Porphyria comprises a group of rare inherited or acquired disorders characterized by defects in the heme biosynthetic pathway, resulting in the accumulation of porphyrins or their precursors. This study presents three cases of porphyria in Puerto Rico, including erythropoietic protoporphyria (EPP) and porphyria cutanea tarda (PCT). Genetic testing revealed a heterozygous mutation in the FECH gene in the EPP case and an HFE gene mutation in a PCT case with hereditary hemochromatosis. A previously undocumented case of PCT with elevated uroporphyrin levels but negative genetic panel results raise

We report a patient homozygous for the UROD c.185C>T (p.P62L) variant who presents with clinical features resembling familial porphyria cutanea tarda (PCT). This case highlights the limitations of rigid UROD-related porphyria classifications and supports the existence of a phenotypic continuum modulated by genetic, epigenetic, and environmental factors.

Chronic kidney disease (CKD) affects 10% of the world's population and 1 in 7 adults in the United States. Dermatologic manifestations are common in CKD patients and are often challenging to diagnose and treat due to a broad differential diagnosis and lack of consensus guidelines. Prompt recognition of dermatologic disease is crucial for timely management and avoidance of complications. In part 1 of this 2-part continuing medical education series, we review dermatologic manifestations affecting CKD patients. Herein, we discuss epidemiology, clinical and histopathological presentations, and man

Porphyria cutanea tarda (PCT) is a disorder characterized by porphyrin accumulation leading to photosensitivity and skin fragility, often triggered by a combination of genetic, autoimmune, and environmental factors. This case describes a 39-year-old woman with primary Sjögren syndrome (pSS) and heterozygosity for the HFE H63D mutation who developed PCT following hydroxychloroquine (HCQ) therapy. Initial symptoms included xerostomia, fatigue, and skin lesions. Laboratory tests showed liver enzyme elevation, leukopenia, hyperferritinemia, and positive anti-SSA/SSB antibodies. Hydroxychloroquine-

Porphyria cutanea tarda (PCT) is caused by decreased activity of uroporphyrinogen decarboxylase (UROD) due to various susceptibility factors, here among pathogenic variants in UROD and the presence of iron overload. Porphyrins are sensitive to light and cause cutaneous symptoms in the form of blisters and ulcers on sun-exposed skin. This review gives an update on diagnosing, monitoring and the treatment of PCT. PCT is often effectively treated with venesection or/and low-dose hydroxychloroquine. Additionally, patients must be informed about sufficient sun protection and reduction of susceptibi

We present two case reports with resolution of PCT following treatment with direct-acting anti-virals for hepatitis C infection (HCV): An 82-year-old woman and a 60-year-old woman with PCT caused by HCV. Both patients were treated with venesection and hydroxychloroquine for their PCT. The patients received treatment with glecaprevir/pibrentasvir, resulting in the eradication of HCV and long-term remission of PCT. The cases illustrate the importance of screening patients with PCT for underlying HCV, and initiating relevant treatment for HCV, as this may lead to remission in PCT.

Clinically manifest porphyria cutanea tarda (PCT) is a rare condition in childhood; however, several cases have been reported in the literature and at our centre. Porphyria Center Sweden is a national knowledge centre, and since 1987, we have diagnosed approximately 1400 new cases of manifest PCT, of which only five have been children. All children have been identified as heterozygous carriers of a pathogenic UROD gene variant and homozygous for hemochromatosis. In our experience, the diagnosis of children with PCT has been delayed, even in the presence of cutaneous symptoms, without known fam

Hepatitis C, an infection of the liver caused by the hepatitis C virus (HCV), is a growing global health concern, with an increasing annual incidence of primary disease and a notable likelihood of developing extrahepatic manifestations, including dermatological conditions. Studies have explored the relationship between hepatitis C infection and dermatologic manifestations, but there has been a limited number of comprehensive review manuscripts. This review aims to provide a comprehensive summary of studies conducted over a 10-year period, highlighting the relationship between hepatitis C infec

