Molecular Dynamics Reveals a Glycan Conformation Underlying IDUA Dysfunction in Attenuated MPS I.

Mucopolysaccharidosis Type I (MPS I) is a rare autosomal recessive lysosomal storage disorder caused by mutations in α-ʟ-iduronidase (IDUA) enzyme, a glycosylated hydrolase responsible for glycosaminoglycan (GAG) degradation. Pathogenic variants are associated with a broad clinical spectrum, from severe Hurler syndrome to attenuated Scheie phenotype. The missense mutations L490P and P533R cause attenuated-intermediate MPS I, retaining residual enzyme activity. To investigate the structural mechanisms underlying these phenotypes, we performed all-atom molecular dynamics simulations -18 trajecto

Inhibition of heparan sulfate synthesis by resveratrol in mucopolysaccharidosis type I and IIIB.

Mucopolysaccharidoses (MPS) are a group of pediatric disorders that are distinguished based on a lysosomal enzyme deficit leading to accumulation of glycosaminoglycans (GAGs). One of the GAGs is heparan sulfate (HS) which accumulates in cells inside the body, but the most devastating symptoms are caused by its accumulation in the central nervous system. Resveratrol, a natural compound, reduces the HS accumulation in both MPS I and MPS IIIB. In this study, we investigated the effects of resveratrol on HS metabolism in cellular models of MPS I and MPS IIIB, with particular focus on its role in G

Autolysosomal enhancer GHF201 attenuates pathophysiology in a mucopolysaccharidosis type I mouse model.

Mucopolysaccharidosis type I (MPS-I) is a hereditary lysosomal storage disorder caused by deficiency in the lysosomal enzyme α-L-iduronidase (IDUA) leading to pathogenic over-accumulation of glycosaminoglycans (GAGs), major constituent of connective tissue and mediators of cell signaling. We tested the therapeutic efficacy of the autolysosomal activator GHF201 in the Idua -/- mouse model of MPS-I. We show that GHF201 reduced GAGs and increased cathepsin activity in Idua -/- mouse embryonic fibroblasts (MEFs) and brain cells. In vivo GHF201 improved aberrant locomotion and behavior, reduced int

Hypomorphic IDUA genotypes are associated with retinitis pigmentosa in individuals without syndromic mucopolysaccharidosis type I.

Bi-allelic variants in IDUA classically cause mucopolysaccharidosis type I (MPS I), a multisystem lysosomal storage disorder. This study describes 14 individuals from 12 families presenting with retinitis pigmentosa, later attributed to bi-allelic IDUA genotypes. Notably, seven individuals, despite comprehensive systemic assessments, showed no features consistent with MPS I at last assessment (aged 46-73 years). Leukocyte α-L-iduronidase activity was reduced but not abolished across all individuals assessed (n = 9), supporting a shared mechanistic basis for the mild, retina-predominant, or exc

Expeditious synthesis of fluorogenic enzyme substrates for mucopolysaccharidosis types I and II.

New, rapid and inexpensive syntheses of 4-methylumbelliferyl fluorogenic substrates for α-L-iduronidase and iduronate-2-sulfatase, have been developed from a common β-D-glucuronosyl fluoride intermediate. The syntheses feature an efficient purple light-promoted, CCl4-free, one-pot Ferrier photobromination/radical debromination sequence for the key C-5 epimerization. The substrates are useful chemical tools for studying mucopolysaccharidosis type I (MPS I) and type II (MPS II).

Isolated Orchialgia as the Sole Manifestation of an Occult L5 Pars Defect: A Case Report.

BACKGROUND: Type 2 diabetes is a major and growing public health challenge worldwide, contributing substantially to premature mortality and disability. Increasing evidence suggests that long-term exposure to air pollution, plays an important role in the development and progression of type 2 diabetes. Countries with high levels of ambient air pollution may therefore experience a disproportionate diabetes burden attributable to environmental exposure. In Iran, where both diabetes prevalence and air pollution levels are high, national- and subnational-level evidence on the burden of type 2 diabet

Influence of Room Illumination on Corneal Densitometry.

IMPORTANCE: Newborn screening (NBS) for lysosomal storage disorders (LSDs) is becoming an increasing concern in public health. However, the birth prevalence of these disorders is rarely reported in the Chinese population, and subclinical forms of diseases among patients identified by NBS have not been evaluated. OBJECTIVE: To evaluate the birth prevalence of the 6 LSDs in the Shanghai population and determine subclinical forms based on clinical, biochemical, and genetic characteristics. DESIGN, SETTING, AND PARTICIPANTS: This cohort study included 50 108 newborns recruited from 41 hospitals

Molecular Profile of Mucopolysaccharidosis Type I Patients in Brazil.

Mucopolysaccharidosis type I (MPS I) is an autosomal recessive disease caused by mutations in the IDUA gene, resulting in decreased activity of the lysosomal enzyme α-l-iduronidase (IDUA) and consequent accumulation of glycosaminoglycans in the lysosomes. There are more than 300 disease-causing variants reported in the IDUA gene, and the mutational profile varies considerably worldwide. In this study, we performed molecular analysis on 119 MPS I patients, representing the largest Brazilian cohort studied so far. Forty-seven different mutations were identified in our sample, and 13 of them are

Case Report: Compound heterozygous mutations in the IDUA gene causing mucopolysaccharidosis type I with uterine developmental abnormality.

