Austrian Pompe Outcome Consensus (APOC): a national Delphi study.

BACKGROUND: Follow-up assessments form the basis for the continuous optimization of therapy and supportive care on an individual level, for confirming treatment efficacy, and for detecting newly emerging or unexpectedly progressive symptoms early enough to permit timely therapeutic intervention. For Pompe disease, evidence based guidelines on which assessments should constitute the minimum standard and which are required in specific situations only, were missing. Therefore, we started the Austrian Pompe Outcome Consensus (APOC) Study. METHODS: APOC was a Delphi process with two classical onli

Successful desensitization to alglucosidase alfa in a very young infant with Pompe disease.

OBJECTIVES: Enzyme replacement therapy (ERT) with alglucosidase alfa markedly improves outcomes in infantile-onset Pompe disease, yet hypersensitivity reactions may interrupt treatment and require desensitization. We report a very young infant in whom ERT was successfully maintained after allergic reactions through a structured desensitization protocol. CASE PRESENTATION: A 3-month-old male with infantile-onset Pompe disease developed generalized urticaria during the seventh alglucosidase alfa infusion despite premedication and infusion rate adjustment. Skin testing was not feasible due to ag

GMPPB-CDG Results in Lysosomal Dysfunction and Acid Alpha-Glucosidase Deficiency.

GDP-mannose pyrophosphorylase B (GMPPB) deficiency is a congenital disorder of glycosylation due to pathogenic variants of the GMPPB gene. GMPPB catalyzes GDP-mannose synthesis, an early step in multiple glycosylation pathways, including N-glycosylation, O-mannosylation, C-mannosylation, and glycosylphosphatidylinositol-anchor formation. In fibroblasts (N = 3), myoblasts (N = 4) and in muscle biopsies (N = 4) from a total of 7 GMPPB-deficient patients we found evidence of glycogen accumulation, both in cytosol and in lysosome-like vesicles, presence of heterogeneous storage material, and expan

Quantitative MRI Biomarkers for Early Muscle Involvement in Late Onset Pompe Disease.

Early treatment in late-onset Pompe disease (LOPD) increasingly depends on detecting subclinical muscle involvement. Quantitative muscle MRI (qMRI) has emerged as a promising tool in this context. We conducted a multicenter prospective study in LOPD patients (≥ 10-years-old), stratified into early-stage (Walton 0-1, FVC ≥ 80%) and symptomatic (Walton 2-6). Thigh MRIs were acquired at 3 T using T1-weighted and STIR sequences. Fat fraction (FF) and water T2 (wT2) were calculated in 11 different regions using 6-point Dixon and 17-echo multi-echo spin-echo acquisitions, respectively. Functional, r

Glycogen Storage Disease in Twins: When Two Lives Reflect One Silent Battle.

Early recognition of hepatomegaly, hypoglycemia, and elevated liver enzymes in infants is crucial for diagnosing glycogen storage disease. Liver biopsy with PAS-diastase staining provides definitive confirmation. Prompt metabolic management and regular follow-up are essential to prevent progression of fibrosis and long-term hepatic complications.

A comprehensive study on the effect of alglucosidase alpha and immunomodulation on survival, motor and cardiac outcome, creatine kinase and antibody titers in classic infantile Pompe disease: the Monza experience.

We report on 13 classic infantile Pompe patients, including four cross-reactive immunological material negative (31%), treated with alglucosidase alpha (rhGAA) at Fondazione IRCCS San Gerardo, Monza, between 2003 and 2024. Median age at rhGAA initiation was 3.3 months, with nine patients (69%) starting on doses > 20 mg/kg/every other week. With a median follow-up of 6.9 years, the 5- and 10-year survival was 92%. Four patients died, and three became ventilator-dependent. Hypertrophic cardiomyopathy normalized in all patients, though electrocardiogram abnormalities persisted in 36%. Walking was

CHK1 inhibition rescues abnormal glycogen buildup in a Caenorhabditis elegans model for glycogen storage disease III.

Glycogen Storage Disease Type III (GSDIII) is a rare genetic disorder caused by mutations in the AGL gene, leading to a deficiency in the glycogen debranching enzyme. This results in the accumulation of abnormal glycogen in various tissues, causing a range of symptoms, including liver enlargement and hypoglycemia. Current animal models do not fully recapitulate the severe phenotypes observed in patients, highlighting the need for improved model systems. To the best of our knowledge, this study presents the first Caenorhabditis elegans model of GSDIII, which successfully exhibits disease-releva

Insights into immunogenicity and therapeutic strategies to mitigate the immune response in infantile-onset Pompe disease: a comprehensive systematic literature review.

INTRODUCTION: Pompe disease, a rare autosomal recessive metabolic myopathy, is primarily treated with enzyme replacement therapy (ERT); however, ERT response depends on several factors, including ERT initiation age, dose, and cross-reactive immunological material (CRIM) status, especially in infantile-onset Pompe disease (IOPD). AIM: This systematic literature review (SLR) focused on three research questions (1): how CRIM status is determined in patients with IOPD in clinical practice, and how CRIM-negative status impacts outcomes (2); how health professionals use CRIM status to inform their

Successful heart transplantation in a patient with glycogen storage disease.

