Beyond Gilbert syndrome: exploring the heterozygous UGT1A1 c.41_40dup variant as a potential modifier of bile composition associated with a cholangiopathic phenotype in adults with unexplained chronic cholestasis.

BACKGROUND: Unexplained chronic cholestasis (UCC) in adults is traditionally interpreted as a hepatocellular or canalicular disorder; however, a subset of patients may instead exhibit a cholangiopathy-driven phenotype. Uridine diphosphate glucuronosyltransferase 1A1 (UGT1A1) plays a key role in bilirubin conjugation and bile composition. The heterozygous UGT1A1 c.41_40dup variant, classically associated with Gilbert Syndrome (GS), is frequently observed in individuals without overt GS. In this setting, reduced UGT1A1 activity has been associated with the production of a bile characterized by a

Case Report: Persistent indirect hyperbilirubinemia caused by Gilbert syndrome misdiagnosed as drug-induced liver injury during tuberculosis treatment.

BACKGROUND: Drug-induced liver injury (DILI) is a common complication of anti-tuberculosis therapy and frequently leads to treatment interruption or modification. In routine clinical practice, liver function abnormalities during tuberculosis treatment are often attributed to DILI. However, isolated indirect hyperbilirubinemia in the presence of persistently normal transaminase levels represents an atypical biochemical pattern that is inconsistent with classical hepatocellular injury and may indicate alternative underlying conditions, posing a diagnostic challenge in tuberculosis management. Genetically confirmed Dubin-Johnson syndrome presenting as long-standing isolated conjugated hyperbilirubinaemia.

Isolated hyperbilirubinaemia with normal liver enzymes is frequently attributed to Gilbert syndrome, but predominance of conjugated bilirubin warrants evaluation for inherited disorders of bilirubin transport. We report a genetically confirmed case of Dubin-Johnson syndrome in a young adult with a 10-year history of intermittent jaundice and persistently conjugated hyperbilirubinaemia, despite normal liver biochemistry and imaging. Extensive evaluation excluded haemolysis, viral hepatitis, autoimmune liver disease and structural biliary pathology. Targeted genetic testing identified a homozygo

Research progress of SPP1 in cancer regulation: Tumorigenesis, metastasis, tumor microenvironment, immunity and therapeutic targets (Review).

OBJECTIVES: Psoriatic arthritis (PsA) has been defined as a systemic, chronic, inflammatory arthritis, usually seronegative for rheumatoid factor (RF), associated with cutaneous psoriasis. The exact prevalence of PsA is unknown and its estimation has been difficult, partly due to the lack of a widely accepted classification criteria. Agreed and validated criteria will facilitate comparison between centres and different countries in the areas of epidemiology, outcome studies and therapeutic trials. A number of classification criteria have been published by Moll & Wright (M & W), Bennett's, Vase

Iturin A disrupts Cryptococcus neoformans membrane integrity, attenuates virulence, and synergizes with amphotericin B.

BACKGROUND: Use of the ergot-derived dopamine receptor agonists (cabergoline and pergolide) is associated with an increased risk of cardiac valvulopathy. Pergolide was withdrawn from the US market in 2007 because of the risk of valvular heart disease, while the European Medicines Agency (EMA) required a reduction in the maximum daily dosage of cabergoline and pergolide from 6 mg/day to 3 mg/day in 2008. In Japan, the package inserts of both drugs were revised in April 2007 to request that physicians conduct periodic ultrasonic cardiography (UCG) examinations for patients taking cabergoline or

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Gilbert's syndrome: The good, the bad and the ugly.

Gilbert's syndrome (GS) is a common hereditary condition characterized by mild increases in serum bilirubin levels due to inherited defects in bilirubin metabolism. This review, based on peer-reviewed articles spanning from 1977 to January 2024 and sourced through the PubMed platform, provides an overview of current knowledge regarding GS. Early studies primarily focused on defining the clinical and genetic characteristics of the syndrome. More recent research has delved into the genetic mechanisms underlying the reduced expression of bilirubin UDP-glucuronosyltransferase, significantly enhanc

An evaluation of the performance of the PAP-PCR method in detecting UGT1A1 gene polymorphisms.

