Structures of the human glucose-6-phosphate transporter provide insights into its transport cycle and substrate recognition.

The human glucose-6-phosphate transporter (G6PT/SLC37A4) mediates the translocation of glucose-6-phosphate (G6P) from the cytoplasm into the endoplasmic reticulum, a process essential for glucose production and the maintenance of blood glucose homeostasis between meals. Dysfunction of G6PT causes glycogen storage disease type Ib (GSD-Ib), a severe metabolic disorder characterized by hypoglycemia, hepatomegaly, and neutropenia. Despite its physiological and clinical significance, the structural basis of G6P recognition and the molecular mechanisms underlying GSD-Ib have remained elusive. Here,

Liver Biopsy-Associated Diagnosis of Glycogen Storage Disease Type IV (Andersen Disease).

Glycogen storage disease type IV (Andersen disease) is caused by a deficiency of the glycogen branching enzyme. We report a case in which liver biopsy played a key role in establishing the diagnosis. A 2-year-old girl presented to our hospital with hepatomegaly. Despite extensive testing, no definitive diagnosis was made, prompting a liver biopsy. The biopsy tissue revealed characteristic cytoplasmic inclusions. Genetic analysis identified a mutation in the GBE1 gene, confirming the diagnosis of glycogen storage disease type IV. Various special stains were helpful in characterizing the accumul

Increased Glycogenin-Exposed Residual Glycogen in Lysosomes Is the Early Pathological Finding in Asymptomatic Pompe Disease.

INTRODUCTION/AIMS: Although asymptomatic or pre-symptomatic Pompe disease is increasingly recognized, early skeletal muscle pathology and the role of residual glycogen at this stage remain unclear. Here, we investigated early muscle pathological changes using human samples and an early-stage Gaa -/- mouse model, focusing on residual glycogen accumulation. METHODS: Clinical, genetic, imaging, and muscle pathological analyses were performed in an asymptomatic Pompe disease patient carrying a novel GAA variant. Skeletal muscle samples from asymptomatic and symptomatic patients, as well as from e

A genome-edited isogenic human embryonic stem cell model of pompe disease recapitulates cardiac and skeletal muscle pathology.

BACKGROUND: Pompe disease is an autosomal recessive lysosomal storage disorder caused by mutations in the GAA gene, leading to acid alpha-glucosidase deficiency and pathological glycogen accumulation, primarily in cardiac and skeletal muscle. While enzyme replacement therapy (ERT) has improved clinical outcomes, its limited efficacy especially in skeletal muscle underscores the need for improved disease models and novel therapeutic strategies. Induced pluripotent stem cells (iPSCs) from Pompe patients have facilitated mechanistic studies; however, their utility is restricted by limited patient

Early enzyme replacement therapy in late-onset Pompe disease diagnosed by newborn screening.

OBJECTIVE: Newborn screening (NBS) now allows for early detection and clinical monitoring in infants and children with late-onset Pompe disease (LOPD), potentially identifying those needing early enzyme replacement therapy (ERT). Early symptom onset has been reported in LOPD, but with a paucity of information on when to initiate ERT and on outcomes with early treatment. We report our experience at Duke University Medical Center with seven symptomatic children diagnosed with LOPD by NBS, compound heterozygous for the common IVS1 variant c.-32-13 T > G and a second pathogenic or likely pathogeni

Multidimensional assessment of diaphragmatic dysfunction in late-onset Pompe disease: a prospective cohort study.

Late-onset Pompe disease (LOPD) often leads to progressive respiratory failure due to diaphragmatic involvement. Conventional pulmonary function tests frequently fail to detect early respiratory muscle weakness. We conducted a five-year prospective cohort study of 15 genetically confirmed LOPD patients to evaluate diaphragmatic function using a multidimensional approach that included spirometry, inspiratory/expiratory pressures (MIP/MEP), sniff nasal inspiratory pressure (SNIP), arterial blood gases, and diaphragmatic ultrasound. Data were analyzed using generalized linear mixed models. Forced

Unifying the Communities of Early-Onset Glycogen Storage Disease Type IV and Adult Polyglucosan Body Disease Through a Genetic Prevalence Study of GBE1-Related Disease.

