A novel mutation in glycogen storage disease type XI presenting with neonatal cholestasis and infection-triggered hepatic flares.

OBJECTIVES: A rare autosomal recessive disorder known as Fanconi-Bickel syndrome (FBS) is caused by variants in the SLC2A2 gene. This gene is responsible for encoding the glucose transporter 2 (GLUT2) protein. FBS is characterised by hepatomegaly, proximal renal tubular dysfunction, glucosuria, rickets and growth retardation. Here, we present a case of an SLC2A2 gene variant with an unusual clinical presentation involving extreme hepatic transaminase elevations during infectious episodes, a feature not commonly associated with FBS. CASE PRESENTATION: A male infant presented at 12 days of age

Mosaic pathogenic PRKAG2 variant and pathologic diagnosis of glycogen storage disease in an infant with negative whole-genome sequencing.

PRKAG2 syndrome is a rare, autosomal dominant, glycogen storage disease of the heart characterized by a triad of ventricular hypertrophy, progressive conduction abnormalities, and pre-excitation. In severe cases, patients may present with neonatal onset of significant ventricular dysfunction with outflow obstruction resulting in sudden cardiac death or the need for transplantation. We present a unique case of an infant with non-diagnostic whole genome sequencing who developed progressive biventricular hypertrophy, ultimately requiring mechanical circulatory support and heart transplantation. The Value of Anti-Drug Antibody Detection in Discriminating Patients from Healthy Controls and Predicting the Gross Motor Functional State in Patients with Pompe Disease.

Anti-recombinant human acid α-glucosidase (anti-rhGAA) antibody formation is a major challenge in patients with Pompe disease receiving enzyme replacement therapy (ERT). The clinical significance of these antibodies and their detection methods remain uncertain. This study aimed to evaluate the diagnostic and functional relevance of anti-rhGAA antibodies in late-onset Pompe disease (LOPD) and to compare the performance of ELISA and Western blot assays. Fourteen patients with LOPD undergoing ERT and 14 age- and sex-matched healthy controls were studied. Serum anti-rhGAA antibodies and their IgG,

Empagliflozin Restores the Colonic Mucous Barrier by an AMPK/SOX4-Dependent Pathway in Glycogen Storage Disease Type Ib.

Glycogen storage disease type Ib (GSD-Ib) is commonly associated with refractory inflammatory bowel disease (GSD-IBD). Although the SGLT2 inhibitor empagliflozin has demonstrated remarkable clinical efficacy, its direct effects on the intestinal epithelium remain poorly understood. This study aimed to determine whether empagliflozin directly restores the impaired colonic mucus barrier and to identify the underlying molecular pathway. In terms of efficacy, we integrated clinical evaluations of GSD-IBD patients with a novel murine model of epithelial injury induced by a defined bacterial consort

Combined omalizumab and desensitization to control IgE-mediated hypersensitivity in enzyme replacement therapy for late-onset Pompe disease.

BACKGROUND: Pompe disease is a rare, progressive lysosomal storage disorder caused by acid α-glucosidase deficiency, leading to glycogen accumulation, proximal muscle weakness, and respiratory decline. Enzyme replacement therapy (ERT) significantly improves survival and stabilizes motor function, but IgE-mediated hypersensitivity reactions (HSRs) can critically compromise treatment, posing a major clinical challenge. Desensitization protocols allow temporary tolerance to ERT, yet breakthrough reactions may occur, necessitating adjunctive strategies such as omalizumab. RESULTS: We report a 40-

Danon Disease: Understanding the Role of LAMP2 Variants in Cardiomyopathy and Multisystemic Involvement.

Danon disease (DD) is a rare X-linked disorder caused by pathogenic or likely pathogenic variants in the lysosome-associated membrane protein 2 (LAMP2) gene. It primarily manifests as hypertrophic cardiomyopathy, myopathy, intellectual disability, and retinopathy, with males typically experiencing a more severe and early-onset phenotype. The pathophysiology of DD is linked to defective autophagy due to LAMP2 deficiency, leading to the accumulation of glycogen-filled vacuoles within cardiomyocytes and other tissues. This results in progressive cardiac hypertrophy, conduction abnormalities, and

Dysregulation of glycogen metabolism in cardiomyopathy caused by glycogenin-1 (GYG1) missense variants.

