Interglobular dentin (IGD) has been used as a biomarker for vitamin D deficiency (VDD) in past populations. However, an evidence-based threshold for the levels of serum vitamin D associated with IGD occurrence has not been established. Here, we explore the association between perinatal serum vitamin D levels, presence of IGD, and biological sex in the deciduous teeth of modern children from Otago, New Zealand. Ground sections were created from the deciduous central or lateral mandibular incisors of 54 five to 6-year-old children (27 M, 27 F) with known perinatal blood vitamin D levels. Vitamin

25-(OH)-vitamin D testing is an example of low-value care whose prevalence has increased dramatically in recent years. In Switzerland, testing rates doubled in less than a decade, resulting in annual costs of over 100 million Swiss francs, without any demonstrated clinical benefit. This article provides an overview of various national and international approaches aimed at reducing low-value care and overtesting. The analysis compares the effectiveness of informative approaches (smarter medicine - Choosing Wisely), decision support approaches (clinical alerts), restrictive approaches (reimburse

BACKGROUND: Vitamin D deficiency and Type 2 diabetes (T2D) are highly prevalent in South Asian populations. Observational studies report inverse associations between serum 25-hydroxyvitamin D [25(OH)D] and T2D risk, but these findings may reflect confounding or reverse causation. We used Mendelian randomization (MR) to assess the association between genetically predicted 25(OH)D and T2D risk in South Asians.
METHODS: We conducted a two-sample MR analysis using publicly available OpenGWAS summary statistics. One variant (rs3755967) met the conventional genome-wide significance threshold (p < 5

BACKGROUND: Cobalamin C (cblC) defect, also referred to as combined methylmalonic acidemia and homocystinuria, is the most common inborn error of intracellular cobalamin metabolism. Late-onset cblC defect often presents with atypical and heterogeneous manifestations, particularly neuropsychiatric symptoms, which may lead to misdiagnosis and delayed treatment.
CASE PRESENTATION: We report a 13-year-old girl with late-onset cblC defect who presented with progressive cognitive impairment, psychiatric and behavioral abnormalities, and motor dysfunction. Because of these atypical manifestations, s

BACKGROUND: Insomnia is a prevalent sleep disorder associated with adverse physical, psychological, and functional outcomes. Emerging evidence suggests that vitamin D deficiency may contribute to sleep disturbances. Women are more likely to have lower serum vitamin D (25-hydroxyvitamin D [25(OH)D]) levels than men, and they also report a higher prevalence of insomnia. Although several studies have proposed that vitamin D supplementation may improve insomnia symptoms, findings remain inconsistent, and evidence from Southeast Asian populations is limited.
OBJECTIVE: This study aims to evaluate

Methylmalonic acid (MMA) is a functional marker of vitamin B12 insufficiency and an emerging metabolic factor associated with skeletal aging and osteoporosis. This review synthesizes epidemiological, experimental, and mechanistic evidence linking the vitamin B12-MMA axis to impaired bone homeostasis. Population studies associate elevated circulating MMA with lower bone mineral density, higher odds of osteoporosis, and lower knee extensor strength, while disturbed vitamin B12 metabolism has also been linked to altered bone turnover. Experimental MMA exposure and models of methylmalonyl-CoA muta

LC-MS/MS enables sensitive quantification of malonic acid (MA), methylmalonic acid (MMA), and ethylmalonic acid (EMA) across clinical matrices. MA provides emerging diagnostic value in metabolic assessments, while MMA remains the primary marker of functional vitamin B12 deficiency. EMA is essential for evaluating mitochondrial and fatty-acid oxidation disorders. Advances in extraction and chromatographic strategies continue to improve analytical performance for these biomarkers.

BACKGROUND: Wernicke encephalopathy (WE) is a potentially reversible neurological emergency that remains frequently missed and undertreated in patients with alcohol use disorder (AUD). Although thiamine is widely accepted as the cornerstone of prevention and treatment, diagnostic uncertainty, limited comparative evidence, and heterogeneous clinical recommendations contribute to substantial variability in real-world management. This scoping review aimed to map the available evidence on the diagnosis and clinical management of WE in patients with AUD or clinically significant alcohol use, with p

