Association between toxic heavy metals and noncancerous thyroid disease: a scoping review.

BACKGROUND: Toxic heavy metals such as chromium (Cr), arsenic (As), cadmium (Cd), mercury (Hg), and lead (Pb) are known to be priority pollutants due to their high degrees of toxicity and widespread presence in the environment. This review aimed to explore the association between heavy metals and noncancerous thyroid diseases by synthesizing findings from observational and experimental studies. This review addressed a critical intersection of environmental health, endocrinology, and public health. The findings would be of interest to a wide range of disciplines given the ubiquitous presence of

Fatal systemic bacterial infections in two harbour porpoises (Phocoena phocoena) secondary to bite wounds from grey seals (Halichoerus grypus).

Mycoplasma phocicerebrale, the causative agent of seal ('speck') finger, a zoonotic disease, is a common commensal in the oral cavity of various seal species. Historically associated with seal hunters, it remains a significant risk for those handling or rehabilitating marine mammals. While primarily known for causing severe cellulitis in humans, M. phocicerebrale can also lead to severe infections in seals, including osteomyelitis and sepsis. Recent studies have revealed the predatory behaviour of grey seals (Halichoerus grypus) on harbour porpoises (Phocoena phocoena). Examination of scarring

Serpin B12 in saliva of pigs: Development and analytical validation of an assay for its quantification and changes in sepsis and stress conditions.

An assay for measuring Serpin B12 in porcine saliva was developed and validated, showing adequate precision, accuracy, and sensitivity. Changes in salivary Serpin B12 concentrations were evaluated under different conditions, including the time of the day, the productive cycle, experimental sepsis and non-septic inflammation, S. suis infection and stress. Serpin B12 were significantly higher at 4 p.m. compared to other times of the day and were higher in piglets before the end of the nursery phase. Significant increases were detected at 6 h and 24 h after the intramuscular administration of Esc

Improving neonatal outcome by studying the incidence of maternal group B streptococcus colonization among pregnant women: A prospective observational study.

AIMS AND OBJECTIVES: Group B streptococcus (GBS) colonization during pregnancy can lead to invasive infections in neonates including meningitis or sepsis. The major risk factor for neonatal disease is the rectovaginal colonization of GBS during pregnancy and delivery. The objectives of this study were as follows: 1) to screen antenatal patients at 35-37 weeks of gestation for GBS colonization, 2) to determine the prevalence of GBS among pregnant women, 3) to formulate an antibiotic policy with respect to antepartum antibiotic prophylaxis and empiric choice of antibiotics for early onset neonat

Exploring Heterogeneity of Fecal Microbiome in Long COVID Patients at 3 to 6 Months After Infection.

An estimated 10% of COVID-19 survivors have been reported to suffer from complaints after at least three months. The intestinal microbiome has been shown to impact long COVID through the gut-lung axis and impact the severity. We aimed to investigate the relationship between the gut microbiome and clinical characteristics, exploring microbiome heterogeneity through clustering. Seventy-nine patients with long COVID evaluated at 3 to 6 months after infection were sampled for fecal metagenome analysis. Patients were divided into two distinct hierarchical clusters, based solely on the microbiome co

Recurrent Necrotizing Enterocolitis in Late Preterm and Full-Term Babies: A Case Report and Literature Review.

Necrotizing enterocolitis (NEC) is a serious inflammatory condition of the intestines that primarily affects premature infants, though it can also occur in full-term infants. Diagnosing and managing recurrent NEC in full-term infants can be particularly challenging, with most requiring surgical intervention upon presentation if medical treatment is insufficient. An approach that emphasizes active medical management while minimizing excessive bowel resection tends to result in better postoperative outcomes, highlighting the need for increased awareness within the healthcare community regarding

Unusual Presentation of Pyogenic Liver Abscess in a Healthy Young Pilot: A Case Report.

Pyogenic liver abscess is a very rare, yet serious, condition, usually accompanied by different risk factors. In this article, we present the case of pyogenic liver abscess in an otherwise healthy male patient. What makes our patient's case unique is the fact that he does not have any past medical history and initially had very vague symptoms that were not very consistent with a diagnosis of a liver lesion, hence the delay in further testing and the conclusion of liver abscess diagnosis. Treatment includes draining of the abscesses, as well as a course of antibiotics. It is important to perfor

Effect of probiotics on necrotizing enterocolitis in preterm infants: a network meta-analysis of randomized controlled trials.

BACKGROUND: Previous studies have suggested that probiotics may have potential benefits for preterm infants. Their efficacy seems to depend on the particular species or combinations used. METHODS: To further investigate the effects of probiotics in preventing necrotizing enterocolitis (NEC) and other related outcomes in preterm infants, we conducted a network meta-analysis of 51 randomized controlled trials involving 11,661 participants. RESULTS: Our study revealed that most probiotics can effectively reduce the incidence of NEC (at or beyond Bell's stage II). Lactobacillus (RR, 0.59; 95% CI

Extracorporeal Membrane Oxygenation-Impella (ECPELLA) for Sepsis-Induced Cardiogenic Shock: A Case Report on Vascular Access Selection and Its Use Without Systemic Anticoagulation.

Sepsis-induced cardiogenic shock (SICS) often necessitates mechanical circulatory support, but the efficacy of venoarterial extracorporeal membrane oxygenation (VA-ECMO) alone remains uncertain due to its potential to increase afterload and worsen cardiac dysfunction. The combination of VA-ECMO and Impella (ECPELLA) has emerged as a potential strategy to mitigate these effects, but its application in SICS remains poorly documented. This case highlights ECPELLA as a viable strategy to improve hemodynamics and facilitate weaning in refractory SICS, particularly in patients with vascular access l

A Rare Case of Ventricular Septal Defect and Double-Chambered Right Ventricle With Bilateral Cardiac Infective Endocarditis.

Infective endocarditis (IE) typically affects the left side of the heart. Multiple valve involvement on both sides of the heart is a rare occurrence. However, congenital heart defects, particularly ventricular septal defects (VSD), can predispose individuals to bilateral endocarditis. We present a case of a 41-year-old patient with congenital VSD and double-chambered right ventricle (DCRV) who presented with fever, generalized fatigue, and unintentional weight loss of 20 kg over two months. Transthoracic echocardiography revealed vegetations on the aortic valve, pulmonary valve, and right vent

Group A Streptococcus Bacteraemia Presenting as Acute Limb Ischaemia in a Neonate: A Case Report.

The reported incidence of Group A Streptococcus (GAS) bacteraemia is generally low, though it remains a significant concern. We report the case of a three-week-old neonate who presented with blackish discolouration of her toes and a rash, later diagnosed with invasive GAS septicaemia and septic emboli. She received antibiotics and anticoagulation, showing significant improvement, with mild residual hyperpigmentation and minor tissue loss at the two-month follow-up. Our case report highlights that early recognition and treatment of neonatal sepsis, including rare presentations such as septic em
