Structural dynamics and stability analysis of a novel ARSK variant confirms its role in MPS type X pathogenesis.

Mucopolysaccharidoses (MPS) are heterogeneous group of disorders caused by the deficiency of lysosomal enzymatic activities. Deficiencies in these enzymes are required for proper degradation of glycosaminoglycans (GAGs), thus referred as inborn errors of lysosomal GAG metabolism. Arylsulfatase K (ARSK) has been recently characterized as lysosomal hydrolase involved in GAG degradation removing the 2-O-sulfate group from 2-sulfoglucuronate and has been associated with MPS X. A 12-year-old female presented with developmental delay, progressive musculoskeletal abnormalities, and skeletal dysplasia

High-Power Surgical Laser for Treatment of Oral Manifestations of Mucopolysaccharidosis VI: A Case Report.

Mucopolysaccharidosis type VI (MPS VI), a rare metabolic disorder, results from glycosaminoglycan accumulation in tissues, frequently leading to hyperplastic gingival alterations that necessitate comprehensive intervention. An 11-year-old male patient diagnosed with MPS VI required hospitalization due to complications associated with a ventriculoperitoneal shunt (VPS) valve. A dental evaluation identified bone malformation in the maxillomandibular complex accompanied by pronounced gingival hyperplasia. The surgical team performed a gingivoplasty in the operating room under general anesthesia,

Human iPSC-derived neural progenitor gene therapy improves outcomes in a neuropathic lysosomal disease.

Mucopolysaccharidosis type IIIA (MPS IIIA) is a neuropathic lysosomal storage disorder caused by SGSH mutations leading to heparan sulfate accumulation in the brain. This drives progressive cognitive decline, neuroinflammation, and behavioral abnormalities. No effective disease-modifying therapy exists, as current approaches fail to deliver sufficient enzyme to the brain or halt neurodegeneration. Here, we report a neural stem cell gene therapy that achieves high enzyme delivery and restores cognitive function, likely through integration of healthy neural cells across multiple brain regions. I

Characterization of Cellular Alterations in a Novel NAGLU Enzyme-Deficient Cellular Model Generated from U-87MG Cell Line.

Mucopolysaccharidosis IIIB (MPS IIIB) is an inherited metabolic disorder caused by mutations in NAGLU gene, resulting in the accumulation of heparan sulphate in lysosomes and cell membranes. This accumulation leads to cellular dysfunction, apoptosis, tissue damage, and organ failure. MPS IIIB is considered a neurodegenerative disease that causes motor impairment, developmental delay, sleep disturbance, and dementia. This is usually diagnosed around 3-4 years of age with an incidence of 1 per 200,000 live births. Current research focuses on cellular defects in the central nervous system, partic

Synthesis of the endogenous non-reducing end heparan sulfate disaccharides for newborn screening and diagnosis of mucopolysaccharidosis types I and II.

There is ongoing newborn screening (NBS) for mucopolysaccharidoses types I and II (MPS I and MPS II) because initiation of treatment early in life leads to better clinical outcomes. NBS starts with measurement of the relevant enzymatic activities in dried blood spots (DBS) on NBS cards. A second-tier test is carried out to eliminate false positives when enzymatic activity is low. In MPS I and MPS II, heparan sulfate accumulates, and previous studies have shown that the non-reducing end disaccharide fragments found endogenously in patient DBS and urine are elevated in patients but not in false

[Anterior segment changes in mucopolysaccharidoses].

BACKGROUND AND AIM: Mucopolysaccharidoses (MPS) are rare lysosomal storage disorders in which the accumulation of undegraded glycosaminoglycans leads to progressive multiorgan and ocular changes. The anterior segment of the eye is particularly affected, with type-specific, distinct corneal and chamber angle alterations. The aim of this study was to characterize anterior segment changes in different MPS types based on structural and functional examinations using clinical and imaging parameters. METHODS: In a clinical cross-sectional study, patients with MPS (types I, II, III, IV, VI) were comp

Beyond Upper Airway Involvement: Evidence of Intrinsic Lung Disease in a Mouse Model of Mucopolysaccharidosis I.

Almost all patients with mucopolysaccharidosis (MPS) develop respiratory dysfunction of varying severity during disease progression. While respiratory disease in MPS has traditionally been attributed to upper airway obstruction caused by glycosaminoglycan (GAG) accumulation in the trachea and bronchi, involvement of the intrapulmonary conducting airways and lung parenchyma remains poorly defined. Here, we characterised lung disease in a mouse model of MPS I using a combination of non-invasive X-ray Velocimetry (XV) functional lung imaging and gold-standard flexiVent respiratory mechanics testi

Intravenous Delivery of a Gene Therapy Vector that Expresses an Antitransferrin Receptor 1 Nanobody-I2S Fusion Protein Rescued Central Nervous System Lysosomal Burden in Mucopolysaccharidosis II Mice.

Hunter syndrome, also known as mucopolysaccharidosis type II, is an X-linked lysosomal storage disease caused by the deficiency of functional iduronate-2-sulfatase (I2S) enzyme, leading to the accumulation of lysosomal glycosaminoglycans (GAGs) affecting multiple organs. Two-thirds of patients have central nervous system (CNS) manifestations. The current standard of care, enzyme replacement therapy (ERT) via weekly intravenous delivery of recombinant human I2S (rhI2S), does not address the neuropathy in the CNS due to its inability to cross the blood-brain barrier (BBB). Next-generation ERTs c

The Critical Role of Fractionated Urine Glycosaminoglycans in the Evaluation of Mucopolysaccharidosis Type II in Four Unrelated Families.

Since 2015, Ann and Robert H. Lurie Children's Hospital has performed diagnostic testing for infants who screen positive for mucopolysaccharidosis type II (MPS II) on the Illinois newborn screen. Preliminary diagnostic testing includes measurement of plasma iduronate-2-sulfatase enzyme activity and urinary glycosaminoglycan analysis, followed by sequencing of the IDS gene. These diagnostic studies rule out or confirm a diagnosis of MPS II for most cases. However, occasionally, a diagnosis of MPS II remains uncertain following initial diagnostic studies. In this case series, we describe our exp

A novel IDS variant associated with an isolated ocular phenotype in Hunter syndrome.

INTRODUCTION: Hunter syndrome (mucopolysaccharidosis type II, MPS II) is an X-linked lysosomal storage disorder caused by iduronate-2-sulfatase (IDS) mutations and is classically associated with multiple-organ-systems involvement. Ocular findings are usually reported in conjunction with systemic manifestations, and isolated ocular presentations have not been well characterized. Here, we report a novel hemizygous IDS variant in a patient who initially presented with isolated corneal and retinal pathology and was subsequently diagnosed with attenuated Hunter syndrome. METHODS: Comprehensive oph
