Spontaneous Bacterial Peritonitis as a Rare Complication of a Pancreatic Duct Leak in a Patient Without Cirrhosis: A Case Report.

Spontaneous bacterial peritonitis (SBP) is classically associated with liver cirrhosis and portal hypertension. Its occurrence in non-cirrhotic patients is rare and poorly characterized. Pancreatic ascites, resulting from disruption of the pancreatic duct (PD) or leakage from a pseudocyst, is an unusual cause of ascites in chronic pancreatitis. The enzyme-rich, exudative nature of this fluid can itself cause chemical peritonitis with neutrophilia, mimicking infection. We report the case of a 50-year-old man with alcohol use disorder and chronic calcific pancreatitis who presented with recurren

Factors Associated With 14-D Reintubation and Outcomes in Surgical Intensive Care Unit Patients.

INTRODUCTION: Reintubation after planned extubation is a clinically significant complication in surgical intensive care unit (SICU) patients, often indicating acute respiratory or systemic deterioration and leading to poor outcomes. However, SICU-specific data on its incidence and risk factors remain limited. METHODS: We conducted a retrospective cohort study of adult postoperative patients admitted to the SICUs of two tertiary medical centers in Taiwan (January 2009 to May 2024). Eligible patients underwent planned extubation following surgery. The primary outcome was reintubation within 14

Androgen-mediated DKK1 upregulation in bronchial epithelial cells suppresses Th2 response and ameliorates asthma.

BACKGROUND: Mutations in the human mitochondrial genome have been suspected to play a significant role in the etiological development of mitochondrial diabetes. Detection of the 3243A>G mutation in the mitochondrial transfer RNALeu(UUR) gene (MTTL1), especially at low heteroplasmy levels, is highly desirable since it facilitates the diagnosis and subsequent management of the disease. The proportions of mutant mitochondrial DNA (mtDNA) can vary between tissues and are usually significantly higher in muscle than in blood, but muscle biopsies from patients with diabetes are rarely available. Epidural Transforaminal Infraneural Infiltration Using Corticosteroids in Lumbar Radiculopathy.

BACKGROUND: A lumbar disc herniation is the primary cause of sciatica, affecting 1% to 5% of the population annually. Epidural corticosteroid injections, whether via the transforaminal or interlaminar route, are an option for patients with refractory sciatica pain. The transforaminal route has the advantage of directly addressing the site of pathology in the anterior epidural space, between the disc herniation and the nerve root. PURPOSE: To evaluate the efficacy of transforaminal infraneural epidural injections using corticosteroids in the treatment of radiculopathy secondary to acute lumbar

Necrotizing Pancreatitis With Concurrent Clostridioides difficile Infection.

Necrotizing pancreatitis is a severe complication of acute pancreatitis that may not become radiographically evident until several days after symptom onset. Although progression from gallstone pancreatitis is well recognized, concurrent postoperative complications and Clostridioides difficile infection can obscure the clinical picture and delay diagnosis. This case adds to the literature by demonstrating how overlapping postoperative findings and concurrent infection masked the evolution of necrotizing pancreatitis despite initial clinical improvement and illustrating treatment approaches in s

India's Draft National Health Research Policy 2026: A Visionary Framework for Transforming the National Health Research Ecosystem.

Chemoport devices are commonly used for the administration of chemotherapeutic medication in cancer patients and can also be used for total parenteral nutrition and long term intravenous therapy. They are usually inserted through the internal jugular vein or the subclavian vein. The common complications of the procedure include pneumothorax, bleeding, arrhythmia and venous thrombosis. One of the rare complications of chemoport is catheter fracture/dislodgement and subsequent migration, with an incidence of 0.1-2.1%. Other rare complications are vascular erosion and embolization, vocal cord pal

Nutritional Management in Patients with Chyle Leak following Head and Neck Surgery: A Systematic Review.

OBJECTIVE: We aim to investigate nutritional management of chyle leaks by a systematic review of the published literature. METHODS: We searched the following databases (Medline, Embase, Cochrane) for literature incorporating nutritional aspects in chyle leak management. We excluded non-English papers, paediatric studies, conference papers, and studies without nutritional intervention. RESULTS: Among 260 patients (out of 26,270), 81.5.% (n=212/260) achieved chyle leak resolution with nutritional management, while 18.5% (n=48/260) required surgery. Median resolution duration with nutritional m

The Clinical Application of Digestive Fluid Reinfusion Technology in the Nutritional Management of Patients With High-Output Ostomies and Fistulas: A Multiple Case Series.