BACKGROUND: Polyglucosan body myopathy is a type of glycogen storage disease characterized by abnormal glycogen structure formation. Progressive heart failure is the primary cause of mortality in affected patients. CASE SUMMARY: Here, we present a rare case of an Azeri teenage boy with advanced, decompensated heart failure associated with a novel sporadic variant mutation in the RBCK1 gene, displaying a polyglucosan body myopathy phenotype. Following multidisciplinary consensus, the patient underwent successful heart transplantation, resulting in discharge two weeks post-transplantation and e

An exercise intervention in children and young adults with McArdle disease: feasibility, acceptability, and clinical outcomes.

INTRODUCTION: Patients with McArdle disease have reduced exercise capacity. Structured exercise programs in adults with McArdle disease can improve aerobic capacity and strength. However, structured exercise has not been evaluated in younger populations. Our aim was to determine the safety, feasibility, and acceptability of an exercise intervention in children and young adults with McArdle disease. METHODS: Children and young adults aged 5–30 years with McArdle disease were recruited through metabolic clinics in New South Wales, Australia, to complete a remote, supervised 12-week exercise inte

Longitudinal characterization of Gaac.1826dupA mice reveals the cardiac, myopathic and biochemical phenotypes of Pompe disease.

Pompe disease (PD) is a rare autosomal recessive disorder caused by acid α-glucosidase (GAA) deficiency, leading to lysosomal glycogen accumulation. Pathogenic GAA variants result in enzyme dysfunction and glycogen storage in cardiac, skeletal and smooth muscle, as well as in the central nervous system, driving both systemic and neurological manifestations. We have previously characterized a transgenic knock-in (KI) mouse carrying the Gaa c.1826dupA variant to 12 weeks of age, showing that it recapitulates key biochemical and phenotypic features of PD. Here, we extend this analysis to present

Urinary glucose tetrasaccharide tracks disease activity in late-onset Pompe disease.

Late-onset Pompe disease (LOPD) is a progressive metabolic myopathy caused by acid α-glucosidase deficiency, leading to glycogen accumulation in skeletal muscle. Reliable biomarkers for monitoring disease progression and treatment response are lacking. Urinary glucose tetrasaccharide (Glc4), a marker of glycogen turnover, is well established in infantile-onset Pompe disease, but its prognostic value in LOPD under longitudinal real-world conditions remains uncertain. Urinary Glc4 was evaluated in 35 genetically confirmed LOPD patients followed for four years with annual functional assessments,

Feeding-regulated glycogen metabolism drives rhythmic liver protein secretion.

The liver has a key role in inter-organ communication by secreting most circulating plasma proteins. However, the mechanisms governing hepatic protein secretion remain unclear. Here we show that hepatic protein secretion follows a diurnal rhythm regulated by food intake in humans and mice. Using liver microsomal proteomics, we find that proteins implicated in the early secretory pathway, such as protein glycosylation and folding in the endoplasmic reticulum (ER) and Golgi apparatus, exhibit a rhythmic expression profile, which is abolished in Bmal1-knockout mice. Mechanistically, we show that

Clinically important improvements in 6-minute walk distance and forced vital capacity in adults with late-onset Pompe disease switching from alglucosidase alfa to cipaglucosidase alfa plus miglustat in the PROPEL study.

We determined the proportion of adults with late-onset Pompe disease (LOPD) with clinically important changes in 6-minute walk distance (6MWD) and/or forced vital capacity (FVC) after switching from alglucosidase alfa (alg) to cipaglucosidase alfa plus miglustat (cipa+mig) in PROPEL (NCT03729362). This post hoc analysis (95 patients) used published anchor- and distribution-based minimal clinically important differences for 6MWD (in meters for anchor-based; % predicted for distribution-based) and a 3 % threshold for FVC (% predicted). For 6MWD, a higher percentage of patients improved after swi

Correlation of biochemical and imaging markers with hepatic adenoma in patients with glycogen storage disease: a retrospective single-center study.

BACKGROUND: Hepatic adenoma is a serious complication of glycogen storage disease, particularly types I and III. However, noninvasive predictors of adenoma presence and progression for use in clinical practice remain limited. In this retrospective study, we included 93 patients with genetically confirmed glycogen storage disease who underwent liver ultrasonography, FibroScan, shear wave elastography, and routine biochemical testing between December 2020 and March 2025. Patients with and without hepatic adenoma were compared to identify discriminative variables, which were used to construct a l

Gastrointestinal manifestations and enzyme replacement therapy in late-onset Pompe disease: insights from a cross-sectional analysis.

BACKGROUND: Late-onset Pompe disease (LOPD) is an autosomal-recessive disorder caused by acid α-glucosidase (GAA) deficiency, typically presenting after the first year of life and resulting in systemic glycogen accumulation. Beyond motor and respiratory manifestations, gastrointestinal (GI) symptoms have been increasingly reported. Although enzyme replacement therapy (ERT) improves skeletal and respiratory function, its impact on GI symptoms remains unclear. METHODS: There were 124 LOPD patients recruited to complete the Gastrointestinal Symptom Rating Scale (GSRS), together with questionnaire
