Healthcare-associated infections in Singapore's long-term acute care hospitals: results from the first national point prevalence survey.

BACKGROUND: While healthcare-associated infection (HAI) surveillance in Singapore's acute care hospitals is well established, comparable data from long-term acute care hospitals (LTACHs), known as community hospitals, are limited. We conducted the first national point prevalence survey (PPS) to determine HAI prevalence in Singapore LTACHs. METHODS: A standardized protocol adapted from the Singapore acute care hospital HAI PPS protocol, which was adapted from the European Centre for Disease Prevention and Control protocol for acute care hospitals, was implemented across 9 LTACHs. Adult inpatie

Spontaneous fundal rupture of an unscarred uterus in the second trimester of twin gestation, presenting with sepsis: a case report.

BACKGROUND: Spontaneous rupture of the uterus during the early weeks of gestation is rare. When it occurs, it is a life-threatening obstetric emergency. Most uterine ruptures happen in scarred uteri and during the late third trimester. This case is unique because it involves a spontaneous rupture in an unscarred uterus during the second trimester. Identified risk factors include antepartum hemorrhage in a previous pregnancy, history of retained placenta, prior manual vacuum aspiration for incomplete abortion, and use of herbal medicine. This case adds to the limited global literature indicatin

Midline and central line catheters usage and blood stream infection: Trends and insights from 2016 to 2024.

BACKGROUND: Central line-associated bloodstream infections (CLABSIs) remain a major source of hospital-related morbidity and mortality. Replacing central venous catheters (CVCs) with midline catheters (MLCs), when clinically appropriate, has been proposed as a strategy to reduce the incidence of CLABSI cases. We hypothesized that increased MLC utilization would correlate with decreased CLABSI incidence. RESEARCH QUESTION: Is increased use of midline catheters, in place of central venous catheters, associated with reduced CLABSI incidence over time? STUDY DESIGN AND METHODS: We conducted a re

Incidence and hospital outcomes of acute adrenal crisis in a Swiss-wide cohort study.

BACKGROUND: Adrenal crises (AC) are life-threatening emergencies for which epidemiological data remain scarce, particularly in older adults. We analyzed nationwide data on AC-related hospitalizations in Switzerland between 2012 and 2022 to determine incidence, clinical outcomes, and predictors of adverse events, emphasizing differences across age groups and etiologies of adrenal insufficiency. METHODS: We conducted a nationwide retrospective cohort study using hospital discharge data linked to the national death registry. The primary outcome was the incidence of AC hospitalizations; secondary

Health Utilities Associated with Pneumococcal Diseases in Children and Adults: A Targeted Literature Review and Meta-analysis.

BACKGROUND: Pneumococcal disease and post-meningitis sequelae (PMS) impact health-related quality of life (HRQoL). However, health utility estimates for these conditions vary considerably in the literature, which may affect the outcomes of economic evaluations. OBJECTIVES: The aim was to generate pooled estimates of quality-adjusted life year (QALY) decrements per episode for pneumococcal disease and pooled health state utility value (HSUV) estimates for PMS in children and adults. METHODS: Global targeted literature reviews were conducted in MEDLINE in June 2024 to identify original studies

Serum vitamin B12 levels and 90-day outcomes in hospitalized patients with dementia: A cohort study.

Elevated serum vitamin B12 levels have emerged as a paradoxical prognostic marker associated with increased mortality across various clinical settings, yet their significance in hospitalized dementia patients remains unexplored. Given the high vulnerability of this population, we aimed to investigate the association between elevated serum vitamin B12 levels and all-cause mortality. This retrospective cohort study evaluated 90-day outcomes in hospitalized adults with dementia who had serum vitamin B12 levels measured within 7 days of admission (2010-2024). Patients were categorized into exposur

Subnational analysis of pediatric sepsis incidence and mortality from official records in Chile and Mexico: a longitudinal study from 2014 to 2024.

