Pulmonary actinomycosis caused by Actinomyces cardiffensis.

We herein present the first case of pulmonary actinomycosis caused by Actinomyces cardiffensis (A. cardiffensis). A computed tomography (CT) examination revealed a nodule with cavitation in the left upper lobe of the lung. One month later, the lesion had almost disappeared, but a new nodule with peripheral consolidation had appeared in the right middle lobe. Because organizing pneumonia was suspected, prednisolone was begun and improvement was seen. However, two months after the initiation of corticosteroid administration, a chest CT scan showed a lung abscess. The patient underwent surgical r

[Thoracic actinomycosis in the differential diagnosis of neoplasm: a propos of a case].

Actinomycosis is an infrequent infection caused by bacteria from Actinomyces genus that manifests as a chronic, suppurative and progressive disease. It's more common in men. Thoracic actinomycosis occurs in 15% of the cases, and infection of the chest wall is less frequent. The clinical presentation mimics tuberculosis or neoplastic processes. In this article we present the case of a 63 year-old man with no comorbidity, with pulmonary actinomycosis involving the chest wall mimicking a neoplastic process, basing the diagnosis on histopathologic findings.

Nonresolving pneumonia.

A 58-year-old man admitted for evaluation of weight loss and dry cough. Initially, he was diagnosed with community-acquired pneumonia. Despite receiving antibiotics, his clinical status deteriorated and was intubated. Fiber optic bronchoscopy revealed a significant amount of mucopurulent secretions. Bronchoalveolar lavage showed marked amount of sulfur granules identified on stain. Microbiology culture was compatible with Actinomyces israelii. Pulmonary actinomycosis is a rare but important and challenging diagnosis to make. It presents with wide spectrum of clinical and radiologic characteris

[A case of thoracic actinomycosis in the left lung coexisting with pulmonary squamous cell carcinoma in the right lung].

An 80-year-old man was admitted to our hospital with chest pain. Chest computed tomography (CT) showed masses in both lung fields. The findings from a biopsy specimen of the left chest wall showed radially filamentous microorganisms, therefore a diagnosis of thoracic actinomycosis was made. The specimen from the mass in the right upper lobe obtained by transbronchial lung biopsy (TBLB) did not yield a definitive, and we started administration of penicillin. After 10 weeks the mass and consolidation in left upper lobe improved, but the size of the mass in the right upper lobe had increased. FDG

[Pulmonary actinomycosis which was clinically suggested lung cancer; report of a case].

A 57-year-old man had been observed a mass-like shadow on a chest radiograph. One year later, chest computed tomography (CT) revealed the enlargement of the shadow. Positive finding of fluorodeoxyglucose-positron emission tomography (FDG-PET) suspected lung cancer, and lingular segmentectomy was performed. The pathological diagnosis was pulmonary actinomycosis. It is necessary to consider pulmonary actinomycosis is a differential diagnosis of lung cancer.

Pulmonary actinomycosis in a male patient with a tracheal bronchus.

BACKGROUND: Tracheal bronchus is a congenital malformation comprising an abnormal bronchus originating from the trachea or other bronchus. This malformation has been associated with recurrent pneumonia in children, but is rarely associated with infection in adults. Actinomyces species are rare causes of necrotizing pneumonias that often masquerade as malignancy, lung abscesses and tuberculosis. METHODS AND RESULTS: A case involving a 46-year-old man with a tracheal bronchus and chronic pneumonia syndrome is presented. Bronchialveolar lavage and transthoracic needle biopsy demonstrated the pre

[Pulmonary actinomycosis simulating Tobias-Pancoast tumor].

Actinomycosis is a rare infection that may involve several organs. Thoracic involvement is uncommon (10-20%). This article describes the case of a 33 year-old patient, HIV negative, complaining of burning pain in the left posterior chest radiating to the left upper arm and reduced strength and paresthesias. The CT scan showed a mass in close contact with the chest wall. Following a thoracotomy, observation verified a solid pulmonary lesion adhered to the left upper lobe, infiltrating the posterior thoracic wall and apex of the cavity with rich vascularization. An incisional biopsy of the lesio

[Surgery for pulmonary actinomycosis that was difficult to differentiate from lung cancer; report of a case].

We herein report a case of pulmonary actinomycosis that was difficult to differentiate from lung cancer. The patient was a 53-year-old man who visited our hospital with a chief complaint of bloody sputum. Computed tomography (CT) showed a tumor in the anterior basal segment (S8) of the left lower lobe that contained a low density area, and because elevated C-reactive protein (CRP) was also observed, the patient was diagnosed with a lung abscess. As no improvements were seen after 4 months of antibiotic administration, and accumulation was observed at the same site on positron emission tomograp

[Disseminated pulmonary actinomycosis with hepatic injury: a misleading form mimicking a polymetastatic picture].

