B0AT1 (SLC6A19) is a neutral amino acid transporter mediating intestinal absorption and renal reuptake of amino acids, including phenylalanine (Phe). Inhibiting B0AT1 enhances Phe excretion, offering a therapeutic strategy for phenylketonuria (PKU). Using cryo-EM, we determined human B0AT1 structures in outward- and inward-open states, revealing an allosteric pocket ~17 Å from the substrate site that is present in the outward-open conformation and has been previously reported. Structure-guided inhibitor design targeting this pocket produced a potent B0AT1 inhibitor that locks the transporter i

BACKGROUND: Phenylketonuria (PKU) is a rare metabolic disorder requiring lifelong dietary management. While clinical stability hinges on strict treatment adherence, this adherence is largely sustained by unpaid family caregiving, particularly in pediatric contexts. Yet, the lived experience of caregivers remains conceptually underexamined and largely absent from PKU research. This integrative review synthesizes empirical findings on caregiver burden in pediatric PKU and maps a multilevel burden structure that traces its structural, relational, and psychological dimensions.

Clinical care and management of inborn errors of metabolism (IEM) are lifelong, complex, and resource-intensive for the healthcare team, patient, and families. A cohort of individuals with IEM who in addition have gender incongruence (GI) presents a unique challenge of patient care in current practices. Here, we discuss four patients with different IEM (phenylketonuria (PKU), 3-Hydroxy-3-methylglutaryl-CoA lyase deficiency (HMGCLD), ornithine transcarbamylase (OTC) deficiency that presented with GI either in their adolescence or adulthood. We review their IEM management and evolving expression

OBJECTIVE: Phenylketonuria (PKU) is an inherited disorder of amino acid metabolism. Despite early dietary treatment, cognitive functioning of patients has been reported as being inferior to healthy controls. Objective of this study was to assess functional connectivity (FC) alterations in PKU in cognition-related brain networks by resting-state functional magnetic resonance imaging. We followed a hierarchical analysis approach partially based on higher criticism (HC) statistics as previously applied in a larger sister-project in fetal alcohol syndrome. RESULTS: After exclusions for excessive h

INTRODUCTION: The approval of pegvaliase (PALYNZIQ®) represented a paradigm shift in the management of phenylketonuria (PKU) by enabling sustained reductions in blood phenylalanine levels and permitting an unrestricted diet. However, as it is a bacterially derived injectable therapy, individuals receiving pegvaliase may experience skin-related adverse events (AEs). Anticipating these AEs and other pegvaliase-related skin concerns and having best-practice strategies to manage them may be crucial to achieving optimal patient outcomes.

BACKGROUND: Effective management of phenylketonuria (PKU) requires consistent blood phenylalanine (Phe) control. However, patients, particularly those with classical PKU, often experience diurnal fluctuations, particularly with early morning Phe levels after an overnight fast. These fluctuations have been linked to neurocognitive and psychological impairments. This 4-week, randomized, controlled crossover study evaluated the efficacy of a prolonged-release amino acid (PR-AA) compared to a conventional free amino acid-based protein substitute (AA) in reducing early morning blood Phe levels in p

Phenylketonuria (PKU) is a rare metabolic disorder causing elevated phenylalanine (PHE) levels requiring lifelong dietary or pharmacological management and regular monitoring. Current PHE monitoring methods, such as tandem mass spectrometry (FIA-MS/MS), are laboratory and sample transport-dependent, leading to delays in obtaining results. This highlights the need for accessible point-of-care (PoC) solutions. Building on a previously developed bioluminescence-based PHE quantification method, we created a PoC PHE sensor integrated with a smartphone app (phenyx app) for real-time monitoring and d

Ethnic differences are crucial when considering the efficacy, safety, and dose of pharmaceuticals across diverse populations. The International Council for Harmonization of Technical Requirements for Pharmaceuticals for Human Use (ICH) guideline E5 addresses the acceptability of extrapolating foreign clinical data taking ethnic factors into consideration. Sepiapterin has recently been approved for the treatment of hyperphenylalaninemia (HPA) in patients with phenylketonuria (PKU) in Europe, the USA, and multiple additional countries worldwide. To date, no clinical trials have been conducted in

Phenylketonuria (PKU) is caused by defective catabolism of phenylalanine (Phe), resulting in Phe accumulation and subsequent neurocognitive impairment. This retrospective study used a large United States claims database linked to laboratory records (6/2018-05/2023) to compare comorbidities, healthcare resource utilization, and healthcare costs between individuals with PKU and matched controls, stratified by age and Phe level, as well as describe pharmacological treatment patterns among individuals with PKU. The study included 5729 and 2648 matched pairs aged ≥ 12 and < 12 years, respectively (

Individuals with phenylketonuria (PKU), caused by different variants of the phenylalanine hydroxylase gene, need to restrict their intake of phenylalanine. This study evaluated dietary patterns and physical activity levels in children with different PKU phenotypes compared to healthy controls. Eighty-two children were recruited (22 classic PKU [cPKU], 21 BH4-responsive PKU, 19 hyperphenylalaninemia, and 20 controls). Anthropometric data, dietary intake, biochemical markers, and physical activity were assessed. Classic PKU (cPKU) subjects exhibited higher carbohydrate and sugar intake than othe

Loss-of-function variants in the human phenylalanine hydroxylase (PAH) gene are the most common genetic causal factors for Phenylketonuria (PKU). Currently, a broad spectrum of variations is recognized in the human PAH gene. However, the molecular function and clinical significance of some novel PAH variants remain unclear. Here, we report on five PKU-affected families carrying three novel PAH variants, including one missense variant (PAH: c.271C>A (p.Leu91Met)) and two deletions (PAH: c.206_208delCTT (p.Ser70del) and PAH: c.541_544delGAGG (p.Glu181Lysfs*13)). These variations constitute diffe

BACKGROUND: Colonoscopy is a diagnostic technique used for gastrointestinal tract pathologies that presents additional challenges when used in individuals with phenylketonuria (PKU). The dietary requirements for colonoscopy preparation conflict with a phenylalanine (Phe) restricted diet essential in classical PKU, and aspartame containing laxatives must also be avoided. This case study highlights the complexities of applying conventional colonoscopy protocols to patients with PKU and emphasis the importance of personalized preparation strategies that consider strict dietary requirements.

OBJECTIVES: While the dietary treatment in phenylketonuria (PKU) is known to cause eating problems and social restrictions in children, knowledge of the prevalence and magnitude of these problems in adults is scarce. This study aimed to evaluate the impact of PKU and its dietary treatment on daily life of adults with PKU.
METHODS: A web-based questionnaire consisting of a PKU-specific questionnaire, the Patient-Reported Outcomes Measurement Information System (PROMIS) short forms, and the 36-Item Short Form Health Survey (SF36) was completed by 40 adults with PKU and 15 age-matched controls w

Phenylketonuria (PKU) is associated with neurocognitive and neuropsychological symptoms despite early treatment and well-controlled phenylalanine (Phe) levels. PKU impairs the conversion of Phe to tyrosine and causes excess phenylalanine to accumulate. This accumulation can competitively block some other amino acids from entering the brain. This may reduce production of key neurotransmitters like dopamine and serotonin, which can affect brain function and development. However, direct correlations between blood Phe levels and central nervous system (CNS) neurotransmitter concentrations have not