Glycogen storage disease type IV (GSD IV) is an autosomal recessive disorder caused by pathogenic variants in GBE1, resulting in deficient glycogen branching enzyme (GBE) activity and formation of abnormal glycogen ("polyglucosan"). GSD IV manifests across a spectrum of clinical dimensions-including hepatic, neurologic, muscular, and cardiac involvement-which vary in severity. The early-onset forms, historically referred to as Andersen disease, present at different stages ranging from in utero to adolescence. The adult-onset form, referred to as adult polyglucosan body disease (APBD), typicall

Restoration of lysosomal membrane integrity in cell models of Pompe disease depends on fatty acid synthase and its product palmitic acid.

BACKGROUND: Lysosomal storage disorders (LSDs) are rare inherited diseases caused by mutations in genes encoding lysosomal proteins, leading to substrate accumulation and lysosomal dysfunction. Whether this build-up compromises lysosomal membrane integrity remains an open question. In this study, we aimed to investigate the presence of lysosomal membrane damage in LSD cells and to identify mechanisms that could reveal potential strategies for improvement of lysosomal function. METHODS: Primary skin fibroblasts from patients with LSDs were used to evaluate lysosomal integrity and function. Lyso

Epigenetic modulation of the gut-muscle axis in pompe disease: Microbiota fingerprints to cellular and molecular pathomechanisms.

Inter-organ cross-talk is increasingly recognised as a fundamental determinant in the pathogenesis of neurodegenerative and neuromuscular disorders, modulating neuroinflammation, protein misfolding, and cellular dysfunction through systemic mediators such as cytokines, adipokines, and growth factors. In neuromuscular diseases, particularly Pompe disease, muscle degeneration is tightly linked to impaired autophagy and chronic inflammation. Recent evidence highlights the gut microbiota as a key regulator of innate and adaptive immune responses, exerting direct effects on skeletal muscle and supp

Effects of Extended-Release Cornstarch Supplementation on Glycemic Stability and Metabolic Parameters in Korean Patients with Glycogen Storage Disease.

Background/Objectives: Patients with hepatic glycogen storage disease (GSD) require frequent nighttime intake of uncooked corn starch (UCCS) to prevent fasting hypoglycemia, which imposes a substantial burden. Glycosade, an extended-release cornstarch, was developed to prolong overnight glucose availability. However, data regarding South Korean patients are limited. Therefore, we aimed to evaluate the efficacy and safety of Glycosade in South Korean patients with hepatic GSD. Methods: In this single-center prospective observational study, patients with hepatic GSD underwent laboratory evaluati

Antibody formation and efficacy of enzyme replacement therapy in adults with Pompe disease: Unlocking long-term insights.

PURPOSE: Anti-recombinant human acid α-glucosidase (rhGAA) antibodies can develop in adult Pompe patients receiving enzyme replacement therapy (ERT), but their long-term impact on clinical treatment outcomes is currently unclear. METHODS: Pompe patients starting ERT ≥18 years of age were monitored for anti-rhGAA antibody titers (at start of ERT and designated time points thereafter) and its neutralizing activity. Six-minute walk test and forced vital capacity were assessed to evaluate clinical outcomes. RESULTS: We studied the antibody titer course (n = 930) of 111 patients. High peak titers

Development and Preliminary Psychometric Evaluation of the Glycogen Storage Disease Type Ia Functional Assessment Diary (GSD FAD).

PURPOSE: Glycogen storage disease type Ia (GSDIa) is a rare, inherited, autosomal recessive condition with deficiency of glucose-6-phosphatase (G6Pase) characterized by fasting hypoglycemia due to an inability to release glucose from hepatic glycogen stores and other metabolic pathways, requiring frequent consumption of exogenous glucose for survival. Patient-reported outcome (PRO) measures are important to assess GSDIa burden, though no disease-specific PRO for GSDIa exists. This research describes the Glycogen Storage Disease Functional Assessment Diary (GSD FAD), a 31-item PRO developed to

Ubiquitination of glycogen and metabolites in cells and tissues.

