Bacteremia in horses undergoing routine dental prophylaxis: Before, during, and after the procedure.

BACKGROUND: Transient bacteremia after dental procedures is well-documented in the literature, but the prevalence and clinical significance in horses remains largely unexplored. OBJECTIVES: To assess whether routine dental prophylaxis in clinically healthy horses is associated with detection of bacteria in blood, using culture and 16S rDNA sequencing. Our central hypothesis is that a percentage of horses would demonstrate detectable but transient bacteremia in the immediate post-procedural period following dental prophylaxis. STUDY DESIGN: Prospective, observational study. METHODS: Venous b

Isolated left-lobe metastatic hepatic abscess due to portal pyemia from microperforated ascending colon carcinoma: a rare case report.

INTRODUCTION AND IMPORTANCE: Liver abscess-like lesions are commonly managed as primary hepatobiliary infections; however, colorectal cancer can be an occult source, particularly when portal pyemia follows a microperforated tumor. Diagnostic complexity increases when right-sided colon cancer presents with an apparently isolated left-lobe hepatic collection mimicking foregut disease, especially in settings where anaerobic culture and immunohistochemistry are limited. CASE PRESENTATION: A middle-aged woman in her fifth decade with an 8-month history of intermittent right iliac fossa pain and re

Real-life impact of clinical metagenomics in the intensive care unit: a multicenter retrospective study in greater paris area hospitals.

BACKGROUND: Infection are the leading cause of intensive care unit (ICU) admission, yet conventional microbiological methods frequently fail to identify the causative pathogen. Metagenomic next-generation sequencing (mNGS) is an emerging, unbiased, pan-pathogen diagnostic tool. However, its real-world microbiological and clinical impact in the ICU remains poorly characterized. This study aimed to assess the microbiological yield and clinical impact of mNGS when implemented in routine ICU practice. METHODS: This retrospective multicenter study was conducted across ten tertiary-care ICUs in the

Shock: Pathophysiology, Classification, and the Role of Lactate in Diagnosis and Prognosis.

Shock is a life-threatening state of circulatory failure characterized by inadequate tissue oxygen delivery and cellular hypoxia. Lactate has emerged as a central biomarker in the diagnosis, risk stratification, and management of shock, yet its interpretation is complex and context dependent. Although traditionally viewed as a marker of anaerobic metabolism and tissue hypoperfusion, hyperlactatemia in distributive shock, particularly sepsis, frequently reflects multifactorial mechanisms, including increased aerobic glycolysis, adrenergic stimulation, mitochondrial dysfunction, and impaired hep

Eravacycline: From broad-spectrum coverage to novel clinical applications.

Eravacycline is a novel, fully synthetic fluorocycline antibiotic with broad-spectrum activity against Gram-positive, Gram-negative, aerobic, and anaerobic pathogens, including multidrug-resistant (MDR) strains. It maintains efficacy despite common tetracycline resistance mechanisms, such as efflux pumps and ribosomal protection proteins. Its pharmacokinetic profile is characterized by extensive tissue penetration, particularly into the epithelial lining fluid and alveolar macrophages. Eravacycline is particularly active against MDR pathogens such as carbapenem-resistant Enterobacterales and A

The Gut Microbiome and Short-Chain Fatty Acid Metabolites in Sepsis.

Sepsis profoundly perturbs the intestinal microbiome and its metabolite output, yet the mechanisms by which these changes influence organ injury remain incompletely defined. In this review, we focus on short-chain fatty acids (SCFAs) as key mediators linking gut microbes to sepsis pathophysiology. We first summarize how sepsis and its treatments reshape gut communities, depleting SCFA-producing anaerobes and altering the gut metabolome. We then examine determinants of SCFA concentrations in the intestinal lumen and describe how gut-blood trafficking of these charged metabolites depends on epit

A case report of bloodstream infection caused by co-infection of Olsenella uli and Christensenella hongkongensis.

We report a case of a Chinese male patient with a history of Hepatolithiasis who was admitted for abdominal pain, fever, and chills. Imaging studies suggested cholangitis and Hepatolithiasis. Blood cultures revealed two rare fastidious anaerobes-Olsenella uli and Christensenella hongkongensis-confirmed by MALDI-TOF MS and 16S rDNA full-length sequencing. These organisms failed to grow on standard solid media but were successfully isolated using a self-prepared solid medium formulated based on the nutrient solution from blood culture bottles. This suggests that for anaerobic bacteria with posit

Evaluation of alternative blood culture systems in detecting pathogenic bacteria: a multi-site, observational prospective study.

BACKGROUND: Sepsis, is a severe, life-threatening response to infection with rising incidence and significant mortality. Although blood culture (BC) systems are critical for sepsis diagnosis and the detection of pathogens bacteria, their high cost limits their use in low-income regions. This study evaluates the performance and affordability of bottles with gold-standard systems. METHODS: From September 2023 to June 2024, we conducted a nine-month evaluation of alternative BC instruments (Meihua Medical [MH] and DL-Biotech [DL]) and BC bottles (BCBs) against bioMérieux and Becton Dickinson (BD

MALDI-TOF misidentification of Sanguibacteroides justesenii: confirmation by 16S rRNA sequencing.

Sanguibacteroides justesenii is a rarely reported Gram-negative anaerobic bacillus of uncertain clinical significance. Its identification is challenging using conventional methods. We report the case of a 58-year-old woman admitted for acute heart failure who developed septic shock following colonic perforation. Blood cultures grew an obligate anaerobic Gram-negative bacillus. MALDI-TOF MS (Bruker®) identified the isolate as Gabonibacter massiliensis (score 2.19). Given the unexpected result, the strain was referred to a reference laboratory, where 16S rRNA gene sequencing identified S. justes

Dynamics of the gut and lung microbiota in severe infections: from observational studies to therapeutic strategies.

BACKGROUND: Severe infections and sepsis are characterized by a disruption of intestinal and respiratory microbial communities. Loss of obligate gut anaerobes and depletion of immunomodulatory metabolites disrupt mucosal integrity, impair immune homeostasis, and increase susceptibility to secondary infection and organ failure. OBJECTIVES: To summarize the current understanding of gut and lung microbiome dynamics during severe infections, describe immunometabolic crosstalk along the gut-lung axis, and identify microbiome-targeted strategies to improve outcomes. SOURCES: Peer-reviewed preclini

A fatal case report of sepsis caused by Robinsoniella sp.

Robinsoniella species are anaerobic, spore-forming, Gram-positive bacilli that are rarely associated with human infections because of their slow growth and the limitations of conventional identification methods. We describe a fatal case of sepsis in an 84-year-old woman with hypertension, diabetes, and chronic renal insufficiency. During hospitalization, she developed impaired consciousness, polymicrobial infections, and multiple organ failure. Despite aggressive antimicrobial and supportive treatment, her condition deteriorated, leading to death. This study identified a Robinsoniella isolate

Impact of HbA1c levels on time to blood culture positivity in bacterial bloodstream infections.

PURPOSE: Poor glycemic control is associated with increased susceptibility to bloodstream infection (BSI), sepsis, and adverse outcomes. Time to blood culture positivity (TTP) is an early microbiological marker with potential clinical implications; however, its relationship with glycated hemoglobin (HbA1c) has not been previously clarified. This study investigated the clinical impact of HbA1c levels on TTP and outcomes in intensive care unit (ICU) patients evaluated for suspected BSI. METHODS: This retrospective study included 904 ICU patients. Blood samples were inoculated into BACTEC Plus A