BACKGROUND: Gilbert's syndrome (GS) is a hereditary disorder caused by mutations in the UGT1A1 gene, leading to unconjugated hyperbilirubinemia. Accurate detection of UGT1A1 gene polymorphisms is crucial for diagnosis and management of GS. This study evaluated the clinical application of pyrophosphorolysis-activated polymerization PCR (PAP-PCR) method in dection of UGT1A1 gene polymorphisms. METHODS AND RESULTS: Whole-blood samples from 53 patients with GS were genotyped using PAP-PCR for UGT1A1 variations, including UGT1A1*60 (c.-3279T > G), UGT1A1*28 ([TA]6 > [TA]7), UGT1A1*6 (c.211G > A, G

Bilirubin Metabolism Does Not Influence Serum Bile Acid Profiles According to LC-MS: A Human Case-Control Study.

In addition to their role in lipid absorption, bile acids (BAs) are also known for several relevant (endocrine) activities including contributing to the regulation of energy homeostasis and some non-communicable diseases (NCDs). Furthermore, moderately elevated unconjugated bilirubin levels, as observed in Gilbert's syndrome (GS), may protect against NCDs. We therefore hypothesized that the BA profile in GS subjects differs from that of normo-bilirubinemic individuals. To test this, we performed a human case-control study, in which GS (n = 60) and controls (n = 60) were matched for age and gen

Low-grade non-muscle-invasive urothelial carcinoma of the bladder in a 16-year-old boy with UGT1A1 polymorphism and Gilbert's syndrome: A case report.

Bladder cancer in the pediatric and adolescent population is exceedingly rare.Genetic susceptibility factors, including polymorphisms in phase II detoxifying enzymes, may play a role in early tumorigenesis. We report the case of a 16-year-old male with recurrent gross hematuria, ultimately diagnosed with low-grade papillary urothelial carcinoma following transurethral resection of bladder tumor (TURBT). Genetic analysis identified a UGT1A1 polymorphism associated with reduced enzymatic activity, a known determinant of impaired detoxification, and the patient had coexistent Gilbert's syndrome.

Spontaneous Bilateral Drusen Regression Without Atrophy in an Octogenarian With Gilbert's Syndrome: A Case Report.

We document significant drusen regression without atrophy (DRwoA) in a patient with dry age-related macular degeneration (AMD) and concomitant hyperbilirubinemia. Serial optical coherence tomography (OCT) and fundus images were used to monitor drusen progression in the patient. Fundus photography of the macula revealed a dramatic improvement in visible drusen in each eye over the course of 2020-2023. Only a few small drusen were visible in the right eye, while moderate to intermediate-to-large soft drusen were present in the left eye. OCT imaging also demonstrated significant improvement in dr

Challenges in the pathological diagnosis of erythropoietic protoporphyria: a case report.

BACKGROUND: Erythropoietic protoporphyria (EPP) is a rare autosomal recessive disorder caused by mutations in the FECH gene, leading to ferrochelatase deficiency and the accumulation of protoporphyrin in various organs. EPP patients with liver damage have atypical histopathological manifestations, which pose challenges for pathological diagnosis. CONTENT: We report a 29-year-old male with recurrent abdominal pain and scleral jaundice. Initial liver dysfunction suggested autoimmune hepatitis, but liver biopsy revealed dense brownish granular deposits with red birefringence under polarized ligh

Prevalence and Data-Driven Exploration of Pre-Diagnostic Symptoms and Features of Gilbert's Syndrome in the UK Primary Care Population.

BACKGROUND: Gilbert's syndrome (GS) is a common genetic disorder marked by elevated bilirubin levels due to UGT1A1 enzyme deficiency. While jaundice and some adverse drug reactions are the primary recognised clinical features, individuals with GS frequently report non-specific symptoms like fatigue, brain fog, and abdominal pain. This study investigates the symptoms and diagnostic triggers of GS using UK primary care electronic health records. METHODS: We analysed data from the IQVIA Medical Research Database, covering over 11 million active UK patients. Individuals with a recorded GS diagnos
