Listeria monocytogenes can cause severe invasive infection in older and medically vulnerable patients, and pinpointing a potential exposure may require a detailed dietary history. We report the case of an 84-year-old woman with end-stage renal disease on hemodialysis, type 2 diabetes mellitus, cardiovascular disease, and vascular dementia who presented with chest pain and mild dyspnea and then developed fever, hypotension, tachycardia, and a single episode of diarrhea, which prompted evaluation for sepsis. Blood cultures turned positive in both aerobic and anaerobic bottles after approximately

BACKGROUND: Transient bacteremia after dental procedures is well-documented in the literature, but the prevalence and clinical significance in horses remains largely unexplored.
OBJECTIVES: To assess whether routine dental prophylaxis in clinically healthy horses is associated with detection of bacteria in blood, using culture and 16S rDNA sequencing. Our central hypothesis is that a percentage of horses would demonstrate detectable but transient bacteremia in the immediate post-procedural period following dental prophylaxis.
STUDY DESIGN: Prospective, observational study.
METHODS: Venous b

INTRODUCTION AND IMPORTANCE: Liver abscess-like lesions are commonly managed as primary hepatobiliary infections; however, colorectal cancer can be an occult source, particularly when portal pyemia follows a microperforated tumor. Diagnostic complexity increases when right-sided colon cancer presents with an apparently isolated left-lobe hepatic collection mimicking foregut disease, especially in settings where anaerobic culture and immunohistochemistry are limited.

BACKGROUND: Gut microbiota alterations are common in critical illness, but differences between critically ill patients with and without sepsis and their clinical relevance remain incompletely understood. We investigated gut microbial diversity, genus-level differences and their associations with inflammatory profiles and clinical outcomes in critically ill patients.
METHODS: This single-centre case-control study enrolled adult critically ill patients grouped by Sepsis-3 criteria. Gut microbiota was profiled by 16S rRNA sequencing. The primary M2 model, adjusted for age, intra-abdominal infect

CONTEXT: Lynch syndrome (LS), also referred to as hereditary non-polyposis colorectal cancer, is a familial cancer syndrome characterised by young age of onset of colorectal and other extra-colonic cancers. Most studies suggest that LS accounts for approximately 1% of all colorectal cancers (CRC). The identification of persons with a mutation for this syndrome is of major clinical importance because regular and life-long surveillance has been shown to improve their survival through early cancer detection. However, the identification of LS among CRC patients is a major challenge because there a

Poly(ADP-ribose) polymerase (PARP) inhibitors constitute the inaugural targeted therapy shown to enhance the prognosis of individuals with hereditary tumors, initially utilized in the management of patients with germline BRCA1/2-associated breast cancer. With ongoing research, PARP inhibitors (PARPi) are currently under extensive investigation for their applicability across a spectrum of diseases, encompassing oncology, cardiovascular diseases, and cerebrovascular diseases. This narrative review provides a comprehensive synthesis of the biological rationale, existing evidence, recent advanceme

BACKGROUND: Infection are the leading cause of intensive care unit (ICU) admission, yet conventional microbiological methods frequently fail to identify the causative pathogen. Metagenomic next-generation sequencing (mNGS) is an emerging, unbiased, pan-pathogen diagnostic tool. However, its real-world microbiological and clinical impact in the ICU remains poorly characterized. This study aimed to assess the microbiological yield and clinical impact of mNGS when implemented in routine ICU practice. METHODS: This retrospective multicenter study was conducted across ten tertiary-care ICUs in the

Shock is a life-threatening state of circulatory failure characterized by inadequate tissue oxygen delivery and cellular hypoxia. Lactate has emerged as a central biomarker in the diagnosis, risk stratification, and management of shock, yet its interpretation is complex and context dependent. Although traditionally viewed as a marker of anaerobic metabolism and tissue hypoperfusion, hyperlactatemia in distributive shock, particularly sepsis, frequently reflects multifactorial mechanisms, including increased aerobic glycolysis, adrenergic stimulation, mitochondrial dysfunction, and impaired hep

