Sex-specific disparities in lipid-based prediction of major cardiovascular events.

AIMS: Lipid-based markers have a profound prognostic potential concerning cardiovascular diseases. However, differences in lipid levels between women and men are evident, and it is unknown whether the differences also extend to the predictive power of the lipid markers. Therefore, this study aims to evaluate potential sex differences concerning the risk estimation of five topical lipid markers: low-density lipoprotein cholesterol (LDL-C), remnant cholesterol, apolipoprotein A-1 (ApoA-1), the triglyceride/HDL cholesterol (TG/HDL) ratio, and ceramide scores (CERT1 and CERT2). METHODS AND RESULT

Targeting Triglycerides in Cardiovascular Disease Prevention: Evidence, Mechanisms, and Emerging Therapies.

PURPOSE OF REVIEW: The goal of this review is to evaluate the evolving role of triglycerides (TGs) and TG-rich lipoproteins (TRLs) in cardiovascular disease (CVD) risk and prevention. We examine the mechanistic rationale, genetic and epidemiological evidence, and therapeutic potential of targeting TGs in residual risk reduction, particularly in high-risk populations. RECENT FINDINGS: Emerging data from Mendelian randomization studies and large clinical cohorts support a causal link between elevated remnant lipoproteins and atherosclerotic CVD, in which apolipoprotein B may be the principal dr

Genome-wide association study identifies novel and confirms established loci associated with serum lipids levels in Brazilians.

Dyslipidemia is an important risk factor for cardiovascular diseases and can result from genetic and environmental influences. Most genome-wide association studies (GWAS) have been conducted in European populations, limiting our understanding of polymorphisms involved in lipid levels in admixed populations such as Brazilians. Therefore, this study aimed to identify genetic variants associated with lipid traits and develop polygenic risk scores (PRS) for dyslipidemia prediction in a large cohort of Brazilians. We performed GWAS of lipid phenotypes using 19,016 Brazilian individuals previously g

Genetic determinants of plasma lipids in Greenlanders.

PURPOSE OF REVIEW: Greenlanders differ from other populations in terms of traditional lifestyle and genetic architecture. This might have a great impact on lipid levels in the population and the spectrum of genetic variants associated with lipid traits. Here, we review recent advances in lipid genetics in Greenlanders and highlight the potential of moving from single lipid trait analyses to more comprehensive lipidomic profiling. RECENT FINDINGS: Genetic association studies in the Greenlandic population have identified variants, including PCSK9 (rs12117661), LDLR (rs730882082), and SI (rs7814

Effects of Vitamin C and/or E Supplementation on Glycemic Control and Cardiovascular Risk Factors in Type 2 Diabetes: A Systematic Review and Subgroup Meta-analysis.

CONTEXT: Type 2 diabetes (T2D) is one of the fastest-growing global health emergencies of the 21st century. However, high antioxidant capacity of supplementation of vitamin C and/or E was inversely associated with insulin resistance. However, each antioxidant possesses a distinct biological function that may be influenced by both dosage and duration of supplementation, potentially resulting in significantly different effects. OBJECTIVE: This meta-analysis aimed to evaluate whether vitamin C, vitamin E, or their combination is more effective in improving glycemic control, blood lipids, and blo

Evaluating Cardiovascular Disease Risk.

There was a steady decrease in cardiovascular disease (CVD ischemic heart disease and stroke) mortality from 1960 to 2020, but since then, this decline has reversed. There have been over 228,000 excess CVD deaths through 2022,1 undoubtedly partially due to the COVID-19 pandemic, but the mortality rate continues to rise (arguably due to the rising epidemic of obesity and diabetes). CVD remains the leading cause of death in developed countries, accounting for over 30% of deaths, and risk estimation is a cornerstone approach to guiding CVD prevention in clinical medicine. Data from the CDC reveal

Risk-weighted apoB: a novel summary metric outperforming traditional lipid biomarkers in predicting coronary heart disease.

BACKGROUND AND AIMS: LDL-C and non-HDL-C do not fully capture coronary heart disease (CHD) risk attributed to all apoB-containing lipoproteins. Use of apolipoprotein B (apoB) as a marker of total atherogenic particle number improves risk prediction, but risk may still be underestimated when triglyceride-rich lipoproteins (TRL/remnants) and lipoprotein(a) [Lp(a)] are elevated. The aim was to formulate a new metric-risk-weighted apoB (RW-apoB)-designed to capture risk from LDL, TRL/remnants, and Lp(a) in a single number. METHODS: Based on previously published estimates of the relative atherogen

Heteroduplex oligonucleotide technology boosts gene knockdown in cardiac and skeletal muscles.

Recent advances in gapmer antisense oligonucleotides (ASOs) have led to the regulatory approval of ASO therapeutics that target the liver and central nervous system. Efficient delivery of gapmer ASOs to muscle tissue will further broaden their therapeutic applications. Here, we evaluated the knockdown activity of lipid-conjugated DNA/RNA heteroduplex oligonucleotides (HDOs) in muscle tissue. Cholesterol-conjugated HDO (Chol-HDO) exhibited effective gene suppression in both cardiac and skeletal muscles, outperforming unconjugated ASO and cholesterol-conjugated ASO (Chol-ASO). Chol-HDO and Chol-

Apolipoprotein E4 in Alzheimer's Disease: Role in Pathology, Lipid Metabolism, and Drug Treatment.