Evidence on the association between cardiovascular health (CVH) and frailty remains limited, and the role of serum 25-hydroxyvitamin D (25(OH)D) in this relationship is not fully understood. This study aimed to examine the association between frailty and CVH and to investigate the independent association of serum 25(OH)D and its potential interaction with frailty. This cross-sectional study used data from the National Health and Nutrition Examination Survey 2007 to 2018, including 8015 adults aged ≥20 years. Frailty was assessed using a frailty index, and CVH was evaluated based on the America

BACKGROUND: Vitamin D metabolites beyond total 25(OH)D may influence fetal development, yet their trimester-specific associations with small vulnerable newborns (SVN) remain underexplored. This study examined longitudinal associations of multiple vitamin D metabolic markers with SVN-related outcomes and identified key determinants during pregnancy.
METHODS: In this prospective cohort of 2,505 mother-infant pairs from China, serum levels of five vitamin D metabolites 25(OH)D2, 25(OH)D3, total 25(OH)D, 3-epi-25(OH)D3, and 24,25(OH)2D3] were quantified using HPLC-MS/MS. Two vitamin D metabolite

BACKGROUND: Thiamine deficiency disorders (TDD) remain a preventable yet underrecognized contributor to maternal and infant morbidity, particularly in low- and middle-income countries. During pregnancy, thiamine deficiency may go undetected despite routine antenatal care and can lead to serious neurological and cardiac complications in breastfed infants. However, data on thiamine status in urban populations receiving standard antenatal supplementation are limited. This study aimed to determine the prevalence of thiamine deficiency in third-trimester pregnant women and to identify associated fa

Vitamin D deficiency is increasingly recognized as a common comorbidity in patients with inflammatory bowel disease (IBD), potentially influencing immune regulation and disease outcomes. Despite high rates of vitamin D deficiency reported globally, data from the Middle East remain scarce. This study aimed to evaluate the relationship between vitamin D concentration and the demographics of patients with IBD. We conducted a cross-sectional comparative study utilizing an IBD clinical cohort from a tertiary care center and a separate, previously published population-based control cohort. Serum 25-

X-linked hypophosphatemia (XLH) (OMIM #307800) is a rare, chronic, and progressive genetic musculoskeletal disorder caused by loss-of-function variants in the PHEX gene located at locus Xp22.11. Clinical manifestations of XLH include short stature, dental abnormalities, and lower limb deformities, although the phenotype is variable. Diagnosis is clinical, biochemical, and radiological, but genetic testing is also recommended. This case report describes a 29-year-old female patient with no family history related to her disease, presenting with short stature, dental abnormalities, lower limb def

Although patients with X-linked hypophosphatemia (XLH) usually present a phenotype characterized by rickets in children or osteomalacia in adults, short stature, lower limb deformities, and dental abnormalities, this condition can manifest with a wide phenotypic spectrum. In attenuated cases, diagnosis is often delayed, preventing early treatment. We present the case of a 50-year-old woman who was referred to our clinic by her son's pediatrician, who had been diagnosed with XLH through molecular testing. Manifestations suggesting attenuated XLH (mild valgus curvature of the lower limbs, bilate

The growing aging population has been accompanied by an increasing prevalence of chronic kidney disease (CKD), highlighting the need to better understand modifiable dietary and metabolic factors that contribute to renal injury. Folate deficiency and excessive fructose intake have been implicated in renal inflammation; however, the underlying cellular mechanisms remain incompletely understood. In this study, we investigated the effects of folate deficiency or fructose exposure on inflammatory responses, lactate metabolism, oxidative stress, and mitochondrial function in human renal tubular epit

Background: Hypophosphatasia (HPP) is a rare genetic disorder caused by a loss-of-function mutation in the ALPL gene, which encodes tissue-nonspecific alkaline phosphatase (TNSALP). Reduced TNSALP activity in HPP primarily leads to bone mineralization disorders. Given the complex and challenging nature of issues related to vitamin D and HPP, it is essential to evaluate each patient individually and adjust the appropriate dose of vitamin D according to their specific needs in order to maintain calcium phosphate homeostasis and normal vitamin D levels. Maintaining proper vitamin D homeostasis in

Background/Objectives: In recent years, the role of vitamin D in the etiology of central and peripheral neurological diseases has been widely investigated, and evidence suggests benefits of vitamin D replacement therapy. However, few studies have evaluated the relationship between serum vitamin D levels and patients with spinal muscular atrophy (SMA). Therefore, this study aimed to analyze serum vitamin D levels in patients with SMA and their relationship with disease type, degree of motor impairment, and the presence of musculoskeletal deformities. Methods: This was a cross-sectional analytic