Eravacycline is a novel, fully synthetic fluorocycline antibiotic with broad-spectrum activity against Gram-positive, Gram-negative, aerobic, and anaerobic pathogens, including multidrug-resistant (MDR) strains. It maintains efficacy despite common tetracycline resistance mechanisms, such as efflux pumps and ribosomal protection proteins. Its pharmacokinetic profile is characterized by extensive tissue penetration, particularly into the epithelial lining fluid and alveolar macrophages. Eravacycline is particularly active against MDR pathogens such as carbapenem-resistant Enterobacterales and A

BACKGROUND: Clostridium perfringens is an anaerobic gram-positive rod that colonizes the gastrointestinal and female genital tracts. It is an uncommon cause of bacteremia but is associated with high mortality when present. The most commonly reported sources include intraabdominal infections, soft tissue necrosis, and malignancy-related processes. Most affected patients are elderly or immunocompromised. CASE PRESENTATION: We present a case of C. perfringens bacteremia in a 52-year-old postmenopausal female with no history of malignancy, recent surgery, or immunosuppression. The patient presente

Sepsis profoundly perturbs the intestinal microbiome and its metabolite output, yet the mechanisms by which these changes influence organ injury remain incompletely defined. In this review, we focus on short-chain fatty acids (SCFAs) as key mediators linking gut microbes to sepsis pathophysiology. We first summarize how sepsis and its treatments reshape gut communities, depleting SCFA-producing anaerobes and altering the gut metabolome. We then examine determinants of SCFA concentrations in the intestinal lumen and describe how gut-blood trafficking of these charged metabolites depends on epit

We report a case of a Chinese male patient with a history of Hepatolithiasis who was admitted for abdominal pain, fever, and chills. Imaging studies suggested cholangitis and Hepatolithiasis. Blood cultures revealed two rare fastidious anaerobes-Olsenella uli and Christensenella hongkongensis-confirmed by MALDI-TOF MS and 16S rDNA full-length sequencing. These organisms failed to grow on standard solid media but were successfully isolated using a self-prepared solid medium formulated based on the nutrient solution from blood culture bottles. This suggests that for anaerobic bacteria with posit

BACKGROUND: Sepsis, is a severe, life-threatening response to infection with rising incidence and significant mortality. Although blood culture (BC) systems are critical for sepsis diagnosis and the detection of pathogens bacteria, their high cost limits their use in low-income regions. This study evaluates the performance and affordability of bottles with gold-standard systems.
METHODS: From September 2023 to June 2024, we conducted a nine-month evaluation of alternative BC instruments (Meihua Medical [MH] and DL-Biotech [DL]) and BC bottles (BCBs) against bioMérieux and Becton Dickinson (BD

We reviewed 8 episodes of Desulfovibrio bacteremia in Japan (2020-2025) and confirmed 4 species by 16S rRNA gene and whole-genome sequencing. We detected β-lactamase genes in 2 D. desulfuricans (blaDES-1-like), 1 D. falkowii (blaMUN-1), and 2 D. fairfieldensis (blaCfiA-like). Mass spectometry failed to identify D. falkowii or D. legallii.

Sanguibacteroides justesenii is a rarely reported Gram-negative anaerobic bacillus of uncertain clinical significance. Its identification is challenging using conventional methods. We report the case of a 58-year-old woman admitted for acute heart failure who developed septic shock following colonic perforation. Blood cultures grew an obligate anaerobic Gram-negative bacillus. MALDI-TOF MS (Bruker®) identified the isolate as Gabonibacter massiliensis (score 2.19). Given the unexpected result, the strain was referred to a reference laboratory, where 16S rRNA gene sequencing identified S. justes

BACKGROUND: Severe infections and sepsis are characterized by a disruption of intestinal and respiratory microbial communities. Loss of obligate gut anaerobes and depletion of immunomodulatory metabolites disrupt mucosal integrity, impair immune homeostasis, and increase susceptibility to secondary infection and organ failure.
OBJECTIVES: To summarize the current understanding of gut and lung microbiome dynamics during severe infections, describe immunometabolic crosstalk along the gut-lung axis, and identify microbiome-targeted strategies to improve outcomes.