Ubiquitin signalling covers a wide range of protein modifications, but its scope may still be underestimated, owing to the ubiquitination of non-proteinaceous substrates, such as sugars, lipids and nucleotides1. The breadth of ubiquitinated non-protein substrates, their abundance and their cellular roles are currently unclear, as current ubiquitinomic and proteomic techniques do not detect non-proteinaceous modifications. Here we report non-protein ubiquitin clipping (NoPro-clipping) as a mass spectrometry-based technique that combines ubiquitin clippases with sortase labelling. Targeted and u

Unmasking Compound Heterozygosity in GYG1 Myopathy: Diagnostic Insights From RNA-Seq and Long-Read Genomics.

Polyglucosan body myopathy type 2 (PGBM2; OMIM #616199) is an autosomal recessive myopathy caused by biallelic variants in GYG1, which encodes glycogenin-1. It is characterized by progressive muscle weakness and PAS-positive, diastase-resistant polyglucosan inclusions on muscle biopsy. We report a 64-year-old woman who developed progressive proximal weakness beginning in her early 50s, with polyglucosan bodies identified on muscle histopathology. Genome sequencing (GS) detected a single heterozygous pathogenic splice-site variant, NM_004130.4(GYG1):c.143+3G>C. Given strong histopathologic evid

Cardiac magnetic resonance in Pompe disease: a systematic literature review.

Pompe disease (PD) is a rare genetic disorder, caused by the lysosomal acid alpha-glucosidase enzyme deficiency leading to accumulation of glycogen, primarily in skeletal and respiratory muscles, but also in the heart. In both known PD forms, namely infantile (IOPD) and late-onset (LOPD), the prevalence and nature of myocardial involvement are still to be clarified. Cardiovascular magnetic resonance (CMR) may help to detect unrecognized alterations contributing to patients' management. This study aimed to systematically review PD CMR features. We conducted a systematic search of three electron

Continuous Glucose Monitoring-Driven Personalization of Cornstarch Therapy in Glycogen Storage Disease: A Retrospective Analysis.

PURPOSE: Hepatic glycogen storage diseases (GSD), including types Ia, Ib, and IXa, are rare inherited disorders characterized by impaired hepatic glucose homeostasis. In GSD Ia, deficiency of glucose-6-phosphatase results in recurrent hypoglycemia and metabolic disturbances. Uncooked cornstarch (UCCS) is the standard therapy due to its slow digestion and sustained glucose release; however, dosing requirements vary according to age, digestion rate, metabolic demand, and lifestyle. Despite the central role of UCCS in long-term management, real-world data describing extended continuous glucose mo

Shifting Towards Empagliflozin First-Line Therapy in Glycogen Storage Disease Type Ib: A Nationwide Real-World Study.

Neutrophil dysfunction and neutropenia are burdensome findings in glycogen storage disease type Ib (GSDIb). Treatment with granulocyte-colony stimulating factor (G-CSF) often corrects neutropenia but fails to improve clinical symptoms like inflammatory bowel disease (IBD). Recently, empagliflozin (EMPA) was shown to correct the neutrophil dysfunction and its clinical consequences. It is increasingly used as first-line monotherapy, but long-term, real-world data are lacking. This nation-wide retrospective study investigated 42 GSD1b patients (36 children) treated with EMPA as first-line monothe

Pompe Disease: Pathogenesis, Molecular Mechanisms, Neurological Aspects, Diagnostics and Modern Therapeutic Approaches.

Pompe disease (PD) is a neuromuscular autosomal recessive disorder caused by mutation in the GAA gene, which encodes acid α-glucosidase (GAA), an enzyme responsible for hydrolyzing glycogen to glucose. Deficiency of this enzyme leads to pathological accumulation of glycogen in almost all tissues of the body, with the most pronounced effects in cardiac and skeletal muscle, as well as in the central nervous system. Two major clinical forms of PD are recognized: infantile-onset PD, characterized by almost complete absence of GAA activity and severe cardiomyopathy and neurological abnormalities, a
