Association between statin therapy as primary prevention and mortality in adults 50 years and older with chronic kidney disease without other indications.

BACKGROUND: Chronic kidney disease (CKD) increases cardiovascular and mortality risk. Guidelines recommend statins for primary prevention in individuals aged ≥50 years with estimated glomerular filtration rate (eGFR) <60 mL/min/1.73 m2, but implementation and outcomes remain unclear. This study examined statin use and its association with all-cause mortality in individuals with CKD and no other indication for statin therapy. METHODS: A retrospective cohort study was conducted using healthcare data derived from Region Halland, Sweden. Adults aged 50-89 years with ≥2 eGFR measurements <60 mL/mi

Syncope as the Sole Manifestation of a Saddle Pulmonary Embolism in a Hemodynamically Stable Patient.

Syncope is a common emergency department presentation with many potential causes. Pulmonary embolism (PE) typically presents with chest pain, dyspnea, and abnormal vital signs, but has been reported without typical signs or symptoms. A 77-year-old man experienced sudden syncope while carrying water cartons. He denied prodromal symptoms or cardiopulmonary complaints. His history included hypertension, chronic kidney disease, glaucoma, and hyperlipidemia. However, he denied major risk factors for PE. On arrival, he was hemodynamically stable, with only a forehead abrasion on exam. Labs revealed

LDL-C target achievement after adding evinacumab in 2 patients with autosomal recessive hypercholesterolemia.

BACKGROUND: Autosomal recessive hypercholesterolemia (ARH) is a rare form of genetic hypercholesterolemia consequent to pathogenic variants in the low-density lipoprotein receptor adaptor protein 1 (LDLRAP1) gene, coding for a protein responsible for moving low-density lipoprotein-receptor (LDL-R) to its site of activity. ARH is characterized by very high levels of low-density lipoprotein cholesterol (LDL-C), leading to aggressive and frequently premature atherosclerotic cardiovascular disease (ASCVD). Lowering of LDL-C is the main target of treatment; however, classical lipid-lowering agents,

In vivo base editing gene therapy for heterozygous familial hypercholesterolemia: a phase 1 trial.

Heterozygous familial hypercholesterolemia is a common genetic disorder characterized by lifelong elevation of serum low-density lipoprotein cholesterol (LDL-C) and premature atherosclerotic cardiovascular disease. YOLT-101 is an investigational in vivo gene therapy that uses adenine base-editing technology, delivered via GalNAc-modified lipid nanoparticles to inactivate PCSK9 and achieve sustained LDL-C reduction. Here we report interim results from an ongoing clinical trial evaluating primary (safety and tolerability) and secondary (lowering of PCSK9 and LDL-C levels) outcomes of a single in

SiRNA-based inactivation of Angptl3 alleviates high-fat diet-induced MAFLD and atherosclerosis in LDLR-deficient hamsters.

BACKGROUND: Angiopoietin-like protein 3 (Angptl3), a factor secreted by the liver, inhibits lipoprotein lipase and other lipases by forming a complex with Angptl4 and 8. However, whether inhibition of Angptl3 can alleviate hepatic lipid accumulation and atherosclerosis remains unclear. Therefore, this study explored the effect of small interfering RNA (siRNA)-based inactivation of Angptl3 on metabolic dysfunction-associated fatty liver disease (MAFLD) and atherosclerosis in male LDLR-deficient hamsters. METHODS: Recombinant adeno-associated virus serotype 9 (AAV9) encoding Angptl3-siRNA or emp

Inflammatory and cholesterol risks and rates of major cardiovascular events among patients with atherosclerotic cardiovascular disease in routine care.

AIMS: Inflammation and hyperlipidaemia play a pivotal role in atherosclerotic cardiovascular disease (ASCVD), and inflammatory risk may outweigh cholesterol risk among statin-treated patients. However, it is unclear how these risks relate to ASCVD outcomes in a real-world population. METHODS AND RESULTS: Observational study of 39 638 ASCVD adults in Stockholm's healthcare (2007-21) who underwent routine testing for high-sensitivity C-reactive protein (hsCRP) and low-density lipoprotein cholesterol (LDL-C). Groups were defined by LDL-C (≥1.8 vs. < 1.8 mmol/L) and hsCRP (≥2 vs. < 2 mg/L): as lo

Effect of lumbrokinase on a rat model of hyperlipidemia-induced erectile dysfunction.