Mucopolysaccharidosis (MPS) represents a group of rare inherited metabolic disorders characterized by abnormal accumulation of glycosaminoglycans (GAGs) due to deficiencies of lysosomal enzymes. Mucopolysaccharidosis type I (MPS I) is caused by biallelic pathogenic variants in the IDUA gene and is inherited in an autosomal recessive pattern. The IDUA gene is located on chromosome 4p16.3 and encodes the lysosomal enzyme α-L-iduronidase, which plays a critical role in the degradation of GAGs, particularly dermatan sulfate and heparan sulfate. Reduced or absent IDUA enzymatic activity leads to th

Mucopolysaccharidosis type I-Hurler (MPS I-H) is a severe lysosomal storage disorder caused by α-L-iduronidase (IDUA) deficiency, leading to glycosaminoglycan (GAG) accumulation and progressive multisystem dysfunction, including the central nervous system (CNS). Early hematopoietic stem cell transplantation remains the primary disease-modifying intervention for MPS I-H despite its clinical challenges. Conventional enzyme replacement therapy (ERT), which has limited CNS efficacy due to blood-brain barrier (BBB) restrictions, is being addressed by the development of novel BBB-penetrating ERT pla

Beyond Detection: Comparing State-Based Newborn Screening Methods for Effective Mucopolysaccharidosis I Diagnosis.

Mucopolysaccharidosis type I (MPS I) results in the accumulation of glycosaminoglycans (GAG) and, for the purposes of newborn screening, is differentiated into two forms: severe (Hurler syndrome) versus attenuated (encompassing Scheie and Hurler-Scheie syndromes). MPS I was added to the federal Recommended Uniform Screening Panel for newborn screening (NBS) in 2016, and as of December 2025, 45 of 54 programs in the United States (US) screen for MPS I. Within the newborn screening program, a second-tier analysis of GAG is thought to reduce false-positive rates, particularly through mitigating t

Liposomal CRISPR/Cas9-Mediated Local Genome Editing for Joint Disease in Mucopolysaccharidosis Type I.

Background/Objectives: Mucopolysaccharidosis type I (MPS I) is a lysosomal storage disorder caused by α-L-iduronidase (IDUA) deficiency, leading to progressive glycosaminoglycan (GAG) accumulation and severe joint involvement. Gene editing represents a promising alternative to restore localized enzyme production. Therefore, this study aimed to evaluate the feasibility, efficacy, and safety of in situ genome editing through intra-articular administration of a nonviral CRISPR/Cas9 system to increase localized IDUA expression in an MPS I mouse model. Methods: Cationic liposomes were formulated to

Trigger Digits in Mucopolysaccharidosis Type I: Clinical Characteristics, Surgical Outcomes, and Histopathological Findings.

PURPOSE: Mucopolysaccharidosis type I (MPS-I), a genetic lysosomal storage disorder, often leads to musculoskeletal deformities such as trigger digits (TDs). Despite systemic therapies, connective tissues such as bone, muscle, and tendons remain difficult for the enzyme to reach, and TDs develop. We performed a retrospective study to assess the prevalence of TDs, evaluate surgical outcomes and recurrence, and explore accumulation of foam cell macrophages contributing to TD development in patients with MPS-I. METHODS: Data were obtained retrospectively from a cohort of 33 children born between

First-in-human intracisternal dosing of RGX-111 in severe MPS I is well tolerated and generates sustained neurodevelopment without HSCT.

Mucopolysaccharidosis type I (MPS I), caused by α-L-iduronidase enzyme (IDUA) deficiency, manifests multisystemic symptoms from tissue accumulation of undegraded glycosaminoglycans. Severe MPS I (Hurler syndrome) is associated with developmental delay and loss of neurocognition. Although IDUA enzyme replacement therapy and allogeneic hematopoietic stem cell transplantation (HSCT) are standard therapies for Hurler syndrome, gene therapy represents potential alternative treatment of neurodevelopmental disease. We report more than 5 years of clinical, biochemical, and neurocognitive outcomes foll

Adult disease burden in patients with mucopolysaccharidosis type I H (Hurler syndrome): A comprehensive literature review with patient case analysis.

INTRODUCTION: Mucopolysaccharidosis type I H (MPS IH; Hurler syndrome) is a rare autosomal recessive disease that causes deficiency of alpha-L-iduronidase - an enzyme responsible for catabolizing glycosaminoglycans (GAGs). While historically considered a pediatric disease with an overall median life expectancy of 8.7 years, advances in MPS IH treatment options have extended patient lifespans into adulthood; however, long-term progression of MPS IH currently remains minimally described. This study aims to provide a comprehensive literature review about the adult disease burden of MPS IH as vali

Delayed diagnosis of mucopolysaccharidosis type I in a patient with spinopelvic instability, short stature, and skeletal dysplasia.

Pathogenic variants in IDUA, encoding α-L-iduronidase (IDUA), cause the autosomal recessive lysosomal storage disorder mucopolysaccharidosis type I (MPS I). Deficiency of α-L-iduronidase leads to the accumulation of partially degraded glycosaminoglycans in various tissues, including the musculoskeletal system. Clinically, typical skeletal manifestations are dysostosis multiplex, joint contractures, and low BMD. Mucopolysaccharidosis type I represents a spectrum of disease severity. Especially in attenuated disease, establishing an accurate diagnosis and initiating disease-specific therapy, inc