BACKGROUND: Pediatric sepsis is a leading cause of global morbidity and mortality, yet high-resolution, granular subnational assessments remain scarce. Chile and Mexico are the only countries in Latin America that possess robust vital registration systems and open access databases with marginal levels of missing cases. This offers a unique opportunity to quantify the subnational burden of pediatric sepsis, identify healthcare system constrictions, and guide targeted public health interventions. METHODS: This retrospective longitudinal study analyzed official hospital discharge and non-fetal d

Phenotypes and Outcomes of Heart Failure First Diagnosed During Sepsis: A Single-Center Retrospective Cohort Study Using ICD-10-Based Subtype Classification.

Background: Sepsis-associated cardiac dysfunction has historically been characterized as predominantly systolic, yet the burden of heart failure with the preserved ejection fraction (HFpEF) phenotype among patients with heart failure (HF) first documented during sepsis remains poorly defined. Methods: In a single-center retrospective cohort of adults (≥18 years) admitted to a tertiary-care hospital between 1 January 2022 and 31 December 2024, we identified patients whose first sepsis diagnosis (ICD-10) coincided with their first HF diagnosis (ICD-10 I50.x) during the same encounter, with no pr

Long-term risk of dementia following encephalitis: a large-scale retrospective cohort study of electronic health records.

BACKGROUND: Encephalitis is an acute inflammation of the brain parenchyma, typically caused by microbial infections or autoimmune mechanisms, and represents a potentially life-threatening neurological condition affecting thousands of individuals each year in the United States. Although survivors may experience persistent neurological sequelae, long-term association of encephalitis and subsequent dementia and related diagnoses across different age groups and encephalitis etiologies remains poorly characterized. This study aimed to quantify the association between encephalitis and the risk of de

Low LDL cholesterol and risk of bacterial and viral infections: observational and Mendelian randomization studies.

AIMS: Low levels of LDL cholesterol may be associated with risk of infectious disease. We tested the hypothesis that low LDL cholesterol due to genetic variation in the LDLR, PCSK9, and HMGCR genes and a polygenic LDL cholesterol score is associated with risk of infectious diseases in the general population. METHODS AND RESULTS: Using observational and Mendelian randomization designs, we examined associations of low plasma LDL cholesterol with risk of bacterial and viral infections in 119 805 individuals from the Copenhagen General Population Study/Copenhagen City Heart Study, 468 701 from th

Diagnostic disparities in inborn errors of immunity: From clinical suspicion to diagnosis.

BACKGROUND: Emerging evidence suggests that inborn errors of immunity (IEI) are underdiagnosed among underserved populations. However, there remains a lack of national studies evaluating diagnostic disparities in IEI. OBJECTIVE: We examined disparities in the timely IEI diagnosis and related health outcomes. METHODS: A retrospective analysis was performed of a US national claims database (years 2007 to 2021). Participants included patients diagnosed with an "unspecified immune deficiency" (uID) and presented with IEI-related symptoms, who later received an IEI diagnosis (n = 1429). We quanti

The real-world analysis of adverse events with azacitidine: a pharmacovigilance study based on the FAERS and WHO-VigiAccess databases.

BACKGROUND: Azacitidine is used to treat myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). It acts as a cytosine analog and DNA methyltransferase inhibitor, inducing DNA hypomethylation to reverse epigenetic modifications and restore normal gene expression. However, adverse events (AEs) associated with azacitidine are mainly reported in clinical trials, with limited real-world evidence. This study aims to assess the AE profile of azacitidine by utilizing data from the Food and Drug Administration (FDA) Adverse Event Reporting System (FAERS) and WHO-VigiAccess databases. METHODS

Examining air medical transport in interfacility emergency general surgery transfers to a quaternary center.

BACKGROUND: Regionalized care has increased interfacility transfers, particularly for emergency general surgery (EGS) patients requiring urgent, specialized care. Air medical transport (AMT) provides rapid transfer with critical care capabilities and is increasingly used for EGS, but its time-saving benefits remain uncertain. This study aims to identify the population of EGS patients undergoing AMT and the distance thresholds where AMT is faster than ground transport. METHODS: We conducted a retrospective cohort and analyzed EGS patients transferred to our quaternary center from January 2021