Kindler Syndrome (KS) is a rare, autosomal recessive genodermatosis caused by mutations in the FERMT1 gene, leading to skin fragility, blistering, photosensitivity, and progressive poikiloderma. We present a unique case of KS in a 6-year-old boy born to consanguineous parents, exhibiting uncommon dermatological, and systemic features. The patient developed multiple erythematous plaques, hemorrhagic crusting, and purulent discharge after birth, with a family history suggesting genetic predisposition. Uniquely, the patient presented with well-demarcated hyperpigmented macules on the abdomen, a f

In humans, an enzyme dysfunction in heme biosynthesis results in a heterogenous group of diseases collectively known as porphyrias. From a clinical standpoint, porphyrias can be classified as erythropoietic (congenital erythropoietic porphyria-CEP, erythropoietic/X-linked protoporphyria-EPP/XLP) or hepatic (acute hepatic porphyrias-AHPs, porphyria cutanea tarda-PCT), according to the site of organ dysfunction deemed to be responsible for the disease. In terms of total heme production, the liver accounts for the second major heme-synthesizing organ, after the bone marrow. In fact, heme is neces

INTRODUCTION: Porphyria cutanea tarda (PCT), the most common porphyria, is caused by a decreased activity of uroporphyrinogen decarboxylase (UROD). PCT is generally sporadic, but in about 25% of cases, the disease is transmitted as an autosomal dominant trait (familial PCT). Few cases of PCT with onset in childhood have been reported to date. The aim was to perform a retrospective analysis of these rare cases, including all patients diagnosed with PCT from 1977 to present, with disease onset occurring before the age of 18.
MATERIALS AND METHODS: We analyzed 28 patients, 15 males and 13 female

BACKGROUND: Despite its rarity, porphyria cutanea tarda (PCT) is globally recognized as the most common form of cutaneous porphyria. This study aims to review the underlying associations and treatment of PCT in Scotland.
METHODS: We retrospectively reviewed data on 27 patients diagnosed with PCT between 1987 and 2022 at the Scottish Cutaneous Porphyria Service.
RESULTS: Males slightly predominated (66.7%). The mean ± standard deviation (SD) age at diagnosis was 55.6 ± 12.5 years. Common associated factors were heavy alcohol intake (88.5%), genetic hemochromatosis (72%), smoking (45.5%), and

Pseudoporphyria is an uncommon dermatosis resembling porphyria cutanea tarda (PCT). The exclusion of true porphyria, especially PCT, is critically essential for diagnosing pseudoporphyria. It has an unknown underlying pathophysiology with a normal or near-normal porphyrin profile. Pseudoporphyria has been associated with chronic renal failure and hemodialysis, medications, and tanning beds. In drug-induced pseudoporphyria cases, eliminating the suspected photosensitizing drug improves the disease typically within weeks to months (on average eight weeks). In genetically predisposed individuals,

Hereditary hemochromatosis (HH) is a genetic disorder leading to excessive iron absorption, impacting multiple organs, notably the skin, nails and mucosae. The objective of this study is to elucidate the dermatologic manifestations, associated symptoms, pathophysiology and management recommendations of HH. We searched five primary databases (PubMed, Embase, Cochrane Library, Scopus and Web of Science) up to April 2023. Non-English articles were included to minimize language bias. The studies were evaluated using Oxford Centre for Evidence-based Medicine standards, with adherence to PRISMA guid

Hepatic porphyrias are a group of metabolic disorders that are characterized by overproduction and accumulation of porphyrin precursors in the liver. These porphyrins cause neurologic symptoms as well as cutaneous photosensitivity, and in some cases patients can experience life-threatening acute neurovisceral attacks. This review describes the acute hepatic porphyrias in detail, including acute intermittent porphyria, hereditary coproporphyria, and variegate porphyria, as well as the hepatic porphyrias with cutaneous manifestations such as porphyria cutanea tarda and hepatoerythropoietic porph

Porphyria cutanea tarda (PCT) is the commonest of the porphyrias (Semin Liver Dis 1998;18:67). It often occurs secondary to an underlying internal disorder, has significant impacts on liver health and longevity, and is a treatable disease. Thus, for the clinician, recognising the disease to make the correct diagnosis, identifying causative underlying diseases, and treating the porphyria and its complications, are crucial. Although reviews on the management of PCT have been written, there have recently been significant advances in the understanding of the factors predisposing to the disease, an
