE) is an increasingly recognized β-hemolytic streptococcus causing invasive infections resembling those caused by Streptococcus pyogenes. Although SDSE bacteremia commonly arises from skin and soft tissue infections, retroperitoneal infection has rarely been described. We report SDSE bacteremia with retroperitoneal lymphatic involvement suggesting possible extension from lower-extremity cellulitis. A 70-year-old man with diabetes and alcoholic liver disease presented with fever and difficulty moving and was admitted with septic shock. Initial c

Ecthyma Gangrenosum Caused by Streptococcus pyogenes: A Case Report.

Ecthyma gangrenosum (EG) is an uncommon infectious vasculitis presenting as erythematous macules that develop into characteristic black eschars most commonly in the setting of bacteremia. While EG is most often associated with Pseudomonas aeruginosa infection, we present a case of EG-like skin lesions caused by Streptococcus pyogenes, an atypical causative organism, in an immunocompromised individual. Resolution was achieved following treatment with disease-specific intravenous antibiotics, adjuvant topical therapies, and stabilization of other underlying medical conditions that precipitated t

Invasive group A streptococcal infections requiring paediatric intensive care in children and adolescents: a nationwide retrospective cohort study in Austria.

Several European countries reported a marked rise in paediatric invasive group A streptococcal (iGAS) infections from mid-2022 onwards. We describe the clinical spectrum and intensive care course of children with iGAS admitted to Austrian paediatric intensive care units (PICUs) during this period. We conducted a nationwide, multicentre, retrospective observational cohort study across all 13 Austrian PICUs. Children aged 0-18 years admitted between 1 September 2022 and 31 March 2024 with microbiologically confirmed iGAS were identified using a standardised, pseudonymised case report form. Data

Rapid-Onset Menstrual-Associated Toxic Shock Syndrome in an Adolescent Female: A Case Report and Clinical Review.

BACKGROUND: Toxic shock syndrome (TSS) is a rare, life-threatening, toxin-mediated illness caused by Staphylococcus aureus or Streptococcus pyogenes. Since its initial recognition in the late 1970s, menstrual-associated TSS has been closely linked to tampon use. While regulatory changes to tampon composition have reduced incidence, TSS continues to be an important cause of fulminant sepsis in adolescents. CASE PRESENTATION: We describe a 16-year-old female who presented with five days of suprapubic pain, nausea, diarrhea, and fever who later developed sore throat, vomiting, rash, and mucous m