Alzheimer's Disease (AD) is a neurodegenerative disorder characterized by cognitive decline and memory loss. Among the genetic risk factors linked to AD, the Apolipoprotein E4 (ApoE4) remains the strongest. It is well known that carrying the ApoE4 isoform is associated with advanced AD pathology, blood-brain barrier (BBB) disruption, and changes in lipid metabolism. In this review, we provide an overview of the role of centrally and peripherally produced ApoE in AD. After this introduction, we focus on new findings regarding ApoE4's effects on AD pathology and BBB function. We then discuss Apo

In situ NMR and integrative proteomics reveal the interaction signature of serum α-synuclein.

Alpha-synuclein (αS) plays a central role in several neurodegenerative diseases. Although predominantly neuronal, αS is also present in peripheral fluids such as serum and cerebrospinal fluid (CSF), where it may contribute to disease propagation. However, its extracellular interactions remain poorly understood. By combining in situ NMR spectroscopy and bottom-up MS proteomics, we reveal that monomeric αS exhibits distinct interactomes in serum versus CSF. Both N- and C-terminal αS regions bind to serum components, while in CSF, αS remains largely unbound. The N- and C-terminal interactions are

VLDL cholesterol and ASCVD risk: A population-based study.

BACKGROUND: Observational and genetic evidence show associations of high remnant cholesterol levels with atherosclerotic cardiovascular disease (ASCVD). New drugs have been developed that substantially lower remnant cholesterol; however, the corresponding absolute risk reduction of ASCVD remains unclear. Remnant cholesterol can be measured directly or calculated, but few studies have analyzed the effects of directly measured remnant cholesterol. OBJECTIVE: To estimate the 10-year absolute risk reductions of ASCVD according to proportional reduction of individual very low-density lipoprotein (

APOE4 and cognition in intracranial atherosclerosis: beyond Alzheimer's pathology.

INTRODUCTION: The apolipoprotein E ε4 (APOE ε4) allele is a major genetic risk factor for Alzheimer's disease, but its relevance to cognition in intracranial atherosclerosis (ICAS) remains unclear. We investigated the association between APOE ε4 and cognition in ICAS. METHODS: Baseline data from a multicenter cohort were analyzed. Patients with radiologically confirmed ICAS underwent APOE genotyping, plasma biomarker assays, magnetic resonance imaging assessment of cerebral small vessel disease (CSVD) and brain atrophy, and standardized cognitive testing. RESULTS: Among 409 patients (mean ag

Predicting Intraocular Pressure From Glaucoma Patients Receiving Medication Treatment Using Explainable Machine Learning.

Glaucoma is a chronic neurodegenerative disease of the visual system, and treatment is targeted toward lowering intraocular pressure. However, some patients fail to respond to treatment and their intraocular pressure levels remain high, risking continuous vision loss. Explainable machine learning provides a mechanism for both individual prognostication and the identification of factors associated with treatment outcome. Here, we used explainable machine learning to predict intraocular pressure for glaucoma patients receiving medication treatment. We accessed the UK Biobank to obtain informatio

A common 19 bp APOE enhancer deletion is protective against Alzheimer's disease in African Americans.

The APOE-ε4/ε4 genotype is the strongest genetic risk factor for sporadic Alzheimer's disease, though the relative risk is diminished in individuals with African ancestry. Through analysis of phased APOE alleles, we identify a 19 bp deletion approximately 1.1 kb distal to the APOE 3'UTR in a SPI1 microglial transcription factor binding site. The deletion is present in 60% of African American APOE-ε4 homozygotes and reduces Alzheimer's disease odds ratio relative to individuals without the deletion. The deletion also delays Alzheimer's disease onset in APOE-ε4/ε4 cases with local African ancest

APOE-stratified Proteomic and Metabolomic Analysis Reveals Mitochondrial Dysfunction Inflammation and Lipid Dysregulation in Alzheimer's Disease.

Apolipoprotein E (APOE) ε4 is the strongest genetic risk factor for Alzheimer's disease (AD). However, it is known that other pathways independent of APOE also play a role in AD. Disentangling APOE-dependent and independent effects is instrumental for understanding the biology of AD. We conducted an APOE-stratified multi-omic analysis in multiple large datasets to identify AD-associated plasma proteins and metabolites. More than 64% of the identified proteins were not found in non-APOE stratified studies, and 17% of the proteins showed APOE-specific trends. Mitochondrial dysfunction was associ

Characterization and response to exercise training of HDL-specific phospholipid efflux.

The HDL-specific phospholipid efflux (HDL-SPE) assay is a novel cell-free measure of HDL function that is inversely associated with coronary artery disease. However, the effect of exercise training on HDL-SPE is unknown. The purpose of this study was to examine the effect of exercise training on HDL-SPE in a large, diverse cohort free of overt disease. Clinical and functional measures of HDL were taken before and after 20 weeks of endurance exercise training in 508 participants from the HERITAGE Family Study. Associations of HDL-SPE with HDL-related traits were examined using Pearson's correla

The impact of cumulative low-density lipoprotein cholesterol on prognosis in coronary heart disease patients: A retrospective study based on time-weighted cumulative exposure.

BACKGROUND: While lipids are known prognostic markers in coronary heart disease (CHD), the long-term impact of cumulative low-density lipoprotein cholesterol (cum-LDL-C) and the duration of high LDL-C exposure remains unclear. OBJECTIVE: This study aimed to evaluate their association with major adverse cardiovascular events (MACE) in patients with CHD. METHODS: We included 1271 patients initially diagnosed with CHD between 2018 and 2023, each with at least 2 follow-up records. Time-weighted cum-LDL-C was calculated and categorized by quartiles (Q1-Q4). Duration of high LDL-C exposure was gro
