Identification and quantitation of phenylethylene glycol in human and rat urine, and its elevation in phenylketonuria.

Phenylethylene glycol has been identified in rat and human urine using gas chromatography/chemical ionization/mass spectrometry. A method was developed for the quantitative analysis in urine of this phenylalanine metabolite and of p-hydroxyphenylethanol, a metabolite of tyrosine, by converting them to the pentafluoropropionyl derivatives and measuring them by selected ion monitoring. In human urine, about 90% of the phenylethylene glycol was present in a conjugated form (releasable by glusulase), but the reverse was true for rat urine, with about 90% being present in the unconjugated form. The

Screening for PKU heterozygosity in bipolar affectively ill patients.

There were no significant differences noted between bipolar manic-depressive patients and normal controls for plasma phenylalanine or tyrosine following an L-phenylalanine loading test given to determine if some affective illness may be related to the heterozygous phenotypic expression of phenylketonuria (reduced liver phenylalanine hydroxylase). The test was able to distinguish known PKU heterozygotes from the other subjects. It is possible that other heterozygous states may be implicated in the development of some psychiatric disorders.

A maximum likelihood map of chromosome 1.

Thirteen loci are mapped on chromosome 1 from genetic evidence. The maximum likelihood map presented permits confirmation that Scianna (SC) and a fourteenth locus, phenylketonuria (PKU), are on chromosome 1, although the location of the latter on the PGM1-AMY segment is uncertain. Eight other controversial genetic assignments are rejected, providing a practical demonstration of the resolution which maximum likelihood theory brings to mapping.

["Transitory" phenylketonuria. A permanent deficit].

Three cases are reported with hyperphenylalaninaemia greater than 1.8 micrometer/ml-1 in the neonatal period, becoming tolerant of a normal regime (3 g protein per kg) without plasma levels of phenylalanine exceeding 0.2 to 0.3 micrometer/ml-1. Atypical kinetics (zero order) of phenylalanine clearance after intravenous perfusion were shown in the three cases at the age of one year and the persistence of the disorder was again demonstrated at the age of five years in two cases by the study of an oral load of phenylalanine. Examination of the parents showed normal fasting levels of phenylalanine

Three social-preference measures in PKU monkeys.

Of twenty-four one-year-old rhesus, half were brain damaged from excessive phenylalanine (PKU) administered either pre- or post-natally. When compared on three measures of affiliation--(a) a simultaneous forced-choice proximity circus test, (b) a sequential free-operant Butler-box test, and (c) a social group in a playroom--the duration measure of the circus gave the best differentiation between stimulus animals, the Butler box the poorest. Both types of PKU monkeys were found to strongly prefer familiar PKU monkeys, and controls to refer unfamiliar followed by familiar controls. This extends

Breast-feeding and infant health.

A mother who breast-feeds makes a significant contribution to her infant's health. Currently researchers are delineating many of the unique nutritional and antiinfective factors in human milk. The psychological advantages that accrue to both the breast-feeding mother and her infant are harder to quantify. Contraindications to breast-feeding are few and relate primarily to maternal illness and need for medications, or to those rare infants who have inborn errors of metabolism. Physicians and other health personnel should advise the mother knowledgeably on breast-feeding and seek imaginative sol

A new variant form of phenylketonuria.

We report a family (parents and two sets of twin girls) in which the propositus presented as a case of phenylketonuria with a somewhat less severe degree of phenylalanine intolerance than occurs in patients with classical phenylketonuria and whose phenylalanine tolerance was further impaired by giving cotrimoxazole. The trimethoprim component of cotrimoxazole reduces the phenylalanine tolerance of normal subjects but does not augment the degree of phenylalanine intolerance in patients with classical phenylketonuria. The results of the present study of the phenylalanine tolerance and the way in

Genetics of the mammalian phenylalanine hydroxylase system. Studies of human liver phenylalanine hydroxylase subunit structure and of mutations in phenylketonuria.

Phenylalanine hydroxylase was purified from crude extracts of human livers which show enzyme activity by usine two different methods: (a) affinity chromatography and (b) immunoprecipitation with an antiserum against highly purified monkey liver phenylalanine hydroxylase. Purified human liver phenylalanine hydroxylase has an estimated mol. wt. of 275 000, and subunit mol. wts. of approx. 50 000 and 49 000. These two molecular-weight forms are designated H and L subunits. On two-dimensional polyacrylamide gel under dissociating conditions, enzyme purified by the two methods revealed at least six

Myelin deficiency in experimental phenylketonuria: contribution of the aromatic acid metabolites of phenylalanine.

Retarded body and brain growth and a deficit of myelin in the cerebral hemispheres and the cerebellum were observed in an animal model of phenylketonuria, the p-chlorophenylalanine and L-phenylalanine treated preweanling rat. These manifestations of phenylketonuria were reproduced in rats treated with phenylacetate in amounts approximating those likely to be produced in phenylketonuria. Young rats treated with equivalent amounts of other metabolites of phenylalanine, namely, phenylpyruvate, phenyllactate, and mandelate, which also accumulate in the brain during hyperphenylalaninemia, did not e

Methods of dietary inception in infants with PKU.

Information on methods of initiating the phenylalanine-restricted diet was obtained from the medical personnel of sixteen clinics in the Collaborative Study of Children Treated for Phenylketonuria. The four dietary methods used initially to lower serum phenylalanine were: Normal dilution Lofenalac, Lofenalac, with phenylalanine added as milk, Lofenalac with milk added to provide 200 mg. phenylalanine per day, and alternating bottles of normal dilution Lofenalac and cow's milk during the first four days of therapy. During the first week of treatment, eight clinics obtained serum phenylalanine d

[Cutaneous dyschromia in three cases of phenylketonuria. Quantitative ultrastructural study of the basal layer of the epidermis (author's transl)].

