Clinical microbiology and infection : the official publication of the European Society of Clinical Microbiology and Infectious Diseases2025eng
BACKGROUND: There is debate on whether cefazolin or antistaphylococcal penicillins should be the first-line treatment for methicillin-susceptible Staphylococcus aureus (MSSA) bacteraemia. Ongoing trials are investigating whether cefazolin is non-inferior to (flu)cloxacillin, but it remains uncertain whether these findings apply to other antistaphylococcal penicillins.
OBJECTIVES: We conducted a systematic review and meta-analysis comparing cefazolin with each of the individual antistaphylococcal penicillins for MSSA bacteraemia.
METHODS: Data sources: We updated a 2019 systematic review but

BACKGROUND: The optimal time cut-off for appropriate antibiotic therapy (AAT) in methicillin-resistant Staphylococcus aureus (MRSA) bacteremia remains uncertain. We assessed the effects of time to AAT on 30-day in-hospital mortality in patients with MRSA bacteremia who received glycopeptides as initial therapy, and applied landmark analyses to adjust for immortal-time bias. METHODS: We conducted a retrospective cohort study of adults with MRSA bacteremia who received an initial course of glycopeptide therapy at two university-affiliated hospitals between 2018 and 2023. Multivariable logistic r

BACKGROUND: Burkholderia pseudomallei, Staphylococcus aureus and Escherichia coli are common causes of bacteraemia in the tropical Asia Pacific region. The 28-day mortality rate of B. pseudomallei bacteraemia in Thailand has been reported to be 66%, higher than the 28-day mortality rate of bacteraemia due to S. aureus (43%) and E. coli (19%) in that country. Individuals living in rural and remote locations in these Thai studies had an even poorer prognosis. The mortality rates of individuals with bacteraemia due to B. pseudomallei, S. aureus and E. coli in countries with well-resourced health

STUDY OBJECTIVE: To compare rates of catheter-related bloodstream infections (CR-BSI) in pediatric patients who received parenteral nutrition (PN) with either soybean oil-based intravenous fat emulsion (SO-IFE) or mixed oil-IFE (MO-IFE). We hypothesized that the use of MO-IFE would be independently associated with reduced infection rates compared with SO-IFE.
DESIGN: Retrospective cohort study.
SETTING: Tertiary referral children's hospital and its associated gastrointestinal rehabilitation clinic (01 January, 2015-31 July, 2019).
PATIENTS: Days of IFE exposure were counted for patients age

BACKGROUND: Published guidelines call for prolonged courses of intravenous (iv) antibiotics for the treatment of MRSA bloodstream infection (BSI) to ensure eradication of deep foci and decrease relapse risk. Sequential iv-to-oral antibiotic therapy has been successfully applied to other serious infections but has not been evaluated for MRSA BSI.
OBJECTIVES: To compare outcomes in adults completing MRSA BSI therapy with oral versus parenteral antibiotics in the outpatient setting [oral outpatient antibiotic therapy (OOAT) versus outpatient parenteral antibiotic therapy (OPAT)].
METHODS: This

The epidemiology of coagulase-negative staphylococcal (CNS) bacteremia among adult ICU patients remains unclear. Decontamination studies among ICU patients provide a unique opportunity to study the impacts of different diagnostic criteria, exposure to various decontamination interventions, and various other factors, on its incidence over three decades. Decontamination studies among ICU patients reporting CNS bacteremia incidence data were obtained mostly from recent systematic reviews. The CNS bacteremia incidence within component (control and intervention) groups of decontamination studies wa

In this study, we evaluated the performance of three blood culture systems in a Chinese tertiary-care hospital. Samples of simulated bacteremia were prepared using 10 mL of fresh blood from healthy humans and bacterial suspensions of known cell density. Portions of the specimens were treated with an antibiotic or antifungal drug at specified concentrations to simulate antibacterial drug treatment. We analyzed three blood culture systems: BACTEC Plus, BacT/Alert, and VersaTREK. Both time-to-detection (TTD) of 10 types of bacteria and five types of yeasts in samples without antibiotic treatment

Staphylococcus aureus bacteremia (SAB) is usually monomicrobial (M-SAB). We reviewed SAB in adults (≥18years old) over a 13year-period and compared polymicrobial (P-SAB) and M-SAB. We encountered 93 P-SAB among 1537 SAB cases (6.1%). The source distribution was comparable; however, source-specified differences were apparent. P-SAB was noted in 12/58 (20.7%) necrotizing soft tissue infections/sacral decubiti and foot gangrene vs. 1/122 (0.8%) cellulitis/abscesses (P<0.001), in 7/64 (10.9%) femoral intravascular catheters (IVC) vs.16/376 (4.3%) IVC in other sites (P=0.03) and 15/134 (11.2%) heal

PURPOSE: Over-the-wire exchange of tunneled dialysis catheters is the standard of care per K/DOQI guidelines for treating catheter-related bacteremia. However, Gram-positive bacteremia, specifically with staphylococcus species, may compromise over-the-wire exchange due to certain biological properties. This study addressed the effectiveness of over-the-wire exchange of staphylococcus-infected tunneled dialysis catheters compared with non-staphylococcus-infected tunneled dialysis catheters.
METHODS: Patients who received over-the-wire exchange of their tunneled dialysis catheter due to documen

BACKGROUND: HIV-infected patients have an increased risk for bacteraemia compared with HIV-negative patients. Few data exist on the incidence of and risk factors for bacteraemia across time in the current era of highly active antiretroviral therapy (HAART).
METHODS: We assessed the incidence of bacteraemia among patients followed between 2000 and 2008 at 10 HIV Research Network sites. This large multisite, multistate clinical cohort study collected demographic, clinical and therapeutic data longitudinally. International Statistical Classification of Diseases and Related Health Problems (ICD)-

BACKGROUND: To allow direct comparison of bloodstream infection (BSI) rates between hospitals for performance measurement, observed rates need to be risk adjusted according to the types of patients cared for by the hospital. However, attribute data on all individual patients are often unavailable and hospital-level risk adjustment needs to be done using indirect indicator variables of patient case mix, such as hospital level. We aimed to identify medical services associated with high or low BSI rates, and to evaluate the services provided by the hospital as indicators that can be used for more

The most important targets of hospital-acquired infection control are to reduce the incidence of surgical-site, catheter-related, and ventilator-associated infections. In this report, we address previously presented infection-control strategies for central venous (CV) line catheterization, using a CV catheter-related infection surveillance system. Data concerning CV catheter insertion were collected from all facilities in our 650-bed hospital, excluding the operating and hemodialysis wards. Collected data included the insertion method, purpose, length of catheter inserted, duration of catheter

Septicemia is a serious problem in hemodialysis patients because it can lead to life-threatening complications and a persistently elevated risk of death. Most analyses have not examined whether there are differences in mortality risk among the organisms that cause these episodes of septicemia. This study was a retrospective cohort analysis of first septicemia hospitalizations during the first year of hemodialysis. Time to death (both in-hospital and within 12 weeks post-discharge) was compared among the different septicemia-causing organisms based on discharge diagnoses in Medicare billing dat