BACKGROUND: Patients with high-output ostomies and fistulas face a significant risk of severe complications such as electrolyte imbalances and renal failure secondary to substantial loss of gastrointestinal fluid. Consequently, these patients present considerable challenges for wound, ostomy, and continence (WOC) nurses. CASES: We report 5 cases of high-output intestinal stomas and 1 case of enteric fistula resulting from gastrointestinal complications following abdominal surgery. An integrated care plan was developed for each patient, which included continuous nutritional assessments to moni

Assessment of Electrolyte Imbalances as an Early Manifestation of Refeeding Syndrome in Critically Ill Patients Receiving Total Parenteral Nutrition at a Tertiary Hospital in Mexico City.

Introduction The nutritional status of critically ill patients is a key determinant of their clinical outcomes. Total parenteral nutrition (TPN) is an essential intervention when oral or enteral routes are not feasible, but it carries important risks such as hepatic and infectious complications, and particularly the development of refeeding syndrome (RFS), a potentially life-threatening condition associated with severe metabolic disturbances. Objectives The primary objective of the study was to determine the primary electrolyte imbalances associated with the development of RFS in critically il

Impact of early enteral nutrition combined with fine nursing in patients with intracranial aneurysm undergoing minimally invasive surgery.

BACKGROUND: Postoperative recovery in patients undergoing minimally invasive surgery for intracranial aneurysms is often hindered by delayed mobilization, inadequate nutritional support, and prolonged hospital stay. Conventional care using parenteral nutrition and standard nursing practices may not sufficiently address these issues. This study evaluated the combined effects of early enteral nutrition and enhanced nursing care on recovery outcomes. NEW METHOD: A total of 138 patients were divided into a control group receiving standard nursing care and parenteral nutrition and an observation g

Clinical and microbiological characteristics of polymicrobial bloodstream infections in hematologic malignancy patients: a cross-sectional study.

Patients with hematological malignancies experience severe immune dysfunction due to the impact of intensive chemotherapy and the impairment of hematopoietic cells. This condition places them at high risk for infectious complications, including bloodstream infections (BSIs), which are associated with increased morbidity and mortality. Polymicrobial bloodstream infections (PBSIs), involving multiple microorganisms, represent a critical clinical condition with high mortality rates in this population. This study aimed to investigate the risk factors for poor clinical outcomes associated with PBSI

Imaging of megacystis-microcolon-intestinal hypoperistalsis syndrome before, during, and after the neonatal period: a pictorial review.

Megacystis-microcolon-intestinal hypoperistalsis syndrome (MMIHS), also known as Berdon syndrome, is a rare genetic congenital disorder of impaired smooth muscle contractility, resulting in functional obstruction of the bladder and bowel. Historically associated with a poor prognosis, recent advances in the use of total parenteral nutrition (TPN), intestinal rehabilitation, and multi-visceral transplantation have led to improvements in survival in patients with MMIHS, with patients now living into the second decade of life. The radiologist plays a key role in the initial workup of these patien

Gastrointestinal involvement in systemic sclerosis: A spotlight on small bowel involvement, complications and management.

Small-bowel involvement in systemic sclerosis is frequent but often underdiagnosed, largely due to the lack of standardized diagnostic protocols for its evaluation. Small bowel dysmotility manifests with symptoms such as abdominal pain, diarrhea, bloating, and distension, and complications including small intestinal bacterial overgrowth, reduced oral intake, malnutrition, and, in severe cases, intestinal pseudo-obstruction. These complications are associated with significant morbidity, dependence on parenteral nutrition, and poor prognosis. In this review, we summarize the current evidence on

Spatulated umbilical cord technique embracing Wharton's jelly for gastroschisis repair.

The purpose was to prospectively analyze outcomes of gastroschisis closure using the Spatulated Umbilical Cord technique (SUCT). In SUCT, the umbilical cord is split open and used to cover the abdominal defect. The procedure involved opening the amnion layer from the base, exposing Wharton’s jelly and ‘spatulating’ using longitudinal incisions and securing it as a patch over the defect. A pilot interventional study was conducted prospectively at 3 centres from 2016 to 2023 with follow-up till 2024. The inclusion criteria included uncomplicated gastroschisis, where primary fascial closure was a

Parenteral Nutrition Management from the Clinical Pharmacy Perspective: Insights and Recommendations from the Saudi Society of Clinical Pharmacy.

Parenteral nutrition (PN) is essential for patients who are unable to tolerate oral or enteral feeding, providing them with necessary nutrients intravenously, including dextrose, amino acids, electrolytes, vitamins, trace elements, and lipid emulsions. Clinical pharmacists (CPs) play a critical role in PN management by ensuring proper formulation, monitoring therapy, preventing complications, and optimizing patient outcomes. In Saudi Arabia, limited literature exists on CPs' involvement in total parenteral nutrition (TPN) administration, health information management (HIM) systems, and pharmac