The authors have studied skin color modifications in 3 cases of phenylketonuria and have observed the characteristic changes; fair skin and fair hair. In addition they noted hundred of pigmented pin point or slightly larger patches in the two more affected patients, in the areas exposed to sunshine. With regard to the ultrastructural study of the epidermis basal layer, the proportion of melanocytes in the two most severe cases was slightly higher than in the normal skin of 6 control subjects. Langerhans cells could not be acertained. The more severe was the disease the greater was the tendency

Experimentally induced and natural recovery from the effects of phenylalanine on brain protein synthesis.

The decrease in the neural polyribosomes produced during hyperphenylalaninemia could not be restored to normal levels by the injection of other single neutral amino acids. All of the neutral amino acids that are transported with phenylalanine were found to produce an alteration of neural polyribosomes similar to that measured with phenylalanine. However, the injection of a balanced mixture of 6 or 7 neutral amino acids could restore the brain polyribosomes to normal states. Although this experimentally induced recovery did not lower brain phenylalanine concentrations, it did restore the acylat

Effects of diet and behavior therapy on social and motor behavior of retarded phenylketonuric adults: an experimental analysis.

The effects of a low phenylalanine diet on six retarded phenylketonuric adults were assessed. An ABA individual-subject design was used in experiment I to assess the effects of a low phenylalanine diet on social and motor behavior. Following a baseline during which the subjects ingested a normal phenylalanine diet (phase A), a low phenylalanine diet (phase B) was administered in a double blind fashion. Finally, the baseline condition (phase A) was reinstated (normal diet). The low phenylalanine diet resulted in few significant behavioral changes for those subjects with which proper methodologi

Comparison of alpha-methylphenylalanine and p-chlorophenylalanine as inducers of chronic hyperphenylalaninaemia in developing rats.

alpha-Methylphenylalanine is a very weak competitive inhibitor of rat liver phenylalanine hydroxylase in vitro but a potent suppressor in vivo. The loss of the hepatic activity (the renal one is unaffected) becomes maximal (70-75% decrease; cf. control) 18h after the administration (per 10g body wt.) of 24 mumol of alpha-methylphenylalanine with or without 52 mumol of phenylalanine. Chronic suppression of hepatic phenylalanine hydroxylase was obtained by injections of alpha-methylphenylalanine plus phenylalanine to suckling rats, and by their addition to the diet after weaning. Screening for phenylketonuria in New York City. Threshold values reconsidered.

In New York City from 1966 to 1970, almost all 736,469 newborns were screened for phenylketonuria (PKU). Among 1,094 infants with presumptive positive test results, 763 were followed up and 46 of them were judged to require preventive treatment. The considerable annual variation observed in the frequency of values of 4 to 6 mg/100 ml bood phenylalanine suggested low reliability at this level. Screening test results of 4 mg/100 ml, with no cases detected among them, represented 53% of all false positive results; newborns with 6 mg/100 ml results yielded 1 infant in need of treatment and account

[Early diagnosis of hereditary metabolic diseases in the newborn by means of microbiological Guthrie tests].

The screening method of so-called bacterial inhibition and growth assay (Guthrie-test) is described and the number of newborn babies tested over the whole of West Germany from 1969--1974 is stated. 626 cases of PKU were detected early and tested after these dates. Only a few german laboratories are able to detect other inborn errors of metabolism such as MSUD (maple syrup urine disease), homocystinuria, histidinemia and galactosemia. The ratio of illnesses therefore can only be estimated and not stated with statistical evidence.

Selenium concentration and glutathione peroxidase activity in blood of New Zealand infants and children.

The blood selenium (Se) concentrations of New Zealand children were lower than those reported for children living in other countries. This low blood Se was primarily determined by the low dietary intake of the children which, in turn, reflects the low Se content of New Zealand soils. Blood Se also varied geographically, with age, and with differences in quantities and types of food eaten. Children with phenylketonuria and maple syrup urine disease on synthetic diets had low Se intakes and blood Se concentrations compared with children on normal diets, and blood Se was seen to decrease with the

Phenylketonuria and scleroderma.

Two mentally retarded siblings, one with severe segmental scleroderma and the other with atrophoderma of Pasini and Pierini, were found at the ages of 6 and 10 years to have phenylketonuria (PKU). The belief that a common pathomechanism exists between morphea and atrophoderma of Pasini and Pierini is supported by the case of the two siblings. Disorders in tryptophan metabolism can occur in both PKU and scleroderma. For a low phenylalanine diet to be effective in PKU, it has to be instituted at an early age. Phenylketonuria should be considered in infants and children with sclerodermatous skin

[Clinical and biochemical heterogenicity of phenylketonuria in adults].

The authors carried out a clinical and biochemical investigation of 23 phenylketonuric patients at the age of 17--49 who were not treated specifically. The clinical examination showed that behavioural reactions, attacks of psychomotor excitation, neurological status, presence or absence of strokes and other symptoms of the disease were heterogenous. It has been established that disturbances of acid, protein, adipose and carbohydrate metabolism have different degrees of expressiveness. The clinical manifestation of the disease is conditioned by heterogeneity of monolocus mutations of the PAH ge