CONTEXT: Glycogen storage diseases are inherited disorders of glycogen metabolism and are commonly associated with hypoglycaemia, hepatic dysfunction, or skeletal and cardiac myopathy, depending on the affected enzyme. Glycogen storage disease type 15 (GSD15) is caused by pathogenic variants in GYG1, which encodes glycogenin-1, the auto-glucosylating primer required for glycogen synthesis. GSD15 is characterized by cardiomyopathy and storage of abnormal glycogen and polyglucosan in cardiomyocytes. OBJECTIVE: To understand the pathobiology of GSD15, we investigated the storage material in hear

Unfavorable cardiovascular risk profile without increased event prevalence in late-onset Pompe disease: an individually matched cohort study.

BACKGROUND: It is unclear whether cardiovascular risk factors are more common or if cardiovascular disease occurs more frequently in adults with late-onset Pompe disease (LOPD) than in the general population. We investigated the prevalence of cardiovascular risk factors and cardiovascular events in adults with LOPD compared to an individually matched control cohort. METHODS: Adults with LOPD were individually matched to control subjects, with the same sex, smoking status, age (± 5 years) and BMI (± 5 kg/m2). Assessments included measurement of height, weight, waist-hip circumference, 30 min b

Quantitative Muscle MRI of the Lower Extremities Reveals Different Patterns of Involvement in Classic Infantile and Young Late-Onset Pompe Patients.

With increased survival due to enzyme replacement therapy, children with classic infantile Pompe disease tend to develop a clinical phenotype with pronounced distal muscle weakness, while late-onset patients typically exhibit proximal muscle weakness. This MRI study aimed to characterize lower limb muscle involvement in classic infantile and young late-onset Pompe patients compared to healthy controls, and examine its relationship with motor function. Quantitative MRI (3-point Dixon and multi-echo spin-echo at 3T) was used to assess fat fraction (FF) and water T2 (T2water) in the leg muscles o

Longitudinal Assessment of Muscle Involvement in Late-Onset Pompe Disease Using Quantitative MRI: A Prospective Cohort Study.

BACKGROUND: Late-onset Pompe disease (LOPD) is a progressive metabolic myopathy characterised by lysosomal glycogen accumulation and leading to secondary disruptions in autophagy and cellular energy metabolism. While enzyme replacement therapy (ERT) has improved outcomes, early detection remains critical, because irreversible muscle damage often precedes overt clinical symptoms. This study investigates whether quantitative MRI (qMRI) can detect subclinical disease progression and correlate with clinical decline. METHODS: Over a 2-year period, 25 participants (10 LOPD (disease duration 131 ± 1

Continuous glucose monitoring shows limited impact of daytime cornstarch on glycemia in glycogen storage disease type I.

BACKGROUND AND AIM: Glycogen storage disease type I (GSD I) is characterized by impaired endogenous glucose production, leading to fasting hypoglycemia and multiple metabolic complications. Cornstarch supplementation is a cornerstone of dietary management, primarily to prevent fasting hypoglycemia. However, its role in modulating daytime postprandial glycemia in adults remains unclear. This study aimed to evaluate the impact of mealtime cornstarch supplementation on postprandial glucose dynamics in adults with GSD I under real-life conditions. METHODS AND RESULTS: In this cross-sectional obse

Hepatocyte-specific Cas9-mediated editing of G6pc and Slc37a4 elicits comparable biochemical and regulatory responses between glycogen storage disease (GSD) type Ia and Ib mice.

BACKGROUND/OBJECTIVE: Glycogen storage disease type I (GSD I) is an autosomal recessive inborn error of carbohydrate metabolism. Patients with GSD type Ia and Ib exhibit overlapping and distinct symptoms and complications. Notably, GSD Ia patients show more severe hypertriglyceridemia and higher risk of hepatic tumors than GSD Ib patients. METHODS: Given the liver's pivotal role in these processes, this study utilized hepatocyte-specific CRISPR/Cas9-mediated somatic gene editing to explore the pathophysiological and biochemical adaptations in hepatic GSD Ia and Ib side-by-side. Additionally,