INTRODUCTION: Thoracic actinomycosis, caused by bacteria of the Actinomyces genus, is a rare infection, with poor prognosis if untreated, whose clinical and radiological picture is misleading, which can simulate a tumoral or tuberculous disease. CLINICAL CASE: This is a case of generalised pseudotumoral pulmonary actinomycosis in a non-smoking, non-drinking 48-year-old man, who for one month has been presenting a cough with haemoptoic sputum, dyspnoea associated with fever and a deterioration in general condition. The clinical examination discovered weight loss, diffuse crackling rales and mu

[Pulmonary actinomycosis. Fine needle aspiration diagnostic].

We present four cases of pulmonary actinomycosis in patients over 40 years of age, two of them with chronic obstructive pulmonary disease (COPD), showing an increase in productive cough, episodes of dyspnea, hemoptysis and long-term fever. Routine chest radiographs revealed segmental air-space consolidation, suggestive of unresolved pneumonia or neoplasm. Computed tomography (CT) scan showed similar findings to the ones previously described. Sputum cultures for mycobacteriae and Mantoux tests were constantly negative. Due to the poor clinical and radiodological outcome of the patients, a fine

Surgery for inflammatory tumor of the lung caused by pulmonary actinomycosis.

BACKGROUND: Actinomycosis is an uncommon chronic suppurative bacterial infection caused by anaerobic bacteria. Pulmonary actinomycosis is even more infrequent and generally simulates a wide variety of pulmonary disorders including tuberculosis and lung cancer. Therefore delayed diagnosis and misdiagnosis is common. Here, actinomycosis was initially confused with pulmonary carcinoma. METHODS: We report on three cases of inflammatory tumors caused by pulmonary actinomycosis. All three patients were male and had a history of alcoholism and poor oral hygiene associated with dental disease. Clinic

Pulmonary actinomycosis presenting as a mass-like consolidation.

Pulmonary actinomycosis presented as a mass-like consolidation. The diagnosis was not established until clinical suspicion prompted special staining of the pathology material. Response to antibiotics was facilitated by a removal of the airway plug through bronchoscopy. Airway obstruction may contribute to the prolonged medical treatment of pulmonary actinomycosis.

Haemoptysis in a patient of achalasia cardia: pulmonary actinomycosis, not tuberculosis.

Actinomycosis is an infectious disease caused by anaerobic gram-positive, non-spore forming bacteria of the genus Actinomyces that affects the oropharynx, digestive tract, and genitalia. Thoracic actinomycosis may affect the respiratory tract and the pleura, even extending to the chest wall. Pulmonary actinomycosis occurs in immunocompetent persons during the fourth and fifth decades of life, with greater prevalence in men, and is generally due to Actinomyces israelii or A meyeri. It is frequently misdiagnosed as primary or metastatic lung cancer or conventional lung infections (eg, tuberculos

[A case of pulmonary actinomycosis mimicking chronic necrotizing pulmonary aspergillosis].

A 60-year-old man was admitted to our hospital with fever, appetite loss, and fatigue. Chest X-ray films and computed tomography scans showed fungus-ball-like lesions in the thoracic cavity, and pleural thickening with surrounding infiltration in the left upper lobe, developing over several months. The white blood cell count (WBC) and serum C-reactive protein (CRP) levels of the patient at the time of admission were 8800/microl and 2.7 mg/dl, respectively. He showed a negative reaction for the serum Aspergillus precipitating antibody, and a positive reaction for the serum Aspergillus antigen (

Pseudoaneurysm of internal mammary artery caused by pulmonary actinomycosis.

Although internal mammary artery pseudoaneurysms are a rare vascular abnormality, they are sometimes seen after sternotomy, diagnostic and therapeutic vascular access or penetrating chest trauma. To our knowledge, internal mammary artery pseudoaneurysm caused by pulmonary actinomycosis has not been reported previously. We report a case of pseudoaneurysm of the left internal mammary artery caused by pulmonary actinomycosis. A 50-year-old woman initially presented with clinical and radiological features of pneumonia, for which she was treated empirically with antibiotics. Later, she developed ha

Organizing pneumonia due to actinomycosis: an undescribed association.

Organizing pneumonia is a pathologic entity characterized by intra-alveolar buds of granulation tissue that can extend to the bronchiolar lumen. It is a non-specific finding reflecting a pattern of pulmonary response to aggression that can be cryptogenic or associated with several causes. Pulmonary actinomycosis is a rare infectious disease, of bacterial aetiology, and of difficult diagnosis. This disease usually causes non-specific respiratory symptoms and radiological findings, and the treatment is based on the use of antibiotics. The authors describe a clinical case of a 53-year-old male sm