BACKGROUND: Erectile dysfunction (ED), which frequently occurs in individuals with hyperlipidemia, seriously affects quality of life. Lumbrokinase exhibits multiple pharmacological effects. There is currently a lack of evidence to support the use of lumbrokinase in treating ED. We investigated the effect and mechanism of lumbrokinase in a rat model of hyperlipidemia-induced erectile function (HLED). METHODS: There were 28 male Sprague-Dawley rats included. Eight rats received a standard diet as controls; 20 rats were fed a high-cholesterol diet to induce hyperlipidemia. After 8 weeks, the apo

Two distinct healthcare utilisation trajectories prior to a COPD diagnosis in a multi-ethnic Asian population: a comparison of transient and chronic users.

INTRODUCTION: Chronic obstructive pulmonary disease (COPD) imposes substantial clinical and economic burdens. Early detection can allow for monitoring and timely treatment to slow its progression. Tracking of patient trajectories prior to their diagnosis can inform the timing and targeting of interventions. We aimed to identify pre-diagnosis healthcare utilisation patterns in a COPD cohort and profile the associated subgroups. METHODS: We conducted a retrospective cohort study of patients with a new inpatient or specialist outpatient clinic (SOC) diagnosis of COPD from 2018 to 2019 in a regio

Barriers and Facilitators to Implementing Guideline Recommendations for Key Aspects of Familial Hypercholesterolemia Management in Australia: Primary Care Integration, Paediatric Management, and Treatment Adherence.

BACKGROUND & AIM: Familial hypercholesterolaemia (FH) is a genetic condition that causes high plasma levels of low-density lipoprotein cholesterol, significantly increasing the risk of premature atherosclerotic cardiovascular disease. FH remains underdiagnosed and undertreated, despite the existence of high-level evidence and clinical practice guidelines. Both under-prescribing and non-adherence to medications have been identified as contributing factors. The aim of this study was to identify the barriers and facilitators or potential solutions to the implementation of improved FH management i

Founder Effect of the c.500G>A Variant in South Asian Patients With Inherited GPD1 Deficiency: Report on 16 Patients and Variant Review.

Only a few studies describe five (or more) patients with inherited glycerol-3-phosphate dehydrogenase 1 (GPD1) deficiency, often termed transient infantile hypertriglyceridemia (HTGTI). We report 18 additional patients with HTGTI (confirmed molecular diagnosis in 16, a variant of uncertain significance in two), most of whom presented in infancy with hepatomegaly. A significant negative correlation was noted between age at presentation and serum triglyceride levels. Except for two, all our patients have homozygous GPD1 variants, wherein the c.500G>A (p.Gly167Asp) variant was the most common (10

Paeoniflorin suppresses the phenotypic transformation of vascular smooth muscle cells during atherosclerosis by regulating the MAPK/NF-κB pathways and ABCA1 expression.

BACKGROUND: The pathological environment of atherosclerosis (AS) is characterized by hyperlipidemia and chronic inflammation, which cause increased heterogeneity among vascular smooth muscle cells (VSMCs). Owing to its lipid-regulating and anti-inflammatory effects, paeoniflorin (Pae) inhibits VSMC phenotypic transformation, making it a promising candidate for AS treatment. METHODS: Mouse aortic VSMCs were treated with oxidized low-density lipoprotein (ox-LDL) and Pae, and the effects on cell phenotype were examined. An AS model was established by feeding ApoE-/- mice a high-fat diet. Serum a

Temporal Trends in Stroke Prevalence and Associated Risk Factors in the United States: Insights From National Health and Nutrition Examination Survey (NHANES) 1999-2020.

INTRODUCTION: Stroke is a leading cause of mortality and disability in the United States (US). Although hypertension, diabetes, and obesity are considered traditional risk factors for stroke, the extent to which these factors have changed their relationship and the prevalence of stroke has not been well-characterized since 1999. Therefore, the objective of this study is to investigate the temporal trends in stroke prevalence among US adults and to determine how stroke risk factors have changed over the past 20 years. METHODS: To achieve this objective, we conducted a secondary analysis of NHA

Inflammatory signalling in diabetic cardiomyopathy: molecular mechanisms and potential therapeutic strategies.

Diabetes mellitus and the associated increased risk of cardiovascular disease is a major health-care issue worldwide. Diabetic cardiomyopathy, a complication of diabetes mellitus, is driven primarily by hyperglycaemia and hyperlipidaemia, which promote cardiac oxidative stress, mitochondrial dysfunction and pathological cardiac remodelling, leading to impaired cardiac function and eventual heart failure. Over the past 30 years, research on diabetic cardiomyopathy and other diabetes-associated cardiovascular diseases has focused on the role of chronic inflammation. Inflammation is a complex pro
